        In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

        The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

      • S-542 Indomethacin aggravates the renal injury by inhibition of adenosine-medited renal protection in AKI

        Background: Ischemia-reperfusion injury (IRI) is a leading cause of acute kidney injury with high morbidity and mortality due to limited therapy. AKI emerges in various clinical settings and is complex with outcome linking oxidative stress, inflammation, and cell death. Therefore protection of AKI is still an unsolved problem. Indomethacin is generally known that it inhibits the production of prostaglandins through the inhibition both cyclooxygenase (COX) 1 and 2. Prostaglandins have a wide variety of effect such as regulation of vasodilation, inflammation, regeneration, pain, fever. Therefore effect of indomethacin in AKI is different according to injury model. We investigated whether indomethacin which inhibits the production of prostaglandins aggravate the renal injury in AKI mouse model. Methods: Male C57/BL6 mice (8-10 weeks old, weight 20~25 g) were used. Acute kidney injury is induced by bilateral kidneys pedicle clamping which were subjected to 20 min or 30 min at both kidneys. Mice were treated with indomethacin at before and after injury. Blood and kidney samples were collected at 24 hr after IRI. The expression level of creatinine, N-gal & Kim-1 were detected in serum. And the expression level of PGE2, cAMP and adenosine were detected in kidney. Kidney Injury score were measured by HE staining and TUNEL. Results: In bilateral AKI model, Serum NGAL level and creatinine level were significantly highest in indomethacin treated group compared to non-treated group (NGAL, p<0.05; creatinine, p<0.01). Indomethacin treated group showed significantly more necrosis and apoptosis compared to non-treated group. Furthermore, Indomethacine inhibited the production of prostaglandins, cAMP and adenosine. Conclusions: Indomethacin inhibits adenosine-mediated renal protection by inhibition of prostaglandin production in AKI. Therefore Indomethacin worsened renal injury by inhibition of prostaglandin production in AKI. * This research was supported by a grant of the Korea Health Technology R&D Project through the KHIDI, funded by the Ministry of Health & Welfare, Republic of Korea (grant number :H15C2212)

        원발성 피부 CD30 양성/ALK 음성 퇴형성 대세포 림프종 1예

        Primary cutaneous CD3O(Ki-1) Positive anaplastic large cell Iymphoma(ALCL) is a rare subset of cutaneous Iymphoma. with a much better prognosis. ALCL is a heterogeneous process that may have a T-cell, B-cell, or indeterminant(null) phenotype and which may or may not express the anaplastic Iymphoma kinase(ALK) oncoprotein. We report a case of ALCL in a 72 year old man. About 4 months ago, multiple erythematous firm ulcerative mass and satellite nodules developed on the right lower leg. The skin lesions rapidly increased in number and size. Some lesions became painful and centrally ulcered. The histologic findings showed a diffuse infiltrate of large Iymphocytes with large nuclei, prominent and multiple nucleoli, and ample cytoplasm. Immunohistochemical stainings for CD3O, CD5 were positive but stainings for LCA, CD3, CD45RO, CD2O, cytokeratin, EMA, and ALK were negative. Therefore, we diagnosed our case as CD+/30ALK- ALCL (Korean J Dermatol 2003;41(5) : 666~669)

      • 과학 포트폴리오 체제의 적용이 초등학생의 사회심리학적 교실 환경에 미치는 효과

        The purpose of the research is to investigate the effects of a portfolio system on students' socio-psychological classroom environments in elementary science class. Two schools were selected from a metropolitan area, a city, and one school from a rural area in central Korea. Two fifth-grade classes were sampled from each school selected, and one class is assigned to experimental group and the other to control group, respectively. The total number of participants of the study is 399. Portfolio system was administered to experimental group, and conventional teaching to control group for two and half month. WIHIC, an inventory to measure socio-psychological classroom environment, was administered as a pre-and post-test. Experimental group gained statistically higher scores on WIHIC after administering portfolio system. They showed more positive perceptions on sub-areas of WIHIC such as teacher support, task orientation, and equity. There is interaction between region and group: the effects of the portfolio system is highest in rural area, lowest in metropolitan area, and the city in between. Portfolio system have positive effects on students' socio-psychological classroom environments. Enhanced socio-psychological classroom environments is expected to contribute to positive self-concept, higher science achievements, and self-directed learning.

        중고등학생들의 과학 그래프 작성 및 해석 능력

        그래프의 상징적인 의미를 학생들이 해석할 수 있다고 교사들이 종종 가정하는 반면, 이러한 가정은 견고한 연구에 기초를 두고 있지 않다. 따라서 그래프를 구성하거나 해석하는 능력을 학생들이 지니고 있는지 알아보는 연구가 필요하다. 또한 불행하게도 많은 학생들이 이러한 그래프 기능을 제대로 갖추지 못하고 있다는 연구결과들을 결부시켜 생각해 볼 때, 이 영역이 연구할 가치와 내용이 많음을 알 수 있다. 따라서 우리나라 7학년에서 12학년에 이르는 학생들의 그래프 능력은 어떠한지 알고자 TOGS(The Test of Graphing in Science)검사를 실시하였다. 학년이 올라감에 따라 그래프 능력도 점차적으로 향상되는 결과를 보였다. 그러나 그래프 능력의 하위요소로 선정된 9가지 요소 중에서 그래프를 작성하는 능력과 관련된 세 가지 하위 요소, 즉 축에 눈금을 매기는 기능, 축에 관련된 변수를 지정하는 기능 및 경향을 알도록 실험데이터로부터 적적한 하나의 선을 그리는 기능에서 부족함을 보였다. 이러한 결과는 그래프와 관련된 교육에서 그래프를 작성하는 것보다 해석하는 쪽에 상대적으로 더 치중하였음을 시사해준다. TOGS검사에서 좋은 점수를 받은 학생들일수록 이러한 차이점이 더 두드러지게 나타났다. Science teachers often suppose that students are able to know the symbolical meaning of graphs when they see the graphs. But such a assumption is not based on the firm theories but a mere image. And we need to search them for holding the abilities to construct and to interpret. In addition, unfortunately, many researchers show that they scarcely have the graphing skills. And then, The Test of Graphing in Science(TOGS) was administered to 535 7th to 12th graders, for we search them for holding the graphing abilities to some degree. Though the higher grade, the better score, they lack the first three among 9 objectives of TOGS which are scaling axes, assigning variables to the axes, using a best fit line, plotting points, translating a graph that displays the data, selecting the corresponding value for y(or x), interrelating/extrapolating graphs, describing the relationship between variables, interrelating the results of the two graphs. It was concluded from this that subjects' graph construction is lower than their graph interpretation in graph skills. It suggests that school science have a bias toward graph interpretation. This tendency represents more strikingly in the case of upper students in TOGS than the others'.

      • 과학개념과 인지적 갈등의 유형이 학생들의 개념변화에 미치는 영향

        The purpose of this study was to classify the types of scientific concepts by theoretical concepts and empirical concepts in physics, and to create cognitive conflict in students with logical statements and demonstrations, and to investigate conceptual changes. It seems that mechanics has much to do with the empirical concepts, and electromagnetics has much to do with the theoretical concepts. The condition of the instrument is intellegible, plausible, fruitful, and able to state and demonstrate. The instrument appropriate for these conditions was developed, which consisted of 6 items in mechanics and 6 items in electromagnitics, and conceptual changes were investigated. Structured interviews were conducted with 32 high school students to create cognitive conflict. We have elicited their ideas three times: pretest, posttest and delayed posttest. As the results of this study, demonstration method was more effective for conceptual change than logical argument method. In case of content areas, the misconceptions on mechanics concepts were changed more easily than those on electromagnetics concepts. In addition, the results of the study showed that the more cognitive conflict, the more the conceptual change was occurred.

        개인의의 개방병원 참여에 대한 의견

        A mailed survey with structured questionnaire was conducted to study the demand of private physicians who were operating their own clinics in the community to be a attending physician at the general hospital. The responding proportion was 21.6 percent of the 960 private physicians. A total of 207 responders : 65.2 percent wanted to be a attending physician. In particular, the physicians who were male, young, surgeon and teaching hospital careered after specialist were more highly motivated. The major activities what they wanted as a attending physician were medical care for the admission patients. They responded that the hospital charges for the medical services and the responsibility of malpractice issues should be fairly shared by attending physician and hospital according to their contributions. There is growing consensus that the need of attending physician at the general hospital will become wide spread, but little organizational preparation to assure the quality of medical care of attending physicians including training of resident physicians and students. In addition, the effective reimbursement system should be develop to compensate appropriately according to the medical achievement of the attending physicians.

      • 소리에 대한 국민학교 학생들의 개념

        The purpose of this study was to investigate Korean children's(1∼5th) conceptions about sound: the production of sound, the transmission of sound. Using interview, data were collected from 80 children, and analysed by category schemes. The major findings of this study were as follows: 1. Younger children thought that sound production was often attributed to the impact or to the properties of the object. 2. Sound transmission was not an idea which was expressed by many children. Some children considered sound to travel through the string on the string telephone, or through air, but what was meant by 'air' was often unclear. Somtimes, they regarded sound as a different kind of matter. 3. A notable feature in children's conceptions on sound was context-dependent.
