      • 정신질환자에 대한 집단음악치료 연구

        약 5주간에 걸쳐 정신질환자 12명을 대상으로 즉흥적 음악요법과 부분적인 Guided Imagery and Music(GIM)을 실시하였다. 집단음악요법이 정신질환자의 증상호전과 예후 판정에 어느 정도 도움을 줄 것인가에 흥미를 갖고 본 연구를 시도하였다. 그 결과 정신질환자들이 음악요법에서 얻은 음악적 활동의 호전은 정신병적 호전과 거의 병행하여 나간다는 사실을 확인하였다. I have had a trial of the group music therapy in 12 psychiatric patients who admitted in Seoul Paik Hospital since Feb 1992. I observe the on going process of patient's status in group MT, and so can summarize the findings. Resistance which the patients feel about the therapist can be relieves gradually with the supportive and encouraged acceptance through the music process. Also improvisational music play offers the organization and the coordination which remove defensive resistance, and contributes to relieve the symptom. I find that patients can judge the one's self behavior and then change it through the music process. The use of music as a tool of group therapy offers to the members being in the process of group therapy, the experiences of 'here and now' which helps patients learning a technical interpersonal relationship. Conclusively, First, the music therapy is effective as adjunctive method of the psychotherapy. Second, the music therapy would make be easy to observe the thought, behavior and feelings of patients for a psychiatrist or a music therapist. Third, the music therapy would be of benefit for assessing the prognosis of the social adaptation and symptomatological improvement of the patients. Forth, the music therapy would be benefit for establishing the rapport of the doctor·patient relationship. Fifth, the music therapy would of benefit for converting the nonverbal communication such as the depressive mood, social withdrawal and mutistic behavior of the patients to verbal communication. Sixth, the music therapy and dancing therapy would be helpful for the chronified patient who would be required the long term admission for treatment to refresh themselves. Finally, although I have had a few cases, the music therapy may be helpful for the patients who were relatively improved in acute negative symptom such as, negativism, autistic behavior, social withdrawal by antipsychotic medications.

        Everyone want to live long without physical problems. In the beginning of 20 century, the Korean average longevity did not exceed 25 years old. However, due to a rapid development of economy and social welfare system, the average longevity had reached 52.4 years old in 1960, 73.5 in 1995, 74.9 in 2000 and it will be 79 in 2030. So the dementia has emerged as a major health challenge, not only for clinician but for a society as a whole. Our faculties in Ilsanpaik hospital hale opened the dementia clinic to provide the better treatment and care service for the elderly. In this purpose we reviewed and summerized the articles about the prevention and management of dementia.

        의학잡지에 게재할 논문의 투고양식 - 신경정신의학 투고규정과 비교를 중심으로 -

        The content of medical paper that will cany for its support may point to the right format and this means that the two components, content and structure, are equally important to evaluate a journal’s quality. The medical literature of our time is so large that no one can read more than small fraction of it and recent development and generalization of computer technology make the people to search an online bibliographic database or its equivalent easily. So, many international index systems, such as Index Medicus, Science Citation Index, MEDIL INE have been developed and widely used. The registration to such a index system may be important because it is one of the way to introduce our papers to the international scientific society. To establish the right format and gratify the minimal requirement of a such system, revision of our journal’s introduction must be carried out. For this purpose, the authors compared our journal’s introduction with that of 85 foreign and 43 domestic journals and also reviewed The 4th edition of the Vancouver style

      • Neuroleptic Malignant Syndrome 2예

        저자들은 증상 및 임상적 경과로 보아 Neuroleptic malignant syndrome으로 생각되는 2예를 경험하였기에 문헌 고찰과 함께 이를 보고한다. Neuroleptic malignant syndrome, first named by Delay and Deniker, is a fetal illness characterized by muscular rigidity, hyperthermia, catatonic stupor and autonomic dysfunctions. Although Delay and Deniker suggest that respiratory in-sufficiency is a cardinal feature, it seems probable that respiratory distress associated with pneumonia is a secondary complication resulting from dysphagia. The pathogenesis of Neuroleptic malignant syndrome is unclear. But the various sins of Neuroleptic malignant syndrome can be explained as disorders of hypothalamus, basal ganglia and brain stem auctions. A possible link between Neuroleptic malignant syndrome and Acute lethal catatonia has been noted. And several authors discussed the relationship of this syndrome to Malignant hyperpyrexia, in which the use of inhaled anesthetics or muscle relaxing agent precipitates tachycardia, tachypnea, rapid temperature elevation and muscular rigidity. The authors present two cases of possible Neuroleptic malignant syndrome. Their detailed clinical features and out-comes are reported here with a brief review of recent therapeutic explorations.

        당뇨성 신경병환자들에서 우울증상

        Before studying the effectiveness of amitriptyline in alleviating the pain of diabetic neuropathy, this study was designed to compare the severity and nature of depressive symtoms of diabetic patients with neuropathy with those of diabetic patients without neuropathy and patients with somatoform disorder whose complaints were mainly somatic pain, respectively. The authors administered Beck Depression Inventory(BDI) to the three groups of patients. The mean total scores of BDI were relatively low in all groups and not significantly different among the three groups. The mean scores of four subscales of BDI were also not significantly different among the three groups. These results might suggest that the pain of diabetic neuropathy did not influence on the severity and the nature of depressive symptoms of preexisting diabetes.

      • 고유수용성 신경근 촉진법 통합패턴이 요통환자의 균형에 미치는 영향

        Purpose : The purpose of this study was to find out The effect of Low back pain on the total pattens of Proprioceptive neuromuscular facilitation(PNF). Methods : The subjects of the study were 62 low back pain patients. They were divided into two groups: 31 in the experimental group and 31 in the control group. the experimental group performed modalities and PNF total patterns three times for a week. the control group performed only modalities and ROM exercise three times for a week. Back muscle strength was measured by a back muscle strength measuring machine, the intensity of pain was measured by the Visual Analogue Scale (VAS), and the level of disability was measured by Oswestry low back pain disability index. Study measurements were compared before and after 6 weeks exercise program. The two groups of subjects were assessed by utilizing two different balance measurement: Static standing balance was measured by balance performance monitor (BPM) and Dynamic standing balance was measured by one leg standing(OLS). The scale for static standing balance was measured by using, sway area, sway path, max velocity. Results : The results of this study were as follow: 1) The score on visual analogue scale shows statistically significant increase on PNF group of post test(p<.05). 2) The score on Oswestry low back pain disability index. shows statistically significant decrease on PNF group of post test(p<.05). 3) The change sway area was statistically significant on pre-test and post-test(p<.05). 4) The change sway path was statistically significant on pre-test and post-test(p<.05). 5) The change max velocity was statistically significant on pre-test and post-test(p<.05). 6) The score on one leg standing shows statistically significant increase on PNF group of post test(p<.05). Conclusion : These results of this study indicated that Proprioceptive Neuromuscular Facilitation Total patterns which performed for six weeks had a statistically significant influence on low back pain. If the exercise for muscle strength is performed along with therapeutic stabilizing exercise, a better effect can be expected on low back pain. We hope that this study will provide a basic data for further research with a bigger group and on a long-term effect.

        Gamma Knife Radiosurgery for Brain Metastases in Patients Harboring Four or More Lesions : Survival and Prognostic Factors

        Objective : This study is performed to evaluate the role of gamma knife radiosurgery(GKS) for treatment of patients with large number of metastatic brain lesions. Methods : The authors retrospectively reviewed the clinical outcome of 99 patients who underwent 124 GKS procedures for a total of 564 metastatic lesions. Sixty-three patients presented with 1 to 3 lesions (group A) and 36 patients presented with 4 or more lesions (group B) at initial GKS. The authors assessed survival according to a variety of possible prognostic factors. Results : Median survival from the GKS for all cases was 28 weeks. The recursive partitioning analysis (RPA) class (p<0.0001) and the total tumor volume (p=0.0174) were significant factors influencing survival. The number of lesions, whole brain radiotherapy, primary tumor site, age, and sex were not significant. Median survival of group A (28 weeks) was not different from that of group B (28 weeks) (p=0.4027). Median survival was 66, 28, and 13 weeks for RPA class I, II, and III respectively in group A and 44, 36, and 11 weeks in group B. Conclusion : It is suggested that GKS provides a survival gain even in patients with a large number (4 or more) of metastatic lesions. Concerning the survival and the selection of patients for radiosurgery, RPA class and the total tumor volume should be considered as more important factors than the number of lesions. Multiplicity of the lesions alone should not be a factor for contraindication of radiosurgery.
