        한국인 주요우울장애 환자에서 혈중 ACTH, Cortisol 농도와 해밀턴 우울 평가 척도의 신체증상 항목과의 상관관계

        Objectives:Hypothalamic-Pituitary-Adrenocortical(HPA) system dysregulation plays an important role in the pathophysiology of major depressive disorder. Medically unexplained somatic symptoms comprised the predominant complaints of korean patients with major depressive disorder. It might be related to dysregulation of HPA system and somatic symptoms. The aim of this study was to investigate the association between Hamilton Depression Rating Scale severity and the Plasma Adrenocorticotropic Hormone(ACTH) & Serum Cortisol level in Korean patients with major depressive disorder. Methods:Our study design was prospective. A total 111 hospitalized patients with major depressive disorder (DSM-IV) at Anam Hospital, Korea University Medical Center between Jan 2009 and May 2011 were selected. Hamilton Depression Rating Scale was measured by psychiatrist. Plasma ACTH, Serum Cortisol levels were analyzed at the time of admission. Results:There are significant correlations(p<0.05) between Hamilton Depression Rating Scale of somatic component(HAM-D 11-15 item) and Plasma ACTH and Serum Cortisol level. However, other HAM-D sub items were not significantly correlated with Plasma ACTH and Serum Cortisol level except somatic component. Conclusion:Our Study results suggest that hyperactivity of HPA axis might be correlation with somatic symptoms in korean patients with major depressive disorder.

      • 만성요통환자에 있어서 근에너지기법과 치료마사지의 복합프로그램 적용 효과

        In order to compare and analyze major techniques employed by the college judo competitors of Korea who participated in the 43rd National Male and Female Judo Championships by Weight Class, the players in the light, middle and heavy weight classes were classified into 3 categories: (1) those who are taller than opponents(by +4 cm or more); (2) as tall as opponents(-3~+3 cm); and (3) smaller than opponents(by -4 cm or less). Scoring points and used techniques were examined from videotapes and matching records, which resulted in the following conclusions: 1. In case of light weight players who are taller than opponents, foot(34.73%) was used most frequently and effectively, followed significantly by hand(28.54%), pressing(15.35%), waist(15.31%) and pull-down(4.25%) sacrifice throw techniques(1.66%) in order(p<0.00l). Those who are as tall as opponents used foot techniques(34.88%) more effectively than hand although hand techniques(38.38%) were employed more frequently. For those who are smaller than opponents, hand techniques(35.03%) were used most frequently and effectively, followed significantly by foot(32.57%), waist(8.39%), pressing(16.12%) and pull-down sacrifice throw techniques(7.8%) sequence(p<.001). 2. In case of middle weight players who are taller than or as tall as opponents, foot(30.72%) was used most frequently and effectively, followed significantly by hand(35.42%), pressing(21.32%), waist(8.15%) pull-down sacrifice throw(0.42%) and cross lock(l.06%) in order(p<0.001). However, for those who are smaller than opponents, hand techniques were used more frequently and effectively than pressing, waist and cross lock

        네트워크다이아그램 기법을 이용한 프로토타입 그린빌딩 환경성능 평가프로그램 구축 연구

        The purpose of this study was to analyze the convenience, dearness and effectiveness of the rating system of the GB. This Green Building uses a rating system which includes a network-diagram; Sorensen-network as well as a Sequence-diagram. This study is consisted of a visualized work sheets that were divided into network-diagram, annual energy consumption, air pollution as well as LCC. It has technique of separated rating for the three departments that of the globe environment conservation, health and convenience, construction ability, as well as highest mark such as the object building's performance was which is not considered with non environmental damage level. This program aimed at establishing a weight given to input to default value and needs constant study for this program. Also this study established a inventory database for estimating air pollution quantity.

        네트워크-다이아그램기법을 이용한 그린빌딩 환경성능 평가방안에 관한 연구

        The purpose of this impact statement was improve to a method of rating and design for the environmental performance of the green building by that an analyze to the weighting point and the structure of the five rating tools: GBTool, LEED, BREEAM, Korean government's method, CAS BEE. For improve to the accuracy, convenience, effectiveness of the rating and design method from the result of analyzed five tools, this impact statement was suggestion followings; 1) Divided assessment and public announcement that 'Global environmental conservation department' and 'Health & convenience department' at assess and design to the environmental performance of the green building. 2) Using by modified Network-diagram method in the EIA method at assess and design to it. 3) Arrange each 25% of weighting points of energy consumption criteria and air pollution criteria according to above divided condition.

        사업장 소음 폭로에 의한 일과성 역치 상승과 회복

        To determine the recovery time from noise-induced temporary threshold shift(TTS), a prospective field study was conducted at three worksites where workers are known to be exposed high level of noise. Subjects were selected according to answers on a questionnaire which inquired about otological history and previous noise exposure, including avocational, military and occupational exposures. After excluding employee with past otologic problems, recent exposure to high level noise, and under medications, total 92 employees participated in the study. Among 92 participants, complete consecutive audiometric examinations were carried out at 0∼2 hours, 5∼7 hours, 14∼16 hours after worktime noise exposure on 26 participants wearing hearing protectors and 22 participants wearing no protective devices. The difference between the hearing level 0∼2 hours after noise exposure and 5∼7 hours is statistically significant by paired t-test(p<0.01). The median recovery times calculated from the data of 22 participants wearing no protective hearing devices are 15.6 hours at 4000Hz, and 7.7 hours, 10.3 hours, 8.4 hours at 1000Hz, 2000Hz and 8000Hz respectively. These data suggest that when measuring the pure tone audiometry for noise exposed workers, at least 16 hours noise-free interval is required.

        Baumann Skin Type in the Korean Male Population

        Background: Research into the Baumann skin type (BST) has recently expanded, with growing interest in the development of an efficient and effective skin type classification system for better understanding of this skin condition. Of the total respondents, OSNW type accounted for the largest percentage and ORPW type the lowest. In terms of Baumann parameters, O type was 53.5%, S type was 56.1%, N type was 84.4% and W type was 57.5%. Several behavioral factors were found to have various relationships with the skin type. Conclusion: The predominant skin type in the Korean male respondents was OSNW type, and the distribution of skin types with regards to age and region was reported to be distinct.         Background: Nearly 25 genetic loci associated with susceptibility to ankylosing spondylitis (AS) have been identified by several large studies. However, there have been limited studies to identify the genes associated with radiographic severity of thedisease. Thus we investigated which genes involved in bone formation pathways might be associated with radiographic severity in AS. Methods: A total of 417 Korean AS patients were classifi ed into two groups based on the radiographic severity as defi ned by the modifi ed Stoke` Ankylosing Spondylitis Spinal Score (mSASSS) system. Severe AS was defi ned by the presence of syndesmophytes and/or fusion in the lumbar or cervical spine (n=195). Mild AS was defi ned by the absence of any syndesmophyte or fusion (n=170). A total of 251 single nucleotide polymorphisms (SNPs) within 52 genes related to bone formation were selected and genotyped. Odds ratios (OR) and 95% confi dence interval (95% CI) were analysed by multivariate logistic regression controlling for age at onset of symptoms, sex, disease duration, and smoking status as covariates. Results: We identifi ed new loci of bone morphogenetic protein 6 (BMP6) associated with radiographic severity in patients with AS that passed false discovery rate threshold. Two SNPs in BMP6 were signifi cantly associated with radiologic severity [rs270378 (OR 1. 97, p = 6. 74 x 10-4) and rs1235192 [OR 1. 92, p = 1. 17 x 10-3]) adjusted by covariates. Conclusions: This is the first study to demonstrate that BMP6 is associated with radiographic severity in AS, supporting the role wingless-type like/BMP pathway on radiographic progression in AS.

        Use of Disease-modifying Antirheumatic Drugs After Cancer Diagnosis in Rheumatoid Arthritis Patients

        Objective: There is no recommendation for the use of disease-modifying antirheumatic drugs (DMARDs) in patients with rheumatoid arthritis (RA) who developed cancer. We examined changes in the DMARDs prescription patterns associated with cancer diagnosis in RA patients. Methods: We reviewed the medical records of 2,161 RA patients who visited rheumatology clinic between January 2008 and February 2017 and found 40 patients who developed cancer during RA treatment. In these patients, we examined DMARDs prescription patterns before and right after cancer diagnosis and at recent outpatient clinic visits. Results: Before cancer diagnosis, methotrexate (MTX)-combined conventional synthetic DMARDs (csDMARDs) were most commonly prescribed (22, 55.0%) and biological DMARDs (biologics) in nine patients (22.5%). For cancer treatment, 19 patients received chemotherapy (including adjuvant chemotherapy) and 21 patients had surgery only. Right after cancer diagnosis, changes in the DMARDs prescription patterns were similar in discontinuation (13, 32.5%), switching (14, 35.0%), and maintenance (13, 32.5%). DMARDs were discontinued more frequently in the chemotherapy group (9/19, 47.4%) than the surgery only group (4/2, 19.0%) (p<0.05). Among the 13 patients who discontinued DMARDs, nine (69.2%) resumed DMARDs after a median of 5.5 months (interquartile range [IQR] 2.9, 18.3) due to arthritis flare. At a median of 4.6 years (IQR 3.3, 6.7) after cancer diagnosis, 25 patients were evaluated at recent outpatient clinic visits. Four patients received no DMARD, three MTX monotherapies, 11 csDMARDs combination therapies, and seven biologics. Conclusion: A significant number of RA patients who developed cancer during RA treatment were still receiving DMARDs including biologics after cancer diagnosis.

        Effects of Change in the Height of Therapy Tables on the Fatigue of Splenius Capitis Muscle and Trapezius Muscle

        The purpose of this study is to analysis of muscle fatigue in the upper trapezius and splenius capitis muscles according to therapy table height variation. The subjects were consisted of 15 healthy adults(10 males, 5 females) who had no medical history of neurological and musculoskeletal problems. In experiment, wireless electrode EMG system was measured for each the upper trapezius and splenius capitis muscles during the treatment performed on table. the differences in the muscle fatigue was compared for 4 types of table height(-6cm, -3cm, 0, +3cm from elbow in 90° flexion position). Muscle fatigue according to therapy table height were significant difference except for left upper trapezius. And muscle fatigue of right upper trapezius and splenius capitis showed significant decrease in +3cm table height compared to -6cm table height(p<.05). Muscle fatigue of right upper trapezius and splenius capitis were the highest in -6cm table height, but those were the lowest in +3cm table height. This study propose to change therapy table height higher than +3cm from elbow in 90° flexion position, if you hope to reduce muscle fatigue.
