        성교육 실시에 따른 성지식, 성태도 변화 연구 : focusing on the 1st grade students in girls' junior high schools 1학년 여중생을 대상으로

        The purpose of this study was to assess the effect of sex education on knowledge and attitude related to sex. The subjects were taken from by 199 students in 3 classes from 1st grade in H girl's junior high school as the study group, and 2 classes from 1st grade in S girl's junior high school as control group. During the survey period(September 21, 1998 to September 30, 1998), 6 times in terms of one-hour class for sex education were taught to the study group. A pre-test was executed on September 19, 1998 and the post-test on September 30. The findings were as follows. 1. According to the research, 20.1% of the subjects have experienced sex education from parents and 89.9% from teacher. They have mostly obtained the sex-related information from teachers(59.8%), following movie, radio, TV, or video tape(40.7%), friends(35.2%), reading materials such as books, cartoons, news papers and magazines(31.7%), parents(15.6%), siblings(7.0%), PC(1.5%) and telephone service(1.5%). 2. 27.1% of the subjects reported that they had sex-related worry concerning from friendship with the opposite sex, following physiological phenomenon(31.5%), sex violence(11.1%), physical characteristics(7.4%), VD and contraception(5.6%), sexual impulse(5.6%), pregnancy and delivery(5.5%), and sexual behaviour(3.7%). The research showed that the adolescents usually solved their problems through the consultation with theifriends(44.4%). However, 16.7% of the subjects were turned out not to request any solution. The other minor routs to settle their problems were written materials such as books, magazines(13.0%), parents(13.0%), movie, radio, TV, or video tape(5.5%), acquainted female elders(3.7%) and teachers(3.7%). 3. The most interesting part regarding sex was the friendship with the opposite sex(61.8%), following adolescent's emotion(55.8%), physiological differences between two genders(52.8%), AIDS(48.7%), VD(46.7%), pregnancy(45.2%), contraception(45.2%), abortion(41.7%), intercourse(41.7%), masturbation(41.2%), sex violence(41.2%) and genital structure and secondary sexual characteristics(28.6%). 4. In regard to characteristics of the subjects influencing sex-related knowledge, the higher educational career of mother, living with at least either parent and the experience of sex education by teachers were statistically significant factors(p〈0.05). 5. In regard to characteristics of the subjects influencing attitudes toward sex, the experience of sex education by parents or teachers was a statistically significant factor(p〈0.05). 6. The analysis of knowledge score comparing results before and after sex education showed that control group's score decreased from 12.5 to 12.44 while the study group's score increased from 12.33 to 21.31, which was statistically significant(p〈0.001). 7. The analysis of the attitude scores before and after sex education showed that the control group's score slightly increased from 55.57 to 56.36, while the study group's score increased from 54.79 to 61.95, which was statistically significant(p〈0.001). 8. The level of sex-related concerns of the study group after sex education marked both the increase in some items and the decrease in others. 9. Most instructive session among the sex education was the third “to be a good friend to the opposite sex”(27.0%).

        초등학교 자녀의 구강보건에 관한 학부모의 인식 및 행동

        The purpose of this study was to collect basic data for the development of oral health education program of parents and to encourage their actual interest and participation in school oral health program. The authors surveyed parents' awareness and behaviors about the oral health of their children. The subjects in this study were 193 parents of elementary school children in Seoul. The collected data were analyzed, the obtained results were as follows: 1. Thirty-five percent of the parents investigated paid a regular visit to a dental clinic. The must common tooth brushing time among them was after dinner, the second was after breakfast, and this was followed by before bedtime and before breakfast. 2. Ninety-four percent of the parents responded that prevention is necessary, 92.2% had a plan to visit a dental clinic to prevent dental caries, and 60.1% experienced receiving pit and fissure sealants. Ninety-seven percent responded that they did check their children's dental caries. 54.9% chocked their children's oral status after tooth brushing. The parents who visit a dental clinic on a regular basis were more likely to check their children's teeth after tooth brushing than those who don't(P<0.05). 3. Eighty eight percent of respondents wanted to keep the fluoride mouth rinse program. 4. It is recommended that an oral health education program be developed for parents to visit dental clinics on a regular basis, to educate as to the right tooth brushing time, and to check out their children's oral status after tooth blushing.

      • 입원 환자 영양검색 전산 프로그램 개발

        Background : About 30% of patients in the hospital are under nourishment. A large portion of people are undernourished when they are admitted to the hospital and in the majority of these, undernutrition develops further while in hospital. Patient-Generated subjective global assessment(PG-SGA), a method of nutritional assessment based on clinical judgment, has been widely used to assess the nutritional status of adults for both clinical and research purposes. Modified PG-SGA can be used as a nutrition screening tool, and a comprehensive nutrition assessment is desirable for those who are malnourished. The purpose of this study is to develop a simple, reliable and valid nutrition screening program(NSP). Methods : We applied the nutrition screening program(NSP) to adult inpatients who are at risk of malnutrition using the Modified Patient-Generated subjective global assessment(PG-SGA) and objective assessment(electrical medical record data). Results : Therefore simple and beneficial NSP is developed for patients with severe malnutrition. Using NSP. nutritional information of the severe malnutrition patient should be shared with the physicians and they should be taken care of by clinical dietitians to improve their nutritional status. Conclusion : The Modified PG-SGA and nutrition screening tool are simple, reliable and valid nutrition screening program(NSP).

        초등학교 고학년 학생의 구강보건에 관한 인식 및 행동

        The purpose of this study was to collect basic data for the development of effective oral health education program in elementary school. The subjects in this study were 604 fourth-year, fifth-year and sixth-year students of an elementary school in Seoul. Oral health awareness and behaviors were surveyed and analyzed. The findings of this study were as follows: 1. The most common time for toothbrushing was after dinner(62.0%) and before bedtime(62.3%). By school year, the largest number who brushed their teeth after dinner were fourth and fifth graders while the greatest number who did so after breakfast were sixth graders Seventy-four percent of respondents brushed the teeth twice or more a day. By school year, the fifth-year students brushed their teeth the least while girls outperformed the boys in daily toothbrushing frequency(p<0.05). Regarding the intake of cariogenic food, 62.1% took that kind of food once or more a day. By school year, the fifth graders took the most (p<0.05), and girls took more cariogenic food than the boys(p<0.05). 2. Ninety percent of the respondents had visited a dental clinic. By grade, the sixth-year students had visited a dental clinic more frequently and girls more frequently than boys. Eighty percent of the respondents visited a dental clinic to receive treatment but just 12.3% visited a dental clinic to prevent oral diseases. Concerning the fetor of dental treatment only 14.4% expressed fear. School year made no difference, yet the percentage of girls who experienced fear was higher than that of boys(P<0.05). Regarding preventive treatment, 39.7% experienced pit and fissure sealing and 24.2% experienced the application of fluorides. All the students hadn't much experience in preventive treatment regardless of school year -- gender didn't make a difference. 3. Fourty-six percent of respondents had a preference for the fluoride mouth rinsing program; 38.4% wanted that grogram to continue. As to the reason for preference, 38.2% preferred it because of the prevention of dental caries, 43.0%, the largest percentage, didn't favor it because they found it too much trouble to do(some of the respondents gave two different answers). 4. Despite an increase in the frequency of toothbrushing at the right time, they still took cariogenic food frequently Their visit to a dental clinics also aimed at treatment more than at prevention, and they had very narrow experience of preventive treatment. Furthermore, the fluoride mouth rinsing program failed to draw their interest. Therefore, it is recommended that a successful real health education program should be developed.

        병원급식 위탁관리의 운영 실태조사

        The purposes of this study were to investigate the current status of contracted hospital food services and to find out the difference in accordance with the number of beds in hospitals. Thirty six hospitals having more than 100 beds in Seoul, Inchon and Kyungkido were the subjects of this study. Data was collected through surveys. The survey was conducted during March and April in 2002. The Questionnaires were mailed to the 36 directors of dietetic departments of the hospitals and 36 managers of contracting patient food services. Statistical analysis was completed using SPSS Win(11.0) for descriptive analysis and t-test. The results of the study are summerized as follows; Ⅰ. Hospital perspective : The range covered by contract food service was 63.3% and 36.7% in hospital food services, and medical nutrition services. The patient and employee food services were in 83.3% and patient food services were in 6.7%. The methods selecting contractors were general, limited, selected and competitive biddings, and private contracts. The responsibility for supervision of contract food services was the dietetic department(51.7%) in most cases. Hospitals having personnel responsible for contracting affairs were in 75.9% of the cases and 24.1% did not have personnel. The biggest reason for contracting was facilitation of personnel management. The most important criteria on selecting food services contractors was the professionality of the contractor. Ⅱ. Contractor's perspective : The cost per meal in the year 2001 was composed of 1,905 won for food cost, 1,081 won for labor cost, 222 won for expenses, 114 won for VAT, 14 won for rent and 146 won for miscellaneous or controllable expense, representing 109 won loss per meal. The profit-and-loss contract cost was higher than the fee-contract cost. The ratios of food cost, labor cost and expenses were higher and the ratios of miscellaneous or controllable expense, VAT, rent and profit were lower in hospitals with more than 400 beds compared with those less than 400 beds. However, no significant differences are present between these two groups of hospitals. The actual contract period was 2.2 years upon initial contract and 1.2 years upon renewal. The initial investment cost was 53 million won and the cost of renovation and repair was 8.5 million won. Significant differences were present between two groups of hospitals. The conditions of employment and number of personnel hired by contractors for contract patient food services were significantly different according to the number of beds.

      • 비만을 동반한 위암 환자에서 두 병을 같이 수술 치료한 경험(증례)

        Obesity is growing problem in Korea. We had a case of bariatric surgery during gastric cancer operation. Patient was 29 year old Korean lady with early gastric cancer located in the lesser curvature side of the middle 1/3 of the stomach. Her body weight was 89 kg, height 163 cm, and thus body mass index was 33.5 kg/m2. Preoperative blood pressure was 130/90 mmHg, hemoglobin 12.9 g/dL, total lymphocyte count 3,280/mL, serum albumin 4.3 g/dL, CEA 1.1 ng/mL, CA 19-9 9.1 ng/mL, and alpha fetoprotein 2 ng/mL. Another associated disease was right thyroid follicular neoplasm. The chest X-ray was normal. After IM injection of 2,500 units of heparin 30 minutes before the induction of general anesthesia, she received curative subtotal gastrectomy and Roux en Y gastrojejunostomy when a malabsorption loop of 120 cm jejunum was incorporated between the Treitz ligament and the end to side jejuno-jejunostmy site at May 29th 2006. Her postoperative course was uneventful except a minor wound seroma and the elevations of serum amylase(up to 4 folds) and lipase(up to 2 folds). She lost her body weight 9 kg in 5th, 21 kg in 7th, and 27 kg in 10th postoperative months to became 61.5 kg. On follow up exams in July 2008, she gained 4 kg to overcome her weakness and fatigue. From the above result, the incorporation of a malabsorption loop during reconstruction phase of gastric cancer operation was a good option for obese gastric cancer patients especially in cases of early gastric cancer.

        Use of Artificial Intelligence for Reducing Unnecessary Recalls at Screening Mammography: A Simulation Study

        Objective: To conduct a simulation study to determine whether artificial intelligence (AI)-aided mammography reading can reduce unnecessary recalls while maintaining cancer detection ability in women recalled after mammography screening. Materials and Methods: A retrospective reader study was performed by screening mammographies of 793 women (mean age ± standard deviation, 50 ± 9 years) recalled to obtain supplemental mammographic views regarding screening mammographydetected abnormalities between January 2016 and December 2019 at two screening centers. Initial screening mammography examinations were interpreted by three dedicated breast radiologists sequentially, case by case, with and without AI aid, in a single session. The area under the receiver operating characteristic curve (AUC), sensitivity, specificity, and recall rate for breast cancer diagnosis were obtained and compared between the two reading modes. Results: Fifty-four mammograms with cancer (35 invasive cancers and 19 ductal carcinomas in situ) and 739 mammograms with benign or negative findings were included. The reader-averaged AUC improved after AI aid, from 0.79 (95% confidence interval [CI], 0.74–0.85) to 0.89 (95% CI, 0.85–0.94) (p < 0.001). The reader-averaged specificities before and after AI aid were 41.9% (95% CI, 39.3%–44.5%) and 53.9% (95% CI, 50.9%–56.9%), respectively (p < 0.001). The reader-averaged sensitivity was not statistically different between AI-unaided and AI-aided readings: 89.5% (95% CI, 83.1%–95.9%) vs.92.6% (95% CI, 86.2%–99.0%) (p = 0.053), although the sensitivities of the least experienced radiologists before and after AI aid were 79.6% (43 of 54 [95% CI, 66.5%–89.4%]) and 90.7% (49 of 54 [95% CI, 79.7%–96.9%]), respectively (p = 0.031). With AI aid, the reader-averaged recall rate decreased by from 60.4% (95% CI, 57.8%–62.9%) to 49.5% (95% CI, 46.5%–52.4%) (p < 0.001). Conclusion: AI-aided reading reduced the number of recalls and improved the diagnostic performance in our simulation using women initially recalled for supplemental mammographic views after mammography screening.
