Gastrointestinal (GI) symptoms in children and adolescents are influenced by diverse psychiatric components such as psychosocial stresses, familial environment, school-related situations, and comorbid psychiatric conditions. Absolutely psychiatric symptoms of pediatric patients are also affected by problems of GI system. Lots of symptoms including anorexia, dyspepsia, nausea and so on are commonly originated from either GI or psychiatric causes or both. Sometimes the negative interactions between GI and psychiatric problems aggravate the severity and eventually decline the functions of children and adolescents with GI symptoms. We summarized the common GI and psychiatric conditions which have GI and psychiatric associations. To a clinician who manages pediatric GI disorders, psychiatric considerations can be beneficial to understand the clinical manifestations of patients and to find the way to relieve them. This short and somewhat superficial review may help to have a bird's-eye view on this topic.

Objectives : I reviewed the fundamental knowledge about normal development and common psychiatric illnesses m adolescence to provide with a lecture note for resident training seminar. Methods : Three textbooks were reviewed and summarized. Results : Adolescence is a bridge period between childhood and adulthood. Through adolescence, the development and achievement of interpersonal relationship, character pattern, learning and occupational capabilities occur. In adolescence, lots of both physical and psychological changes are observed. Also the requirement of social role and duty increased. Because of them, many psychiatric illnesses and adaptive problems are developed and exert negative influences on the quality of life of adolescents and their family. Conclusion : Adolescence is important period in human development and many psychiatric disabilities are developed and aggravated in this period.
중추신경자극제를 사용할 수 없는 주의력결핍과잉행동장애 아동에서의 Modafinil 효과 : 예비 연구

Objectives : This study was aimed to examine the efficacy and safety of modafinil in psychostimulant-inapplicable children with attention deficit hyperactivity disorder (ADHD) due to insufficient treatment response or intolerable adverse effects. Methods : Fifteen children (8.8±1.8 years, 14 boys, total IQ 107.22±13.4) were recruited for this study. ADHD was confirmed by K-SADS-PL. The efficacy was assessed by ADHD rating scale (ADHDRS), Conner's parental rating scale-revised(CPRS-R), ADHDRS school version, clinical global impression (CGI) and computerized neurocognitive function test. Mean dose of modafnil was 250. 0±103.3 mg. Results : Total score (p=0.012) and hyperactive/impulsive score (p=0.021) of ADHDRS for investigator, as well as CPRS-R score (p<0.01) and CGI-severity score (p<0.01) were significantly reduced after 39.2±8.7 days of modafmil administration. Response time of word color test was also significantly decreased after medication. Decreased appetite (2 subjects), insomnia (2 subjects), irritability (1 subject) and alopecia (1 subject) were observed during this study period. Conclusion : Modafinil was effective and well-tolerated for the psychostimulant-inapplicable children with ADHD.

This study aimed to identify the association between gamma-aminobutyric acid-A (GABA-A) receptor subunit β3 (GABRB3) gene and autism spectrum disorders (ASD) in Korea. Fifty-eight children with ASD [47 boys (81.0%), 5.5 ± 4.1 years old], 46 family trios, and 86 healthy control subjects [71 males (82.6%), 33.6 ± 9.3 years old] were recruited. Transmission disequilibrium test revealed that, 183 bp long allele in GABRB3 gene was preferentially transmitted in families with ASD (p = 0.025), whereas a population-based case-control study, however, showed no association between ASD and GABRB3 microsatellite polymorphism. Our data provide preliminary evidence that GABRB3 gene is associated with ASD in Korea.
