In recent years, gonorrhea has been pandemic and remains one of the most common STDs in the world, especially in developing countries. For the detection of a more effective therapeutic regimen and assessing the prevalence of Penicillinase Producing Neisseria gonorrhoeae(PPNG), we have been trying to study the patients who have visited the Venereal Disease Clinic of Choong-Ku Public Health Center in Seoul since 1980 by menas of the chromogenic cephalosporin method. In 1998, 93 strians of N. genorrhoeae were isolated, among which 60(64.5%) were PPNG. The prevalence of PPNG in Seoul, which had been decreased to 39% in 1996 after a peak of 74.3% in 1993, is increased to 64.5% in 1998.

OBJECTIVES: Our goal is to validate diagnosing and characterizing epilepsy based on a medical record survey by external reviewers. METHODS: We reviewed medical records from 80 patients who received antiepileptic drugs in 2009 at two hospitals. The study consisted of two steps; data abstraction by certified health record administrators and then verification by the investigators. The gold standard was the results of the survey performed by the epileptologists from their own hospital. RESULTS: The specificity was more than 90.0% for diagnosis and activity, and for new-onset seizures. The sensitivity was 97.0% or more for diagnosis and activity and 66.7-75.0% for new-onset epilepsy. This method accurately classified epileptic syndromes in 90.2-92.9% of patients, causes in 85.4-92.7%, and age of onset in 78.0-81.0%. Kappa statistics for inter-rater reliability and test-retest reliability ranged from 0.641-0.975, which means substantial to near-perfect agreement in all items. CONCLUSIONS: Our data suggest that epilepsy can be well identified by external review of medical records. This method may be useful as a basis for large-scale epidemiological research.

<P><B>Abstract</B></P> <P>Limited regenerative capacity of the nervous system makes treating traumatic nerve injuries with conventional polymer-based nerve grafting a challenging task. Consequently, utilizing natural polymers and biomimetic topologies became obvious strategies for nerve conduit designs. As a bioinspired natural polymer from a marine organism, mussel adhesive proteins (MAPs) fused with biofunctional peptides from extracellular matrix (ECM) were engineered for accelerated nerve regeneration by enhancing cell adhesion, proliferation, neural differentiation, and neurite formation. To physically promote contact guidance of neural and Schwann cells and to achieve guided nerve regeneration, MAP was fabricated into an electrospun aligned nanofiber conduit by introducing synthetic polymer poly(lactic-co-glycolic acid) (PLGA) to control solubility and mechanical property. <I>In vitro</I> and <I>in vivo</I> experiments demonstrated that the multi-dimensional tactics of combining adhesiveness from MAP, integrin-mediated interaction from ECM peptides (in particular, IKVAV derived from laminin α1 chain), and contact guidance from aligned nanofibers synergistically accelerated functional nerve regeneration. Thus, MAP-based multi-dimensional approach provides new opportunities for neural regenerative applications including nerve grafting.</P> <P><B>Statement of significance</B></P> <P>Findings in neural regeneration indicate that a bioinspired polymer-based nerve conduit design should harmoniously constitute various factors, such as biocompatibility, neurotrophic molecule, biodegradability, and contact guidance. Here, we engineered three fusion proteins of mussel-derived adhesive protein with ECM-derived biofunctional peptides to simultaneously provide biocompatibility and integrin-based interactions. In addition, a fabrication of robust aligned nanofiber conduits containing the fusion proteins realized suitable biodegradability and contact guidance. Thus, our multi-dimensional strategy on conduit design provided outstanding biocompatibility, biodegradability, integrin-interaction, and contact guidance to achieve an accelerated functional nerve regeneration. We believe that our bioengineered mussel adhesive protein-based multi-dimensional strategy would offer new insights into the design of nerve tissue engineering biomaterials.</P> <P><B>Graphical abstract</B></P> <P>[DISPLAY OMISSION]</P>

Purpose : The aim of this study was to investigate the timing and sequence of eruption of permanent canine and premolars, and to evaluate tooth calcification stage on emergence in Korean children. Materials and Methods : The sample was comprised of 1,266 children (male 720, female 546) aged from 7-13 years. Tooth eruption and calcification stages were determined through oral and panoramic radiographic examination, respectively. Probit analysis was used to calculate the timing of tooth eruption and tooth calcification stage from these cross-sectional data. Results : In both males and females, eruption occurred around the time when one third of tooth root or more was formed. The sequence was as follows: first premolar, canine, and second premolar in maxilla, and canine, first premolar and second premolar in mandible. Tooth eruption occurred earlier in girls compared with boys, averaging 0.63 years. Conclusions : Eruption sequence is identical in males and females with a trend for females to erupt earlier than males. Tooth eruption becomes earlier over the past decades in Korean children.

Objective: The purpose of this study is to determine the prevalence of sinus disease and abnormalities in patients scheduled for dental implant in maxillary posterior area using cone beam CT. Patients and Method: One hundred five maxillary sinuses in eighty-seven patients who underwent cone beam CT for dental implant in maxillary posterior area were included. Any patients who had previous history of sinus operations were not included. The sinus abnormalities were classified as follows ; normal (membrane thickness 〈2 mm), mucosal thickening (membrane thickness ≥ 2 mm and 〈6 mm), partial opacification (membrane thickness 〉6 mm but not full), full opacification and mucous retention cyst. The relationship between the remaining bone height, sinus symptoms and maxillary sinus abnormality was statistically surveyed. Results: Of 105 maxillary sinuses in 87 patients, 80 (76%) maxillary sinuses showed abnormalities ; 4 of 4 symptomatic patients and 76 of 101 asymptomatic patients. Mucosal thickening was the most common sinus abnormality. Only 3 (4%) of 80 maxillary sinus abnormalities were caused by the odontogenic origin. The prevalence of maxillary sinus abnormalities was higher in the symptomatic group than asymptomatic one (p<0.05). Conclusion: Maxillary sinus abnormalities were very common in the patients who were planning implantation in maxillary posterior areas. This result supports that thorough evaluation for maxillary sinus is recommended when implant treatment is planned for those areas.
허베이 스피리트호 유류 유출이 초등학생들의 알레르기 질환 유병률에 미친 영향

연구목적: 본 연구는 2007년 12월 7일 크레인 부선과 유조선 허베이 스피리트호가 충돌해 원유가 유출된 사고에 따른 인접한 지역 내 초등학생을 대상으로 유류노출과 알레르기 질환과의 관련성을 알아보았다. 연구방법: 지리정보 시스템을 활용하여 사고 지점으로부터 거주지까지 20 km 안쪽에 거주하는 학생을 고노출군, 20 km초과 거주자를 저노출군으로 분류하고, 방제작업 참여여부에 따라 노출수준을 분류하여 ISAAC 설문과 피부단자 검사를 사고1년 6개월이 지난 시점에서부터 실시하였다. 연구결과: 조사 대상자의 지난 12개월 동안의 알레르기 증상경험 유병률은 천식이 9.4%, 알레르기성 비염 39.1%, 알레르기성 피부염 17.0%, 알레르기성 결막염 13.6%로 나타났다. 알레르기성 질환에 영향을 줄 수 있는 다른 환경요인을 통제하여 유출 유류 노출 강도에 따른 영향만을 보기 위하여 혼란요인을 보정한 후 분석한 결과, 사고지점으로부터 거주지까지의 거리에 따른 지난 12개월 동안의 알레르기성 비염의 교차비(odds ratio)가 저노출군보다 고노출군에서 1.88배(95% CI: 1.08 -3.29) 높은 것으로 나타났다(p<0.05). 알레르기성 질환에 영향을 줄 수 있는 다른 요인을 보정한 후, 방제작업 참여유무에 따른 알레르기성 질환 유병률의 차이를 분석한 결과, 지난 12개월 동안의 알레르기성 비염의 교차비가 미방제작업군보다 방제작업군에서 1.93배(95% CI: 1.12 - 3.30) 높은 것으로 나타났다(p<0.05). 결론: 초등학생의 알레르기 질환 중 알레르기성 비염의 경우 노출군에서 유의하게 높은 위험도를 나타내었으며, 전체적으로 유류 유출 피해지역에 거주하는 초등학생의 알레르기 질환의 유병률이 높은 것으로 보아 유류 유출이 초등학생의 알레르기성 비염 발생에 일부 영향을 미쳤을 것으로 생각된다. Objective: The relationship between the degree of oil spill exposure and the allergy prevalence rate of elementary school child living nearby the spot where the crude oil was spilled from the collision between the Korean crane barge and registered oil tanker Hebei Spirit on December 7, 2007. Methods: The research was conducted by dividing the subjects into two groups of the highly exposed and the low exposed according to the distance from the point of accident to their residence, and categorizing the oil exposure degree depending on the participation in clean-up works, with the ISAAC questionnaire and skin prick testing. Results: The allergy prevalence rate of the subjects for the last 12 months stood at 9.4% for asthma, 39.1% for allergic rhinitis, 17.0% for atopic dermatitis, 13.6% for allergic conjunctivitis. The study revised general confusion factors for allergic to analyze environmental factors that may influence allergic. In terms of the prevalence rate of allergic rhinitis by distance, the highly exposed group suffered allergic rhinitis 1.88 times (95% CI: 1.08 - 3.29) more than the low exposed group for the past 12 months, with significant relationship (p<0.05). In terms of the rate by participation in clean-up activities, the participated group experienced allergic rhinitis 1.93 times (95% CI: 1.12 - 3.30) more compared to the other (p<0.05). Conclusion: This study that the allergic diseases of the elementary school child living in the oil spill area would have posed a threat to their health.

Objective : The purpose of this study is identify the operation status of the neurosurgical care units (NCUs) in neurosurgical residency training hospitals nationwide and determine needed changes by comparing findings with those obtained from the Korean Neurosurgical Society (KNS) and Korean Society of Neurointensive Care Medicine (KNIC) survey of 2010. Method : This survey was conducted over 1 year in 86 neurosurgical residency training hospitals and two neurosurgery specialist hospitals and focused on the following areas : 1) the current status of the infrastructure and operating systems of NCUs in Korea, 2) barriers to installing neurointensivist team systems, 3) future roles of the KNS and KNIC, and 4) a handbook for physicians and practitioners in NCUs. We compared and analyzed the results of this survey with those from a KNIC survey of 2010. Results : Seventy seven hospitals (87.5%) participated in the survey. Nineteen hospitals (24.7%) employed a neurointensivist or faculty member; Thirty seven hospitals (48.1%) reported high demand for neurointensivists, and 62 hospitals (80.5%) stated that the mandatory deployment of a neurointensivist improved the quality of patient care. Forty four hospitals (57.1%) believed that hiring neurointensivist would increase hospital costs, and in response to a question on potential earnings declines. In terms of potential solutions to these problems, 70 respondents (90.9%) maintained that additional fees were necessary for neurointensivists' work, and 64 (83.1%) answered that direct support was needed of the personnel expenses for neurointensivists. Conclusion : We hope the results of this survey will guide successful implementation of neurointensivist systems across Korea.
