        철결핍성빈혈 진단과 RDW, MCV

        자동혈구계산기에서 산출되는 RDW나 MCV가 철결핍성빈혈의 구분진단에 어떠한 도움이 되는지를 보기 위하여 어는 대학병원의 최근 5년 동안의 의무기록과 건강진단결과를 이용하여 227명의 빈혈환자와 143명의 건강인을 선정하고 그들의 혈액검사 결과를 비교분석해 보았다. 분석결과 빈혈환자로서는 철결핍성빈혈과 재생불량성빈혈 환자가 많았으며, 그 외에 만성질환과 관련된 빈혈환자도 많은 것으로 나타났다. 정상인 들에 비하여 빈혈환자에서 RDW는 높게, 그리고 MCV는 낮게 나타났는데 특히 철겹핍성빈혈에서는 다른 빈혈에 비하여 19.3±4.8로 현저하게 높게 나타난 반면, MCV는 62.9±13.7fl로 현저하게 낮게 나타났으며, 그 빈혈의 정도에 따라 그 변화가 크게 나타났다. 빈혈관련수들을 이용하여 D.F.=0.26-0.012MCV-0.130MCH+0.073MCHC+0.052RDW+0.003PLT와 같은 판별함수가 도출되었고, 이를 토대로 철결핍성빈혈과 다른 빈혈을 구분하는데 있어서는 MCV, MCH, MCHC, RDW등이 판별력이 있다는 것을 확인 할수 있었다. RDW and MCV are thought to be the highly sensitive blood cell parameters in the differentiation of iron deficiency anemias. Through the medical records of 227 anemic patients and the physical checking results of 143 healthy persons in a General Hospital during the recent five years, the authors evaluated various blood cell parameters including RDW and MCV. Iron deficiency anemia, aplastic anemia and other anemias associated with chronic discase were shown as the three major causes of anemias in Korea. In the patients of iron deficiency anemia MCV was very low(62.9±13.7fL), while RDW was very high(19.3±4.8) showing much lower MCV and much higher RDW in severe IDA compared with in mild IDA. To differentiate iron deficiency anemias form other anemias, a discriminant function was developed from some blood cell parameters like MCV, MCH, MCHC, RDW and platelets(D.F.=0.26 -0.012MCV -0.130MCH+ 0.073MCHC +0.052RDW +0.003PLT).

      • 국내에서 유행하는 HIV의 전파 경로에 따른 Subtype 분포

        Background : Previous data have been reported that subtype B is prevalent in South Korea, but neither the extent nor the proportion of subtypes could be evaluated. This study was designed to analyze the distribution of HIV-1 subtypes, temporal instructions and transmission dynamics between epidemiological groups. Methods : 1,280 Koreans had been diagnosed as HIV seropositive during the period 1985 to 2000. Among them, 134 individuals were selected for this molecular epidemiological study. 134 DNAs were isolated from uncultured or cultured peripheral blood mononuclear cells. V3-V5 (0.7 kb) fragment of HIV-1 env gene was amplified by nested polymerase chain reaction and was sequenced. Results : HIV-1 isolates from thirty-seven homosexuals were all subtype B (100%). On the other hand, 66 isolates from 94 heterosexuals were subtype B (70%) and 28 were non B subtypes (30% : 13 A, 4 C, 2 D, 8 E , 1 G). Only subtype B strains were isolated from 73 males who were infected with HIV inside Korea while 16 B and 20 non B subtype strains were isolated from 36 males who were HIV infected outside of Korea. However, B and non B strains were isolated half and half from females who were infected inside Korea except one. Conclusion : The HIV-1 subtype B strains are prevalent in Korea from the early HIV infection until present in both homo and heterosexuals. Non B strains have been transmitted from men who were infected outside Korea to their spouses and casual partners. So, we need further study to monitor subtype B and non B HIV transmission in epidemiological groups of Korea, (Korean J Infect Dis 33:311∼318, 2001)

      • 대학생의 노인에 대한 태도에 관한 연구

        The purpose of this study is to identify the perception, the future and images, and prejudice toward old people. And to examine difference the group of gerontological education (educated group) with the non-educated group. 189 students in K university were participated in this study. The educated group was composed 101 students who enrolled liberal arts course that learned this title. Data were collected from May to June, 2002. The results are as follows; 1) Many students perceived the elderly that was started 60~64 years. 2) The perception, the future and images, and prejudice of both groups were some positively. 3) Each variables were not significantly difference. But the mean level of the educated group were more higher. 4) The educated group was found significantly related to the perception and prejudice(r=.303, p=.002). The non-educated group was found significantly related to the perception and the future and image(r=.421, p=.000), the perception and prejudice(r=.321, p=.003), the future, image and prejudice(r=.272, p=.012).

        구치부 레진 수복 재료의 가수분해

        우수한 심미성 수복재료로서 복합레진은 그 사용 빈도가 증가하고 있다. 이런 증가 추세에도 불구하고 복합레진의 부적절한 마모저항성 때문에 구치부 수복에서의 사용이 제한되어왔다. 이와 관련된 인자로 수복물의 표면하 분해가 고려되고 있다. 본 연구에서는 복합레진의 마모에 미치는 환경적 분해의 효과를 알기 위해 알카리성 용액(0.1 N NaOH)에 현재 많이 사용되는 Definite(Degussa-Hu¨ls AG, Germany), Prodigy(Kerr, USA), Pyramid(Bisco, USA) 및 Synergy(Coltene, Swiss)등 4종의 복합레진을 보관하였을 때 각 제품의 분해과정을 평가하고자 하였다. 각 제품 당 3개의 시편을 제작한 후 0.1N NaOH 용액에 저장하여 60℃에서 보관하였다. 2주 후 제거하여 HCI로 중화, 세척 후 60℃에서 건조하였다. 무게 손실, 분해층 깊이, Si 농도 등을 기준으로 분해저항성을 평가하여 다음과 같은 결과를 얻었다. 1. 무게 손실은 Synergy에서 1.24±0.002%로 가장 높은 값을 보였으나 각 제품간 유의한 차이는 보이지 않았다. 2. 분해층 깊이는 Synergy에서 107.83±2.52㎛로 가장 높은 값을 보였고, Synergy를 제외한 다른 제품에서는 유의한 차이를 보이지 않았다. 3. Si 용출량에 있어서는 4가지 제품 모두 차이를 보이지 않았다. 4. 무게 손실과 분해층 깊이 사이에는 높은 상관 관계를 보였다(r=0.6127, p<0.05). 5. 무게 손실과 Si 용출량, 분해층 깊이와 Si 용출량 사이에 상관 관계는 없는 것으로 나타났다. 6. 주사전자현미경 관찰시 NaOH 용액에 보관한 후 레진 기질과 필러 사이의 결합 파괴를 관찰할 수 있었다. The use of resin composites has continued to increase over the last several years. In spite of their growing popularity, composites continue to exhibit a number of undesirable characteristics. One of the major deficiencies of composite restorative resins is their inadequate resistance to wear. Of the multitude of factors that have been associated with wear, subsurface degradation within the restoration is considered to be one. The aim of this study was to evaluate the resistance to degradation of four commercial composite resins in an alkaline solution. This solution with a high concentration of hydroxyl ions is a convenient medium for accelerated degradation of silane coupling and filler particles. The brands studies were Definite(Degussa-Hu¨ls AG, Germany), Prodigy(Kerr, USA), Pyramid(Bisco, USA) and Synergy(Coltene, Swiss). Preweighed discs of each brand were exposed to 0.1N NaOH solution at 60℃. After 14 days they were removed, neutralized with HCI, washed with water and dried. Resistance to degradation was evaluated on the basis of following parameters: (a) mass loss(%)-determined from pre-and post-exposed specimen weights; (b) Si loss(ppm)-obtained from ICP-AE analysis of solution exposed to specimens; and (c) degradation depth(㎛)-measured microscopically (SEM) from polished circular sections of exposed specimens. The results were follows: 1. Mass loss of Synergy was 1.24±0.002%. it was the highest, there was no significant difference among the materials. 2. The degree of degradation layer depth of Synergy was 107.83±2.52㎛, it was the highest, there was no significant difference among any other materials than Synergy. 3. There was no difference among the four materials in Si loss. 4. The correlation coefficient between mass loss and degradation depth was relatively high(r=0.06, p<0.05). 5. There was no coefficient correlation between Si loss and mass loss, the degree of degradation layer depth and Si loss. 6. When observed with SEM, destruction of bonding is observed between resin matrix and filler.

      • 오매(Fructus Mume)의 항산화물질의 분리 및 특성연구

        천연물로부터 새로운 항산화제를 개발하기 위한 연구의 일환으로 한방에서 사용되는 한약재 중의 하나인 오매(烏梅)의 물 추출물과 메탄을 추출물 및 순차 분획물에 대해 팜유, 우지와 돈지를 기질로하여 유지저아시험, Rancumat method, 전자공여작용 등으로 항산화성을 확인한 결과, EtOAc 분획물에서 강한 항산화 활성을 나타내어 column chromatography, thin layer chromatography를 통하여 활성성분에 관한 연구를 시도하였다. 1. 오매 용매분획물 중 EtOAc 분획물(400 ppm)은 유지저장실험(60℃)에서, 팜유, 우지와 돈지에 모두 우수한 효과를 나타내었으며, 특히 우지에서 높은 활성을 나타내었고, BHA(200 ppm)와 α-tocopherol(200 ppm)보다 활성이 높았다. 2. 오매 EtOAc 분획물(400 ppm)은 Rancimat method에서는 팜유, 우지와 돈지에 비교적 우수한 효과를 나타내었으며, 우지에서 AI가 4.39로 상당히 높은 활성을 나타내었다. 3. 오매의 용매분획물 중 EtOAc 분획물이 DPPH라디칼에 대한 강한 소거활성을 나타내었으며, 그 활성은 EtOAc 분획물의 농도에 의존하여 증가하였고, SC_(50)은 21.2μg/ml이었다. 4. 오매 EtOAc 분획물에서 분리한 8개 획분 중 E4에서 SC_(50)이 16.7μg/ml으로 높은 DPPH 라디칼 소거활성을 나타내었으며, 원인 물질은 여러 가지 스펙트럼 측정 결과 수종 물질이 혼합물인 것으로 밝혀져, 보다 순수한 형태로의 분리 후 정확한 동정의 시도가 요구되었다. Fructus Mume is the roasted hits of Prunus mwme and has been used as tradibonal Chmese medicine. The anboxidatwe effect of the extract from Fructus Mume was measured using different methodsn such as, electron donating abdihes(EDA), Rancimat and oven tests. The methanol extract obtained from the Fructus Mume was solvent-fractionated with ethyl acetate, n-butanol and water, respectively. The ethyl acetate fraction showed the highest antioxidative activity among them. To identify the compounds with antioxidative activity, the isolates from thm layer chromatography were studied further. This study demonstrates that Fructus Mume might be useful for the prevention or treatment of disorders associated with oxidative damage.

        수종 치과용 광중합형 복합레진의 화학적 분해

        이상적인 수복재료의 요건으로 치아의 저작기능과 심미성을 충분히 회복시킬 수 있는 물리적. 화학적 성질을 갖추는 동시에 생물학적인 적합성과 구강내 환경변화에 따른 내구성 등을 들 수 있다 그러나 복합레진을 이용하여 치아를 수복하였을 때 수복물은 구강내에서 시간이 흐름에 따라 타액내에서 서서히 분해되고 교합력이나 음식물에 의해 마모되어가는 운명을 거친다. 본 연구에서는 현재 수복재료로서 많이 사용되고 있는 Definite(Degussa-Hiils AG. Germany), Revolution(Kerr, U.S.A.) Unifil(GC, Japan) , Palfique(Tokuyama, Japan)의 4종의 복합레진을 알카리성 용액 (0. IN NaOH)에 보관하였을 때 각 제품의 분해저항성을 평가하고자 하였다. 각 제품당 3개의 시편을 제작하여 무게측정을 한 후 0.IN NaOH에 저장하여 60℃에서 보관하였다. 2주 후 제거하여 HCI로 중화, 세척 후 60℃에서 건조하였다 무게손실, 표면하분해층 깊이, 용출된 Si농도를 기준으로 각 레진의 분해저항성을 평가하여 다음과 같은 결과를 얻었다. 1. 무게손실량은 Unifil에서 3.21%로 가장 높았으나 각각 제품간에 있어서의 차이를 보이지는 않았다. 2. 분해층 깊이는 107.69∼47.407n의 범위였고, Unifil, Palfique. Revolution, Definite 순으로 감소하였으며 Palfique. Definite를 제외한 다른 제품간에는 유의한 차이를 보였다(p(0.05). 3. Si 용출량은 Palfique가 8940.Oppm으로 가장 많은 용출량을 보였으며 Revolution과 Definite를 제외한 각 제품간에는 유의한 차이를 보였다(p<0.05). 4. 각 제품의 무게손실과 분해층 깊이 사이에는 높은 상관관계를 보였다(r= 0.06, p<0.05) 5. 각 제품의 무게손실과 Si 용출량, 분해층 깊이와 Si 용출량 사이 에는 유의한 상관관계를 보이지 않았다. 6. 주사전자현미경 관찰시 NaOH용액에 보관한 후 레진 기질과 필러 사이의 결합의 파괴를 관찰할 수 있었다. There was significant difference among the materials except Palfique and Definite. 3. The Si loss of Paltique was 8940.Oppm, it was the highest. There was significant difference among the materials, except Revolution and Definite(p<0.05). 4. The correlation coefficient between mass loss and degradation depth was relatively high(r = 0.06. p<0.05). 5. There was no significant coefficient correlation between Si loss and mass loss, and/or the degree of degradation layer depth and Si loss. 6. When observed with SEM, destruction of bonding is observed between resin matrix and filler. Above results suggested that the hydrolytic degradation is considered as evaluation factor of composite resins.

      • The development of the conditionally replication-competent adenovirus: replacement of E4 orf1-4 region by exogenous gene

        <B>Background</B><P>Tumor or tissue specific replicative adenovirus armed with a therapeutic gene has shown a promising anti-cancer therapeutic modality. However, because the genomic packaging capacity is constrained, only a few places inside it are available for transgene insertion. In the present study, we introduce a novel strategy utilizing the early E4 region for the insertion of therapeutic gene(s).</P><B>Methods</B><P>We constructed the conditionally replication-competent adenovirus (CRAd), Ad5E4(mRFP) by: (i) replacing the E4/E1a promoter by the prostate-specific enhancer element; (ii) inserting mRFP inside the E4orf1-4 deletion region; and (iii) sub-cloning enhanced green fluorescent protein controlled by cytomegalovirus promoter in the left end of the viral genome. Subsequently, we evaluated its replication abilities and killing activities in vitro, as well as its in vivo anti-tumor efficacy in CWR22rv xenografts.</P><B>Results</B><P>When infected with Ad5E4(mRFP), the number and intensity of the mRFP gene products increased in a prostate cancer cell-specific manner as designed, suggesting that the mRFP gene and E4orfs other than E4orf1-4 were well synthesized from one transcript via alternative splicing as the recombinant adenovirus replicated. As expected from the confirmed virus replication capability, Ad5E4(mRFP) induced cell lysis as potent as the wild-type adenovirus and effectively suppressed tumor growth when tested in the CWR22rv xenografts in nude mice. Furthermore, Ad5E4(endo/angio) harboring an endostatin-angiostatin gene in E4orf1-4 was able to enhance CRAd by replacing mRFP with a therapeutic gene.</P><B>Conclusions</B><P>The approach employed in the present study for the insertion of a therapeutic transgene in CRAd should facilitate the construction of CRAd containing multiple therapeutic genes in the viral genome that may have the potential to serve as highly potent cancer therapeutic reagents.
