        초음파 기관지 내시경으로 진단한 기관기관지 골연골형성증 1예

        Tracheobronchopathia osteochondroplastica (TO) is a rare benign disease of unknown etiology characterized by accumulation of calcium phosphate in the submucosa of large airways and benign proliferation of bone and cartilage resulting in nodular formation. We report a case of tracheobronchopathia osteochondroplastica diagnosed by Endobronchial ultrasonography in a 56-year-old man. Chest Computed Tomography revealed thickening of tracheal and bronchial wall, and multiple nodules through whole trachea. Endobronchial ultrasonography showed numerous submucosal nodules with hetero-echogenecity in the third and fourth layers. Histopathological examination revealed nonspecific bronchitis with squamous metaplasia and metaplastic ossification. We confirmed tracheobronchopathia osteochondroplastica. The patient's symptoms were successfully treated with antibiotics and oxygen supplyment. endobronchial ultrasonography can helpful diagnosis in tracheobronchopathia osteochondroplastica.

      • Alkali 감령처리된 Polyethylene Terepthalate Filament 직물에 나타난 흰얼룩의 원인분석

        A study has been undertaken to know the reason why the white specks appear in the polyethylene terephthalate (PET) filament fabrics which were made alkali weight loss treatment in the dyeing and finishing factories of Taegu area. The results are as follows 1. Main component of the white spects in PET filament fabrics is CaCo₃. 2. The CaCO₃was extracted from the washing water and adhered to the PET filament fabrics during the dyeing and finishing process. 3. When PET filament fabrics were treated with hard water, the warp sizing agent did not dissolve to make specks on dyed fabrics. Then the insoluble sizing agent can not be got rid of by alkali treatment. 4. On account of using Na₂CO₃ in washing PET filament fabrics, white spects also can be appeared.

        Gastrointestinal stromal tumor (GIST) is the most common non-epithelial neoplasm arising in the gastrointestinal tract, but this tumor is rarely seen in association with type l neurofibromatosis (NFl) Generally, the majority of GISTs are isolated neoplasm and they are sporadically found in the stomach, but the GISTs in NFl patients are usually multiple and usually found in the small intestine. We report a case of multiple GISTs in the jejunum of a 63-year-old woman diagnosed as NFl accompanied by complication of gastrointestinal bleeding, In this case, Patient had multiple cafe-au lait spots and neurofibroma on skin and had freckling on axilla and groin, and then, we made a diagnosis of NFl. Gastrointestinal bleeding is controlled by resection of multiple GISTs. Generally, only 3-5% of all gastrointestinal bleeding comes from the small bowel. Causes of small intestinal bleeding are angiodysplasia, neoplasm, NSAIDs induced ulcer etc. but, If patients have NFl accompanied by complication of gastrointestinal bleeding, GISTs on small bowel must be considered.

      • 세가지 형상의 부유식 방파제 특성에 대한 비교연구

        본 연구는 2차원 유한요소모델을 이용한 부유식 방파제의 파랑감쇠특성에 대한 이론적 연구이다. 유체의 운동은 선형적이고, 2차원적인 포텐셜 흐름으로 가정하고 수치해석을 수행하였으며, 2차원 유한요소해석과 무한요소해석을 채택하여 부유식 방파제 영역에 적용하였다. 따라서 2개의 영역에 있어 방파제 근처 영역에서는 유한요소해석을 적용하였으며, 그 외의 영역에서는 무한요소해석을 사용하였다. 방파제의 형식은 직사각형, 원형, 사다리꼴의 세가지 형상으로 하였으며, 그 결과를 기존의 실험결과와 비교하였다. A theoretical investigation on the wave-attenuation characteristics of floating breakwaters are presented. The fluid motion is idealized as linearized, two dimensional potential flow. A two-dimensional finite element model is adopted to study the performance of floating breakwaters. The near field region surrounding the solid body is modeld using conventional finite elements, however, the far field region is represented appling the infinite elements. Numerical analysis is carried out for floating breakwaters with three different cross-sectional shapes, rectangular, circular and trapezoidal shapes, respectively. Comparison with hydraulic experimental results measured in the past is carried out to prove the validity of the numerical results.

      • 간경변 환자에서 Oro-cecal Transit Time의 측정

        Background: Neurotransmitter like substances in the gut have been proposed to contribute to the overall portal systemic encephalopathy (PSE). The source of the ammonia in patients with PSE is mainly the small bowel, as a result of the conversion of glutamine in the diet to glutamate and ammonia. PSE is highly responsive to changes in diet, to antibiotic therapy and to ingestion of nondigestible disaccharides such as lactulose. This study was performed to evaluate the relationship between PSE and oro-cecal transit time. In addition, performed to evaluate the effect of severity, cause, complications of liver cirrhosis on oro-cecal transit time. Methods: Between August 2006 and February 2007, 48 cirrhotic patients consecutively was enrolled in study their oro-cecal transit time after informed consent was obtained. A non-invasive hydrogen breath test was used to study oro-cecal transit time (OCTT). Result: The OCTT were delayed in cirrhotic patients with PSE compared without PSE (123.3 ± 42.7 min vs 170.0 ± 58.3 min, p<0.05). OCTT tended to prolongate further as the damage in the liver got worse, nevertheless the result was statistically meaningless (p<0.111). Neither presence of ascites or cause of PSE had any influence on gastrointestinal transit. Conclusions: Cirrhotic patients with PSE have more delayed OCTT than without PSE. Further research is needed to find out the relationship between OCTT and severity of liver cirrhosis.

        롤로기어캠의 모델링에 관한 연구

        In this paper, we developed an automated program for the modeling and emulation of three-dimensional roller gear cam mechanisms. The three-dimensional modeling of a roller-gear cam employs coordinate transformations based on the con-tact condition. The wire frame modeling followed by the shading technique using triangular meshing elements incorporat-ed the "tangent path technique" to reduce the number of data and computational load. Computer simulations for two kinds of products for roller-gear cam mechanism illustrate the relative motion between the modeled roller-gear cam and a turret and thus show the effectiveness of the proposed modeling.

        인터루킨-1α를 기관지 투입 후 나타난 폐세척액에서의 대식세포의 미세구조적 변화

        인터루킨-1을 흰쥐의 기관지 내로 투여하였을 때 급성 부종성 폐손상(acute edematous lung injury)이 유발되며, 폐세척액 내 호중구가 증가되고 surfactant의 양도 증가한다. 대식세포는 증가된 surfactant에 기인한 탐식작용 활동이 증가하여 세포 내 환상의 구조물들이 증가하나 정상군과 비교하여 형태적으로 세포기관의 구조에서 차이를 보이고 있다. 이러한 세포구성은 세포간 기능상의 차이를 나타내는 것이며, surfactant의 순환과 연관성이 있을 것으로 사료된다. 본 연구에서 정상군에 있어서 폐포강 내 대식세포가 환상의 구조물들을 합성하는 형태를 보였으며, 이것은 surfactant의 재합성과 분비작용과 매우 밀접한 연관성이 있음을 보여 주는 것이다. In order to investigate the recycling of the pulmonary surfactant in association with morphological changes in macrophage after interleukin-1 α(IL-1) induced lung injury, an acute lung injury was induced by instillation of IL-1 into the trachea, Numbers of neutrophils and phospholipid content were increased significantly(P<0.01) in IL-1 treated BAL(brochoalveolar lavage) compared to control rat. By increased phagocytosis, the lamellar structures in the macrophges of IL-1 treated rats' BAL were increased and the compositions of the cellular organelles were changed in comparison to control rat. This difference in compositions of cellular organelles denotes difference of functions in macrophages between control and IL-1 treated rats. As macrophages have been said to implicate (in the difference in the recycling of pulmonary surfactant, it is highly probable that the difference in compositions of cellular organelles is closely related to the recycling of pulmonary surfactant. In the present study circular structures were synthesized in the cytoplasm of the macrophages in BAL of normal rats. Based on these experimental results, it is suggested that macrohages might synthesize surfactant during recycling of surfactant in the lung.

      • 저온유통용 골판지 포장상사 개발에 관한 연구 (Ⅰ) : 골판지의 흡습특성 시험 Characteristics of moisture absorption to corrugated fiberboard

        농산물의 저온저장 및 고습조건에서 안전하게 유통할 수 있는 골판지 포장상자를 개발하기 위하여 원지의 구성, 라이너의 발수도, 골형태의 차이에 따라 수분흡습 특성과 압축강도와의 관계를 구명하기 위해 각각 원지특성을 달리하여 이중 양면 골판지를 재료로 사용하여 시험하였다. 온도변화에 따른 골판지 함수율 변화는 골판지 원지 종류, 발수도 차이등에 관계없이 거의 일정한 값을 나타냈다. 상대습도 변화에 따른 골판지의 함수율 변화는 상대습도에 크게 영향을 받는 것으로 나타났으며 골판지의 수분흡습은 발수도 보다는 원지에 구서에 따라 영향을 많이 받는 것으로 나타났다. 또한 수직압축강도는 상대습도가 증가함을 따라 크게 감소하여 상대습도가 수직압축강도에 미치는 영향이 매우 크며 상대습도가 증가함에 따른 수직압축강도의 저하비율은 골판지의 종류에 관계없이 거의 같은 것으로 나타났다. 따라서 저온고습 조건에서 안전하게 내용물을 보호할 수 있는 저온유통용 농산물 골판지 상자의 개선을 위해서는 고품질 원지의 생산기술 개발 및 골판지 상자의 구조개선 연구가 우선되어야 할 것으로 판단된다. The hardness of corrugated packaging box decreased during storage period because of high humidity. Therefore, the strength-decreasing factor of corrugated packaging box had to be investigated to determine the compression strength of the corrugated packaging box. This study was conducted to define the characteristics of moisture absorption as well as compression strength of corrugated fiberboard. (1) The moisture content of the corrugated fiberboard was rarely influenced by the kinds of raw materials and water repellent. However, the moisture adsorption of the corrugated fiberboard was shown to be clearly affected by air temperature changes. (2) The moisture content of the corrugated fiberboard was significantly affected by relative humidity. The moisture adsorption of corrugated fiberboard was also greatly influenced by composition of raw materials rather than water repellent. The results indicated that the improvement of raw materials was more effective than the increase of water repellent of corrugated fiberboard to reduce the moisture adsorption of the corrugated fiberboard. (3) The vertical compression strength was significantly decreased with increasing relative humidity. The reduction rate of vertical compression strength was not charged with the increase of relative humidity, which was regardless of kinds of corrugated fiberboard. (4) For improving the strength of the corrugated packaging box, new technique should be found to develop better quality of raw materials. Moreover, the physical construction of corrugated packaging box should be clearly understood because the corrugated packaging box for the agricultural products was usually used for the circumstances of high humidity and low air temperature.

        The purpose of this study is to modify the curriculum of department of dental hygiene. The specific objectives of this study are: 1. To analyze the curriculum of dental hygiene in Korean junior college. 2. To analyze the curriculum of dental hygiene in foriegn junior college. 3. To analyze the questionnaire on the complementary modification of the curriculum of department of dental hygiene. In order to accomplish the above objectives, curriculum of department of dental hygiene in Korean 10 junior colleges and the curriculum of the ministry of education were analyzed varietily also, curriculum of the department of dental hygiene in foriegn some junior colleges were analyzed by the researcher and 56 dentists, 22 professors of the department of dental hygiene and 11% dental hygienists were randomly selected in Korea a s subjects of the study. The instruments such as modification of the curriculum scale developed by Prof. Nam, Prof. Kim and Prof. Ma were used in this study. However, some items of these scales were modified by the researcher. The statistical method employed in this study was a percentage. From this analysis the following conclusions were drawn : 1. In curriculum of the department of dental hygiene in Korean junior college, establishing hours, credits, subjects in a subject of special study turned out to be scant for the curriculum of the ministry of education. 2. In curriculum of the department of dental hygiene in Korean junior college, an overall complection credit, a credit of a subject of special study, turned out torriculum of the department of dental hygiene in Korean junior college, out to be scanty for the American colleges while the completion credit of a general course turned out to be ample and an overall completion hours and subjects of a subject of special study turned out to be scanty for Holland and Japan. 3. In questionnaire analysis Professors hopped that a acquistion of technician qualification subject of specal study increase in completion unit. Dental hygienists hopped increasing of subject of special study, insureing of clinical practice, and insureing of clinical treatment of patients. Dentists hopped dental of patients and occupational moral.

      • 신경인성 방광 1례에 대한 임상고찰

        From January 10th 2002 to February 17th 2002, the clinical study was carried out I case of patient in trouble with voiding difficulty such as urinary incontinence, enuresis and urinary frequency after the L₄-L_(5) HN P operation, who had been hospitalised in the Dept. of Internal Medicin e, Cheon-An Oriental Medical Hospital, Daejeon University. The results were as following 1. For this case, we could get the better result using Bo-Joong-lk-Gi-Tang(補中益氣湯) in the treating period. 2. We could get the effective result that the case of voiding difficulty after HNP operation and cerebral infarction by conservative therapy of oriental medicine such as acupuncture, moxibustion and herb medicine.
