        대한민국 청소년의 주간 졸음증에 관련된 요인 분석

        Background: Daytime sleepiness, a common phenomenon among adolescents focused on academics, has negative effects on aspects such as growth and overall learning. However, research on various drugs and diseases affecting daytime sleepiness is lacking in the reality. Therefore, this study aims to investigate the factors influencing daytime sleepiness in adolescents with daytime sleepiness. Methods: This study was conducted through a survey of 2,432 middle and high school students, aged 14 to 19. The questionnaire consisted of information on socio-demographic characteristics, overall health status, and sleep patterns. The Pediatric Daytime Sleepiness Scale (PDSS), translated into Korean, was used to assess daytime sleepiness. Daytime sleepiness was measured by calculating the total score for each item of the PDSS, and divided into two groups based on the cutoff value of 19, which was the upper quartile. Results: We analyzed a total of 1,770 students including 799 boys and 971 girls. Students with a PDSS score of 19 or higher made up 33.3% of boys and 66.7% of girls. In multivariate analyses, females, smoking, poor self-reported health level, sleep after 12 am, not feeling refreshed in the morning, headache, muscle pain, and scoliosis increased the risk of daytime sleepiness significantly. The AUROC of PDSS, including significant factors in multivariate analyses, was 0.751 (95% CI 0.725~0.776). Conclusions: Daytime sleepiness in adolescents affects growth, academic performance, and emotional stability. Therefore, it is important to manage medications, diseases, and other factors that affect daytime sleepiness on a social level.

        Predictors of Excessive Daytime Sleepiness in Korean Snoring Patients

        Background : Excessive daytime sleepiness is one of the most common symptoms in snoring patients. How-ever, the reason why some individuals complain of daytime sleepiness and others do not is unclear. In this study,we evaluated snoring individuals and examined several demographic and polysomnographic profiles in an attemptto identify predictors of excessive daytime sleepiness. Methods : The inclusion criteria for patients were the fol-lowing: 1) patients who underwent an overnight polysomnograph, 2) patients with the chief complaint of snoring,and 3) patients who completed the Korean version of the Epworth sleepiness scale. We used the Epworth sleepi-ness scale to estimate excessive daytime sleepiness. We quantified correlations between the Epworth sleepinessscale and the demographic/polysomnographic parameters. We also analyzed the parameters affecting excessivedaytime sleepiness using multiple linear regression analysis. Results : One hundred nineteen patients met theinclusion criteria for this study. Multiple regression analysis showed that young age was the only independentvariable showing statistical significance for predicting excessive daytime sleepiness, and was well-correlated withthe Epworth sleepiness scale. However, there were no polysomnographic parameters that were predictive. Con-clusions : Clinicians need to be cautious when using the Epworth sleepiness scale for the diagnosis of obstructivesleep apnea and determining the response to treatment

      • Relationship among smartphone use, sleep quality, and daytime sleepiness in college students

        Aim(s): Electronic devices such as computers and smartphones are being used for classes, team activities, and meetings to reduce contact between people due to COVID-19. The portability of smartphones enables people to access it continuously throughout the day, which affects sleep. The purpose of this study is to identify the relationship among smartphone use, sleep quality, and daytime sleepiness in college students. Method(s): This study used a descriptive research design. The convenience sample consisted of 159 college students in D city of South Korea. The data were measured with the Smartphone Overuse Scale, Pittsburgh Sleep Quality Index, and Epworth Sleepiness Scale through an online survey in May 2021. Descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients were applied to analysis using MedCalc 20.0 program. Result(s): About 76% of the subjects were using their smartphones before going to bed. The subjects mainly used smartphones from 8 pm to 4 am next morning (65%), and 88% of them used it in bed. There was no significant difference between smartphone use pattern and sleep quality. However, the group that used the smartphone before going to bed had a significantly higher daytime sleepiness score than the group that did not use it (p<.002). There was a statistically significant correlation among smartphone overuse, sleep quality (p<.001), and daytime sleepiness (p<.001). Conclusion(s): Daytime sleepiness symptoms were found in the group who mainly used smartphones in bed during the night and early morning. There were correlations among smartphone overuse, quality of sleep, and daytime sleepiness. As the non-face-to-face situation continues, excessive use of smartphones at night may change into pathological daytime sleepiness in college students. The study findings suggested that continuous guidance on the correct use of smartphone should be provided.

        일 도시 고등학교 학생들의 수면형태와 주간졸음증의 예측인자

        Objectives Various sleep patterns may be shown in adolescents to be busy for studying. The aim of this study was to investigate sleep-wake patterns and to evaluate predictors of daytime sleepiness. Methods Among 1011 high school students living in Cheonan-si, demographic variables and self-reported questionnaires, including items about menstruation, Morningness-Eveningness Questionnaire (MEQ), Beck Depression Inventory (BDI), Pittsburgh Sleep Quality Index, and Epworth Sleepiness Scale (ESS) were admitted. Results A total of 839 students (male=453, female=386) completed questionnaires. Significant differences in sleep pattern, categorized by score of MEQ (p=0.004), ESS (p<0.001), and BDI (p=0.012) were observed between male and female subjects. Logistic regression analysis showed that excessive daytime sleepiness was predicted by female [odds ratio (OR)=2.039, p=0.022] and sleep quality (OR=1.238, p=0.004). In the regression analysis of female students, dysmenorrhea (β=0.108, p=0.029), eating of caffeine (β=-0.114, p=0.019), MEQ (β=-0.108, p=0.027), insomnia (β=0.180, p=0.002), and depression (β=0.116, p=0.029) might be associated with daytime sleepiness. Conclusion In Korean high school students, female and sleep quality might be risk factors of daytime sleepiness. Daytime sleepiness was known to be related with cognitive dysfunction. Therefore, an education program for improving sleep quality in these adolescents should be considered for mental health. 본 연구는 839명의 고등학생을 대상으로 시행한 연구로서, 남녀학생에서 일주기 리듬에 따른 수면형태에서 차이를보였다. 수면형태에 따른 비교에서 저녁형 학생에서 아침형과 중간형 학생보다 수면의 질이 낮고, 주간졸음증이 더 심하며, 불면증상과 우울증상이 높았다. 주간기능에 영향을 준다고 알려진 주간졸음증 발생에는 여성과 수면의 질이 유의한 위험인자로 평가되었다. 특히 여학생에서는 월경곤란과저녁형 수면형태가 주간졸음증과 연관성이 있는 것으로 나타났다. 따라서, 청소년 학생들의 정신건강과 효율적인 학업수행을 지지하기 위해서, 수면의 질을 향상시키고 올바른 수면위생을 배울 수 있는 프로그램 개발과 교육이 중요할 것으로 생각된다.

        간호대학생의 스마트폰 중독이 주간 졸음에 미치는 영향

        본 연구는 간호대학생의 스마트폰 중독과 주간 졸음 정도를 파악하고, 스마트폰 중독이 주간 졸음에 미치는 영향을 파악하기 위한 서술적 상관관계 연구이다. 간호대학생을 편의 표집하여 스마트폰 중독, 우울, 수면의질, 주간 졸음 등이 포함된 설문지를 이용하여 조사하였다. 187명의 자료를 분석한 결과 스마트폰 중독은 24.6%이었으며, 수면의 질은 22.4(±11.97)점, 주간 졸음 점수는 6.2(±3.20)점으로 주간 졸음증은 13.9%였다. 주간 졸음에영향을 미치는 변수는 수면의 질(β=.44 p=.000)과 스마트폰 중독(β=.17, p=.010)으로 나타나 주간 졸음의 22.5%를설명하였다 그러므로 간호대학생의 수면 건강을 증진하기 위해 건강한 스마트폰 사용 가이드를 포함한 수면 위생교육 프로그램의 개발 및 운영, 그리고 상담 프로그램의 활성화가 필요하다. The purpose of the study was to determine the extent of smartphone addiction and daytime sleepiness and the effect of smartphone addiction on daytime sleepiness among nursing students. A total of 187 nursing students were convenience sampled and surveyed using a questionnaire that included smartphone addiction, depression, sleep quality, and daytime sleepiness. Data analyses showed that the prevalence of smartphone addiction was 24.6%, sleep quality was 22.4 (±11.97), daytime sleepiness was 6.2 (±3.20), and the prevalence of excessive daytime sleepiness was 13.9%. The variables affecting daytime sleepiness were sleep quality (β =.44, p=.000) and smartphone addiction (β=.17, p=.010), which explained 22.5% of daytime sleepiness. Therefore, the development and implementation of sleep hygiene education programs, including guidelines for healthy smartphone use, and the activation of counseling programs are needed to promote sleep health among nursing students.

        지연성 수면위상증후군 청소년에서 기상 시 어려움과 주간 졸림에 대한 Dawn Simulation 효과 연구

        Objectives : Delayed sleep phase syndrome (DSPS) is characterized by difficulties in falling asleep and waking up at a desired time. Dawn simulation is a technique using a light that gradually increases in intensity before awakening in the morning, to imitate a natural sunrise. It has been found to be effective in decreasing both morning drowsiness and difficulty in awakening as well as treating symptoms of seasonal affective disorder. The aim of this study was to determine whether dawn simulation is helpful in decreasing difficulty in morning awakening and daytime sleepiness in adolescents with DSPS. Methods : Twelve adolescents with DSPS participated in a 2-week dawn simulation trial. Each subject self-assessed level of difficulty in awakening, morning drowsiness and daytime sleepiness during a 2-week baseline period and a following 2-week trial period with dawn simulator. Subjects used Stanford Sleepiness Scale (SSS) for measuring morning drowsiness and Epworth Sleepiness Scale (ESS) for measuring daytime sleepiness. Difficulty in awakening was assessed by a single-item questionnaire. Results : Dawn simulation trial decreased morning drowsiness (p=0.016) and daytime sleepiness (p=O.013) significantly compared to baseline. It also seemed to improve difficulty in awakening, but the effect was not statistically significant (p=0.092). Conclusion : Dawn simulation may help waking up in the morning and may improve daytime functioning by deceasing both morning drowsiness and daytime sleepiness in adolescents with DSPS.

      • 수면박탈이 주간 시간대별 졸리움에 미치는 영향

        STUDY OBJECTIVES : The purpose of this study was to evaluate the effect of total sleep deprivation on daytime sleepiness measured by MSLT and VAS/SSS. PARTICIPANTS : 10 healthy, normal young male college students participated in this study. Participation subjects were in good physical and psychological health and were asymtomatic as to sleep/wake complaints. INTERVENTION : Subjects visited our laboratory by 08:30 at the first day, and they had baseline MSLT (from 09:00 to 17:00 at 2 hours' interval) while they had 5 naps during the day. Each time, the subjects completed VAS and SSS before they underwent an MSLT to evaluate their sleepiness. They spent the night without sleep at the laboratory. Next day, they repeated the same process as those of the first day. RESULT : As compared to baseline, subjects 1) had significantly shorter sleep latency, 2) had significantly more stage 2 sleep but less stage 1 sleep without significance, and 3) had significantly more subjective sleepiness after total sleep deprivation. CONCLUSION : Additional study is needed to correalte between subjective and objective measures of daytime sleepiness.

        The Characteristics of Sleep in Headache Patients

        Purpose: To investigate the relationship between headache and sleep by evaluating sleep quality, daytime sleepiness, and specific features related to sleep-disordered breathing (SDB). Methods: One hundred one subjects with headache and 118 healthy controls were enrolled. To collect various information on headache attacks, headache group completed self-reported questionnaire about the characteristics of headache attacks and the migraine disability assessment (MIDAS) questionnaire. The subjective quality of sleep was evaluated in all of the subjects using the Pittsburgh sleep quality index (PSQI) and Epworth sleepiness scale (ESS). In addition, the following specific features of sleep were evaluated in 28 subjects selected randomly from each group: apnea-hypopnea index (AHI), prevalence of SDB, nocturnal oxygen saturation (SaO2), and oxygen desaturation index (ODI) as measured using a portable monitoring device. Results: The global PSQI and the prevalence of poor sleeping (global PSQI >5), ESS scores and the prevalence of daytime sleepiness (ESS score >10) were significantly higher in the headache group (both p<0.0001, respectively). The mean scores on the numerical rating scale and the MIDAS were significantly higher in the poor-sleeper group than in the good-sleeper group (p=0.0347 and p=0.0016, respectively). The global PQSI and prevalence of daytime sleepiness were significantly higher in the chronic-headache group than in the acute-headache group (p=0.0003 and p=0.0312, respectively). Conclusions: There is a significant association between headache and sleep. Especially, severity and chronicity of headache were significantly associated with sleep quality and daytime sleepiness.

        사무직 근로자의 주간졸음, 직무스트레스가 삶의 질에 미치는 영향

        본 연구는 일부 사무직 근로자들의 삶의 질에 영향을 미치는 요인으로 주간졸음과 직무스트레스와의 관련성을 평가하고자 시도하였다. 대전광역시에 거주하며 사무직에 근무하는 근로자 221명을 대상으로 연구하였으며, 2015년 2월 2일부터 2월 27일까지 구조화된 무기명 자기기입식 설문조사를 실시하여 다음과 같은 결론을 얻었다. 주간졸음과 직무스트레스는 양의 상관관계를 보였고, 삶의 질과 주간졸음, 삶의 질과 직무스트레스는 음의 상관관계를 보였다. 전체 연구대상자의 삶의 질에 영향을 미치는 요인으로 연령, 주관적 사회경제상태, 주관적 건강수준, 교육수준, 주간졸음이 35.5 의 설명력을 보였으며, 직무스트레스의 추가로 50.8%의 설명력을 보여 직무스트레스가 삶의 질에 큰 영향을 미치고 있음을 알 수 있었다. 따라서 근로자의 삶의 질을 개선시키기 위해 직장 내 직무스트레스를 감소시키기 위한 노력이 필요할 것으로 사료된다. The purpose of this study was to assess the association between daytime sleepiness and job stress, two factors affecting the quality of life for office workers. Respectively, for daytime sleepiness. Positive correlation was found between daytime sleepiness and job stress and negative correlation was found between the quality of life and daytime sleepiness and between the quality of life and job stress. As for the factors affecting the quality of life for the respondents in general, age, subjective socio-economic status, subjective health status, education level, and daytime sleepiness accounted for 35.5% and the addition of job stress to these factors accounted for 50.8%. This result has confirmed that job stress may strongly affect the quality of life. It is therefore necessary to make efforts to reduce job stress at workplace with the objective of improving the quality of life for workers.

        This case study is to report the effectiveness of Hyungbangdojeock-san in the treatment of daytime sleepiness. The patient was diagnosed as a Soyangin, one of the 4 constitution types in Korean medicine, and treated with herbal medicine, Hyungbangdojeock-san and acupuncture treatment for 14 days. The severity of symptom was assessed using Epworth Sleepiness Scale (ESS). After treatment, the severity of daytime sleepiness was reduced. ESS score was decreased from 11 to 2. As a result this case showed the effect of Hyungbangdojeock-san on daytime sleepiness.
