        Use of mesenchymal stem cells for chronic kidney disease

        Despite advances in management of chronic kidney disease (CKD) with medications and renal replacement therapy, CKD remains a significant health issue. The regenerative ability of the kidney is limited, and usually inefficient to prevent progression of fibrosis. Unfortunately, currently available pharmacologic agents cannot halt tissue injury in CKD. So, CKD may finally progress toward end-stage renal disease, and morbidity and mortality of patients with CKD remains high [1]. Much effort to identify novel therapies, to retard kidney damage in CKD, has been made. Of such efforts, mesenchymal stem cells (MSCs) are of interest, because of their potential therapeutic effects, regarding kidney disease from a few years ago. The effectiveness of MSCs in treatment of kidney disease, has been extensively investigated in preclinical models, and systemic review of data, suggested that results are promising [2]. Therapeutic potential of MSCs is mediated by multiple mechanisms, such as immunomodulatory effects through secretion of regulatory cytokines, activation of regulatory immune cells, and the capacity to increase cellular repair through secretion of anti-apoptotic, anti-fibrotic, and up-regulation of renal developmental markers as well [3] (Fig. 1). These multiple functions of MSCs, supposedly lead to multifaceted strategies in various models of kidney disease.

        전립선비대증 환자에 대한 Doxazosin GITS의 안전성과 유효성 평가

        Purpose: The doxazosin-gastrointestinal therapeutic system(GITS) greatly minimizes the need for titration by changing the drug-delivery rate and pharmacokinetic profiles. This multi-center study was designed to evaluate the efficacy and safety of doxazosin GITS in patients with BPH when used under the standard usage conditions. Materials and Methods: A total of 487 men, aged 40 years or over, with clinical evidence of BPH were enrolled. Demographic profiles were gathered at the baseline and the patients followed up at 4 and 8 weeks for dose adjustment and final assessment of the efficacy, respectively. The efficacy, measured in terms of the International Prostate Symptom Score(IPSS), quality of life(QOL), maximum urinary flow rate(Qmax) and post-voiding residual volume were assessed on each visit. Results: On completion of the study, the mean change in the IPSS and QOL from the baseline were -6.9±5.7(p<0.01) and -1.4±1.1(p<0.01), respectively. The Qmax and post-voiding residual volume were significantly improved compared to the baseline(p<0.01). Decreases in the mean systolic and diastolic blood pressures from the baseline in hypertensive patients(n=139) were significantly greater than in normotensive patients(n=309). There were 25 cases(5.1%) among 487 patients in which adverse events, such as dizziness, impotence, dryness of mouth and postural hypotension, were reported. Conclusions: In treating BPH, doxazosin GITS significantly improved the IPSS, QOL, Qmax and post-voiding residual volume. Doxazosin GITS is a safe and effective drug for the treatment of BPH in Korean men when used in a standard clinical setting. (Korean J Urol 2004;45:667-673)

        Direct Measurement of the Force Generated by a Single Macrophage

        During immune responses, macrophages play important roles in phagocytosis and antigen presentation by engulfing pathogenic micro-organisms and cell debris. Since the function of a macrophage highly depends on a series of physical steps including migration, direct contact and strong binding to its target, deployment of cytoplasm and membrane, and intake of the target, here we have investigated the mechanical behavior of a macrophage by manipulating it with a flexible pipette that was used as a force sensor and transducer. We examined the response of a macrophage to mechanical pulling by a positively charged pipette. We observed that the macrophage initially formed strong binding to the pipette and migrated along the direction of pulling for some early period. After that period, the macrophage exerted a huge traction force to pull the pipette back and attempted to retract itself towards its original location. We found that whether it was able to return to the original location depended on the level of applied force. Since the traction force generated by a single macrophage had not been characterized accurately, we measured the force for the first time to our knowledge and found the maximum traction force to be around 80 nN. This quantitative measurement was made possible by a new and convenient method used to calibrate the stiffness of the pipette. Through the study, we acquired a better understanding of the mechanics of and the force generation by a macrophage.t?`t

        The authors present a case of subperisoteal orbital hematoma which is extended from subgaleal hematoma. A 15-year-old-male was admitted with a complaint of multiple visual symptoms of left eye following blunt head trauma. He has suffered from Wilson's disease. Several coagulative laboratory findings were abnormal(fibrinogen and coaguation factor V, X). Computed tomography, sonography and magnetic resonance imaging established the subperiosteal orbital extension of subgaleal hematoma into the orbital cavity. Needle aspiration of orbital subperiosteal hematoma was failed but symptoms and signs of the patient were much improved spontaneously following decrease of subgaleal hematoma. The literature is reviewed and management is discussed.

        전립선비대증 환자에 대한 Doxazosin GITS의 12개월 투여 후 유효성과 안전성 평가

        Purpose: The aim of this study was to assess the long-term efficacy and safety of doxazosin gastrointestinal therapeutic system(GITS) in Korean patients with benign prostatic hyperplasia(BPH) under usual care conditions. Materials and Methods: In this a 12-month, open-label, multicenter, noncontrolled, flexible-dose study, 475 men(≥40 years), with clinical evidence of BPH, were enrolled from 40 centers. Patients were evaluated at the baseline, and at 1, 2 and 6 months for dose adjustment, based on their treatment response(efficacy), and 12 months to assess the final efficacy. The primary efficacy variable was the Clinician's Global Assessment of Change(CGAC; improved, no change, or worse) and the secondary were the International Prostate Symptom Score(IPSS), quality of life(QoL), maximum flow rate(Qmax) and post void residual(PVR). The adverse events(AEs) and blood pressure(BP) were also recorded. Results: 186 patients completed the study. Most patients(n=155; 83.8%) improved, but 31(16.2%) had no change in symptoms based on the CGAC. The mean changes in the IPSS and QoL from the baseline were -9.0±6.8 and -1.6±1.4, respectively(both p<0.05). The Qmax and PVR were significantly improved compared with the baseline(Qmax 10.5±4.3 vs. 13.7±6.3, PVR 39.1±37.0 vs. 23.2±33.7, p<0.05). The decrease in the systolic and diastolic BP from the baselines in hypertensive patients(n=52) was significantly greater than in normotensive patients(n=134)(SBP/DBP -9.5±18.4/-13.4±10.9 vs. -3.3±12.5/-1.4±9.5, p<0.05). A total of 34 AEs related to the medication were reported(7.1%). The most frequently reported AEs were dizziness(2.7%), impotence(1.1%), dry mouth(0.8%) and postural hypotension(0.4%). Conclusions: Treatment with doxazosin GITS for 12 months resulted in significant improvements in the IPSS, QoL, Qmax and PVR, demonstrating its effectiveness and good tolerability as a long-term therapy for Korean BPH patients.
