        Surgical technique for single-port laparoscopy in huge ovarian tumors: SW Kim`s technique and comparison to laparotomy

        Objective This study aimed to introduce a method to remove huge ovarian tumors (≥15 cm) intact with single-port laparoscopic surgery (SPLS) using SW Kim`s technique and to compare the surgical outcomes with those of laparotomy. Methods Medical records were retrospectively reviewed for patients who underwent either SPLS (n=21) with SW Kim`s technique using a specially designed 30×30-cm2-sized 3XL LapBag or laparotomy (n=22) for a huge ovarian tumor from December 2008 to May 2016. Perioperative surgical outcomes were compared. Results In 19/21 (90.5%) patients, SPLS was successfully performed without any tumor spillage or conversion to multi-port laparoscopy or laparotomy. There was no significant difference in patient characteristics, including tumor diameter and total operation time, between both groups. The postoperative hospital stay was significantly shorter for the SPLS group than for the laparotomy group (median, 2 [1 to 5] vs. 4 [3 to 17] days; P<0.001). The number of postoperative general diet build-up days was also significantly shorter for the SPLS group (median, 1 [1 to 4] vs. 3 [2 to 16] days; P<0.001). Immediate post-operative pain score was lower in the SPLS group (median, 2.0 [0 to 8] vs. 4.0 [0 to 8]; P=0.045). Patient-controlled anesthesia was used less in the SPLS group (61.9% vs. 100%). Conclusion SPLS was successful in removing most large ovarian tumors without rupture and showed quicker recovery and less immediate post-operative pain in comparison to laparotomy. SPLS using SW Kim`s technique could be a feasible solution to removing huge ovarian tumors.

      • 간 세포암에서 VEGF, TGF-β1, b-FGF 발현의 의의

        Purpose: Angiogenesis is important for the proliferation and the metastasis of solid tumors. The growth of a solid tumor is widely recognized to depend on the process of neovascularrozation. Without angiogenesis, tumors cease to grow beyond even a few milimeters in diameter. It has been shown that tumor vascular density is an independent prognostic marker in several types of human tumors and is known to correlate with poor prognosis. To date, many angiogenic factors have been identified, such as transforming growth factor-α(TGF-α), transforming growth factor-β(TGF-β), fibroblast growth factor family(FGF), vascular endothelial growth factor(VEGF), platelet derived endothelial cell growth factor(PD-ECGF), tumor necrosis factor-α(TNF-α), and angiogenin. Hepatocellular carcinoma(HCC) is the second most common tumor in Korean males and is known as a typical hypervascular tumor with frequent portal vein invastion. The authors identified the expreesion of VEGF, TGF-β1, and b-FGF in HCC specimens and evaluated the relationship between these growth factors and the clinicopathologic characteristics of HCC. Method: We reviewed the medical records of 30 patients who were diagnosed as hepatocellular carinoma treated with hepatic resection between January 1994 and December 1998 in Soonchunhyang University Chunan Hospital. The selection of the cases was decided according to the condition of paraffin block fixation. The prognostic factors such as age, sex, tumor size, concentration of serum α-fetoprotein, presence of liver cirrhosis, presence of tumor emboli in portal vein, TMN stage, amount of transfusion during the operation, hepatitis B virus(HBV) infection, and Edmonson-Steiner(E-S) grade were investigated. Relationship between the prognostic factors and the immunopathologic expression of the TGF-β1, b-FGF, and VEGF was examined. Result: Thirty patients (24 males, 6 females) were included in the current study. The patient's mean age was 50.6 years and the age ranged from 36 to 65 years. The mean size of the tumor was found to be 5.2cm. All the patients were follewed up for 7 to 63 months. Child's classification A patients were 23(76.7%)cases, B patients were 7(23.3%)cases, and C was none. Immunohistochemical staining of HCC tumor mass in VEGF expression patients were 17(56.7%), b-FGF expression patients were 10(33.3%), and TGF-β1 expression patients were 10(33.3%). VEGF expression or more than one positive expression among the three factors correlated with tumor size and the stage of HCC but did not correlated with other clinicopathological characteristics. TGF-β1 and b-FGF did not correlate with any clinicopathological characteristics. Conclusion: The results suggest that the expression of VEGF or more than one positive expression among the three factors in HCC cells may be a significant prognostic factor of HCC.

      • 林土의 有機物의 分解速度와 無機養料의 垂直分布에 關한 考察

        Kim Choon Min, Chang Nam Kee and Chung Whan Ho: Decomposition rate of plant residue and the vertical distibution of mineral nutrients in the woodland soil. The relationship between decomposition rate of litter and the amount of mineral nutrients dispersed to the soil horizons were studied on the oak and pine forest stand. The results are as follows: 1. Decomposition rate of litter was highest in the F horizon of the oak stand, however in the pine stand it was highest in H horizon. 2. The correlation coefficients between decomposition rate and humus, organic carbon, moisture content, calcium, phosphorus, and nitrogen in the pine stand are; 0.90, 0.98, 0.76, 0.98, and 0.88, in oak stand; 0.97, 0.93, 0.92, 0.97, 0.74 and 0.73. 3. Factors which affect the decomposition rate of litter are humus, organic carbon, moisture content, calcium, phosphorus and nitrogen. 4. Among the variables, PH, moisture content and phosphorus under the oak and pine forest showed significant differences in each horizon. 5. C/N ratio of pine stands is higher than that of oak stands studied. 6. The vertical distribution of mineral nutrients is highly related with the amount of humus accumulated on top soil.

        Mycobacterium mageritense에 의한 당뇨병성 족부 감염증 1예

        Diabetic foot infection is one of the important complications in patients with advanced diabetes mellitus. Limb threatening infections such as osteomyelitis, abscess, and necrotizing fasciitis are frequently accompanied by the disease. Non-tuberculous mycobacterium (NTM) is a rare causative organism of diabetic foot infection. Thus, if one is not suspicious or meticulous, infection due to NTM will be easily overlooked and this will result in delayed diagnose and treat. Therefore, it is necessary to consider NTM as the causative organism if the wound does not respond to the conventional antibiotic treatment and the culture from the adequately obtained specimen reveals atypical acid-fast bacilli. We present a case of diabetic foot infection with osteomyelitis and abscess due to Mycobacterium mageritense, one of the rapid growing mycobacteria, that was successfully treated with surgical debridement and appropriate antibiotic treatment.

        반복적인 부비동염으로 내원한 Good 증후군 1예

        Good 증후군은 흉선종과 면역결핍이 동반되는 질환으로, B세포의 감소로 인한 범저감마글로불린혈증과 CD4^(+)T세포의 감소로 인한 세포성 면역저하가 동시에 나타나는 것을 특징으로 한다. 본 증례는 18개월 전부터 발생한 반복적인 부비동염을 주소로 내원한 43세 남자 환자로, 내원 1년 전 흉선종절제수술을 시행받았으며, 면역학적 검사상 범저감마글로불린혈증과 T세포의 감소 등의 이상 소견이 발견되었다. 정주 면역글로불린과 경험적 항생제(amoxicillin/clavulanic acid) 사용 후 부비동염이 호전되었으며, 예방접종과 정기적인 면역글로불린 투여 후 특이 합병증없이 경과 관찰 중이다. Good's syndrome is the association of thymoma with immunodeficiency, characterized by hypogammaglobulinemia, B-cell lymphopenia and variably defects in cellular immunity with CD4^(+) T-cell lymphopenia and an inverted CD4^(+):CD8^(+) T-cell ratio. We report a 43-year-old male patient who presented with a 18-month history of productive cough and postnasal drip. One year ago, he underwent the operation for resection of a thymoma. Despite of appropriate management, sinusitis relapsed multiple times. He was found to have hypogammaglobulinemia with nearly absent B cells(4/μL). The CD4^(+) T-cell count was 554/μL with an inverted CD4^(+):CD8^(+) T-cell ratio of 0.6. His symptoms and signs improved with antibiotic treatment and monthly administration of intravenous immunoglobulin (IVIG, 400 mg/kg).

        소아의 상완골 원위부 과간 골절

        상완골 원위부 과간 골절은 소아에서는 매우 드물게 발생되는 골절로 대부분 주관절의 굴곡상태로 주관절 후방부에 직접적인 충격에 의해 발생하는 것으로 보고되고 있다. 전위가 없거나 경미한 경우 도수적 정복술 혹은 도수적 정복술 후 경피적 핀고정술을 시행하며, 전위가 심한 골절이거나 도수적 정복술이 불가능한 경우 관혈적 정복술 후 핀고정술을 시행하는 것으로 알려져 있다. 저자들은 전위된 상완골 원위부 과간 골절 환아 2예에서 각각 도수적 정복술과 관혈적 정복술 후 K 강선 고정술을 시행하여 만족한 결과를 얻었기에 보고하는 바이다. Intercondylar fractures of the distal humerus are very rare in children. The pattern of the fracture would suggest that the mechanism of injury involves a fall directly on the flexed elbow. Most agree that the undisplaced fracture can be managed conservatively. With increasing displacement and comminution, the opinions tend to differ. We suggest that closed reduction with percutaneous K wire pinning and open reduction with percutaneous K wire pinning offer a satisfactory methods by which to treat displaced intercondylar fracture in children.

        Erythrophagocytosis of Lead-Exposed Erythrocytes by Renal Tubular Cells: Possible Role in Lead-Induced Nephrotoxicity

        <P>Background: Nephrotoxicity associated with lead poisoning has been frequently reported in epidemiological studies, but the underlying mechanisms have not been fully described.</P><P>Objectives: We examined the role of erythrocytes, one of the major lead reservoirs, in lead-associated nephrotoxicity.</P><P>Methods and results: Co-incubation of lead-exposed human erythrocytes with HK-2 human renal proximal tubular cells resulted in renal tubular cytotoxicity, suggesting a role of erythrocytes in lead-induced nephrotoxicity. Morphological and flow cytometric analyses revealed that HK-2 cells actively phagocytized lead-exposed erythrocytes, which was associated with phosphatidylserine (PS) externalization on the erythrocyte membrane and generation of PS-bearing microvesicles. Increased oxidative stress and up-regulation of nephrotoxic biomarkers, such as NGAL, were observed in HK-2 cells undergoing erythrophagocytosis. Moreover, TGF-β, a marker of fibrosis, was also significantly up-regulated. We examined the significance of erythrophagocytosis in lead-induced nephrotoxicity in rats exposed to lead via drinking water for 12 weeks. We observed iron deposition and generation of oxidative stress in renal tissues of lead-exposed rats, as well as the histopathological alterations such as tubulointerstitial lesions, fibrosis, and up-regulation of KIM-1, NGAL, and TGF-β.</P><P>Conclusions: Our data strongly suggest that erythrophagocytosis and subsequent iron deposition in renal tubular cells could significantly enhance nephrotoxicity following lead exposure, providing insight on lead-associated kidney damages.</P><P>Citation: Kwon SY, Bae ON, Noh JY, Kim K, Kang S, Shin YJ, Lim KM, Chung JH. 2015.       • 스타 토폴로지에서 다중경로정보를 이용한 효율적 요약기법

        We aggregate multi-links information between boundary nodes using the line segment scheme that aggregates topology information within PG referring bandwidth and delay parameter in this paper. The proposed scheme search multi-links efficiently using the depth priority method based on hop count instead of searching all links. To do this, we propose a modified line segment algorithm using two line segment method that represents two points which consist of delay-bandwidth pair to reduce topology information and provide a flexibility to the multiple-links aggregation. And we apply it to current star topology aggregation. To evaluate performance of the proposed scheme, we compare/analyze the current method with the proposed scheme with respect to call success rate, access time and crankback rate. Through the simulation result analysis, the proposed star topology aggregation scheme presents the better performance than existing scheme.

      • 미생물에 의한 수용성 절삭유의 劣化에 관한 연구

        In order to study on the degradation of water soluble cutting oil by microbial cells, the optimal growth conditions of bacteria and Saccharomyces cerevisiae ATCC 26603 and degradation phenomena by activity of microbial cells were examined. The object of this experiment was to find measures for preserving the quality of cutting oils by which an overall cost could be cut down. As antibiotics for Saccharomyces cerevisiae ATCC 26603, p-chlorophenol, formaldehyde, boric acid, potassium sorbate, sodium benzoate, and sodium propionate were used to soluble-type water soluble cutting oil. For the comparison of degradation rate, Saccharomyces cerevisiae ATCC 26603 was added to emulsion-type and soluble-type cutting oil (seed culture 2ml of Saccharomyces cerevisiae/cutting oil 100-ml). The results obtained in this experiment were as follows: 1) Saccharomyces cerevisiae ATCC 26603 utilized mineral oil, surface additive agent, inorganic salt, and fatty acid as the sources of nourishment. Then the concentration of cutting oil was decreased 2) Optimal growth conditions of Saccharomyces cerevisiae ATCC 26603 in emulsion-type cutting oil were at pH 3.0, 37.5℃, and the dilution ratio of 100 : 1(water: original liquid). The degradation was started at pH6.5, refraction 1.3323, and cell number 10^5 pieces/ml. In case of soluble-type cutting oil, the optimal growth conditions of Saccharomyces cerevisiae ATCC 26603 were the same as those of emulsion-type, but the degradation was started at pH 8.3, refraction 1.3343, and cell number 8 × 10 exp(5) pieces/ml. 3) From the experiment of antiseptics, it was found that effectiveness was in inhibition was in the following order: p-chloropheno>sodium benzoate>sodium propionate>boric acid/potassium sorbate>formaldehyde.
