        대도시 노인의 질병, 주관적 신체증상 및 영양상태가 우울에 미치는 영향

        Purpose: The purposes of this study were to investigate factors related to the depression of the elderly dwelling in a big city, and to explore the influence of physical health, cognitive symptom and nutritional status on the elders’ depression. Methods: After the approval of the Institutional Review Board (IRB) and obtaining the participants’ consent form, a face‐to‐face and private interview was conducted with each participant from May, 2007 to Aug, 2007 by trained graduate level students. A total of 201 elders aged over 65 years participated in the study. The questionnaire consisted of K‐GDS, PHQ‐15 and Nutritional Screening Initiative (NSI). Collected data were analyzed with the SPSS 13.0 program, which was used for frequency, percentage, mean, standard deviation, Pearson correlation coefficients, and multiple regression. Results: The major findings of this study were as follows. 1) 28.4% of the subjects belonged to the depression group, 78.1% had one or more diseases, 85.6% had experienced somatic symptoms, and 49.7% belonged to the nutritional risk group. 2) Older age, lower educational level, more pocket money, a larger number of diseases, more severe cognitive symptoms and poorer nutritional status were significantly related to a higher depression score. 3) Significant factors influencing depression were nutritional status, cognitive symptoms and the number of diseases, which explained 38.1% of the variance of elderly depression, and nutritional status was the most influencing factor. Conclusion: The finding of this study gives useful information for developing assessment tools and interventions for elders’ depression.

        운동 자기효능감과 운동 유익성 및 장애성이 노인대상자의 운동 수행에 미치는 효과

        Purpose: The purpose of this study was to identify the effects of exercise self-efficacy and exercise benefits/barriers on doing regular exercise among elderly. Methods: The subjects were 484 older adults who were eligible and agreed to participate in this study. Data were collected from July 27 to September 21, 2007 by face-to-face, and private interviews. The questionnaires consisted of Exercise Self-efficacy, and Exercise Benefits/Barriers. The collected data were analyzed with the SPSS 13.0 program, which was used for frequency, percentage, mean, standard deviation, t-test, ANOVA, and logistic regression. Results: The major findings of this study were as follows: 1) Approximately 56.8% of participants were doing regular exercise. Study participants’ exercise self-efficacy mean score was 36.30, exercise benefits 2.74, and exercise barriers 2.03; 2) There was significant correlation between exercise self-efficacy and exercise benefits; 3) Significant factors influencing regular exercise were self-efficacy, exercise benefits and exercise barriers. Conclusion: These results suggested that nurses should emphasize exercise benefits/barriers and reinforce self-efficacy to improve regular exercise in the elderly.

        일부 간호대학생의 혈액매개질환 예방 지침에 대한 지식과 실천 정도

        The purpose of this study was to identify knowledge of universal precautions and its performance in practice. The research was conducted from November 2 to 30, 1998. A total 515 student nurses ; 249 from a baccalaureate nursing college and 266 from second and third year of a 3-year community nursing college were surveyed. The results are as following : 1. The average score for universal precautions knowledge was 270.41$\pm$19.43/300(range 150-300). The results showed that 99.2% of students avoid injury from used needles, 98.6% answered that they always wash their hands if they had contact with the patient's blood and they always dispose of used needles in special collectors (97.7%) for needles. But, 39.2% responsed that they dispose of used needles after recapping them. 2. The average score for universal precautions knowledge of the senior students in the 4-year college was the highest (277.65$\pm$13.99). 3. The average score for the performance of universal precautions knowledge was 53.18 $\pm$5.91(range 14-70). The items : ‘I cautiously avoid injury from the used needles’(4.92$\pm$0.33), ‘I always wash my hands if there has been contact with the patient's blood’(4.91$\pm$0.34), and ‘I always disposed of used needles in the appropriate collector’(4.89$\pm$0.42) showed the highest performance. However ‘I always dispose of used needles after recapping them’(2.19$\pm$1.39) and ‘I always use protection goggles when in danger of contamination’(2.19$\pm$1.20) showed low performance level. 4. The highest average score for universal precautions performance was shown among the second year students in 3-year nursing college (54.19$\pm$6.92) between the groups. It showed that the level of the universal precautions performance was higher for those who had education on university precautions prior to performance of the universal precautions than for those without any prior education. 5. The percentage of students who reported the experience of direct contact with patients' blood and/or body fluids was 42.30%. The experience of direct contact with blood and/or body fluids of the educational group was significantly higher than those were not educated. 6. The most frequent cause of the direct contact was ‘needle pricking and/or skin cut’(63.04%). The most frequent substance with which the students contact was ‘blood’(59.85%). The majority of the sample had answered that the mode of contamination was ‘unknown’(63.54%). The majority of the sample answered that strategies used after contamination included ‘washing with soap’(33.61%). Reviewing the chart of patients or asking other health professionals(28.85%). 7. The number of students who had the experience of a needle stick and/or skin cut was 145(28.16%). The clinical practice places where the incidents occurred were mainly in the internal medicine unit (45.07%) and the surgical unit (31.92%) followed by the intensive care unit and the emergency unit in order. The experience of a needle stick and/or skin cut happened during on intra-muscular injection 47.34% and intravenous injection 21.81%. The causes of the needle stick and/or skin cut were ‘putting the needle cap back on 77(35.81%)’. The number of students who took an appropriate post management blood test and/or vaccination was 27(18.62%). 8. The Pearson Correlation Coefficient between the knowledge of universal precautions and performance of universal precautions in practice showed a positive correlation.

        노인대상 자원봉사자 교육을 위한 기초 조사

        The purpose of this study was to identify the educational needs of volunteers caring for the elderly through investigation of their activities and the level of knowledge. The subjects of this study were 142 volunteers who were working at an institute for the elderly in J city. Data were collected using self-report questionnaires from October 1 to November 20, 2000. The data was analyzed by SPSS/PC. The results of this study were as follows ; 1. The most frequent activities of volunteers were physical care, followed by spiritual care, environmental care, and psychosocial care. 2. The areas revealing relatively high level of volunteers' knowledge were psychological and social characteristics of the elderly and social welfare for the elderly. Those areas which showed relatively low level of knowledge were nutrition, elimination, and skin care. In conclusion, those volunteers who work for the elderly were lacking in consistency of activities and knowledge. They were also lacking in the clinical practice. Therefore, education for volunteers need to emphasize practice with the aged people.

        자기효능 증진 방법을 사용한 자조관리 과정이 관절염 환자의 건강증진에 미치는 효과

        It is reported that self-help education is effective to increase self-efficacy for the patients with chronic illness. Also self-efficacy is proved to be an important variable to effect the physical and psychological health state. Based upon this theoretical framework, KRHPS (Korean Rheumatology Health Professions Society) has administered for 2 years the self-help education increasing self- efficacy for the patients with arthritis. The group had a weekly session for 6 weeks. During the session self-efficacy was increased by 1) actually performing a behavior 2) listening to other patients talking about a successful experience or seeing other patients performing a behavior, 3) instructor's or another patient's verbal persuasion and 4) physiological cues experienced by themselves. Every group had 4-15 participants and 2 instructors. Data were collected by self-administered questionnaire for 211 out of 361 participants. One hundred and fifty two subjects were analyzed because of missing values. As a result, it was confirmed that self-help education was effective to increase self-efficacy and self-efficacy was an important variable to improve pain, depression, aid ADL. Also it may fairly be said that nurses were right health-professionals to lead self-help groups for the patients with chronic illness. S0 it is important to facilitate many other self-help education for the patients with chronic illness.
