        타액선 내시경과 외부접근법으로 치료한 이하선관타석증 치료 1예

        We introduce a case of surgical removal for Stensen’s duct stone with sialendoscopy. This case was that the patient was presented with postprandial swelling in infra-auricular lesion. CT scan shows that this symptom was induced by the Stensen’s duct stone. The identification of the stone was done with sialendoscopy, and the removal of the stone was performed via cervical minimal incision without injury of the facial nerve. There were limitations of the removal of the Stensen’s duct stone via only external approach, but combination of sialendoscopic and external approach removes the stone more easily and safely. The combined operation offers a further option for minimally invasive treatment in case with Stensen’s duct stone.

        한국의 지반거동을 고려한 교량과 송전철탑의 지진취약도 분석

        The Korean peninsula has known as a minor-to-moderate seismic region. However, some recent studies had shown that the maximum possible earthquake magnitude in the region is approximately 6.3-6.5. Therefore, a seismic vulnerability assessment of the existing infrastructures considering ground motions in Korea is necessary. In this study, we developed seismic fragility curves for a continuous steel box girder bridge and two typical transmission towers, in which a set of seven artificial and natural ground motions recorded in South Korea is used. A finite element simulation framework, OpenSees, is utilized to perform nonlinear time history analyses of the bridge and a commercial software, SAP2000, is used to perform time history analyses of the transmission towers. The fragility curves based on Korean ground motions were then compared with the fragility curves generated using worldwide ground motions to evaluate the effect of the two ground motion groups on the seismic fragility curves of the structures. The results show that both non-isolated and base-isolated bridges are less vulnerable to the Korean ground motions than to worldwide earthquakes. Similarly to the bridge case, the transmission towers are safer during Korean motions than that under worldwide earthquakes in terms of fragility functions.

        A 19 year-old female patient with multiple impacted teeth of unknown cause was treated by space regaining, orthodontic traction following surgical exposure, or surgical exposure only. The results were as follows. Normal occlusion was obtained. In orthodontic traction following surgical exposure of bucally impacted teeth, loss of alveolar bone and marginal tissue occurred.

      • 백서 하악골의 기능적 전방위가 하악골 성장에 미치는 영향

        The purpose of this study was to investigate the growth changes of the mandible and associated structure in response to postural hyperpropulsion and changes after removal of postural hyperpropulsor. The experimental animals were four-week-old Sprague-Dawley males rats. The animals were worn the postural hyperpropulsor diurnally for 10 hours per day. The animals were sacrified after 1-week, 2-week, 4-week postural hyperpropulsion and 4-week postural hyperpropulsion-4-week removal period. The growth changes of rat manidible and associated structures following postural hyperpropulsion on the growing rat mandible were observed biometrically, radiographically and histologically. The findings were as follows. 1. The angle between the chief axis of the bone trabeculae in the condyle and the mandibular plane of rats observed for 4 weeks after worn the hyperpropulsor for 4 weeks was directed posteriorly as compared with that of control rats. 2. The ratio of mandibular length to maxillary length of experimental rats was higher than that of control rats. 3. The tooth axis of mandibular first molar of rats worn the postural hyperpropulsor for 4 weeks was more mesially inclined as compared with control rats. 4. Histologically, the cartilage layer at the superior region of the condyle of rats worn the postural hyperpropulsor for 2 weeks appeared thicker than that of same aged normal rats, and generalized increasing of the cartilage layer was shown on the condyle of rats worn the postural hyperpropulsor for 4 weeks. 5. There was no significant histologic difference between rats observed for 4 weeks after worn the postural hyperpropulsor for 4 weeks (8 week experimental rats) and same aged normal rats. 6. The newly formed bone at anterior region of articular fossa of rats worn the postural hyperpropulsor for 2 weeks and 4 weeks was thicker than that of same aged normal rats. J. Kyungpook Nat. Univ.       • 치아이동에 의한 백서 삼차신경절의 CGRP 면역양상 신경세포 분포의 변화

        이 연구의 목적은 중추신경계에서는 동통의 전달을 조절하며 말초조직에서는 혈관이완, 면역체계의 조절, 탐식기능 조절 등의 염증에 관여되며 골 생성에도 관여하여 치아이동시 중요한 기능을 할 것으로 츠측되고 있는 CGRP의 생성장소인 삼차신경절내의 세포체의 크기에 따른 CGRP 면역 반응성의 변화를 관찰하여 삼차 신경절내 CGRP 면역양성을 띄는 각 크기의 세포들의 치아이동의 시간 경과에 따른 반응을 밝혀보고자 하는 것이다. 생후 9주령의 Sprague-Dawley계 백서 30마리를 정상 대조군 6 마리, 3 시간 교정력 적용군 5 마리,12 시간군 4마리,1 일군 5마리, 3일군 5마리, 7일군 5마리로 나누어 실험군은 각 해당 시간동안 상악 우측 제 1대구치에 30gm내외의 교정력을 가한 후 희생 시켰다. 희생시킨 백서의 삼차 신경절을 적출 하여 냉동 절편을 형성항 후 토끼의 항체를 이용하여 면역화학 염색을 시행하였다. 삼차 신경절의 CGRP 면역양성 신경세포체를 크기에 따라 소형(20 μm미만), 중형(20-35 μm), 대형(35 μm초과)으로 나누어 교정력 적용시간에 따른 변화를 관찰하였다. .삼차 신경절내 전 신경세포체중 CGRP 면역양성 신경세포체가 차지하는 비율은 정상 대조군이 33.0%였으며 교정력적용후 1일 후에는 24.5%로 감소하였으나 7일 후에는 41.8%로 증가하였다. .정상 대조군에서 CRGP 면역양성 신경세포체중 소형,중형,대형은 각각 51.3%, 44.0%, 4.7%였다. .소형의 CRGP 면역양성 신경세포체는 3 시간, 12 시간 군에서 높게 나타났다. .중형의 CRGP 면역양성 신경세포체는 3 일, 7 일군에서 대형의 CGRP 면역양성 신경세포체는 7 일군에 높게 나타났다. 위를 종합하여 볼 때 치아이동 초기에는 CGRP 면역양성 신경세포체중 소형의 세포가, 후기에는 중형, 대형의 세포가 반응한다는 것을 알 수 있었다. CGRP was known as the modulator of pain transmission in central nervous system and local effector to peripheral tissue causing vasodilation, increased blood flow, modulation of immune system, stimulation of endothelial cell proliferation, and stimulation of bone formation. Numerous study, therefore, were done to elucidate involvement of CGRP to tooth movement. To investagate the response of CGRP immunoreactive nerve cells according to cell size in trigeminal ganglion during tooth movement, immunohistochemical study was performed using rat. Experimental rats(9 weeks old, 210 gm) were divided as six groups(normal(n=6), 3 hour group(n=5), 12 hour gorup(n=4), 1day group(n=5), 3 day group(n=5), 7 day group(n=5)), and were applied orthodontic force (approximately 30gm) to upper right maxillary molar. After frozen sections of trigeminal ganglions were immunostained using rabbit antisera, the changes of CGRP immunoreactive cells in regard to cell size distribution(small cell(upto 20 μm), medium cell(20-35μm), large cell(above 35 μm) were observed. The results were as follows The percentage of CGRP immunoreactive cells to all nerve cells in trigeminal ganglion was 33.0% n normal control group, was decreased to 24.5% in 1 day group, and was increased to 41.8% in 7 day group. . The percentage of small, medium, and large cells expressing CGRP immunoreactivity in normal trigeminal ganglion to all CGRP immunoreactive cells were 51.3%, 44.0%, 4.7%, respectively. . The percentage of small cells with CGRP immunoreactivity to all CGRP immunopositive cells was increased in 3 hour and 12 hour groups. . The percentage of medium cells with CGRP immunoreactivity was increaed in 3 day and 7 day groups. . The percentage of large cells with CGRP immunoreactivity was increaed in 7 day group. Conclusively, the small cells with CGRP immunoreactivity in trigeminal ganglion respond to orthodontic force during initial phase of tooth movement, and later the medium and large cells with CGRP immunoreactivity respond
