      • 조산의 예측 지표로서의 fetal fibronectin의 유용성

        Objective: The purpose of this study was to evaluate clinical efficacy of fetal fibronectin as a marker to predictor of preterm labor. Materials & Methods: The study group consisted of sixty-eight healthy pregnant women, at gestational age between 24 and 36 weeks, who visited our department with vaginal discharge from April, 1995 to December, 1997. For comparison, we divided all of the pregnant women into two groups by the result of ROM-Check^®, positive groups(N=21) and negative groups(N=47). When preterm labor appeared, 50mg of ritodrine hydrochloride(Yutopar^®) mixed I liter of 5% dextrose solution was administered slowly intravenously. Results: 1) The gestational age at delivery and the interval time from admission to delivery in the positive groups were significantly shorter than those of the negative groups(p<0.05). 2) The positive groups had significantly lower birth weight and Apgar scores than the negative groups(p<0.05). 3) Among the negative groups, in case of tocolytics administration, the interval time from admission to delivery significantly delayed more than that time in case of no tocolytics administration(p<0.05). Conclusions: The detection of the fetal fibronectin in vaginal discharge had a useful method as a predictor of preterm labor. Tocolytics was more effective in the negative groups than in the positive groups.

      • 폐경기 여성에서 호르몬 대치 요법시 병행 투여된 프로제스토젠 투여 방법에 따른 혈청 지질 및 지단백에 미치는 영향

        Objective : The purpose of our study is to evaluate the effect of different administration method of progestogen on lipid profiles including total cholesterol, LDL(low density lipoprotein)-cholesterol, HDL(high density lipoprotein)-cholesterol, triglycerides during hormone replacement therapy in postmenopausal women. Methods and Materials : Seventy-one postmenopausal women who were assigned to following groups (Group I : conjugated equine estrogen(CEE), 0.625 mg/day plus continuous medroxyprogesterone acetate (MPA), 2.5mg/day, Group II : CEE, 0.625 mg/day plus cyclic MPA, 5mg/day for 12days/month. And we measured serum total cholesterol, high density lipoprotein (HDL)-cholesterol, low density lipoprotein (LDL)-cholesterol, and triglycerides before hormone replacement therapy(HRT) and 12 months later after hormone replacement therapy. Results: 1. Total cholesterol decreased in group I,III. 2. HDL-cholesterol increased in group I,II,III. 3. LDL-cholesterol decrease in group I,II. 4. Triglycerides increased in group I,II,III. There was significance in serum total cholesterol and LDL-cholestrerol among three groups. Conclusions : Estrogen alone or in combination with progestogen both in cyclic and continuous use improves lipid profiles, so medroxyprogesterone acetate(MPA) does not seem to blunt estrogenic effect on lipid profiles.

      • Rocuronium의 애별투여방법이 모지내전근의 이완에 미치는 영향

        Objective : There are controversies in pnmmg technique of rocuronium whether it can or can't shorten the onset time. We want to compare the effects of two different priming doses and intervals of rocuronium. Materials and Methods : After giving propofol, 50 patients were randomly assigned to 5 groups. Group I received a placebo, followed 3 min later by rocuronium 0.6 mg/kg. Priming doses and intervals of Group II, Ill, N, V were 0.06 rng/kg and 2 min, 0.12 mg/kg and 2 min, 0.06 rug/kg and 3 min, 0.12 mg/kg and 3 min, respectively. Total dose of rocuronium was 0.6mg/kg. Train of four (TOF) count or ratio and 1st twitch height of TOF (T1) were monitored with 15 secondsintervals. TOF ratio of each groups just before bolus injection, duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were compared. Results : Duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were decreased in priming groups compared to Group 1 (P < 0.01) In group V, significant depression of TOF ratio just before bolus injection was seen (P < 0.05). Conclusion : Priming technique with rocuronium is effective method to shorten the onset of neuromuscular block. But priming dose of rocuronium itself could cause hypoxia and aspiration in awake patients and should be used carefully. Priming dose of rocuronium 0.06 mg/kg and 2 minutes interval maybe relatively safe method.

      • 전격성 경과를 취한 만성 호산구성 폐렴 환자 1예

        Chronic eosinophilic pneumonia(CEP) is a rare disease of unknown etiology characterized by chronic infiltration of the lung with eosinophils. It presents with profound systemic symptoms comprising fever, night sweats, weight loss, dyspnea and blood eosinophilia with nonsegmental air-space consolidation confined to the outer third of the lung, the "photographic negative of pulmonary edema". Histopathologic features of chronic eosinophilic pneumonia are intraalveolar and interstitial infiltrations with eosinophils, histiocytes, giant cells, scattered lymphocytes and plasma cells. Glucocorticoid therapy cause prompt resolution of symptoms as well as disappearans of blood eosinophils, elevated serum Ig E levels and the roentgenographic lesions. Infrequent radiographic findings include nodular infiltrates, consolidations, cavitations, atelectasis and pleural effusions. Deaths from CEP although rare, have been reported, but the majorities of CEP have benign courses and do not need a therapy with ventilator. We report a case of chronic eosinophilic pneumonia, which had a sudden course associated with diffuse pneumonic consolidations on the both lung and bilateral pleural effusion.

        A Case of Critical Pedagogy to Promote EFL Learners’ Critical Thinking: Flipped Humanities Convergence Class *

        To enhance EFL students’ critical thinking (CT) skills and students-centered learning, this study explored how to integrate literacy works into the EFL classroom. For this, a critical literary pedagogy (CLP) was designed that combines three methods; interdisciplinary learning converging literature, language and philosophical ideas to provide various knowledge with linguistic development, self-cognitive learning and the combination of in-and out-classes for self-regulated learning. First, to create the balance between social practice and self-regulation, an SNS-based flipped classroom model using Kakao Talk was adopted, which allowed EFL Korean undergraduates to acquire knowledge about the class content on an individual basis. Then, they participated in critical literacy activities including argumentation in class. While analyzing Thomas Hardy’s Tess of D’Urbervilles along with Foucault’ power relations and discourse, EFL learners could interpret a real world by comparing their own opinions with those of their peers’. Quantitative methodologies were conducted to test the validity of this model on CT-efficacy. The result through a multiple-regressive analysis (R 2 = .599) found scaffolding input was a key component to promote CTefficacy. A CLP model emphasizing scaffolding input, self-cognition and selfregulation showed the potential to maximize CT-efficacy by stimulating students’ selfstudy and self-monitoring attitudes.

      • 계면활성제의 분리막에 대한 투과거동

        The molecular weights of surfactant SDS(M.W. 288) and SLS(M.W. 420) are smaller than the molecular weigh cur-off of cellulose acetate membrane used in this study. However, 20% of SDS and 67% of SLS were rejected by cellulose acetate membrane. The higher rejection rate of SLS than that of SDS is due to the longer hydrophobic chain and greater molecular weight. There was no resistance to permeation rate by membrane fouling. Most of permeation resistance was due to the concentration polarization. Permeation rate was declined with operating time and with increase in concentration of surfactant due to concentration polarization.

        식빵의 관능적 특성 평가를 위한 묘사분석 절차 개발

        This study was conducted to develop the descriptive analysis procedures for evaluating the sensory characteristics of yeast leavened breads. Eleven highly trained panelists identified the following 23 sensory attributes in the bread and defined the terminology for each attribute; yellowness of crumb, roughness of surface, uniformity of cell. density of cell, brownness of crust for appearance characteristics, yeast fermented, chemical, roasted flour. buttery. milky, boiled flour. sweet, and salty for flavor characteristics. springiness, ease to tear, moistness on surface, adhesiveness to lip, hardness, stickiness, cohesiveness of mass, moisture absorption, chewiness. and loose particles for textural characteristics. Reference samples for the flavor attributes were determined. There were significant differences in all of the 23 sensory attributes of commercial bread samples. The principal component analysis (PCA) was performed to summarize the sensory data. The first two principal components explained 89% of the variation of the original variables indicating reliability of procedure developed in this study.

        A new reducing agent, thexylbromoborane-methyl sulfide complex (ThxBHBr.SMe₂), was synthesized from the reaction of tetramethylethylene and monobromoboranemethyl sulfide complex. The reducing characteristics of the reagent to the representative organic compounds have been studied systematically under the standardized conditions (0℃, methylene chloride). Thus, the reagent appeared as a mild selective reducing agent, especially on the partial reduction of carboxylic acids to the corresponding aldehydes.

      • Tetracaine이 개의 大腦皮質 Na^+-K^+-ATPase의 活性에 미치는 影響

        본 實驗은 tetracaine이 개의 大腦皮質의 Na^+ - K^+ - ATPase의 活性에 어떻게 影響을 미치는 가를 알아보기 위하여 실시한 것이다. 그 酵素에 대한 tetracatine의 沮害效果와 그 機作은 酵素反應速度論的 分析方法 즉 Dixon-plot, Lineweaver-Burk plot, Arrhenius-plot 등으로 分析하였다. 그 結果는 다음과 같다. 1. Na^+ - K^+ - ATPase는 全 ATPase와 Mg^++ - ATPase의 活性보다 tetracaine에 의하여 더 많은 沮害를 받는다. 2. Tetracaine의 50% 沮害效果는 沮害常數 5㎎/㎖와 동일하였다. 3. Tetracaine은 그 酵素의 Km값을 변화시키지 못하였으나 最高速度(Vmax)는 변화시키기 때문에 tetracaine은 可逆的 非競爭的 沮害劑로 作用한 것 같다. 4. Q_10 값은 tetracaine의 농도가 증가함에 따라 감소되는데 이 값으로 Arrhenius-plot를 作成해 본 結果 切點(transition state)이 나타나지 않으므로 tetracaine은 脂質에는 작용하지 않고 오직 Na^+ - K^+ - ATPase에 작용하는 것 같다. The present experiments were disigned to investigate the effect of tetracaine on the dog brain cortex Na^+ - K^+ - ATPase activity. The inhibitory effects of tetracaine on the enzyme avtivity and its mechanism were analyzed by Dixon plot, Lineweaver-Burk plot, and Arrhenius plot. The results obtained here were as follows : 1. The Na^+ - K^+ - ATPase activity was inhibited more than the total ATPase and Mg^++ - ATPase in the presence of tetracaine. 2. The concentration of 50% inhibition for tetracaine was identical with the inhibition constant (Ki), the Ki value was 5 ㎎/㎖ for tetracaine. 3. In this study tetracaine did not change the Km value (2.5) but decreased the Vmax of the Na^+ - K^+ - ATPase activity, suggesting that tetracaine acts as reversible and noncompetitive inhibitor on the enzyme. 4. Q_10 value and energies of activation (20.1 Kcal/mole) decreased as the concentrations of trtracaine increased and the transition state did not appeared. These results suggest that trtracaine inhibits the enzyme activity by reversible and noncompetitive inhibition.

        Clozapine-유발성 무과립구증의 G-CSF치료경험 1례

        저자들은 국내 CPMS하에서 최초로 발생한 clozapine-유발성 무과립구증 환자에게 G-CSF를 투여하여 성공적으로 치료한 경험을 보고하였다. Clozapine-유발성 무과립구증의 발생원인은 아직 명확하지는 않으나, HLA 표현형 등의 면역학적 기전과 desmethylclozapine등의 세포독성 기전이 연구되고 있다. 다만, 최종적 공통 경로가 골수내의 골수성 전구세포의 증식억제라는 것은 분명하며, 이것이 CSF사용의 이론적 근거가 된다. CMPS를 통하여 매주 백혈구 수치를 측정하도록 하고 있으나, 최근에도 clozapine-유발성무과립구증의 발생률은 약 1%로 알려져 있으며, 무과립구증의 발생시 급격한 발생 양상, 연장되는 경과, 골수성 전구세포의 부재 등의 특징들로 인하여 2차 감염으로 인한 사망률이 매우 높은 치명적인 상태에 빠지게된다 그러므로, clozapine사용 환자의 선정에 있어서, 고령 및 여성 등 위험인자를 고려하여야 할 것이며, 사용 중에는 백혈구의 스파이크 등 예측인자에 대한 주의가 요망된다고 하겠다. Clozapine-유발성 무과립구증이 발생하면, 우선 골수생검 등을 통한 정확한 진단이 이루어져야 하며, 즉각적 clozopine의 투약중단, 혈액학 및 감염학 전문의에게 자문의뢰하고, 2차감염을 막기 위한 격리 및 예방적 항생제 투여 등과 함께, G-CSF나 GM-CSF투여를 통한 무과립구증 기간 단축 등의 적극적인 치료가 반드시 요망된다. The authors present a successful treatment case of clozapine-induced agranulocytosis with granulocyte colony-stimulating factor(G-CSF). It is the first case in Korean that occurred despite proper monitoring by the Clozaril Patient Monitoring System(CPMS) and full clinical vigilance. The mechanism of clozapine-induced agranulocytosis is unknown. Various studies are attempting to identify the pathogenic mechanism involved, and whether it is immunologic like human leukocyte antigen(HLA)-associated or toxic like desmethylclozapine-associated. However, it is clear that the final common pathway is suppression of myeloid proliferation in the bone marrow. The theory that clozapine-induced agranulocytosis is caused by suppression of colony forming units of granulocytes and macrophages(CFU-GM) forms the rationable for the use of GCSF or GM-CSF. The management of clozapine-induced agranulocytosis should include accurate diagnosis of agranulocytosis through bone marrow examinations, prompt discontinuation of clozapine, consultation with a hematologist, infectious disease specialist. Reverse isolation and administration of prophylatic antibiotics are need for prevention of secondary infection. A potential decrease of recovery time achieved by G-CSF obviously lowers the risk of secondary infectious disease.
