TERUDERMIS를 이용한 골 혹은 건이 노출된 창상의 치료

Deep skin and soft tissue defects with exposed bone and tendon is difficult to treat, because skin graft rarely survives and flap surgery is sacrifice of donor site. Since "Stage I" membrane was developed by Yannas and Bruke in 1980, numerous kinds of artificial skin have been developed. The adaptability of "Terudermis", developed by the Terumo Co., as an artificial skin composed of sponge made of a fibrillar atelocollagen and a heat-denatured atelocollagen, was clinically evaluated on application to 13 cases presenting deep skin and soft tissue defect with exposed bones and tendons from October 1997 to march 1998. Terudermis has the advantage of allowing early incorporation of fibroblasts and capillaries into its collagen sponge due to very weak dehydrothermal cross-linking. Before Terudermis graft, several days of wet dressing and debridement were required to prepare healthy well- vascularized bed because Terudermis was weak on unsanitary wounds. After bed preparation, Terudermis was grafted like usual skin graft. Tie-over bolster dressing or compressive dressing was used case by case. The dressing was opened 2~3 days after Terudermis grafting. Wet dressing was done daily until the skin graft was done. Autologous skin graft was done 2∼3 weeks after Terudermis graft. Our clinical results indicated that Terudermis was beneficial in treating 77% of our patients. Through the use of this new method, treatment of severe skin and soft tissue defects that are usually treated by musculocutaneous or other conventional skin flaps can be replaced by Terudermis as an new artificial dermis.

선천성 구순열 및 구순구 개열의 임상적 고찰 : -최근 5년간 경험에-

Cleft lip and/or palate has been one of the commonest congenital anomalies. The etiological factors of these anomalies have been statistically and experimentally studied by many scholars, bu they are not yet understood perfectly. So this study was performed to provide the important statistical data in cleft lip and palate. From the clinical data of the 50 patients with cleft lip and cleft lip c palate who had been admitted and operated at Dong Kang Hospital from January 1983 to September 1987, we have obtained the following results; 1. The incidence of cleft lip and/or palate was 2.5 per 1,000 live births. 2. The ratio of cleft lip: Cleft lip with palate was 1.0:0.67. Cleft lip and cleft lip with palate were more common in male. 3. The ratio of left side: right side : bilateral type was 2.8:1.2:1.0. 4. According to the severity, the ratio of complete type:incomplete type was 1.0:2.8 in cleft lip, 9.0:1.0 in cleft lip with palate. 5. There was no statistical singificance in parent's age at birth and birth order. 6. There was statistical significance in season of birth. 7. The rate of positive prenatal history during the first trimester of pregnancy was 30% in medication and 32% in disease of total patients. 8. The rate of positive family history was 8%. 9. The rate associated with other congenital anomalies was 24%, and tongue tie was the most frequent anomaly.

연약지반상 측방유동에 영향을 미치는 요인에 관한 수치해석

When road or river embankment on soft clay foundation constructed, the foundation occur a lateral flow because of consolidation settlement and shear deformation at the same time. A lateral flow happen heaving case and settlement case by which thing superior cause. The study on lateral flow is most composition of soft clay foundation and specially about lateral movement of the abutment piles is studying lateral movement judgement methods by Japanese insitution but phenomenon on lateral flow is not establish the foundmental analysis. In this study, collecting the data measured at the foundation if abroad trial embankment detail introducing soil parameter and field condition have a system analysis. The sensitivity of factor is examined by varing the value of heigth of embankment, thickness of soft clay, shear strength of soft clay of major cause of lateral flow generation.

가돈 피부의 조직 확장후 표면적 변화와 피부의 두께 변화

The objective of this study was focused on the change fo the surface area and skin thickness after full expansion fo the skin of 4 female pigs with 300cc round shaped tissues expanders for 6 weeks. The following is the method of experiment. At first we shaved the backs of 4 female pigs by diameter 15cm sized round and tattooed one by diameter 9cm round and by reqular square of 4cm. One determined the control group and the other determined the expanded group. and we put 300cc round shaped tissue expander on them of the expanded group. We have injected 50cc normal saline a week and fully expanded for 6 seekds. At 6 weeks when we have finished fully expanding in state of puting it in the bakcs. we measured the change of surface area in reqular square, and the thickness of epidermis and dermis after punching the tissues from the control group and the expanded group. After removal of the fully expanded tissue expander, we measured the thickness of dermis and epidermis at intervals of 2 weeks for the 6 weeks. The results were as follows. 1. The surface area of the expanded skin was increased, on an average, 38.64% in comparison with the area of control skin(p<0.05). 2. When the tissues expanders were fully expanded for 6 weeks, the thickness of epidermis in the expanded group, 117.91 ±2.23um, became thicker than that of the control group, 91.68 ±2.47um and the thickness of dermis in the expanded gruop, 3003 ±117.84um, became thinner than that of the control group, 3475 ±223.38um(p<0.05). 3. After 6 weeks following removal of the fully expaneded expanders, the thickness of epidermis in the expanded group. 109.97±9.43㎛, became almost egual to that of the control group, 109.99 ±11.51㎛ and the thickness of dermis in the expanded group, 3495 ± 70㎛, became thicker but is still thinner than that of the control group, 3615 ± 182.30㎛.

관광정보채널에 따른 음식관광의 경험적 가치와 지역 이미지에 관한 연구

This studied empirical value of food tourists through various tourism information channel, there was difference on empirical value of food tourists by food information channel and the purpose of this research was to discover which influence these kind of empirical value affect toward regional image. We conducted survey to collect data for verifying hypothesis and conducted element analysis to confirm credibility and validity about questions of survey. The findings and implications can be summarized as follows: First, we conducted one-way ANOVA for hypothesis verification that there will be difference about empirical value of consumers by purchasing channel and it was shown that tourism information channel affect regardful influence in emotional value, service value and informational value as a result, utilitarian value and playful value didn``t show regardful difference. Utilitarian value was shown high in personal experience or recommendation by acquaintance, service value was shown high in travel related booklets/publications, informational value was shown high in internet advertisement, and playful value was shown high in travel agency/travel exhibition. Second, we conducted multiple regression analysis by assorting total food tourists and food tourists by tourism information channel for hypothesis verification that empirical value of food tourism will affect regardful influence over regional image. As a result of that all 5 empirical value was shown as they affect statistically regardful influence over regional image in total food tourists.

매몰이의 교정술

Cryptotia is a rare anomaly in caucasians, but more common in orientals. This congenital anomaly is an abnormal adherence of the upper part of the auricle to he head, and it has been called "Pocket ear". In cryptotia there is often a deformity of the antihelix, especially its superior crus. The key points of surgery are (1) to cover the skin defect on the posterior aspect of the ear after it is released from the head, and (2) to repair the cartilaginous deformity. We have repaired 4 cryptotic deformities in two patients with aesthetically satisfactory results using Ishii method and advancement of scalp flap.
