        A dog which referred to animal hospital with chief compliment of left hindlimb lameness, was died under the examination, so we observed this case histopathologically. Clinical signs were hindlimb lameness, dyspnea, exercise intolerance, lethargy, hemoptysis, anorexia, and coughing. The gross findings were 5cm in diameters nodule in the caudal abdomen, swelling and milky nodules of the lung, swelling and gray-white patches of the liver, ovarian cyst and dirofilariasis in the heart. In histopathologic examination, metastatic tumors and microfilaria were observed in the lung, liver, spleen, adrenal gland and kidney.

        작문매체와 학습자의 메타인지가 작문의 질과 길이 및 작문태도에 미치는 영향

        본 연구의 목적은 작문에서의 컴퓨터(워드프로세서) 사용이 최근 작문에서 중요하게 논의되고 있는 학습자의 메타인지와 함께 작문의 결과에 미치는 영향을 알아보는 것이었다. 이를 위해 광주에 있는 U중학교 3학년 남학생 80명을 대상으로 메타인지 수준별(상·하)과 워드집단과 지필집단으로 무선 배정한 후 수업시간에 작문을 하도록 하고 작문 결과와 작문 태도 검사를 통해 작문의 길이와 질, 그리고 작문에 대한 태도 및 그 상호작용 효과를 실험하였다. 실험 결과 워드를 사용한 집단과 메타인지 상위 집단의 작문의 길이와 질은 더 좋았으나, 작문의 태도나 작문 매체와 메타인지 수준에 따른 작문의 길이와 질, 태도의 상호작용 효과는 의의 있는 차이가 없었다. 이에 작문 매체와 메타인지 수준은 작문의 길이와 질에 영향을 준다는 것과, 작문의 태도 및 매체와 메타인지 수준의 상호작용 효과는 다양한 실험 설계와 충분한 시간, 워드프로세서를 이용한 작문 기회의 확대 등과 더불어 다시 논의되어져야 할 것임을 알 수 있었다. The purpose of this study was to examine the effects of media and metacognition and their interactive effects on writing quantity, quality, and students' attitudes toward writing. The subjects of this experiment were selected 80 male students from third grades at U middle school in Kwangju city. The research design was 2x2 factorial design with four groups by levels of metacognition and writing medm. On the basis of metacognition test scores, the subjects were divided into two groupshgh vs. low), and these groups were randomly assigned to ane of word-processor writing group and hand-writing group, forming four groups in total. After pretest of writing attitudes, each group were given 45 minutes writing time and took the posttest of writing attitudes after a week. ANOVA and ANCOVA were employed to analyze the data and the significance level, alpha was set at a .05 level. The results were as follows: First, the word-processing group was superior to the hand writing group in writing quantity and quality. second, the high-metacognition group was superior to the low metacognition group in writing quantity and quality. But, writing media and metacognition didn't have a significant effects on student' s attitudes. third, there was no interaction effects between the writing media and the levels of prior metacognition on writing quantity, quality and students' attitudes. Last, in the word-processing group, high metacognition subjects received higher scores than low metacognition subjects.         Fracture healing is a complex physiological process that involves the coordinated participation of several cell types. Unlike other tissues that heal by the formation of poorly organized scar, in fracture healing the original tissue, bone, is regenerated and the properties of the preexisting tissue largely are restored. Fracture healing generally proceeds in an orderly fashion when appropriate biological and mechanical component are present. Many complications may occur with repair and subsequent healing of fracture, including osteomyelitis, implant loosening, and fracture instability. All of these complications may result in delayed union, nonunion or malunion of the fracture. A 1-year-old male, Collie sustained a closed, transverse femoral fracture in a mortor vehicle accident. His initial treatment with open reduction and bone plating was failed. A second surgery was done to remove the plate and perform corrective osteotomy, and bone grafting. Histopathological observation of the bone fragment from fracture end was carried.

        Effects of Phenytoin and Diazepam on the Seizure Activity in the Cortical Dysplasia Animal Models

        Dysplasia-associated seizure disorders are markedly resistant to pharmacological intervention. Relatively little research has been conducted studying the effects of antiepileptic drugs(AEDs) on seizure activity in a rat model of dysplasia. We have used rats exposed to methylazoxymethanol acetate(MAM) in utero, an animal model featuring nodular heterotopia, to investigate the effects of AEDs in the dysplastic brain. Pilocarpine was used to induce acute seizure in MAM-exposed and age-matched vehicle-injucted control animals. Field potential recordings were used to monitor amplitude and numbers population spikes, and paired pulse inhibition in response to stimulation of commissural pathway. Two commonly used AEDs were tested: diazepam 5, 2.5mg/kg;phenytoin 40, 60mg/kg. Diazepam(DZP) and phenytoin(PHT) reduced the amplitude of population spike in control and MAM-exposed rats. However, the amplitude of population spike was nearly eliminated in control rats as compared to the MAM-exposed rats. Pharmaco-resistance was tested by measuring seizure latencies in awake rats after pilocarpine administration(320mg/kg, i.p.) with and without pretreatment with AEDs. Pre-treatment with PHT 60 mg prolonged seizure latency in control rats, but not in MAM-exposed animals. The main findings of this study are that acute seizures initiated in MAM-exposed rats are relatively resistant to standard AEDs assessed in vivo. These data suggest that animal model with cortical dysplasia can be used to screen the effects of potential AEDs.

      • 류마티스성 승모판 협착증의 치료를 위한 경피적 승모판 성형술의 치료성적

        연구목적 : 류마티스성 승모판 협착증에 대한 유용한 치료법인 경피적 승모판 성형술의 본원의 단기 치료 성적을 알아보고자 하였다. 방법: 1993년 10월에서 1999년 4월까지의 이대목동병원에 내원한 류마티스성 승모판 협착증 환자 21명에서 투시조명 하에 경피적 승모판 성형술을 시행하였다. 시술전에 경흥부심초음파와 경식도초음파가 시행되었고, 시술후에는 경흉부초음파를 사용하여 혈역학적, 임상학적 변화를 비교하였다. 연구대상의 거의 모두가 NYHA class Ⅱ에 속했고, 5명이 심방세동을 가지고 있었으며, 모두 심초음파점수가 8이하였다. 좌심방에 혈전이 있는 경우로 혈전을 용해시킨 후 시술한 경우가 2예였다. 결과 : 경피적 승모판 성형술 이후의 평균 승모판구면적은 시술전의 1.16±0.36㎠에서 2.06±0.33㎠로 증가되었다. 경승모판 이완기 평균 압력차는 시술전 11.60±5.54mmHg에서 시술 후 4.93±2.53mmHg(p<0.001). 좌심방 크기는 46.41±14.66mm에서 42.03±15.01mm로(p=0.042), 그리고 심박출량은 4.21±1.25L/min에서 6.88±9.57L/min로(p<0.0001) 의미있는 호전을 보였다. 3도 이상의 심한 승모관폐쇄부전이나 심각한 시술후 합병증은 없었다. 결론 : 본원에서 약 6년간 시행된 경피적 승모판 성형술는 그 사례가 적고 시술전 예상되는 난이도 면에서 특이한 것은 없었지만, 시술후 합병증이 거의 발견되지 않았으며 혈역학적 측정상에서도 성공적인 시술을 보여주었다. 승모판구면적에 따른 시술후 단기내 호전의 정도는 거의 예측할 수 없었으나, 심초음파점수는 모두가 8점이하로 높은 성공율의 조건을 제공하였다. Objective : Percutaneous mitral valvuloplasty(PMV) became a treatment modality or mitral stenosis because of its low morbidity, short hospital stay, and low cost. We reviewed clinical and hemodynamic results after PMV for the patients with mitral stenosis in Ewha Womans University Mokdong hospital. Methods : We compared the results of echocardiographic, hemodynamic, and clinical parameters before and after PMV. PMV was performed under fluoroscopic guidance in 21 patients(M:4, F:17, mean age 43±12 years) with mitral stenosis from October 1993 to April 1999. Transesophageal echocardiography(TEE) and Transthoracic echocardiography(TTE) were performed for the evaluation of mitral valve, chamber size, and the presence of left atrial thrombus before procedures. TIE was also used for follow-up evaluation. On presentation, all patients showed at least NYHA class Ⅱ. Five patients had atrial fibrillation. Two patients with thrombus in the left atrium were included to study group after thrombolytic treatment with coumadin. Echo-score of our patients was not greater than 8. Results : Mean mitral valve area(MVA) by 2 dimensional or Doppler echocardiography was increased from 1.16±0.36㎠ before PMV to 2.06±0.33㎠ after PMV. There were marked improvements in transmitral gradients(11.60±5.54mmHg before PMV vs 4.93±2.53mmHg after PMV, p<0.001), left atrial dimension(46.41±14.66mm vs 42.03±15.01mm, p=0.042), and cardiac output(4.21±1.25L/min vs 6.88±9.57L/min, p<0.0001) following PMV, Severe(≥GⅢ) mitral insufficiency or severe postprocedural complications were not noted. This suggested that all procedure was successful. Conclusion : The Procedural success rate of PMV in Ewha Womans University Mokdong hospital was 100%. Low echo score of our patients might explain this high procedural success rate. Long-term-follow-up is warranted in the near future.

        조혈모세포이식 환자에서 침습성 진균 감염에 대한 Micafungin의 예방 효과 및 안전성

        Background: Micafungin, a potent inhibitor of 1,3-β-D-glucan synthase, is a novel antifungal agent of the echinocandin class. In vitro study showed that micafungin was effective against Aspergillus species as well as Candida species, but clinical data on the prophylactic efficacy against invasive fungal infections (IFIs) other than candidiasis are still lacking. Materials and Methods: We identified 60 consecutive adult hematopoietic stem cell transplantation (HSCT) recipients who received at least 3 doses of micafungin during neutropenic period. Micafungin was started as an alternative in patients who were intolerant or had adverse events (AEs) to primary prophylactic antifungal agents. We retrospectively reviewed the medical records and analyzed the efficacy and safety of micafungin for prophylaxis against IFIs. Results: The patients either had autologous (n=9) or allogeneic (n=51: 1 syngeneic, 24 sibling, 26 unrelated donor) HSCT. Itraconazole oral solution (n=58) was the most frequently used first line antifungal agent for prophylaxis and was administered for median 11 days. The most frequent cause of switch to micafungin was vomiting (n=42). The duration of neutropenia and micafungin administration was median 13 and 12 days, respectively. A successful outcome was achieved in 45 (75%) patients. Empirical antifungal therapy was initiated in 13 (22%) patients. There were 2 cases (3.3%) of breakthrough fungal infections which comprised a probable invasive pulmonary aspergillosis and a possible invasive fungal sinusitis. There was no case of invasive candidiasis. A total of 53 (88%) patients experienced at least one AE regardless of causality during micafungin administration. The most frequent AEs were hypokalemia, vomiting, diarrhea, and elevated serum aspartate aminotransferase or alanine aminotransferase. Among the aforementioned AEs, only 1 case of diarrhea could be classified as a probable relation with micafungin when causality was assessed. There was no AEs that caused discontinuation of micafungin. Conclusions: Micafungin seems to be a safe and effective agent for prophylaxis of IFIs including aspergillosis as well as candidiasis in HSCT recipients. However, further large, prospective, and randomized comparative studies are warranted for aspergillosis.

      • 관상동맥 내 빛간섭단층촬영술을 활용한 관상동맥 내 석회화 병변의 중재시술 후 경과 예측: 전향적 관찰 연구

        Objective Imaging modalities such as intravascular ultrasonography or computed tomography or angiography have shown limitations in assessing coronary calcification. In this study, we investigated whether quantitative indices of optical coherence tomography (OCT) in calcified lesions are correlated with the late outcome of a coronary stent. Methods We consecutively enrolled patients who had more than grade 2 coronary calcification on fluoroscopy. OCT was performed at baseline, immediately after stenting, and at 9 months after stenting. We analyzed qualitative and quantitative characteristics of plaque, calcification, and stent-related indices. Results All images of 3-time points were available in a total of 10 patients. Calcified lesions were frequently associated with thin cap fibroatheroma (100%), plaque erosion (20%), or rupture (20%) before the procedure. Thrombus was found in 100% of cases in the OCT immediately after stenting. Maximal calcium area before stenting was strongly correlated with late luminal loss assessed by 9-month follow-up OCT (r= 0.766, P= 0.01). Conclusion Preprocedural OCT assessment on calcified coronary lesion may predict high-risk procedure and late stent outcome. Further studies are warranted to confirm these findings.

      • VRSA에 대한 항생물질을 생산하는 방선균의 분리

        본 연구가 VRSA에 대한 신규 항생물질 생산 균주를 얻기 위하여 실시하였다. 국내외에 채취한 토양시료에서 분리한 방선균 중, 임상분리 VRSA와 돌연변이 유발 VRSA에 대해 항균성을 나타내는 JY-24를 선별하였다. 선발 균주는 원통형의 포자 모양을 갖고 있으며, Bennett's 배지에 배양시 aerial mycelium color는 흰색을 나타냈으며, reverse side color는 대체로 노란색을 나타내었다. 항균물질 생산을 위한 탄소원으로는 fructose, mannose, 질소원으로는 yeast extract가 가장 우수하였으며 glucose의 첨가로 항균 물질 생산성이 증가하는 것으로 나타났다. 최적 초기 pH는 pH 7로 나타났으며, 28 ℃, 37℃, 45 ℃의 온도에서 균이 성장하는 것으로 나타났다. 또한 배양기간에 따라 항균물질 생산성을 조사한 결과, 임상분리 균주에 대해 5일 배양 시 가장 높았으나, 3일 배양과 7일 배양 시 동일한 생산성을 나타내 생산속도가 빠르며, 안정한 물질인 것이 파악되었다. 향후 균주동정 및 항균물질 정제에 관한 연구를 수행하고자 한다. An antibiotic substance producing strain JY-24, effective to the VRSA (Vancomycin-resistant Staphylococcus aureus), was isolated from soil. The spore chain of JY-24 was cylindrical, and its sulface was smooth. The aerial mass color of the strain was white, and its reverse side color was yellow. The strain did not produce soluble pigment. For the production antibiotics, fructose, mannose and yeast extract were favorable as carbon and nitrogen sources. Optimal initial pH for the production of the antibiotic was pH 7. Accumulation of the antibiotic in the culture broth reached at maximal level after 5 days cultivation in Bennett's medium.

      • 전립선암에 대한 초음파 영상유도 방사선치료의 Setup 오차 분석을 통한 재현성 평가

        목 적: 전립선암 환자에 대한 비침습적이고 피폭선량에 대한 부담이 없는 초음파를 이용한 영상유도방사선치료의 Setup 오차 분석을 통해 치료의 재현성을 평가해 보고자 한다. 대상 및 방법: 치료가 종료된 전립선암 환자 26명의 1,105영상에 대하여 앞-뒤(anterior-posterior, AP), 좌-우(right-left, RL), 위-아래(superior-inferior, SI) 방향의 Setup 오차에 대해 평균, 표준편차와 3D-error을 분석하였다. 또한 오차범위 0-1 mm, 1-3 mm, 3-5 mm, 5 mm 이상에 대한 각 오차 빈도수(frequency, %) 분석을 통하여 AP, RL, SI 방향에 대한 방향성을 확인 하였다. 결 과: Setup 오차는 AP, RL, SI 방향에 대하여 평균과 표준편차는 $1.87{\pm}1.36mm$, $1.73{\pm}1.22mm$, $2.01{\pm}1.40mm$였다. 3D-error는 평균 $3.63{\pm}1.63mm$였다. 각 방향에 대한 Setup 오차값의 빈도(frequency, %)는 AP 방향에 대한 오차범위 0-1 mm에서 29 %, 1-3 mm에서 50.2 %, 3-5 mm에서 19.6 %, 5 mm 이상에서 1.3 %였다. RL 방향에 대하여 오차범위 0-1 mm에서 31.3 %, 1-3 mm에서는 52.5 %, 3-5 mm에서는 15.8 %, 5 mm 이상에서 0.5 %였다. 마지막으로 SI 방향에 대한 오차범위 0-1 mm에서 26.3 %, 1-3 mm에서 50.2 %, 3-5 mm에서 22.4 %, 5 mm 이상에서 1.1 %였다. 결 론: Setup error는 SI 방향에서 $2.01{\pm}1.40mm$로 가장 컸으며 오차 빈도는 모든 방향에서 50 % 이상 오차 범위 1-3 mm에 들어온 것으로 확인되었다. $Clarity^{TM}$ Auto scan을 이용한 초음파영상유도 방사선치료는 치료 전에 방광의 전처치 상태, target의 구조 등을 관찰이 가능하고, 치료 중 target의 움직임을 실시간으로 monitoring하여 빠른 repositioning이 가능하다. 결과적으로 초음파를 이용한 실시간 영상유도방사선치료는 치료의 재현성을 높일 수 있을 것으로 사료된다. Purpose: To evaluate the reproducibility of image-guided radiotherapy using ultrasonography which is non-invasive, without radiation exposure for prostate cancer patients. Materials and Methods: We analyzed the setup variation of 1,105 images for 26 prostate cancer patients and the mean, standard deviation and 3D-error in AP, RL and SI directions. Setup variations were classified 0-1 mm, 1-3 mm, 3-5 mm, 5 mm and more. Results: The mean and standard deviation of setup variation in AP, RL and SI directions was $1.87{\pm}1.36mm$, $1.73{\pm}1.22mm$ and $2.01{\pm}1.40mm$. The 3D-error in AP, RL and SI directions was $3.63{\pm}1.63mm$. The frequency of setup variation in AP direction was 29 % in the range from 0 mm to 1 mm, 50.2 % in the range from 1 mm to 3 mm, 19.6 % in the range from 3 mm to 5 mm and 1.3 % in the range of 5 mm or more. In RL direction, the frequency was 31.3 % in the range from 0 mm to 1 mm, 52.5 % in the range from 1 mm to 3 mm, 15.8 % in the range from 3 mm to 5 mm and 0.5 % in the range of 5 mm or more. SI direction, the frequency of errors in the range from 0 mm to 1 mm was 26.3 %, 50.2 % in the range from 1 mm to 3 mm, 22.4 % in the range from 3 mm to 5 mm, and 1.1 % in the range of 5 mm or more. Conclusion: The setup error was highest in the SI direction of $2.01{\pm}1.40mm$. The frequency in each direction was the highest in more than 50 % in the range from 1 mm to 3 mm. $Clarity^{TM}$ Auto scan is possible to monitoring the motion of the prostate during the treatment and to repositioning the patient. In conclusion real-time image-guided radiotherapy using ultrasonography will be increase the reproducibility of radiation therapy.
