        Upper respiratory colonization and subsequent postoperative pneumonia caused by gram-negative bacilli are increased in patients undergoing surgical procedure. The present study was to investigate the effect of surgical procedure on postoperative respiratory colonization and subsequent infection caused by gram-negative bacilli. Among patients undergoing various major and minor operations at Hanyang university hospital during the months of November and December of 1985, 50 patients were chosen to evaluate the changes in upper respirtory colonization by gram-negative bacilli and receptivity of buccal epithelial cells to Pseudomonas aeruginosa, a common respiratory colonizer and pathogen. Upper respiratory colonization by gram-negative bacilli was significantly increased from preoperative 10% (5/50) to postoperative 29% (13/45). Klebisiella pneumoniae was the most frequent postopertative isolate (4 strains). Viability of buccal epithelial cells was decresed postoperaively (P<0.05). Receptivity of buccal epithelial cell to Pseudomonas aeruginosa was increased postoperatively (P<0.001) in mean numbers of attached bacteria per cell and in numbers of cells with minimal numbers of attached bacteria and these two parameters were extremely correlated (=0.999). Upper respiratory colonization rate of gram-negative bacilli and increase in receptivity of buccal epithelial cells to Pseudomonas aeruginosa was significantly greater in patients whose operation took more than 2 hours than in those whose operation lasted less than 2 hours (P<0.001). Data obtained in the present study suggest that changes in surface binding properties of upper respiratory epithelium may be an important factor in predisposing to postoperative respiratory infection by gram-negative bacilli.

        The main purpose of patent law is to promote the progress of science and technology by encouraging research and disclosure of results. To support this goal, the patent system gives the inventor a property right and protection from infringement. On the other hand, inventors are also required to specify their patent rights so that the public may know what they are prohibited from doing during theexistence of the monopoly……" Although the patent system intends to protect the patented inventor from would be infringers, there is strong public policy for limiting patent protection to the terms of the claim. patent infringement occurs where either the alleged infringing device li-terally falls within the scope of the claim, or where the accused device performs substantially the same function, in substantially the same way, to obtain substantially the same result as the claimed invention. The latter is the doctrine of equivalents designed to prevent a patented invention from being pirated by minor and unsubstantial changes. This doctrine may be a significant tool for proving infringement, and its use is becoming important to patent infringement litigation. One issue important in applying the doctrine of equivalents concerns the means plus function claim stated in the patent Act of 1952, §112(6). The purpose and the scope of this section has often been confused with the application of doctrine of equivalents. The CAFC has distinguished statutory equivalents from the doctrine of equivalents in interpreting the means plus function claims. Until 1982, infringement actions were heard in district courts and of Appeals of the several circuits. The courts in each circuit decided the same issue independently, and a great inconsistency arose among the circuits. After the Court of Appeals for the Federal Circuit (CAFC) was establishedby merging the Court of Customs and Patent Appeals (CCPA) and the Appellate Division of the Court of Claims (Ct. Cl.), the CAFC addressed several major issues concering claim interpretation in patent infringement litigation, including the doctrine of equivalents. In the application of the doctrine of equivalents by the Federal Circuit, there has been much controversy over the judicial standards for infringement analysis. A most controversial and inconsistent issue is whether infribnge-ment under the doctrine of equivalents should be determined by looking at the invention as a whole" or on an element by element" basis. This controversy relates to the balancing requirement between public policy for limiting the patent claim by restrictive claim interpretation and protection of patented inventions by expensive interpretation of patent claims.

        불평형을 고려한 절삭력 모델에 관한 연구

        This paper is on a new cutting force model which includes the force caused by unbalance. The cutting forces under the different additional masses are measured with the spindle speed changed. The model is justified through correlation between simulation and experimental result.

        TERUDERMIS를 이용한 골 혹은 건이 노출된 창상의 치료

        Deep skin and soft tissue defects with exposed bone and tendon is difficult to treat, because skin graft rarely survives and flap surgery is sacrifice of donor site. Since "Stage I" membrane was developed by Yannas and Bruke in 1980, numerous kinds of artificial skin have been developed. The adaptability of "Terudermis", developed by the Terumo Co., as an artificial skin composed of sponge made of a fibrillar atelocollagen and a heat-denatured atelocollagen, was clinically evaluated on application to 13 cases presenting deep skin and soft tissue defect with exposed bones and tendons from October 1997 to march 1998. Terudermis has the advantage of allowing early incorporation of fibroblasts and capillaries into its collagen sponge due to very weak dehydrothermal cross-linking. Before Terudermis graft, several days of wet dressing and debridement were required to prepare healthy well- vascularized bed because Terudermis was weak on unsanitary wounds. After bed preparation, Terudermis was grafted like usual skin graft. Tie-over bolster dressing or compressive dressing was used case by case. The dressing was opened 2~3 days after Terudermis grafting. Wet dressing was done daily until the skin graft was done. Autologous skin graft was done 2∼3 weeks after Terudermis graft. Our clinical results indicated that Terudermis was beneficial in treating 77% of our patients. Through the use of this new method, treatment of severe skin and soft tissue defects that are usually treated by musculocutaneous or other conventional skin flaps can be replaced by Terudermis as an new artificial dermis.

      • 성인원외폐렴의 원인미생물에대한 전향적 다기관 연구 : 성인원외폐렴의 원인으로 세균의 역할을 중심으로

        Background : Communite-acquired peumonia (CAP) is one of the leading causes of mortality and morbidity. Despite progress in diagnostic techniques and treatments, management of pneumonia remains challenging, because the precise etiology remains uncertain in as many as 49% of cases. The limitaions of identifying etiologic agents make it necessary to use empiric antibiotics in almost all patients, and furthermore emergence of antibiotic-resistant organisms pose difficulties to the selection of an empiric antibiotic regimen. For the optimal choice of empirical antibiotics, we should know the frequency of etiologic agents and antimicrobial resistance rates in the community. Methods : A prospective multicenter study of community-acquired pneumonia in Korea was carried out between May 1997 and April 2000. The microbiologic diagnosis was based on the results of sputum culture, blood culture and pleural culture. Results : Five hundred eighty eight cases of community-acquired peumonia in 562 patients admitted to the hospitals. The mean age was 59.9 with male predominance (58.3%), and 370 (63%) had underlyin gillness. The etiologic agents were identified in 38.3%, and the list of individual agents, in decreasing order, was Streprococcus pneumoniae (21.7%), Klebsiella pneumoniae (14.8%) Pseudomonas aeruginosa (9.8%), Staphylococcus aureus (9.5%), viridans group streptococci (5.7%), Enterobacter cloacae (4.2%), Hemophillus Influenza (3.8%), The rates of admission to the intensive care unit was 10.4%. The motality was 7.1%. Susceptible rates of S. pneumoniae to penicillin was 36.6% and showed multidrug resistant. Forty percents of S. aureus were methicillin-resistant S. aureus. K. penumoniae were susceptible to cephalosporin and quinolone. Conclusion : In Korea, S.pneumoniae is the most important agent causing community-acquired pneumonia. Susceptible rates of S. pneumoniae to penicillin was 36.6% and showed multidrug resistant. Gram negative bacteria such as K. pneumoniae, P. aeruginosa showed high incidence when compared with that of other countries. Most of them had underlying diseases including bronchiectasis and chronic obstructive pulmonary diseases. (Korean J Infect Dis 33:1∼7, 2001)

      • 내시경적 점막절제술로 치험한 식도 과립상 세포종 1례

        과립상 세포종은 Schwann 세포 기원으로 생각되며 인체에 비교적 드물게 발생한다. 과립상 세포종은 전신 어느 곳에서나 발견될 수 있으나 주로 혀, 구강, 피부 혹은 유방 등에서 호발하며 드물게 위장관에서 발견된다. 위장관에서는 식도에서 가장 호발하며 다음으로 위, 대장 순이다. 과립상 세포종은 대부분, 특히 위장관에서는 양성이며 소수의 악성 병변이 보고되었다. 이러한 이유와 함께 수술 전의 진단이 어렵기 때문에 과립상 세포종에 대한 근본적인 치료는 현재까지 외과적 절제술이다. 최근에 시도되는 치료방법들로는 내시경적 레이저 치료, 용종절제술, 내시경적 점막 절제술 등이 있다. 저자들은 상부 소화관 내시경검사를 시행하여 식도 과립상 세포종을 진단하고 내시경적 점막 절제술을 시행하여 합병증 없이 퇴원하여 현재 재발없이 경과 관찰중인 1례를 경험하였기에 보고하는 바이다. Granular cell tumors, which occur infrequently, are probably of Schwann cell origin. They can occur almost anywhere in the body but usually affect the tongue, oral cavity, skin, or breasts and are rarely found in the gastrointestinal tracts. The esophagus is the most frequent gastrointestinal site, followed by the stomach and the colon. Granular cell tumors are generally benign, especially in the gastrointestinal tract, some malignant lesions have been reported. For this reason, and also because preoperative diagnosis is difficult, the standard treatment for granular cell tumor has until now been surgical excision. In recent years, other therapeutic methods is endoscopic laser therapy (ELT), polypectiomy, endoscopic mucosal resection (EMR). We report a case of esophageal granular cell tumor which was diagnosed by an endoscopy and managed using an endoscopic mucosal resection without complication.

        Etiologic agents of horses respiratory diseases and development of management technologies

        Respiratory diseases among horses cause economic losses in foals and productivity decreases in horses being raised and in adult horses. In South Korea, fundamental studies of horses` respiratory diseases and those related to disease occurrence are insufficient. Therefore, the aim if the current study was to use diverse hematological indicators, obtained through hematological tests and measurment of blood antibody level in horses suffering form respiratory diseases, to monitor disease occurrence and prognosis evaluation. Investigations of infection states by horses` bacterial respiratory diseases, susceptible drugs, and resisted drugs according to pathogens showed differences between farms. Among the β-lactam-based antibiotics, cephalosporin-based antibiotics showed some efficacy against Streptococcus spp. To review age-based changes in erythrocytometer hemograms in hematological tests of riding horses, red blood cell (RBC) tended to gradually decrease with increasing age this tendency was statistically significant. In addition, leukocytometer hemograms analyzed by age showed that total white blood cell counts tended to decrease with increasing age. These results of drug susceptibility tests and hematological tests, which have not been presented thus far, will be useful as standardized data in future breeding management and disease tests of Jeju riding horses.
