        뇌졸중 노인을 돌보는 주간호제공자의 부담감과 예측요인 : 한방병원 입원 노인을 중심으로

        Purpose : The purpose of this study was to investigate factors influencing burden in primary caregivers caring for an elderly person who has had a stroke. Method : A convenience sample of 124 primary caregivers of elderly people with a stroke admitted to an oriental medicine hospital participated in this study. Interviews were done with a standardized questionnaire including Caregiver Burden Scale (Chang, 1995). Result: Average burden score was 93.19, indicating a moderate to severe level of burden. Time-dependent burden had the highest score of the sub-domains. For general characteristics, the factors significantly influencing caregiver burden were gender and age of the elderly person, age and educational level of the caregiver, whether living together or not, and the presence of another caregiver. Multiple regression analyses demonstrated that level of ADL of the elderly person, relationship between the elderly person and the caregiver prior to the stroke, caregivers' relation to the elderly person and primary income were significant factors in explaining variance in overall burden. Conclusion : Recognition of high levels of caregiver burden and factors influencing burden will allow us to develop different nursing strategies to decrease caregiver burden in those caring for an elderly person with a stroke.

      • 수소화된 비정질 규소와 다결정 규소 박막의 반송자 이동도

        열처리는 규소 박막의 구조적, 광학적 성질뿐만 아니라 반송자 이동도 특성을 변화시킨다. 반송자 이동도는 주파수 응답 또는 시간 응답을 통해 소자의 동작에 영향을 끼친다. 본 논문은 수소화된 비정질 실리콘(a-Si:H)의 이동도 변화를 열처리 온도의 함수로써 살펴보았다. 이동도 측정에서 사용된 방법들에 대한 장단점들에 대해 검토하였다. 홀 효과와 Haynes-Shockley 방법은 a-Si:H 박막의 이동도 결정에 대한 오류를 나타내었다. TOF법은 a-Si:H에는 적용 가능하지만 높은 암전류때문에 다결정 실리콘(poly -Si)에는 적용할 수가 없다. 전도를 제한하는 공간전하, 박막 트랜지스터, 과도 전류 관찰방법등은 a-Si:H와 poly-Si의 이동도를 결정할 수 있었다. 규소 박막의 전계효과 이동도는 700℃이상의 고온처리와 RF 플라즈마 재수소화후에 20∼67㎠로 상승된 값을 얻었다. 우리는 박막 트랜지스터(TFT)와 과도 전류 관찰방법은 박막 실리콘 분석에 적당하다는 결론을 얻었다. The carrier mobility influences the device behavior through its frequency response or time response. This paper summarizes the mobility changes of the a-Si:H as a function of anneal temperature. The anneal treatment changes the carrier mobilities of thin film Si as well as optical and structural properties. The advantages and disadvantages were discussed for the employed methods in mobility measurement. Hall effect and Haynes-Shockley (HS) method exhibited some errors for a-Si:H mobility determination. A time of flight (TOF) method was attractive for the a-Si:H but not applicable for the poly-Si films because of high dark current. Space charge limited conduction (SCLC), thin film transistor (TFT), and transient current observation methods were able to determine the mobilities of a-Si:H and poly-Si films. A very high field effect mobility of 20-67㎠/V.s was observed after the high temperature anneal above 700℃, and grain boundary passivation using a RF plasma rehydrogenation. We learned that thin film transistor and transient current observation methods are suitable for the analysis of thin film Si.

      • Ru(Ⅲ) : 거대고리 착물의 합성과 이를 촉매제로 이용한 올레핀의 산화반응 Macrocyclic Complexes and Catalytic Oxidation of Olefins

        The complexes were prepared by the controlled dropwise addition method . Reactions of K_2[RuCl_2·H_2O] with cyclam, meso, and rac (cyclam=1,4,8,11- tetraazacyclotetradecane, meso =C-meso-5, 7, 7, 12, 14, 14-hexamethyl-1,4,8,11-tetraazacyclotrtradecane, rac=C-rac-5, 7, 7,12, 14,14-hexamethyl-1,4,8,11-tetraazacyclotetradecane) in ethanol yielded trans-[RuLCl_2]Cl (L= cyclam, meso, rac) complexes. The ligand-to-metal charge-transfer transition in the UV vis spectrum of trans-[Ru(meso)Cl_2] Cl determined at 20℃ has been appeared at 370nm and 314㎚, while the spectrum for trans-[Ru(cyclam)Cl_2]Cl complex is considerably red-shifted. Oxidation of olefins using trans-[RuLCl_2]Cl complexes as catalyst was investigated in dichloroethane, in with sodium hypocholrite, hydrogen peroxide and tert-butyloxide were used as terminal oxidant and olefins were styrene,cyclohexene, 1-hexene, cyclooctene, and 1-octene. In this experimental condition, sodium hypochlorite was rather efficient terminal oxidant than hydrogen peroxide and tert-butylperoxide. In addition trans-[Ru(meso)Cl_2]Cl complex was rather efficient catalyst than trans-[Ru(cyclam) Cl_2]Cl complex, when sodium hypocholrite was used as terminal oxidant.

      • 악성림프종 환자에서 복합화학요법으로 유발된 백혈구 감소증에 대한 인형 재조합 과립구 집락형성-촉진인자(rhG-CSF: Neutrogin)의 효과

        Clinical effects of rhG-CSF were investigated in 17 patients receiving chemotherapy for non-Hodgkin's lymphoma. Patients were given G-CSF 2㎍/㎏/day after 2nd cycle chemotherapy for least 14 consecutive days, and clinical and laboratory results were compared to results of the first cycle. Treatment with G-CSF resulted in increase of mean WBC count(952.9±152.2㎜³vs 1882.4±350.4/㎜³) and neutrophil count(212.9±53.0/㎜³vs 841.5±238.0/㎜³), reduced the duration of leukocytopenic days(12.4±1.3 days vs 5.5±1.0 days, WBC count less than 4000/㎜³) and neutropenic days(4.5±0.9 days vs 2.3±0.7 days, neutrophil count less than 500/㎜³), and reduced the duration of neutropenic fever(5.8±1.1 days vs 0.9±0.5 days). The side effects of G-CSF were not so significant. These results demonstrated that G-CSF is safe and useful for the treatment of neutropenia induced by anticancer chemotherapy of non-Hodgkin's lymphoma and reducing the incidence of febrile periods and duration of antibiotic administration.

        광기능성 폴리머의 소산장 결합을 이용한 측면연마 광섬유형 자외선 센서

        측면연마된 광섬유와 광기능성 색소가 분산된 평면도파로 결합기를 이용한 새로운 형태의 자외선센서를 제작하여 그 특성을 측정하였다. 자외선 조사에 의해 유효굴절률이 변화하는 광변색성 색소가 분산된 폴리머를 평면 도파로 재료로 사용하였다. 박막 광도파로는 광변색성 색소인 스파이록사진을 사용하였다. 자외선 조사시간을 20초, 30초, 40초로 증가시켰을 경우 센서의 파장응답은 1.44nm/mW, 1.64nm/mW, 1.78nm/mW였고 1550nm 파장에서 센서의 90% 복귀시간은 100초로 나타났다. In this paper, we investigated a novel UV sensor using evanescent field coupling between the side polished fiber and photo-functional polymer waveguide. It was found that resonant wavelength shifts occur due to variation in the refractive index of polymer planar waveguide for its photo-functional properties on exposed UV. Spiroxazine (photochromic dye) was used as the planar waveguide. The resonant wavelength response were exhibited at 1.44 nm/mW, 1.64 nm/mW, and 1.78 nm/mW when UV irradiations were exposed for 20 seconds, 30 seconds, and 40 seconds, respectively. The recovery time of sensor was independent of UV exposure power and 90% recovery time was 100 seconds.

      • 동종 조혈모세포이식 후 발생한 크립토콕쿠스 척추염 1예

        Skeletal cryptococcosis is an uncommon infection. Cryptococcus is a common cause of meningitis and infects 7∼10% of patients with AIDS. As well as AIDS, the infection may be seen in association with leukemia, lymphoma, Hodgkin's disease, sarcoidosis, tuberculosis and diabetes, also in patients on steroid medication. But there is no case report of skeletal cryptococcosis following allogeneic hematopoietic stem cell transplantation. A 40-year-old woman was admitted to the hospital because of low back pain. She had chronic myelogenous leukemia for 2 years and underwent allogeneic hematopoietic stem cell transplantation 8 months ago. She have been treated with steroid and cyclosporine orally because of chronic graft versus host disease. On examination she was afebrile and had posterior lower lumbar tenderness. But, she had no reduced strength of low extremities. Open biopsy was underwent. Histology demonstrated budding, round-to-oval, refractile yeast-like organisms within debris. The results of a lumbar puncture were unremarkable and cerebrospinal fluid culture failed to grow bacteria and yeast. The patient was treated with amphotericin B (1 gram) and AmBisome□ (2.8 gram) over 6 weeks. Three months after cessation of therapy, the patient was doing well.(Korean J Infect Dis 33:298∼301, 2001)

        Detection of Near-membrane Calcium Ions in Live Tissues with a Two-Photon Fluorescent Probe

        A two-photon fluorescent probe (ACaCL) that can detect near-membrane $Ca^{2+}$ is reported. This probe can be excited by 780 nm fs pulses, shows high photostability and negligible toxicity, and can visualize near-membrane $Ca^{2+}$ in live cells and deep inside live tissues by two-photon microscopy.

      • Functional Analysis of Two iap Genes (iap2 and iap3) of Lymantria xylina Multiple Nucleopolyhedrovirus (LyxyMNPV)

        Baculoviral anti-apoptotic genes, p35 and iap (inhibitor of apoptosis), play important roles in the initiation stage of viral infection. However, some iap genes are not involved in the anti-apoptotic activity. To investigate the anti-apoptotic activity of the iap genes of Lymantria xylina multiple nucleopolyhedrovirus (LyxyMNPV), two ly-iap genes (ly-iap2 and ly-iap3) were cloned from LyxyMNPV. From a 5′ RACE analysis, a late promoter motif (TAAG) was found in the upstream (-15 bp) of ly-iap2, but ly-iap3 only posited an enhancer-like element (CGTGC) in the upstream (-22 bp) of 5′ UTR. Gene expression were detected by RT-PCR; the ly-iap2 and ly-iap3 genes began to express in the host cells (IPLB-LD652Y cell line) infected with LyxyMNPV 6 hours post-infection (p.i.) and reached the peak 72 hours p.i., followed by decline 3 to 5 days p.i. Functional assay of the iap genes were performed by an over-expression method in Sf9 cells. Full-length domains of LY-IAP2, LY-IAP3 and LY-IAP2-BIR could differently inhibit the apoptosis which induced by Drosophila RPR protein (DRPR). Interestingly, LY-IAP2-RZF domain was important for LY-IAP2 to rescue apoptosis, but it might be also involved in the ubiquitin activity leading to the degradation of LY-IAP2 protein. LY-IAP3-RZF might be working as a “helper domain” to inhibit DRPR-induced apoptosis. These results can be used to figure out the roles of the ly-iap genes in the apoptosis of host cells.
