Surgical technique for single-port laparoscopy in huge ovarian tumors: SW Kim`s technique and comparison to laparotomy

Objective This study aimed to introduce a method to remove huge ovarian tumors (≥15 cm) intact with single-port laparoscopic surgery (SPLS) using SW Kim`s technique and to compare the surgical outcomes with those of laparotomy. Methods Medical records were retrospectively reviewed for patients who underwent either SPLS (n=21) with SW Kim`s technique using a specially designed 30×30-cm2-sized 3XL LapBag or laparotomy (n=22) for a huge ovarian tumor from December 2008 to May 2016. Perioperative surgical outcomes were compared. Results In 19/21 (90.5%) patients, SPLS was successfully performed without any tumor spillage or conversion to multi-port laparoscopy or laparotomy. There was no significant difference in patient characteristics, including tumor diameter and total operation time, between both groups. The postoperative hospital stay was significantly shorter for the SPLS group than for the laparotomy group (median, 2 [1 to 5] vs. 4 [3 to 17] days; P<0.001). The number of postoperative general diet build-up days was also significantly shorter for the SPLS group (median, 1 [1 to 4] vs. 3 [2 to 16] days; P<0.001). Immediate post-operative pain score was lower in the SPLS group (median, 2.0 [0 to 8] vs. 4.0 [0 to 8]; P=0.045). Patient-controlled anesthesia was used less in the SPLS group (61.9% vs. 100%). Conclusion SPLS was successful in removing most large ovarian tumors without rupture and showed quicker recovery and less immediate post-operative pain in comparison to laparotomy. SPLS using SW Kim`s technique could be a feasible solution to removing huge ovarian tumors.

Purpose: The purpose of this study was to investigate the relationships between the degree of smart phone addiction among adolescents and their depression and anxiety levels. Method: In this study, 379 high school students were selected using the convenient sampling method. The instruments used for this study were smart phone addiction measure, the CES-D (Center for Epidemiological Studies-Depression Scale), and the STAI (State-Trait Anxiety Inventory). Data were analyzed using the SPSS 20.0 program with descriptive statistics, t-test, ANOVA with Scheffe test, and Pearson’s correlation coefficient. Result: The mean score for smart phone addiction was 40.45±17.27, depression was 36.56±9.58, state anxiety was 44.00±9.61, and trait anxiety was 45.70±9.75. There were significant differences between the degree of smart phone addiction and the following variables: gender (t=-4.953, p<.001), hours of smart phone use per day (F=12.259, p<.001), types of frequently used features (F=3.485, p=.008), and satisfaction level for smart phone (F=5.18, p<.001). There were statistically significant relationships (p<.001) among degree of smart phone addiction, depression level, and state and trait anxiety levels. Conclusion: The results of this study suggested that there was a significant relationship between smart phone addiction and mental health, specifically, depression and anxiety among adolescents. Further research is needed to develop nursing strategies to provide care for adolescents who frequently utilize smart phones.

Purpose: The purpose of this study was to reclassify the advanced nursing practices of critical care nurse practitioners(CCNPs) in intensive care unit and measure the time and frequency of CCNP's activities. Method: Practices of ICU nurses are divided into RN's and CCNP's practices by a panel of ICU nursing experts. Each practice of CCNP is defined and CCNP's working time and service frequencies are monitored in general hospitals. Result: Practices of CCNP were classified into 4 domains and 32 practices. Fourteen practices by CCNPs were completed in 10 minutes and the other 12 practices consumed 10-30 minutes. A priority of practice in respiratory therapy was given to artificial airway management, management of tracheostomy patient, lower respiratory care, and the priority of CRRT was management of anticoagulation. Conclusions: Advanced nursing practices of CCNPs were recognized from those of RNs. A further research of CCNPs practices should be extended to other advanced practices and it is required to evaluate economic value of advanced nursing practice in the national health insurance system.

Inhibition of Cytokine-Mediated Nitric Oxide Synthase Expression in Rat Insulinoma Cells by Scoparone

<P>Cytokines produced by immune cells infiltrating pancreatic islets are important mediators of β-cell destruction in insulin-dependent diabetes mellitus. Scoparone (6,7-dimethoxycoumarin) is known to have a wide range of pharmacological properties <I>in vitro</I>. In this study, the effects of scoparone on cytokine-induced β-cell dysfunction were examined. Presence of scoparone significantly protected interleukin-1β (IL-1β) and interferon-γ (IFN-γ)-mediated cytotoxicity of RINm5F, a rat insulinoma cell line, and preserved glucose-stimulated insulin secretion in rat pancreatic islets. Scoparone also resulted in a significant reduction in IL-1β and IFN-γ-induced nitric oxide (NO) production, a finding that correlated well with reduced levels of the inducible form of NO synthase (iNOS) mRNA and protein. The molecular mechanism by which scoparone inhibited iNOS gene expression appeared to involve the inhibition of NF-κB activation. These results revealed the possible therapeutic value of scoparone for the prevention of diabetes mellitus progression.</P>

Case Reports : A Case of Rheumatoid Arthritis Presenting as an Intra-articular Mass of the Wrist Joint in a Patient with Chronic Monoarthritis

Rheumatoid arthritis (RA) mainly affects polyarticular joints and is characterized by inflammation of the synovial membrane leading to joint destruction. We report on an unusual case of RA presenting as an intra-articular mass invading bone of the wrist joint in a patient with chronic monoarthritis. A 43-year-old man presented with left wrist joint pain and swelling lasting several years. A plain radiograph showed a non-specific osteolytic lesion in the distal ulna but a magnetic resonance image demonstrated an intra-articular irregular mass-like lesion with eccentric bone erosion the distal radioulnar joint. Synovial biopsy detected hyperplasia of the synovial lining cell layer and finger-like protrusions of inflamed and edematous fibrovascular stroma containing dense inflammatory infiltrates, mainly plasma cells, B cells, and CD4+ T cells. Rheumatoid factor and anti-citrullinated protein antibody were highly positive. The patient was diagnosed with RA and treated with disease-modifying anti-rheumatic drugs, showing a good response on further follow-up. (J Rheum Dis 2015;22:298-302)

Analysis of KAP1 expression patterns and human endogenous retrovirus Env proteins in ovarian cancer

Background Human endogenous retroviruses (HERVs) constitute around 8% of the human genome and have important roles in human health and disease, including cancers. Previous studies showed that HERV envelope (Env) proteins are highly expressed in cancer tissues and co-related with cancer progression. KAP1 has been reported to play a key role in regulating retrotransposons, including HERV-K, through epigenetic silencing. Objective The relationship between KAP-1 and HERV Envs expressions was analyzed only in tumor cell lines and has not yet been studied in cancer tissues. In this study, we analyzed the expression patterns and relationship between KAP1 and HERV Env proteins in ovarian cancer tissues. Method The expression patterns of KAP-1 and HERV Env proteins, including HERV-K and HERV-R, were analyzed in ovarian cancer tissue microarrays that contained 80 surgical specimens, including normal ovary and malignant ovarian cancers. Results The expression of HERV-R Env and KAP1 proteins is signifcantly higher in ovarian cancer compared with normal ovary tissues. However, the expression of HERV-K Env did not change signifcantly in cancer tissues. The expression patterns of HERV-K Env and HERV-R Env signifcantly increased in early stages of cancer and KAP1 expression was higher in certain stage and types of cancers. However, the expression of HERV-K Env, HERV-R Env, and KAP1 did not change in diferent age groups. The correlation between the expression of KAP1 and HERV-Env, including HERV-K and HERV-R, was not signifcantly correlated. Conclusions The results of this study showed that there was no signifcant correlation between the expression of KAP1 and HERV Env proteins in ovarian cancer tissues, unlike studies with cell lines in vitro. These results suggest that the actual expression of HERV Env proteins in ovarian cancer tissues may be regulated through various complex factors as well as KAP1.

The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

여대생의 체중조절 경험과 비만도, 신체상, 스트레스대처에 관한 연구

Purpose: The purpose of this study was to investigate about weight control experience and to search the relationship between body image and stress coping in college female students according to obesity. Method: The subjects of this study consisted of 187 college female students. The data was collected from November to December of 2003. A self-rating questionnaire was used to collect data. The data was analysed by the SPSS computer program and it included descriptive statistics, frequency, percentage, X² -test, t-test, ANOVA and the pearson correlation coefficient. Result: The 67.9 percent of subjects had a experience of the diet, why of the diet was because of the appearance at 94.5% and because of the health at 5.5%. The mean score on body image was 111.61(SD=13.58). There was a significant difference in body image among under weight, normal weight, and overweight groups. The mean score of body image in under weight was the highest compared to others. The mean score on stress coping was 73.61(SD=7.91). There was not a significant difference in stress coping among the three groups. There was a significant positive correlation between body image and stress coping. Conclusion: The findings of this study give useful information to construct further studies in intervention program relating overweight control for college female students.

Generally, the edge preparation of the ship water ballast tank inside is worked with just one pass grinding. According to the SSP(Secondary Surface Preparation) rule, the edge preparation must keep 2R or 3-pass grinding. The collar plate edge grinding of the ship inside is manually progressed by worker. In addition, the poor working condition cause grinding workers to avoid working in them, and the quality is determined by the skill-level of a worker. So, this research, successfully focused on overcoming the difficulties in working condition and development of automated grinding machine for standard collar plates.
