Multiple primary cancer means that more that two cancers occur independently in an individual. Recently, the incidence of multiple primary cancer has increased with lengthened survival, of cancer patients, development of new diagnostic technique and increased clinical evaluation. We report a patient who had adenocarcinoma of stomach combined with squamous cell carcinoma of esophagus simultaneously.

Femoral pseudoaneurysm is important complication after diagnostic femoral catheterizationor more complex procedure. With the increasing use of larger-size percutaneous instruments and periprocedual anticoagulant or antiplatelet agent the incidence of postcatheterization femorl artery injuries ncluding pseudoaneuiysm has increased in the past few years. Duplex ultrasonography and addition of color- flow Doppler provides an accurate, noninvasive. risk-free diagnosis and faster detection of intraaneurysrnal blood flow and the track betweenthe injured artery and the pseudoaneurysm. Though early surgical repair of the arterial defect is usually recommended because of severeand life-threatening complication such as rupture, fhrornboembolism, compression neuropathyetc, Ultrasono-Guided Compression Repair(UGCR) is to be first-line treatment for its advantagesuch as high success rate, low morbidity and cost-effectiveness. The authors report 2 cases of femoral psoudoaneurysrns treated using UGCR with nlanualcompression with C-clamp at the same time as a nonsurgical treatment.
간경변증 환자에서 도플러 초음파를 이용한 신저항 계수(RI)의 임상적 의의

Cirrhotic patients may exhibit circulatory derangement and renal dysfunction during the clinical course. Renal dysfunction on cirrhosis can occur without specific causative factors. This functional renal failure in cirrhosis is considered as a consequence of renal vasoconstriction. These alteratons of renal hemodynamics are already present in the early phase of the disease, even in the condition that the conventional kidney function tests are normal. A new method for noninvasive evaluation of arterial tone is duplex Doppler sonography. Among the various sonographic indices proposed, the renal resistive index(RI) is the most widely used for the estimation of intrarenal arteriolar vascular resistence. This study was performed to evaluate the role of Doppler sonography in early detection of renal dysfunction and to assess the clinical significance of RI. In 25 cirrhotic patients without overt kidney failure and ten control subjects, the RI of the acurate artery in both kidneys was measured by Doppler sonography. The mean RI of cirrhotic group was significangly higher than that of control subjects(0,68±0.08 vs -0.62 ±0.05,p<0). Accordiing to Child class, the RI showed increasing tendency from A to C, through witout statistical significance. In this study, the RI was significantly inversely correlated with 24hr urinary sodium amount(r=-0.39, p<0,05)and correlated with serum creatinine(r=6.60,p<0.01). This study indicates that the measurement of RI is a sensitive method to assess intarenal hemodynamics and to detect early changes of the renal dysfunction in cirrhotic patients.
Subsea X-mas tree에서의 blockage 처리 실패에 대한 위험도 저감 방안 분석

해저 화학품 주입 시스템(subsea chemical injection system)은 해저 생산 시스템(subsea production system)의 문제 발생 요인 중 blockage를 처리하기 위해 화학품 주입을 담당한다. 생산 중단이 치명적인 이익 손실을 야기하는 해저 생산 시스템에서는 발생된 문제를 빠르게 처리하는 것이 중요하기 때문에 타 산업보다 상대적으로 시스템의 높은 신뢰도 수준을 요구한다. 따라서 본 연구에서는 제어 시스템(control system)과 연계되어 있는 subsea X-mas tree에 화학품을 주입하는 해저 화학품 주입 시스템을 분석하였다. 이를 통하여 FSA(Formal Safety Assessment)를 따라서 위험 요소를 정의하고 FTA(Fault Tree Analysis)와 ETA(Event Tree Analysis)를 이용한 정량적 위험도 분석을 하였으며 그 결과를 이용하여 위험도 저감 방안에 대한 타당성 평가를 수행하였다.

Thrombohemorrhagic complications in chronic myeloproliferative disease(CMPD) including chronic myelogenous leukemia(CML) are not rare. Lower incidence of disordered hemostasis is reported in CML compared with other CMPD. The mechanism of thrombohemorrhagic complications might be a consequence of qualitative platelet abnormalites and prolonged bleeding time rather than that of thrombocytosis. Although defect of platelet function has been extensively investigated, there was no established consistent correlation between clinical bleeding and number and function of platelet. The most common site of bleeding complications in the CMPD is superficial mucosa. Bleeding in deep tissue and viscera is very unusual. We report a case of CML which developed a huge spontaneous retroperitoneal hematoma.
