        한국인 성인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 비교

        본 연구는 비만도에 따른 한국인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 특성을 파악하기 위하여 시행되었다. 이를 위하여 계명대학교 의과대학 동산의료원 수면클리닉에 수면장애를 주소로 내원하여 수면다원검사 후 치과에서 측모 두부방사선계측사진 촬영을 한 87명의 성인 환자들을 체질량지수(BMI)와 수면무호흡지수(AHI)에 따라 비비만 단순코골이군(Non-obese, simple snorers), 비만 단순코골이군(Obese, simple snorers), 비비만 수면 무호흡군(Non-obese, OSA patients), 비만 수면무호흡군(Obese, OSA patients)의 4군으로 나누어 비교하였다. 그결과, 4군 중 비만 수면무호흡군의 수면무호흡지수가 가장 컸으며, 비만 수면무호흡군보다 비비만 수면무호흡군의 하악각이 더 크고 혀 길이는 더 작았다. 또한, 비비만 수면무호흡군보다 비만수면무호흡군의 설골이 더 전하방에 위치하였고, 수면무호흡지수에 영향을 미치는 기여 인자는 비만 수면무호흡군에서는 혀 길이, 비비만 수면무호흡군에서는 설골의 후방위치였다. 이처럼 비만 수면무호흡 환자와 비비만 수면무호흡 환자의 측모 두부방사선계측학적 특성과 기여 인자가 다르게 나타나므로, 치료방법도 따라서 다르게 선택해야 할 것이다. 비만 수면무호흡 환자들에게는 먼저 체중감량이 권고되어야 할 것이고, 비비만 수면무호흡 환자들은 폐쇄부위에 따라 구강 내 장치나 Nasal CPAP (continuous positive airway pressure), UPPP (uvulopalatopharyngoplasty) 등이 추천될 수 있을 것이다. Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.

        불법 주정차 차량 단속을 위한 차량 검지 및 추적 기법

        본 논문은 불법 주정차 단속을 위한 정지 차량 검지 및 추적 기법을 제안한다. 제안하는 알고리즘은 크게 네 부분으로 구성되어 있다. 먼저, 입력 영상으로부터 움직이는 차량을 구분하기 위하여 향상된 코드북 물체 검지 알고리즘을 이용한 차량 검지 알고리즘을 제안한다. 두 번째로 차량의 기하학적 특징을 이용하여 차량이 아닌 물체는 제외시키는 전처리 기법을 사용한다. 그런 다음, 검지된 결과 차량들을 히스토그램 추적 기법과 칼만 필터를 결합한 추적 알고리즘을 이용하여 추적한다. 추적 결과를 더 정확하게 하기 위하여, 히스토그램 추적 결과를 칼만 필터의 측정 데이터로 사용한다. 마지막으로, 정지 차량 검지 알고리즘의 신뢰성 있고 정확한 성능을 위하여 실제 정지 카운터 (RSC)를 제안한다. 실험결과로부터 제안한 시스템은 복잡한 실제 도로 환경에서도 여러 차량을 동시에 추적할 수 있고, 정지 차량을 성공적으로 검지해냄을 확인한다. This paper presents a robust vehicle detection and tracking algorithm for supervision of illegal parking. The proposed algorithm is composed of four parts. First, a vehicle detection algorithm is proposed using the improved codebook object detection algorithm to segment moving vehicles from the input sequence. Second, a preprocessing technique using the geometric characteristics of vehicles is employed to exclude non-vehicle objects. Then, the detected vehicles are tracked by an object tracker which incorporates histogram tracking method with Kalman filter. To make the tracking results more accurate, histogram tracking results are used as measurement data for Kalman filter. Finally, Real Stop Counter (RSC) is introduced for trustworthy and accurate performance of the stopped vehicle detection. Experimental results show that the proposed algorithm can track multiple vehicles simultaneously and detect stopped vehicles successfully in the complicated street environment.

We report a patient who had adenocarcinoma of stomach combined with squamous cell carcinoma of esophagus simultaneously.

        A system is designed and developed to acquire accurate images and to detect cell defects of Flat Panel Displays. Detailed analysis and experiments are performed to design and select the lighting, optical and camera sub-system as well as mechanical motion and position control sub-system. Sample images are acquired using the developed system and the processed result shows the good performance in detecting simple cell defects.

      • 토끼에서 근육주사시 입자 크기에 따른 amoxicillin의 비교 약물동태학

        To investigate the pharmacokinetic difference between the two amoxicillin (AMX) particles in rabbits after intramuscular injection (i.m.), both of AMX-S (particle size: 10 ㎍) and AMX-L (particle size: 100 ㎍) were injected into New Zealand White rabbits (1.2±0.3 ㎏) at a dose rate of 10 ㎎/㎏ of body weight. In this study, serum AMX concentrations were detected by microbiological assay with E. coli BE 1186 which shows high antibiotic sensitivity. After i.m. administration, AMX-S and AMX-L were best fitted as 1-compartment model with the absorption and elimination phase. The biological half-life (T_1/2, _k10) of AMX-S is 4.06±1.09 h and that of AMX-L 4.76±0.69 h. The serum maximal concentration time (T_max) of AMX-S and AMX-L were 0.88±0.17 h and 0.77±0.11 h, respectively. Maximal AMX concentration (C_max) (AMX-S: 5.71±0.62 ㎍/㎖, AMX-L: 5.04±0.25 ㎍/㎖) in serum showed a significant difference (p<0.05). In terms of bioequivalance, however, there was no difference between the two AMX's after i.m. injection in the basis of C_max and AUC.

      • 두경부에서 생긴 림프종에서 EBV와 HTLV-1의 검색

        Background and Objectives : Extranodal non-Hodgkin's lymphoma(NHL) of the head and neck(H&N) accounts for 10-20% of all cases of NHL. Despite their frequency, the cause of these lymphomas is still poorly understood. Recently, the role of viral origin in NHLs, including Epstein-Barr virus(EBV), as the main cause of sinonasal lymphomas of T/NK cell phenotype and HTLV-1 as a cause of acute T-cell lymphoma/leukemia has been well documented. We investigated the clinicopathologic findings, immunophenotypic profile, and status of EBV and HTLV-1 DNA of patients with H&N lymphoma. Materials and Methods : Twenty-seven patients with NHL of H&N region were studied. There were 15 males and 12 females with the median age of 50 years. All patients were reclassified according to the Working formulation(WF) and REAL classification. EBV genome DNA and HTLV-1 RNA were surveyed by PCR assay using formalin-fixed and paraffin-embedded tissue blocks. Results : The tonsil was the most commonly involved site(44.4%) followed by nasal cavity(18.5%), nasopharynx(18.5%) and orbit(7.4%). Immunophenotyping revealed 19 cases of B cell lineage, 7 cases of T cell lineage and one case of null cell type. Most of B-cell lymphomas were diffuse large cell lymphomas(58%). Tonsillar lymphomas were all B-cell origin. Four of the five nasal cavity lymphomas and one nasopharyngeal lymphoma showed an angiocentric T/NK cell phenotype with strong association with EBV. EBV genome was detected in 15 of 26 H&N NHLs(57.7%). Seven of 19 B-cell lymphomas(36.8%) and all T/NK or null cell type lymphomas were positive for EBV DNA. However, there was no HTLV-1 positive cases found. Conclusions : It could be concluded that the high incidence of EBV of angiocentric T/NK-cell lymphomas of the nasal cavity may indicate a probable role of EBV in the development of these lymphomas.

        분포 증발산량 산정 결과의 격자형 GIS 표현

        본 연구에서는 원격탐사나 지리정보시스템 등을 적응하여 ASCII형태의 수지표고자료와 원격탐사 자료를 이용하여 분류한 토지피복도를 이용하여 각 토지피복별로 알베도와 바로크계수, 증발효율 등을 이용할 수 있는 격자형 토지피복자료를 격자단위로 분할하여 모형의 입력값으로 이용하였기 때문에 유역의 공간적인 특성을 반영한 덜다 정확하고 신뢰성있는 자료의 구축이 가능했고, 기존의 열수지법을 이용하여 증발산량의 시 공간적인 분포양상을 계산할 수 있는 격자기반의 분포형 증발산량 추정 프로그램을 포트란언어고 개발하였으며, 격자형 증발산량 산정 모형은 향후 연될 격자기반의 분포형 강우-유출모형과 통합할 수 있도록 고려하였다. 또한, 격자기반의 계산으로 증발산량에 대한 유역내 특정지점에서의 시7)적 변화 양상과 공간적 분포 양상을 GIS상에서 나타낼 수 있도록 구성하였다. 모형의 적용성 검증을 위해서 비교적 정화한 기상자료와 수문자료를 보유하고 있는 일본의 쇼나이강 유역(532㎢)을 대상으로 적응한 결과, 유역 연평균 증발산량은 825.4mm로 나타났다. A Grid-based distributed evaporation prediction model which calculates temporal and spatial evaporation with a heat balance method was developed. And, the model was considered as the integration with distributed hydrological model in near future. This model was programmed by fortran language and used ASCII formatted map data of DEM (Digital Elevation Model) and land cover map extracted by remote sensing data. Also, temporal variations and spatial distributions of evaporation are presented by using GIS. To verify the applicability of the model, it was applied to the Shonai river basin (532㎢) which has sufficient meteorological and hydrological data, Japan. The result shows that the estimated mean annual evaporation was 825.41nm, and this value is estimated as suitable things in considering rainfall and discharge data in study area.

        불소 테잎 적용 후 시간변화에 따른 타액 내 불소농도

        이 연구의 목적은 카르복시메틸셀룰로오스 기저재에 불소를 분사하여 제작한 불소 테잎을 피실험자의 구강 내에 도포 후 잔류하는 불소의 농도를 측정하고자 하는 것이다. APF gel(60seconds taste^(R), 1.23% APF gel, Group Ⅰ), Fluoride varnish(CavityShield™, 5% NaF, Group Ⅱ), Fluoride tape(SCMC-T-5, 5% NaF, Trial product, Group Ⅲ)을 군별로 9명씩 건강한 20대 남녀성인의 구강에 도포 후 1시간, 3시간, 5시간, 7시간, 1일, 2일, 3일 후 비자극성 전타액내 불소농도를 불소이온전극을 이용하여 측정하였으며 다음과 같은 결과를 얻었다. 1. 1, 2, 3일 후와 비교하여 모든 그룹에서 7시간까지 초기농도보다 유의하게 높은 불소농도를 보였다. 도포 후 1, 2, 3일에는 초기농도와 통계적으로 유의한 차이를 보이지 않았다(p>0.05). 2. 도포 후 1, 3, 5, 7시간에 타액 내 불소농도는 모든 시간에서 2군, 3군, 1군 순으로 높았다. 3. 도포 후 1시간과 3시간 후에 2 군의 불소농도는 3 군에 비하여 유의하게 높았으나(p<0.05) 1군과 3군 사이에는 통계적으로 유의한 차이가 없었다(p>0.05). The purpose of this study is to investigate the residual fluoride concentration of fluoride tape in oral cavity which made by spraying NaF on carboxymethylcellulose base. After 1. 3, 5, 7 hours and 1. 2, 3 days of applicating APF gel(60seconds taste^(R), 1.23% APF gel. Group Ⅰ), Fluoride varnish(CavityShield™, 5% NaF, Group Ⅱ) and Fluoride tape(SCMC-T-5, 5% NaF, Trial product, Group Ⅲ) in oral cavity of 27 healthy adults in their twenties, the result of fluoride concentration in unstimulated whole saliva which measured by using fluoride sensitive electrode made up to following conclusion. 1. Until 7 hours after application in every group, it showed significantly higher fluoride concentration in saliva than baseline value but at 1, 2, 3 days after application, there were no significant differences between measurements and baseline value(p>0.05). 2. Until 7 hours after application at every time, mean fluoride concentration in saliva was higher in the order of Group Ⅱ, Ⅰ and Ⅲ. 3. 1 hour and 3 hours after application, Group Ⅱ revealed significantly higher fluoride concentration than Group Ⅲ(p<0.05), but there were no significant differences between Group Ⅰ and Group Ⅲ in every time.

        Ⅱ급 부정교합 치료후의 하악골의 위치변화에 관한 임상적 고찰(영구치열에서)

        The purpose of this study was to find out some relationship between the positional change of mandible during treatment and the prognosis after treatment of ClassⅢ malocclusion. The patients selected for this study were sucessfully treated two patients, and three patients who were also sucessful in active treatment, but showed relapse during observation. Serial Cephalograms, photos, models, treatment record of these patients were analysed in each term of treatment and observation period. The results might be summarized as follows ; The patient with swing back type in which mandible rotated posteroinferiorly during active treatment showed good dental and skeletal relationship during observation. Among the patients with Y-axis type, one who had harmonious growth between maxilla and mandible showed stable dental relationship during observation, the other who had not showed relapse. The swing back type was not changed during and after treatment but the patients who had Y-axis type were unchanged in some patients, and changed into forward type in the others. During active treatment, was lower facial height(ANS-Me) increased in all 5 patients, and in observation period the patients showing increase in lower facial height had stable occlusion bur the others having decrease showed unstable dental relationship.

      • 백서 하악골의 기능적 전방위가 하악골 성장에 미치는 영향

        The purpose of this study was to investigate the growth changes of the mandible and associated structure in response to postural hyperpropulsion and changes after removal of postural hyperpropulsor. The experimental animals were four-week-old Sprague-Dawley males rats. The animals were worn the postural hyperpropulsor diurnally for 10 hours per day. The animals were sacrified after 1-week, 2-week, 4-week postural hyperpropulsion and 4-week postural hyperpropulsion-4-week removal period. The growth changes of rat manidible and associated structures following postural hyperpropulsion on the growing rat mandible were observed biometrically, radiographically and histologically. The findings were as follows. 1. The angle between the chief axis of the bone trabeculae in the condyle and the mandibular plane of rats observed for 4 weeks after worn the hyperpropulsor for 4 weeks was directed posteriorly as compared with that of control rats. 2. The ratio of mandibular length to maxillary length of experimental rats was higher than that of control rats. 3. The tooth axis of mandibular first molar of rats worn the postural hyperpropulsor for 4 weeks was more mesially inclined as compared with control rats. 4. Histologically, the cartilage layer at the superior region of the condyle of rats worn the postural hyperpropulsor for 2 weeks appeared thicker than that of same aged normal rats, and generalized increasing of the cartilage layer was shown on the condyle of rats worn the postural hyperpropulsor for 4 weeks. 5. There was no significant histologic difference between rats observed for 4 weeks after worn the postural hyperpropulsor for 4 weeks (8 week experimental rats) and same aged normal rats. 6. The newly formed bone at anterior region of articular fossa of rats worn the postural hyperpropulsor for 2 weeks and 4 weeks was thicker than that of same aged normal rats. J. Kyungpook Nat. Univ.
