        우리나라 지역 직업성질환 감시체계의 현황과 전망

        기존의 특수건강진단과 작업환경측정을 통한 직업병 관리가 진폐증, 소음성난청 등의 소수 특정질환에 국한되고 실제 직업병 발생 규모 파악이나 신종 직업병의 발견에 한계를 보인다는 사실은 산업의학전문가들 사이에서도 공감을 이루고 있다. 미국과 영국 등에서의 직업성질환 감시체계에 대한 경험은 우리 나라의 직업성질환 감시체계 구축에도 새로운 자극제가 되면서, 1998년이후 인천, 대전, 여천, 구미, 부울경 지역에서 지역 직업성질환감시체계를 산업보건관리의 중요한 시스템으로 구축하려는 노력이 확산되고 있다. 새로이 구축되어지는 이들 지역 직업성질환 감시체계는 감시하고자 하는 대상질환, 활용 가능한 인적자원 및 자료원, 지역 의료체계의 특수성 등에 따라서 목적과 방법을 달리하면서 독특한 형태로 발전을 하고 있다. 각 지역단위 감시체계들이 그 상황에 맞게 독특한 목적과 전략들을 발전 시키면서도, 향후 발전할 국가적인 차원의 직업성질환 감시체계 구축을 위하여 직업성질환 감시의 기본 전략 등을 공유하는 등의 노력이 필요하다. 환례 정의 및 기본적인 등록 서식의 공유, 직업성질환 감시 자료원의 발굴, 공동의 정보 네트워크 및 직업성질환 감시 데이터베이스 구축 등 직업성질환 감시활동을 지원하기 위한 여러 기초 인프라 구축에 힘을 모아야 할 것이다. 우리 나라에서 직업성질환 감시체계를 성공적으로 구축하기 위해서는 수집된 자료의 질 관리를 위한 직업성질환 감시의 원칙 제정과 감시 전략의 공유 등이 필요하며, 전국적인 직업성질환 감시체계의 하부구조라고 할 수 있는 지역감시체계의 기초 토대 마련과 강화작업이 절실하게 필요하다.

        신생아에서 발생한 국균증에 의한 다발성 뇌농양 1예

        저자들은 고열, 기면과 경련양 운동을 주소로 내원하여 중증 고나트륨혈증성 탈수와 급성 신부전을 동반한, 면역 기능부전 등의 소견없이 원발성으로 발생한, 생후 15일 남아에서의 뇌국균증을 수술적요법과 항진균제 투여로 치료하였기에 문헌 고찰과 함께 보고하는 바이다. Fungal infection of the central nervous system tends to occur in immunosuppressed patients. In the pediatric population, it is usually seen in severely immunocompromised patients, particulary in children with chronic granulomatous disease, hematopoietic malignancies, and those receiving chemotherapy or corticosteroid therapy. Literature indicates rare survivors from neonatal aspergillosis. We report a case of multiple brain abscess caused by Aspergillus in a 15-day-old male neonate, who had suffered from high fever, generalized seizure, severe dehydration, and azotemia. The patient was immunologically competent and successfully treated with surgical removal and antifungal agents.

        A major source of the hazardous waste generated is from chemical industries producing plastics, herbicides, pesticides and chlorinated solvents. All of these processes produce a class of hazardous waste termed the chlorinated hydrocarbons (CHCs), either directly or from undesirable side reactions. In this study, the destruction characteristics of hazardous waste through incineration is investigated by the development of a reliable computer program. Especially a nonequilibrium combustion model was used to describe the effect of the chemical kinetics due to the flame inhibition characteristics of CCl_(4) which was used as the surrogate of hazardous waste.

      • 여수 선소(船所) 주변지역의 황화합물 악취 특성

        The objective method, combined sensory and instrumental analysis, was desired to evaluation of intensity as well as odor active compounds because malodor is sensory pollution. The Seonso area was classified into seaside, residential and reference area. Target compounds were sulfur compounds such as hydrogen sulfide(H₂S). methyl mercaptan(CH₃SH), dimethyl sulfide((CH₃)₂S)and dimethyl disulfide((CH₃)₂S₂). The odor intensity at Seonso area in Yeosu city was set in order as follows : reference area < residential area < seaside area. There was a difference of the odor intensity between the summer and the winter at seaside area. Most of the odor intensity identified at seaside area(S-3) was exceeded the Korean odor regulations(the level 3.0). The concentrations of dimethyl sulfide, dimethyl disulfide were not significantly changed with sampling time and site. However, the concentration of hydrogen sulfide and methyl mercaptan at S-3 site were higher than other sites and these concentrations of the summer were higher than that of the winter. The concentrations of hydrogen sulfide at the sediments of R-1, S-1, S-2, S-3 sites by the Gastec detection tube were 0.16, 1.26, 0.43 and 2.20㎎/g-dry respectively. The order of major odor active compounds related to Seonso area were dimethyl disulfide < dimethyl sulfide < methyl mercaptan < hydrogen sulfide.

      • SOL-GEL법을 이용한 강유전체 BaTiO₃박막 및 Ceramics 제조

        Concentrated solution was prepared for a sol-gel process of BaTiO₃. Barium hydroxide and Ti-isopropoxide were used as the raw materials with methanol and isopropanol as solvents. Dense ferroelectric polycrystalline BaTiO₃thin films with the perovskite structure were fabricated by spin coating of the prepared concentrated solution on a few substrates. From XRD investigation, it was confirmed that tetragonal phase of polycrystalline BaTiO₃powders are formed after heat treatment above 550℃ in air and grain-oriented BaTiO₃thin films are obtained by coating of 0.06M concentrated solution on the substrates. It was observed that surfaces of thin films deposited on the substrates by our methods were formed crack-free and uniformly. The thickness of thin film which depends the concentration of the solution was 400∼900Å for 1 time coating, which was confirmed by SEM analysis. Ceramics and thin films show dielectric constant of 4600 (at 1㎑) and capacitance 300∼800㎊ (at 1㎑) at room temperature, respectively.

      • Southern Blot Hybridization법을 이용한 악성 림프종의 면역유전형에 관한 연구

        Immunogenotyping using gene rearrangement analysis has emerged as a precise laboratory aid in the diagnosis and classification of malignant Iymphoid neoplasms. The lineage and clonality of the malignant Iymphoid neoplasms can be identified by the demonstration of rearrangements of antigen receptor genes of the immunoglobulin and T-cell receptor genes. The analysis of the gene rearrangements on the malignant Iymphoid neoplasms are also useful as a sensitive unique clonal markers to detect early recurrence in patients with malignant Iymphoid neoplasms after treatment. To analyze the sensitivity and specificity of gene rearrangements in the diagnosis of malignant Iymphoid neoplasms. 24 cases of malignat Iymphoma were examined by Southern blot hybridization using CTβ-T cell receptor β chain gene-DNA probe and JH-immunoglobulin heavy chain gene-DNA probe. The results of the immunogenotypings using Southern blot hybridization disclosed high correlation between the immunophenotyping using immunohistochemical stain with monoclonal antibodies (B-cell Iymphoma 84.2%. T-cell Iymphoma 75% ). The analysis of the gene rearrangement of the angioimmunoblastic lymphadenopathy(AILD) and unclassifiable Iymphoma using immunohistochemical stain could resolve the monoclonality and lineage . Rearranged bands to the CTβDNA probe were observed in one case out of 2 cases of AILD. One case of unclassifiable Iymphoma showed rearranged bands to the CTβ DNA probe. There were no rearrangements in reactive follicular Iymphoid hyperplasia and paracortical Iymphoid hyperplasia. In conclusion, DNA gene rearrangement study should be applied to differentiate the clonality and cell lineage in the malignant Iymphoma with indistinctive immunophenotype.

        전기절연유의 절연파괴에 미치는 전극간격 및 곡률반경의 영향

        In this paper, we investigated the influence of gap length and tip radius on breakdown of mineral based insulation oil. Applied voltages were DC and AC voltage. Electrode system was needle-plane structure. The tip radius of needle electrode was 5, 10, 20 and 25μm, respectively. We measured breakdown voltage for each of tip radius with increasing electrode gap, 2mm to 12mm. Electric breakdown strength at tip was calculated using Mason's equation contained geometric figure. As gap length increased, breakdown strength increased linearly. But, as tip radius of needle increased, breakdown strength decreased exponentially. It can be explained by the phenomenon that electron is easily injected, as tip radius increases, and effective work function decreases. When appling DC voltage, breakdown strength was higher when polarity of needle was negative than positive. It is because of the space charge effect in accordance with the influence of liquid motion.

        내인성 혈장 리튬 농도와 정신분열병

        Objectives : The aim of this study was to compare endogenous plasma lithium concentrations among schizophrenic patients classified by DSM-IV subtype and control groups and to investigate the correlation of endogenous plasma lithium concentration and psychotic symptoms in schizophrenia. Methods : Schizophrenic patients were selected among psychiatric inpatients without lithium medication and then classified by DSM-IV schizophrenia subtype, Schizophrenic patient groups were composed of 15 disorganized type, 15 paranoid type and 15 undilferentiated type schizophrenic patients. The control group was composed of 15 healthy subjects without any psychiatric disease,Endogenous plasma lithium concentrations were estimated by inductively coupled plasma atomic emission spectrometer. The psychotic symptoms in schizophrenic patients were classified as positive symptoms, negative symptoms and disorganized symptoms according to andreasen classificationl about SANS and SAPS items. Endogenous plasma lithium Con-centration among three subtypes of schizophrenia and control group was compared, and correlation between endogenous plasma lithium concentrations and psychotic symptoms was examined. Rcsults : 1) Schizoprenic patients showed higher endogenous plasma lithium concentration than control groups (p=0.033). Endogenous plasma lithium concentrations were significantly different among three subtypes of schizophrenia (p=0.001). Compared with the control group, disorganized type showed higher endogenous plasma lithium concentration, but paranoid type and undifferentiated type were not significantly different. 2) Disorganized symptoms correlated with endogenous plasma lithium concentration (r=0.416, P=0.004), but negative symptom and positive symptom did not significantly correlate with endogenous plasma lithium concentration (r=0.202, P=0.184. r= -0.216, P=0.155). Conclusion : These results suggested that schizophrenic patients with disorganized symptom show the differences in utilization or distribution of endogenous lithium.

      • 덤프 소각기에서 CCI₄/C₃H_(8)분해 특성에 관한 연구

All of these processes produce a class of hazardous waste termed the chlorinated hydrocarbons(CHCs), either directly or from undesirable side reactions. Generally, hazardous waste is physical characteristics of difficult destruction at high temperature. We investigated the destruction characteristics of hazardous waste through incineration. A nonequilibrium combustion model was used to describe the effect of the chemical kinetics due to the flame inhibition characteristics of CCU which was used as the surrogate of hazardous waste. In this study, the numerical simulation data for 3-dimensional dump combustor shows a quantitative good agreement with experimental data. Also dump incinerator shows high destruction efficience of CCl_(4). And parametric screening studies was carried out through numerical simulation with changed CCl_(4)/C_(3)H_(8) ratio, waste injection velocity and waste equilibrium ratio.
