        정신분열병에 대한 리스페리돈의 효과 및 안정성

        연구목적 : 본 시험의 목적은 임상시험 시작전에 연구자들을 대상으로 PANSS Workshop을 통하여 PANSS, ESRS에 대한 국내에서의 표준화 작업을 구축하고 새로운 정신병 치료제인 리스페리돈의 효과와 안정성을 재확인하여 리스페리돈 사용에 대한 적정화를 이루는데 있다. 연구방법 : 1996년 4월부터 1996년 9월까지 국내 39개 대학병원 정신과에 입원중인 혹은 증상이 악화되어 입원하는 정신분열병 환자 377명을 대상으로 다시설 개방 연구를 시행하였다. 1주일간의 약물 배설기간을 가진후, 리스페리돈을 8주간 투여하였고, 기준점, 1주, 2주, 4주, 그리고 8주후에 평가되었다. 용량은 제1일에는 리스페리돈 1mg씩 1일 2회, 제2일에는 2mg씩 1일 2회, 제3∼7일에는 3mg씩 1일 2회 투여하였다. 이후 환자의 임상상태에 따라 임의로 증량할 수 있으며, 최대 일일 16mg을 초과하지 않도록 하였다. 추체외로 증상을 조절하기 위한 투약을 허용하였다. 임상증상 및 부작용의 평가는 PANSS(Positive and Negative Syndrome Scale), CGI(Clinical Global Impression) 그리고 ESRS(Extrapyramidal Symptom Rating Scale)을 사용하였다. 연구결과 : 377명중 343명(91%)이 8주간의 연구를 완결하였다. 치료 종결시점인 8주후 PANSS 총점수가 20% 이상 호전된 경우를 약물 반응군으로 정의할때, 약물반응군은 81.3%였다. 리스페리돈에 반응하는 예측인자로는 발병연령, 이전의 입원 횟수, 유병기간이 관련 있었다. 리스페리돈은 1주후부터 PANSS양성, 음성, 및 일반정신병리 점수상에 유의한 호전을 보여 효과가 빨랐다. CGI의 경우도 기준점에 비해 1주후부터 유의한 감소를 나타내었다. ESRS의 경우, 파킨슨 평가점수는 기준점과 비교해 투여 1주, 2주, 4주후 유의하게 증가되었다가 8주후 기준점과 차이가 없었다. Dystonia 평가점수는 1주후만 유의한 증가를 보였으며, dyskinesia 평가점수는 유의한 차이가 없었다. 혈압, 맥박수의 생명징후 및 일반 혈액학 검사, 생화학적 검사, 심전도 검사에서 유의한 변화는 없었다. 결 론 : 이상의 다시설 개방 임상 연구를 통해 리스페리돈은 정신분열병 환자에서 양성증상뿐만 아니라 음성증상 및 전반적인 증상에도 효과적인 것으로 사료된다. 보다 명확한 평가를 위해서는 다른 항정신병약물과의 이중맹검 연구가 필요할 것으로 생각되며, 또한 장기적 치료에 대한 평가도 함께 이루어져야 하겠다. Objective : The purpose of this study was to investigate the efficacy and safety of risperidone in the treatment of Korean schizophrenic patients. Method : This multicenter open study included 377 schizophrenic patients drawn from 39 university hospitals. After a wash-out period of 1 week, the schizophrenic patients were treated with risperidone for 8 weeks and evaluated at 5 points ; at baseline, and 1, 2, 4 and 8 weeks of treatment. The dose was increased from 2mg/day(1mg twice daily) to 6mg/day(3mg twice daily) during the first week and adjusted to a maximum of 16mg/day over the next 7 weeks according to the patient's clinical response. Medication to control extrapyramidal symptoms was permitted. The psychiatric and neurological status of the patients was assessed by PANSS, CGI, and ESRS scales. Results : 343(91%) of 377 patients completed the 8-week trial period. Clinical improvement, as defined by a 20% or more reduction in total PANSS score at end point, was shown by 81.3% of patients. The predictors of response to risperidone were associated older age, shorter duration of illness, fewer previous hospitalization. Risperidone had rapid onset of action ; a significant decrease of the total PANSS and three PANSS factor(positive, negative, general), and CGI was already noticed at the end of first week. For the ESRS, parkinsonism rating scores were significantly increased until week 4 comparing with baseline. Dystonia rating scores were significantly increased until week 1, and dyskinesia rating scores were not significantly changed during the study. Laboratory parameters including vital sign, EKG, hematological, and biochemical values showed no significant changes during the trial. Conclusions : This study suggests that risperidone is generally safe and effective against both the positive and negative symptoms in our group of patients.

        SrCI_2:Eu^2+ 형광체의 광발광 및 광자극발광 특성

        고상반응법으로 SrCl_2:Eu^2+ 형광체를 제작하고, 제작한 형광체의 광자극발광과 광발광 특성을 조사하였다. SrCl_2:Eu^2+ 형광체와 광발광 및 광자극발광은 Eu^2+ 의 5d->4f 천이에 기인되었으며, 355 nm의 광으로 여기시켰을 때 광자극발광과 광발광 스펙트럼의 파장범위는 모두 380~440nm이었고, 피이크 파장은 407nm이었다. SrCl_2:Eu^2+형광체의 선량의존성은 2.5 mGy~200mGy영역에서 우수한 선형성을 나타내었으며, 상온에서 광자극발광의 fading은 20분에 60%이었다. SrCl_2:Eu^2+ phosphors were prepared by the solid phase reaction method, and their photostimulated luminescence(PSL) and photoluminescence(PL) characteristics were investigated. The PSL and PL peak of the SrCl_2:Eu^2+ phosphors are due to the 5d->4f transition of Eu^2+ ions in phosphors. The PSL and PL spectrum obtained by the 355 nm exitation was observed in 380~440 nm region with the peak at 407 nm. The dose response of the PSL phosphors were linear within 2.5 mGy ~ 200 mGy of 100kV X-ray. The fading of the phosphors at room temperature was approximately 60% after 20 min.

      • CsI(Ag) 단결정의 육성과 섬광특성

        CsI(Ag) single crystals doped with 0.001, 0.003 and 0.005 mole % silver as activator were grown using the Czochralski method. The gamma ray energy spectrum is measured by the CsI(Ag) single crystals coupled to photomultiflier tube. It was confirmed that the crystal structure of grown CsI(Ag) was bcc, and that its lattice constant was 4.568Å. The energy resolution of CsI(Ag) for Cs-137 gamma ray was maximum when CsI(Ag) was doped 0.003 mole % silver, and its value was 9.84% the pulse height from the scintillation detector system using the CsI(Ag) single crystals was linear to gamma ray energy.

        BaFBr:Eu^2+ 형광체의 열발광 및 광자극발광 특성

        BaFBr:Eu^2+ 형광체를 제조하고, 이 형광체의 열발광 특성과 광자극발광 특성을 조사하였다. 이 형광체의 열발광 g1ow 피이크 온도는 352K와 448K였으며, 주 피이크(352K)에 관여하는 트랩의 활성화에너지는 약 0.96eV이었다. 또한 이 형광체의 광자극발광 스펙트럼의 파장범위는 350~450nm 사이였으며, 광자극발광에 기여하는 트랩의 활성화에너지는 약 0.98eV이었다. 열발광트랩과 광자극발광 트랩의 활성화에너지는 실험오차 내에서 일치하였다. BaFBr:Eu^2+ phosphors were prepared, and the thermoluminescence(TL) and photostimulatedluminescence(PSL) of the prepared phosphors were measured. Two glow peaks around 352 and 448 K are observed for x-ray irradiated BaFBr:Eu^2+ phosphors, and the activation energy of the main glow peak(352 K) was about 0.96 eV. The spectral range of the PSL was 350 ~450 nm, and the activation energy of the trap giving rise to PSL was about 0.98 eV. The activation energy of the traps giving rise to TL is agreed to those giving rise to PSL within experimental error.

      • 도시 고형폐기물의 발생원 관리

        The management of generating source was studied for effect treatment and disposal of municipal solid waste(MSW). Two house with boiler using briquette, one apartment with central heating system and Bukhan National Park were selected in this study as generating source. Generation rate, composition and properties such as moisture volatile solid(V.S), fixed solid(F.S) and lower heating value(LHV) of each materials in the MSW were measured weekly, and the collection system of MSW in Seoul city were investigated. From this experimental results, the following conclusions were obtained. 1. The annually average generation rate of MSW was 377g/cap.day in the case of apartment and 1350g/cap.day in the case of house which involved briquette ash in 75wt%. 2. As the generation rate changes in a large range in our country(the number of month within 15% above or below yearly mean were more than 33%), It is very difficult to determine the size of collection and treatment facilities. 3. It was possible to recover directly papers and plastic to 90% and 80%of them respectely from the generating source. In this case, the waste resources could be with good quality within 10% moisture content. 4. The weight proportions of garbages(vegetable, food and fruits) and plastic were 46.9 and 9.7wt% in annually average respectively, which were higher than those in other country. The moisture content and V.S. content of MSW were 45.3 and 41.4/wt% in annually average. 5. The lower heating value of MSW was measured to be 2170㎉/㎏ in wet basis after briquette separation, but to go down 800㎉/㎏ after papers and plastic recovery. 6. The equations for estimating LHV in wet basis were obtained in This equation (7) and (8), was found to be more correct than other equations. LHV = 4500Vc + 9500Vp-600W[㎉/㎏]···(7) LHV = 7260 Pi + 2950Pa + 640(P_(G)+P_(F))+4000Pc-600W[㎉/㎏]···(8) 7. The collection system containing separated collection in Seoul city was ineffective owing to the lack of landfill site and loading place of MSW. 8. It is necessary for as to take proper management system for solid waste in the National Park because visitors and generation quantity of solid waste change largely according the place and season. 9. The major collection facilities in the National Park were waste basket and incineration box, but the incineration box was too large and its collection period was irregular, so it was cause to the congestion of solid waste and bad smell. Therefore, we must set up the more waste basket which able to fine within 40∼50 m from the origination place of solid waste and induce the visitor to throw the solid waste, and remove the incineration basket to one-third of total volume for the only purpose of open burning of solid waste at elevated place.

        In this paper, we propose the data storage model which supports the efficient updates and retrieval for XML documents. For proposed system, we adopt the DTD-independent schema form, and change the virtual fragmentation model for storing XML documents. And we design new identifier(called position ID) which does not update the position values of the other elements after many element insertion and deletion.

      • Helicobacter pylori 편모 유전자의 클로닝 및 염기서열 분석

        A λgt11 expression libary of H. pylori DNA in E. coli Y1090 was screened with flagellin-specific rabbit antiserum for molecular cloning of the flagellin gene of H. pylori. A positive clone, λHPF4, was obtained and the recombinant antigen expressed from λHPF4 was a fusion protein with the molecular weight of 168kd. Sequence analysis of antigen-encoding DNA showed that an open reading frame composed of 1,536 nucleotides encodes a polypepride with a oredicted molecular size of 54kd. This open reading frame did not show the homology with flaA gene encoding 56kd protein of H. pylori and was confirmed as a unique sequence through homoligy searching. Therefore, the cloned antigen is supposed to be the carboxy-terminal region of the other flagellin protein of H. pylori, flaB, with the molecular weight of 58kd.
