        심방 중격과 승모판 전엽 양쪽에 동시에 부착된 좌심방 점액종 1예

        저자 등은 심계항진을 주소로 내원한 59세 여자 환자에서 심초음파에 의해 진단하여 종양 절제를 성공적으로 시행한 좌심방내 심방 중격과 승모판 전엽 양쪽에 동시에 부착된 점액종 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Myxoma is the most common primary cardiac tumor. It usually develops on the interatrial septum, and occurs in the left atrium in more than 75% of cases. Myxoma in the ventricle is much less common and accunts for only 5% of cases. A myxoma arising from mitral valve is exceedingly rare and is normally located on the atrial side of valve, with and eqivalent distribution between the anterior and posterior leaflets. We report a case of myxoma that arose from both the interatrial septum and anterior mital leaflet. which has not been previously reported in the literature.

      • 2-Fluorenyl chalcone 유도체의 합성과 가수분해반응에 관한 연구

        Fluorenyl chalcone 유도체를 합성하였고 25 ±1℃의 20% dioxane-H?O 혼합용매 중에서 자외선 분광광도법을 이용하여 pH 1.0~13.0까지의 넓은 pH범위에서 가수분해 반응속도를 측정하였다. 또한 Fluoreny chalcone 유도체의 pH변화에 따른 반응속도로 측정하고, 일반염기효과, 치환기효과 및 가수 분해 생성물 확인 등으로부터 Fluorenyl chalcone 유도체에 대한 가수분해 반응속도상수를 구하였다. 이들 유도체들은 pH와 무관한 부분과 수산화 이온농도에 비례하며, pH와 무관한 부분에서는 중성의 물분자가 첨가된다. 반응속도 상수 측정으로부터 Fluorency chalcon 유도체의 가수분해 반응은 비가역 1차 반응임을 확인하였고, 치환기효과로부터 전자유인성기에 의해 반응이 촉진되는 것을 알 수 있었다. 또한, 가수분해 최종 생성물은 확인 결과 2 ~acetyl fluorene과 벤즈알데히드였다. 이러한 실험결과를 토대로 하여 Fluorenyl chalcone 유도체에 대한 가수분해 반응 메카니즘을 제안하였다. It maybe concluded that a part was unrelated to pH and another part was in proportion to concentration of hydroxide ion: Above pH 10.0, It was in propotion to concentration of hydroxide ion, a part having no concern with pH was added to the neutral H?O molecule. From the result of measurement the reaction rate, hydrolysis of fluorenyl chalcone derivatives confirmed to the irreversible first order through measurement the substituent effect, It was found that reaction rate was accelerated first order through measurement the substituent effect, It was found that reaction rate was accelerated by electron attracting group. Also, from the result of final product, there were 2-acetyl fluorene benzaldehyde.On the basis of these findings, Hydrolysis for the fluorenyl chalcone derivative was proposed a fitting mechanism.

      • Thiopental이 백서 자궁 평활근의 수축에 미치는 영향

        Background and Objectives: Thiopental has been used as an intravenous induction agent for cesarean section. The aim of the present study was to investigate the effects of thiopental on the isolated rat uterine smooth muscle. Materials and Methods: Uterine smooth muscle tissues were obtained from non-pregnant rats(n=20). The muscle strips were suspended in tissue baths and the isometric tension was recorded. After spontaneous uterine activity had been accomplished, thiopental in various concentrations was applied cummulatively to the bath and the effects were continuously monitored. Results: Thiopental induced a dose-dependent inhibition of spontaneous myometrial contractile activity, Complete muscular relaxation on the spontaneous uterine contractility was obtained at in a dose of 10_(-3)M of thiopental. Conclusions: The spontaneous contractile activity of the uterine smooth muscle is inhibited by thiopental.

        Furfurylidene acetophenone 유도체의 합성과 가수분해 반응

On the basis of general base catalysis, substitutent effect, confirmation of hydrolysis products, it was measured the reaction rate of furfurylidene acetophenone derivatives for the pH change. It maybe concluded that a part was unrelated to pH and another part was in proportion to concentration of hyfroxide ion : Above pH 10.0, It was in proportion to concentration of hydroxide ion, a part having no concern with pH was added to the neutral H_2O molecule. From the result of measurement the reaction rate, hydrolysis of furfurylidene acetophenone derivatver confirmed to the irreversible first order. Through measurement the substituent effect, It found that reaction rate was accelerated by electron attracting group. Also, From the result of final product, There were furfural and acetophenone derivative was proposed a fitting mechanism.

        전기장 인가에 따른 DNA 신장에 관한 AFM 연구

        본 연구에서는 마이카 표면상에서 전기장에 의한 극히 낮은 농도의 DNA 신장에 대한 효율적인 방법론을 제시하였다. 전기장에 의한 DNA 신장을 AFM형상으로 분석한 결과, DNA 농도범위가 약 57×10 exp(-3)ng/mL-57×10 exp(-6)ng/mL일 때 매우 잘 적용되며, 신장된 DNA는 두 전극 중앙에 존재하였다. 또한, 일반 마이카, Mg^2+ 처리된 마이카, 그리고 AP-마이카 표면들에 대한 신장 효율의 차를 기술하였다. 동일한 실험조건 하에서 AP-마이카의 표면이 가장 좋은 효율의 DNA 신장 결과를 나타내었다. An effective method of DNA stretching using electric field on mica surface was proposed for extremely low solution concentration of DNA. DNA stretching based on electric field was observed by AFM and well applied on the concentration range from 57×10 exp(-3)ng/mL to 57×10 exp(-6)ng/mL. Stretched DNA existed in the middle of two electrodes. The difference in stretching efficiency for the different surfaces of bare mica, Mg^2+ soaked mica and AP-mica was discussed. The best performance of stretching was found from the surface of AP-mica under the same experimental condition.

        평활근종으로 오인된 샘근육증에서 기원한 투명세포암

        The incidence of cancer from adenomyosis is rare. Previously, only two cases of clear cell adenocarcinoma (CCA) arising from adenomyosis have been reported in English literature. Here, we report a case of CCA arising from adenomyosis. A 52-year-old postmenopausal Korean woman presented with complaints of vaginal bleeding and back pain. Endometrial biopsy revealed endometrial polyp with atrophic change and transvaginal ultrasonography showed myomas with cystic change. Magnetic resonance imaging revealed a cystic degenerative mass consistent with leiomyoma in the posterior portion of uterus body. And the serum level of CA-125 was 17.7 U/mL. Hysterectomy revealed a yellow-ten solid mass in the myometrium that was diagnosed as CCA arising from adenomyosis, The tumor was mainly located in the myometrium and transition between adenomyosis and CCA along with endometrial stromal cell was identifi ed, Malignant tumor arising from adenomyosis could be considered as a differential diagnosis when the patient with adenomyosis and intact endometrial surface complained of vaginal bleeding.

        외안각 절제술과 경결막 절개에 의한 안와 접근법

        Various surgical approaches to the orbits have been used. But the transconjunctival approach, also called the inferior fornix incision, provides adequate access to the orbital floor with esthetic results. Additionally, transconjunctival approach with lateral canthotomy allows more wide surgical field and good access to the orbital area. Generally, preseptal approach is more used rather than postseptal approach in the transconjunctival incision for the prevention of fat herniation during surgery. There are some complications in transconjunctival approaches but these complications can be prevented with careful surgical techniques. We report clinical results in 12 patients who were treated by preseptal transconjunctival incision with lateral canthotomoy for the treatment of orbital trauma. We were satisfied with this technique for reduction and reconstruction of the orbital trauma and we could also observe relatively good esthetic results.

        아세클로페낙 연질캡슬(클란자 에스 연질캡슬)의 개발

        To develop and aceclofenac soft capsule, four preparations with various solubilizers were prepared and their dissolution test was carried out. Among four preparations tested, a preparation with ethanolamine was selected a formula of aceclofenac soft capsule (Clanza S^(™), since it showed the fastest dissolution rate. Bioequivalence of aceclofenac tablet, Airtal^(™)(Dae-Woong Pharmaceutical Co., Ltd.) and aceclofenac soft capsule, Clanza S^(™)(Korea United Pharmaceutical Co., Ltd.) was evaluated according to the guideline of KFDA. Fourteen normal male volunteers (age 20-25 years old) were divided into two groups and a randomized 2×2 cross-over study was employed. After oral administration of one tablet or capsule containing 100 ㎎ of aceclofenac, blood ws taken at predetermined time intervals and the concentration of aceclofenac in plasma wa determined with an HPLC method under UV detector. The pharmacokinetic parameters (C_(max), T_(max) and AUC_(t)) were calculated and ANOVA was utilized for the statistical analysis of parameters using logarithmically transformed AUC_(t), C_(max) and T_(max) between Airtal tablet and Clanza soft capsule were 2.89%, 0.18% and 43.0%, respectively. There were no sequence effects between two formulations in these parameters. The 90% confidence intervals using logarithmically transformed data were within the acceptance range of log(0.8) to log(1.250(e.g.log(0.81) - log(1.23) and log(0.89) - log(1.14)) for AUC_(t) and C_(max), respectively. Thus, the criteria of the KDFA guidelines for the equivalence was satisfied, indicating that Clanza S^(™) soft capsule is bioequivalent to Airtal^(™) tablet.
