        Lidocaine, Thrombin, Epinephrine 의 항균효과

        목적 : 기관지내시경 검사에서 흔히 사용되는 국소마취제인 lidocaine과 내시경시 지혈목적으로 사용되는 thrombin과 epinephrine이 각종 균주에 미치는 항균효과에 대해 알아보고자 하였다. 재료 및 방법 : 균주는 가톨릭대학교 성모병원에서 2004년 3월부터 2004년 9월까지 임상검체 에서 동정된 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 각각 42, 42, 42, 43주를 대상으로 하였다. Lidocaine, thrombin, epinephrine 감수성 검사는 National Committee for Clinical Laboratory Standards (NCCLS, 2002)의 기준에 따랐다. 결과 : Lidocaine은 S. aureus, S. pneumoniae, P. aeruginosa에서 MIC_(50), MIC_(90) 모두 20,000 ㎍/mL 이었다. K. pneumoniae는 각각 10,000 ㎍/mL이었다. Thrombin은 S. aureus와 P. aeruginosa에서 MIC50 500 lU/mL 과 MIC_(90) 500 IU/mL 이상이었고, K. pneumoniae에서는 MIC_(50)과 MIC_(90)이 모두 500 lU/mL이상이었으나 S. pneumoniae에서는 MIC_(50)과 MIC_(90)은 125 IU/mL이었다. Epinephrine은 K. pneumoniae, S. pneumoniae에 대한 MIC_(50), MIC_(90)가 모두 >500 ㎍/mL이었고, S. aureus와 P. aeruginosa에 대한 MIC_(50), MIC_(90)가 모두 500 ㎍/mL이었다. 결론 : 기관지 내시경 검사에서 흔히 쓰이는 lidocaine, thrombin, epinephrine 등의 약제들이 호흡기 질환의 흔한 병원균인 S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa 균주들에 대해 항균 효과가 있을 수 있어 세균배양검사에 영향을 미칠 수 있겠다. Background : We performed this stody to find out about antimicrobial effect of lidocaine which is commonly used local anesthetic, and thrombin and epinephrine used for hemostasis during bronchoscopic procedures. Materials and Methods : The microorganisms that were cultured from specimens obtained during bronchoscopy were Staphylococcus aureus (n=42), Streptococcus pneumoniae (n=42), Klebsiella pneumoniae (n=42), and Pseudomonas aeruginosa (n=43) collected from St. Mary's Hospital, from March to Sep 2004 were used for susceptibity testing. Susceptibility to lidocaine, thrombin, and epinephrine were tested according to the National Committee for Clinical Laboratory Standards. Result : MIC_(50) and MIC_(90) of lidocaine for S. aureus, S. pneumoniae, P. aeruginosa were all 20,000 ㎍/mL and that for K. pneumoniae were 10,000 ㎍/mL. MIC_(50) and MIC_(90) of thrombin for both S. aureus and P. aeruginosa was 500 IU/mL and above 500 IU/mL, respectively; that for K. pneumoniae were all above 500 IU/mL and for S. pneumoniae they were 125 IU/mL, MIC_(50) and MIC_(90) of epinephrine for K. pneumoniae and S. pneumoniae were above 500 ㎍/mL; that for S. aureus and P. aeruginosa were 500 ㎍/mL. Conclusion : We observed possible antimicrobial effect of lidocaine, thrombin, and epinephrine in vitro against pathogens such as S. aureus, S. pneumoniae, K. pneumoniae, P. aeruginosa, which are common respiratory microorganisms. The use of these agants could affect the result of bacterial culture.

      • CD34+ 조혈 모세포 이식 2례

        Background: In most solid tumors, the CD34 antigen has not been detected, so positive selection of CD34+ cells may reduce tumor cell contamination and the CD34+ cells are capable of reconstituting hematopoiesis. We tried CD34+ cell transplantation in two patients. Method: CD34+ cells from chemotherapy + G-CSF mobilized PBPCs or bone marrow were positively selected with an avidin-biotin immunoadsorption column (CEPRATE SC system). Case 1. One course of chemotherapy using cyclophosphamide(200㎎/㎡) and etoposide (4.2g/㎡), combined with G-CSF(5㎍/㎏) S.C. was used in a relapsed lymphoma patient. This patient responded to the induction chemotherapy. CD34+ cells from harvested bone marrow were selected by the CellPro immunoadsorption column. The total number of mononuclear cells loaded onto the CellPro was 2.4×10^(8)/㎏, with 1.1% CD34+ cells. After column separation, the total number of positively selected cells was 5.16×10^(6)/㎏. The number of CFU-GM was 76.8×10⁴/㎏. This patient was treated with melphalan (140㎎/㎡) and TBI (1200cGy) and the positively selected CD34+ cells were infused. The time to neutrophil recovery greater than 0.5×10^(9)/L was 19 days and the time to platelet recovery greater than 50×10^(9)/L was 21 days. Case 2. Two courses of mobilizing chemotherapy were given 4 weeks apart using taxol(210㎎/m2) and adriamycin(60㎎/m2), combined with G-CSF(5㎍/㎏) S.C. in a breast cancer patients with 7 axillary node metastasis. CD34+ cells from each single leukapheresis product were selected by the CellPro immunoadsorption column. In the first collection, the total number of nucleated cells was 4.4×10^(8)/kg, with 0.42% CD34+ cells. In the second collection, the total number of nucleated cell was 2.8× 10^(8)/㎏ with 0.43% CD34+ cells. After colum separation, the total numbers of collected cells were 4.0×106/kg and 4.8×10^(6)/kg, the total number of CD34+ cells were 1.2×10^(6)/㎏ and 0.82×10^(6)/㎏. Colonogenic assays of positively selected CD34+ cells gave rise to myeloid erythroid, and multilineage colonies, with a median of 190 CFU-GM, 190 BFU-E, and 164 CFU-GEMM per 1×10³ adsorbed cells, respectively. High-dose chemotherapy with cumulative doses of 40mg/㎡ mitoxantrone, 750mg/㎡ thioptepa, and 1000mg/㎡ carboplatin was administered. Positively selected CD34+ cells were rapidly infused 24 hours after the end of high-dose chemotherapy. The time to neutrophil recovery greater than 0.5×10^(9)/L was 16 days and the time to platelet recovery greater than 50×10^(9)/L was 20 days.

        Probiotics는 "장내균종을 개선시켜 줌으로써 숙주동물에게 유익한 영향을 주는 것", "사람 또는 동물 장내 미생물의 균형을 개선시켜 숙주에 효과를 주는 살아있는 미생물 단독 또는 혼합배양물"이라는 의미의 용어이다. Probiotics의 조건에는 효능성, 안전성, 안정성, 분리성의 4가지 조건을 만족시켜야 하며, 식품의 상품성 향상, 면역증강, 유해효소의 합성 억제, 유당 불내증 개선, 장내 유해균의 증식 억제, 장내 균총의 정상화 등 다양한 효능을 가지고 있다. 이런 다양한 효능을 가진 probiotics는 건강식품 및 의료용의 소재, 화장용 소재, 사료 첨가제, 생 백신 개발 재료, 천연 항균물질 생산 등 다양한 곳에 응용되어 사용이 가능하다. Probiotics are living non-pathogenic micro-organisms, which as food ingredients also beneficially affect host's health. These non-pathogenic micro-organisms may be lactic acid bacteria, such as lactobacilli, lactococci, and bifidobacteria, or yeasts, such as Saccharomyces cerevisiae subspp. boulardii, or bacillus, such as B. subtilis, B. natto, B. licheniformis. Probiotics were originally used to influence both animal and human health through intestinal microbiota alterations. At present, the specific live microbial food ingredients and their effects on human health are studied both within food matrices and as single or mixed culture preparations. Probiotics such as lactobacilli have been reported to effect positively, gastrointestinal disorders such as diarrhoea (rotavirus-associated and antibiotic -associated), inflammatory bowel disease, pouchitis, irritable bowel syndrome, colorectal cancer, lactose absorption, Helicobacter pylori infection and constipation and are increasingly used as a treatment modality. Basic and clinical research should thus be associated to meet the enthusiasm that patients show for a therapeutical approach that they feel to be safer than most drugs currently proposed to them.

        생쥐에서 치주조직 형성 과정에 따른 치주인대 세포-특이 PDLs17 단백질 발현 변화

        PDLs17, PDL-specific gone, had previously identified the cDNA for a novel protein from cultured PDL fibroblasts using subtraction hybridization between gingival fibroblasts and PDL fibroblasts. The purpose of this study was to observe the immunohistochemical localization of PDLs17 on the developing periodontium of mice. Developing mice (post-natal mice; P4, P10, P15) were recovered and decapitated, and the heads were fixed overnight at in a freshly prepared solution of 4% paraformaldehyde. Some specimens were decalcified for 2-4 weeks in a solution containing 10% of the disodium salt of ethylenediamine-tetraacetic acid (EDTA). Next, tissues were dehydrated, embedded in paraffin and sectioned serially at 6㎛ in thickness. Polyclonal antiserum raised against PDLs17 peptides. SEAVHETDLHDGC, were made, and immunostained the tooth, periodontium, developing bone and bone marrow of developing mice. The results were as follows. 1. Immunohistochemical analysis showed that PDLs17 is Preferentially expressed in the developing tooth follicle, but not in tooth pulp and overlying gingival mucosa of 4-day-old developing mouse. PDLs17 also expressed in preosteoblast-likec ells in the developing alveolar bone of 4-day-old mouse. 2. PDLs17 expressions were observed in developing tooth follicle, tooth pulp and alvolar bone of 10-day-old developing mouse. 3. PDLs17 protein were observed in PDL cells, tooth pulp and surrounding alveolar bone of 15-day-old tooth-eloping mouse. However, there were no PDLs17 expression in ameloblasts and odontoblasts. These results suggested that PDLs17 might expressed at the initial stage of differentiation when the undifferentiated mesenchymal cells in the bone marrow and PDL, differentiatiation from tooth follicle. However, more research needs to be performed to gain a better understanding of the exact function of PDLs17 during the differentiation of tooth pulp cells and PDL cells.

      • Mongolian gerbil 심장동맥의 해부학적 연구

        The arterial supply to heart of mongolian gerbil has been described. Observations were made by the specimens injected with latex after the flowing through in left ventricle. The results were summaried as follows; 1. A. coronaria arose from the initial portion of Aorta in heart and A. coronaria dextra and sinistus arose in Aortic sinus on 10 specimens. A. coronaria sinistra arose more near boundary of Aortic sinus than A. coronaria dextra. 2. A. coronaria sinistra which arose from Aortic sinus divided into Ramus interventricularis sinistra and Ramus circumflexus sinistra. A. coronaria dextra which arose from Aortic sinus only runed continously as Ramus interventriculares dextra. 3. Ramus interventricular sinistra not reach the heart apex through the sulcus interventricula but stopped in thoracocostal plan extending to under part. 4. R. circumflexus runed theough sulcus coronarius, thereafter it runed from left ventricle to Sulcus interventricularis dorsalis. 5. A. coronaria dextra runed to diaphargm side through sulcus interventricula dorsi thereafter it became Ramus interventricular dextra. It distrubuted left and right ventricle wall on 8 specimens and 2 specimen stopped margo obtusus.

      • 장구균의 동정에 대한 PCR 방법의 유용성

        Background : Enterococci are important cause of nosocomial infections. Recently, vancomycin-resistant enterococci (VRE) has been increasingly reported as significant nosocomial pathogens. Therefore, accurate identification of enterococcal species is a prerequisite step for the appropriate antibiotic treatment and epidemiologic surveillance. We wanted to know the usefulness of PCR method compared with Vitek automatic identification system. Methods : Totally 105 isolates were identified on the species level by Vitek (GPI card and software version R06.1), methyl-α -D-glucopyranoside test, and PCR methods. Results : Among 105 enterococcal isolates, 59 were identified as E. faecium, 11 E. faecalis, 6 E. gallinarum by Vitek. But 29 isolates (28%) were unidentified. Subsequently all of these isolates were analyzed by PCR, the results of which were as follows : 17 E. faecium, 5 E. casseliflavus, 7 E. gallinarum. Two isolates identified as E. gallinarum by Vitek were reidentified as E. casseliflavus by PCR and other methods far phenotypic characterization. Conclusoin : PCR method was more accurate and sensitive than Vitek for the identification of enterococci species. (Korean J Infect Dis 33:123∼127, 2001)

        중성자에 조사된 원자로 압력용기 재료(SA508)의 Magneto-acoustic emission 효과

        원자로 압력용기 재료인 SA508 steel을 온도 70 ℃와 대기압하에서 최고 10 n/㎠까지 중성자를 조사시켜 조사량에 따른 미세경도 변화와 magneto-acoustic emission(MAE) 에너지를 측정하였다. 중성자 조사에 따른 경도의 변화는 조사량이 10 /㎠까지는 거의 일정하였으나, 조사량이 10 /㎠이상에서 급격히 증가하였다. MAE 에너지의 변화는 중성자 조사량에 따라 경도의 변화와 같은 형태로 변하였으나 그 변화량은 감소하여 그변화의 추이는 경도의 변화와는 역의 형태였고, 또한 MAE에너지의 상대적 변화와 경도 변화사이에는 아주 좋은 선형성을 보였다. 이러한 결과에서 SA508 강재는 10 /㎠이상의 중성자에 조사될 경우에 재료에 중성자 조사에 의한 미세 결함이 급격히 증가하여 전위(dislocation)이동에 대한 저항성을 나타내는 마찰경화의 증가가 경도의 증가를 유발하고, 또한 이러한 미세 결함은 자기장과의 반응에서는 90°자벽의 운동중에 자기탄성 변화를 유도하여 MAE 에너지의 감소를 유발함을 알 수 있었다. 그리고 경도의 변화량보다 MAE 에너지의 변화량이 더 크게 나타나, 중성자 조사에 의한 미세결함은 기계적 성질보다 자기적 성질에 더 민감하게 반응한다는 것을 알 수 있었다.따라서 MAE가 중성자 조사에 의한 재료의 미세 구조 결함을 비파괴적인 방법으로 평가하는 강력한 도구의 가능성이 있음을 알 수 있었다. Magneto-acoustic emission (MAE) energy and hardness were measured in the reactor pressure vessel steel (SA508 Steel) for the various neutron fluence, irradiated dose up to 10 n/㎠. The hardness was nearly a constant up to 10 n/㎠, but it was rapidly increased with an increase of the neutron irradiation above 10 n/㎠. It may be considered that the increase of hardness is due to the hindrance of dislocation motion induced defect clusters by irradiation. On the other hand, the MAE energy was slowly decreased as the neutron irradiation increased up to 10 n/㎠ and it was rapidly decreased with an increase of the neutron irradiation increased up to 10 n/㎠ and it was rapidly decreased with an increase of the neutron irradiation above 10 n/㎠. The decrease of the MAE energy may be considered as an increase of the defect clusters which is very sensitive to the 90°domain wall motion. Furthermore, the change of MAe energy and hardness had nearly a linear relationship, but the change of MAE energy was more significant than the change of the hardness. Therefore, MAe may be considered as a very useful technique for the nondestructive evaluation of irradiation damage.

        청소년의 직업 및 진로의식에 관한 연구

        The purpose of this study was to analyze the perceptions of the youth toward their career choice, awareness and their needs of placement office in the university. The questionnaire was consisted of demographic information, career choice and exploration, desired career fields and the needs of placement office system in the university. The data were collected from the youth and analyzed by SPSSWIN 7.5, and the statistical techniques used for this study were crosstabs(X^2-test), frequencies, and percentage. The findings were : (1) the youth usually have considered their career choice before and after their graduation. (2) the youth want to get their own business for avoiding the uncertainty of job markets. (3) the youth stressed mainly of the key competencies to get a job. (4) the placement office system was not existed or not operated enough for preparing their career preparation and choice. (5) the youth perceived that it was hard to get the information regarding job qualifications and career markets. (6) only 4.7% of the respondents visited the job infromation center or placement office. Based on the findings, recommendation was derived that the job information center or placement office should be improved or set it up for their career guidance and job search, and the key competencies on the career should be emphasized on the curriculum planning.

        중성자에 조사된 원자로 압력용기 재료의 Barkhausen 노이즈에 관한 연구

        원자로 압력용기 재료인 SA508 steel을 온도 70℃와 대기압하에서 최고 10 n/㎠까지 중성자를 조사시켜 조사량에 따른 자기이력곡선과 Barkhausen noise amplitude(BNA), Barkhausen noise energy(BNE) 및 미세경도 변화를 측정하여 이들 사이의 상관관계를 구하였다. 중성자 조사에 의하여 자기이력곡선은 변화가 없었으며, BNA, BNE 및 경도는 중성자 조사량에 따라 세 단계의 변화를 보였으며, 중성자 조사량이 10 n/㎠에서는 급격히 변하여 이들 사이의 상관관계가 있음을 알 수 있었다. 따라서 BNA와 BNE가 중성자 조사에 의한 재료의 미세 구조 결함을 조사하는데 비파괴적인 방법으로 가능성이 있음을 알 수 있었다. Hysteresis loop, Barkhausen noise(BN), and hardness were measured in the neutron irradiated RPV steel for various fluence, irradiated dose up to 10 n/㎠. The coercivity, remanence and maximum induction of neutron irradiated samples did not change significantly, but the BNA and BNE were decreased as the neutron irradiation increased. The changes of BNE and BNA were characterized by three stages with respect to neutron dose. The BNA and BNE were decreased with an increase of neutron dose to 10 n/㎠, and remained nearly constant up to 10 n/㎠, then were decreased rapidly with an increase of the neutron dose above 10 n/㎠. On the other hand, the hardness was observed revesely with the change of BNA and BNE.

      • 복합구조물의 파괴에 관한 치수효과의 중요성

        In this paper, the importance of the size effects on the strength ratio is demonstrated by numerical results. The rate of decrease of tensile strength is for glass fiber, based on the experience of a composite manufacturing specialist. For other material, similar procedure may be used until detailed test result on such material is available. The strength criteria used is that of Tsai-Wu for stress space. The factors influencing the ratio are, reducing the tensile strength alone or both tensile and compression strengths, selection of the normalized interaction term, that is, the generalized Von Mises criterion or the Hill's criterion, and the status of applied stresses. Some of the numerical results are presented for a guideline for the furture study.
