Background: Osteoprotegerin (OPG) plays protective roles against the development of vascular calcification (VC) which greatly contributes to the increased cardiovascular events in patients with chronic kidney disease (CKD). The present study aimed to find the non- traditional, kidney-related cardiovascular risk factors correlated to serum OPG and the effect of serum OPG on the arterial stiffness measured by brachial ankle pulse wave velocity (baPWV) in patients with the pre-dialysis CKD. Methods: We cross-sectionally analyzed the data from the patients in whom baPWV and the serum OPG were measured at the time of enrollment in a prospective pre-dialysis CKD cohort study in Korea. Results: Along with traditional cardiovascular risk factors such as age, diabetes mellitus, pulse pressure, and baPWV, non-traditional, kidney-related factors such as albuminuria, plasma level of hemoglobin, total CO2 content, alkaline phosphatase, and corrected calcium were independent variables for serum OPG in multivariate linear regression. Reciprocally, the serum OPG was positively associated with baPWV in multivariate linear regression. The baPWV in the 3rd and 4th quartile groups of serum OPG were higher than that in the 1st quartile group after adjustments by age, sex and other significant factors for baPWV in linear mixed model. Conclusion: Non-traditional, kidney-related cardiovascular risk factors in addition to traditional cardiovascular risk factors were related to serum level of OPG in CKD. Serum OPG level was significantly related to baPWV. Our study suggests that kidney-related factors involved in CKD-specific pathways for VC play a role in the increased secretion of OPG into circulation in patients with CKD. Purpose To analyze the success and complication rates and factors associated with technical failure of the ultrasound (US)-guided percutaneous thrombin injection of femoral artery pseudoaneurysms caused by vascular access. Materials and Methods Records of 30 patients with post-catheterization femoral artery pseudoaneurysms who had been treated with US-guided percutaneous thrombin injections in the department of radiology between March 2009 and June 2019 were retrospectively analyzed. The lesion was diagnosed based on US or contrast-enhanced CT. The characteristics of the patients and their lesions were analyzed. Results The mean patient age was 67.8 years. The mean diameter of the pseudoaneurysmal sac was 20.88 mm (5–40 mm). Twenty patients (66.6%) obtained complete thrombosis after the primary injection, while 10 patients (33.3%) obtained partial thrombosis. The number of patients with a low platelet count (< 130 k/μL) was significantly higher in the partial thrombosis group than in the complete thrombosis group (p = 0.02). No substantial procedure-related complications were found in any patient. Conclusion The US-guided percutaneous thrombin injection is considered an initial treatment option for pseudoaneurysms caused by vascular access because of its safety and efficacy.
이행성 척추에 동반된 골수부종 환자의 복합한방치료: 증례보고

This study was to investigate the effectiveness of combined Korean medicine treatment on a patient who was diagnosed on lumbosacral transitional vertebra (LSTV) with localized bone marrow edema. The patient was treated with acupuncture, pharmacopucture, cupping and herbal medicine. We evaluated patient's progress using range of motion (ROM), EuroQol-5 dimensions (EQ-5D), Oswestry disability index (ODI), numerical rating scale (NRS), patient global impression of change, self-walkable distance. NRS of back pain decreased from 7 to 5, at the same time NRS of lower limb decreased from 4 to 3. EQ-5D score increased from 0.24 to 0.646. ODI score improved 80 to 62. Self-walkable distance and lumbar spine ROM also increased. As a result, clinical improvements were found in a patient. In conclusion, this study shows that Korean medicine treatment can be considered as effective conservative care for LSTV with localized bone marrow edema. (J Korean Med Rehabil 2022;32(1):125-131)
In silico 예측과 전기생리학적 방법을 이용한 벼의shaker type K+ 채널 유전자의 선별

Five rice cDNAs that are most likely to encode putative voltage-gated K+ channels were selected by the in silico sequence homology and membrane topology analyses of transmembrane domains (TMs) and of a wellpreserved K+ selectivity filter (TXXTXGYG) in reference to Arabidopsis shaker type K+ channel (KAT1). The five candidate cDNAs werefurther subcloned into pSP64T vector, a Xenopus expression vector, and then in vitrotranscribed to generate cRNAs. Each cRNAs were microinjected into Xenopus oocytes, and their K+ channel conductance was measured elecrophysiologically by using two electrode voltage clamping (TEVC). Among them, one of the rice cDNAs (clone J023118D04) gave rise to K+ current with biophysical characteristics similar to those of the shaker type K+ channels. Collectively, cloning and functional characterization of plant ion channels could be greatly facilitated by combining the in silico analysis and heterologous expression in Xenopus oocytes. 본 연구는 식물에 내염성을 줄 수 있는 유전자인 shaker type K+ 채널을 인코딩하는 유전자를 벼의 유전자로부터 선별하기 위해서 수행하였다. 먼저, 전압의존성 K+ 채널의 공통적인 특징인 6개의 막 관통부위와 K+ 이온선택필터 보존서열 (TXXTXGYG)을 보유하는지 여부에 따라 30,778개의 벼 게놈 유전자로부터 K+ 채널을 지정할 가능성이 가장 높은 5개의 유전자를 TMHMM과 CLUSTALW를 사용하여 in silico 방식을 통해 선별하였다. 선별된 5개의 cDNA를 pSP64T Xenopus 발현 벡터에 서브클로닝한 후 in vitro 전사로 cRNA를 제작하였다. 합성된 cRNA를 개구리 난모세포에 미세 주입시킨 후 발현된 K+ 채널의 이온투과도를 two electrode voltage clamping (TEVC) 방법을 이용하여 측정하였다. 그 결과, 클론 J023118D04가 shaker type K+ 채널의생물리적 특성을 갖는 것으로 확인하였다. 수많은 벼 유전자로부터 shaker type K+ 채널을 인코딩하는 유전자를선별하는데 in silico 예측 및 개구리 난모세포 이형발현과 전기생리학적 측정 방법의 결합이 매우 유용함을 확인하였다.
