        In vivo study for the hemostatic efficacy and foreign body reaction of a new powder-type polysaccharide hemostatic agent

        Purpose: Various hemostatic agents have been introduced in therapy as postoperative bleeding is a poor prognostic factor for postoperative outcomes. These products can be divided into those that directly promote the hemostatic cascade and those that physically form a barrier by absorbing blood. The latter, powder-type hemostatic agents have the advantages of being inexpensive and more absorbable with less foreign body reactions (FBRs) and are applicable to a relatively wide area. This study was conducted to verify the safety and efficacy of a newly invented polysaccharide product (OOZFIX, Theracion Biomedical), which improves blood absorption and hemostatic effects. Methods: Two separate animal experiments were performed. The first evaluated FBRs histologically at 3 days, 2 weeks, and 4 weeks, after implantation of OOZFIX in rats, and the second compared hemostatic performance of OOZFIX and Arista AH (Bard) in the porcine liver punch biopsy model. Results: We found minimal FBRs in the 3-day group and no reactions in both the 2-week and 4-week groups after implantation of hemostatic agents. The time to hemostasis of OOZFIX was not significantly different from that of Arista AH (median [interquartile range]: 9 [6–10] minutes vs. 8 [6–10] minutes, respectively; P = 0.522). When comparing the serial bleeding grade tendency, there was no statistical difference between OOZFIX and Arista AH (P = 0.656). Conclusion: OOZFIX caused a minimal FBR that disappeared within 2 weeks in vivo, and its hemostatic performance was comparable with that of an existing agent, Arista AH. Further clinical studies are required in the future.

        학령전기 여아의 하반신 체형 유형분석 : 부산 및 경남지역을 중심으로

        This study was done to provide the fundamental data for scientific and rational children's clothing sizing system by investigating their somatometric characteristics and classifying somatotypes. The subjects were 269 elementary schoolgirls aged from 7 to 8 years old living in Pusan and Kyungsangnam-do. Data from each girl comprises 28 anthropometic measurments and 4 photographic measurments, related to the lower half of body. To analyze somatotypes of the lower half of body, factor analysis, cluster analysis, discriminant analysis were performed far statistical analysis of the data. As to the analysis to draw somatometric factors by this age group, five factors which explain 76.85% of the whole variances were extracted. The first and second factors which explain more than 60% of the whole variances represent 'horizontal size' and 'vertical size', which characterize most aspects of the body shape of the subjects. On the basis of the cluster analysis, three different lower half of body types were categorized. Type I has biggest horizontal size, average vertical size and most protruded belly. Type Ⅱ has average decree of horizontal size, quite big vertical size and most protruded hips. Type Ⅲ has smallest horizontal and vertical size. According to the analysis to discriminate somatotypes of the lower half of body of this age group, weight and waist circumference of discriminant function 1 and abdominal circumference of discriminant function 2 have coefficient values.

        This case study aimed to improve low self-esteem and positive behavior development of a child who lives in a rehabilitation facility. The research questions included the following : 1. Do art therapy effective in improve self-esteem of the child who lives in rehabilitation facility? 2. Do art therapy effective in positive behavior development of the child who lives in rehabilitation facility? The subject of this study was six-years-old girl who lives in S-rehabilitation facility located in Gwangju. Art Therapy for the child has been progressed 16 sessions, twice a week, for 60 to 90 minutes. This case study used House-Tree-Person test supposed by Buck and Behavior Development Checklist reconstructed by the researcher as Measure apparatus. In conclusion, Art Therapy Program was effective in improving self-esteem and positive behavior development of the child who lives in rehabilitation facility.

        열공성 뇌경색과 뇌내출혈의 위험인자 비교

        Cerebral small vessel disease is the most important cause of lacunar infarction (LI) and intracerebral hemorrhage (ICH). It is generally postulated that, in its early stages, the vascular lesion engenders vessel wall fragility and ICH. But if rupture dose not occur, segmental vessel occlusion evolves, producing LI. It may be a common aging phenomenon that is exacerbated by hypertension and diabetes mellitus. This study was performed to evaluate risk factors of LI and ICH. We reviewed retrospectively the medical records of 690 patients (254 LI, 436 ICH) with stroke who were admitted to Keimyung University Dongsan Medical Center from January 1995 to December 1998. The major risk factors including age, sex, hypertension, diabetes mellitus and several minor risk factors were analyzed in these patients. Older age (p<0.01), male sex(p<0.01), hypertension with treatment(p<0.01), hypertension with long duration(p<0.01) and hypertriglyceridemia(p<0.01), cigarette smoking(p<0.01) were significantly more frequent in LI. Younger age(p<0.01), female sex(p<0.01) and hypertension without treatment(p<0.01), hypertension with short duration(p<0.01) were more frequent in ICH. The prevalence, duration and treatment of diabetes mellitus were not different between these two groups. This study show that the age, sex, treatment of hypertension, duration of hypertension and cigarette smoking seem to be significant risk factors between LI and ICH.

        Actinomycosis is a chronic suppurative infection, whitch infiltrates mucosa-associated tissues. Thoracic, pelvic and cervicofacial infections of actinomyces are not uncommon, but endobronchial actinomycosis is rarely reported. We report a case of a 29 year old man who presented with a recurrent pneumonia and chronic cough. Fiberoptic bronchoscopy revealed occlusion of the right. lower medio-basal segment bronchus with exophytic endobronchial mass. The diagnosis was confirmed histologically by demonstration of sulfur granules in the bronchoscopic biopsy of the mass. Intravenous administration of amoxacillin and clindamycin followed by oral amoxacillin and clindamycin therapy resulted in complete recovery.

      • Identification of Novel Functional Null Allele of <b><i>SLC26A4</i></b> Associated with Enlarged Vestibular Aqueduct and Its Possible Implication

        <P>Abstract</P><P>Mutations in the <I>SLC26A4</I> gene, which encodes pendrin, cause congenital hearing loss as a manifestation of Pendred syndrome (PS) with an iodide organification defect or nonsyndromic enlarged vestibular aqueduct (NSEVA, DFNB4). There have been reports of differences between PS and NSEVA, including their auditory phenotypes and molecular genetic bases. For appropriate genetic diagnosis and counseling, it is important to functionally characterize <I>SLC26A4</I> variants. In this study, we identified and evaluated a novel null mutation of <I>SLC26A4</I> and report our method of assessing the pathogenic potential of mutations in <I>SLC26A4</I>, one of the most frequent causative genes of deafness in humans. A 3-year-old female with progressive sensorineural hearing loss and her parents were recruited. They underwent clinical, audiological, radiological and genetic evaluations, which revealed that the female patient had an enlarged vestibular aqueduct and an incomplete partition type II anomaly in the cochlea bilaterally. Sanger sequencing of the <I>SLC26A4</I> gene was also performed. For a confirmatory genetic diagnosis, we first characterized the anion/base exchange ability of mutant pendrin products in HEK 293 cells and, if necessary, evaluated whether the mutant pendrin traffics to the plasma membrane in COS-7 cells. We also expressed a null function mutant, p.H723R, and a previously documented polymorphism, p.P542R, as controls. The pure tone average was 66 dB HL in the right ear and 75 dB HL in the left ear. Sequencing of <I>SLC26A4</I> revealed a known pathogenic mutation (p.H723R) and a novel missense variant (p.V510D) as a compound heterozygote. When we expressed the p.V510D mutant pendrin in mammalian cells, the rate constants for Cl<SUP>-</SUP>/HCO<SUB>3</SUB><SUP>-</SUP> exchange were 10.96 ± 4.79% compared with those of wild-type pendrin. This figure was comparable to that of p.H723R, indicating p.V510D to be another pathogenic mutation with a null function. The p.V510D pendrin product was shown to be entrapped in the endoplasmic reticulum (ER) at 24-30 h after transfection, and not trafficked to the plasma membrane in COS-7 cells, suggesting retention in the ER and abnormal trafficking as the pathogenic mechanism. This was similar to p.H723R, which is a null function founder mutant in this population but is a candidate variant for future drug therapy to rescue the abnormal cell trafficking. Impaired cellular trafficking due to ER retention and abolished exchange activity of the newly detected p.V510D indicates the pathogenic potential of this variant. These missense variants may be good candidate variants for drug therapy if the intrinsic exchange activity is not damaged by the change.</P><P>© 2014 S. Karger AG, Basel</P>

      • Exposure to hypergravity increases serum interleukin-5 and pulmonary infiltration in mice with allergic asthma

        <P><B>Objective</B></P><P>We evaluated the effect of acute hypergravity (HG) on the immune response in a murine model of allergic asthma.</P><P><B>Material and methods</B></P><P>Twenty-eight BALB/c mice were used. Group A (control group, n = 7) mice were sensitized and challenged with normal saline. Group B (control HG exposure group, n = 7) mice were sensitized, challenged with saline, and exposed to acute HG (+10 Gz) for 4 hours. Group C (asthma group, n = 7) mice were challenged with intraperitoneal and intranasal ovalbumin (OVA) to induce asthma. Group D (asthma HG exposure group, n = 7) mice were exposed to HG for 4 hours after the induction of asthma. We estimated the total and OVA-specific serum IgE, serum titers of various cytokines, and the number of eosinophils, neutrophils, and lymphocytes in bronchoalveolar lavage (BAL) fluid. Histopathology of the lung was also evaluated.</P><P><B>Results</B></P><P>The serum level of interleukin (IL)-5 was significantly higher in Group D (12.9 ±4.9 pg/ml) compared to that in Group C (4.7 ±6.5 pg/ml, p = 0.017). In BAL fluid, the number of neutrophils was significantly increased in Group D compared to Group C (p = 0.014). Group D demonstrated a higher infiltration of inflammatory cells (9973.8 ±1642.7 cells/mm<SUP>2</SUP>) compared to Group C (7666.3 ±586.5 cells/mm<SUP>2</SUP>, p = 0.017). This tendency of increase in infiltration was not significant in non-asthmatic animals (Group A: 4488.8 ±176.1 cells/mm<SUP>2</SUP> vs. Group B: 4946.3 ±513.7 cells/mm<SUP>2</SUP>, p > 0.05).</P><P><B>Conclusions</B></P><P>Acute HG exacerbated the allergic response by increasing serum IL-5 levels and promoting pulmonary infiltration of inflammatory cells.</P>
