        단기교육으로 시행가능한 심실기능평가법

        Background: The initial history, physical examination, and ECG assessment should focus on identification of potentially serious noncardiac or cardiac disorders, including coronary artery disease, congestive heart failure, and electrical instability at the emergency room. additionally, it is essential to define disease severity, stability and need for emergency therapy. echocardiography is a useful tool for this purpose. especially Doppler echocardiography may be more sensitive and time-saving diagnostic tool for the evaluation of patients presenting with cardiogenic symptoms. So we evaluate the feasibility of the echocardiographic measurement by emergency physicain after short-term course. Method and Results: Twenty volunteers(10 male, 38.8±9.3 years) were included in the study for measurement of myocardial performance index and established parameters of ventricular function using conventional echo-Doppler methods. Myocardial performance index: (ICT+IRT)/ET, was obtained by subtracting ejection time(ET) from the interval between cessation and onset of the mitral inflow velocity to give the sum of isovolumic contraction time(ICT) and isovolumic relaxation time(IRT). The most of mean values of echocardiogrphic parameters were not significantly different between those of cardiologist and those of emergency physicians(p<0.01). The duration for measuring myocardial performance index was shortest among echocardiographic parameters. the validity of echocardiographic parameters measured by emergency physicians was proved relatively good. Conclusion: It is proved to be feasible for emergency physician to perform echocardiographic evaluation of ventricular function after short-term course

        Associations of serotonergic genes with poststroke emotional incontinence

        <P><B>Objectives</B></P><P>Poststroke emotional incontinence (PSEI) has been associated with serotonergic dysfunction. Polymorphisms of serotonin transporter (5‐HTT) and serotonin 2a receptor (5‐HTR2a) genes may regulate serotonergic signaling at brain synapses, and this study was to investigate associations with PSEI in an East Asian population.</P><P><B>Methods</B></P><P>In 276 stroke cases, PSEI was diagnosed by Kim's criteria. Covariates included age, gender, education, history of depression or stroke, current depression, and stroke severity and location. Genotypes were ascertained for 5‐HTT gene‐linked promoter region (5‐HTTLPR), serotonin transporter intron 2 variable number tandem repeat, 5‐HTR2a 1438A/G, and 5‐HTR2a 102 T/C. Associations with PSEI were estimated by using logistic regression models, and gene–gene interactions were investigated by using the generalized multifactor dimensionality reduction method.</P><P><B>Results</B></P><P>PSEI was present in 37 (13.4%) patients. The 5‐HTT gene‐linked promoter region <I>s</I>/<I>s</I> genotype was independently associated with PSEI. No associations with STin2 VNTR and 5‐HTR2a genes were found, and no significant gene–gene interactions were identified.</P><P><B>Conclusions</B></P><P>Stroke patients with 5‐HTTLPR <I>s</I> allele had higher susceptibility to PSEI, which underlines the potential role of serotonergic pathways in its etiology.         Carcinosarcoma of the stomach is regarded as a rare malignant neoplasm composed of both carcinomatous and sarcomatous components in a given tumor. Few cases have been reported since 1904. This is a case of carcinosarcoma of the stomach in a 61-year-old man. He suffered from indigestion, vomiting, and epigastric pain. Endoscopic finding showed a huge protruding mass with intact mucosa on the posterior wall of the antrum up to the pylorus. Surgery was performed and carcinosarcoma with pancreatic invasion was confirmed by pathology.

        박탈과 증오의 바람 : 이만희 감독의 <휴일>(1968)을 중심으로

        본고는 이만희 감독의 영화 <휴일>을 당대 사회의 반영이나 재현이 아니라, 정동(affect)의 차원에서 분석함으로써 영화가 이데올로기나 선전도구가 아니라 자율적인 영역을 구축했다는 점을 주장한다. 달리 말해 <휴일>은 서사 중심의 영화가 아니라, 인물들의 관계에서 발생하는 힘에 주목하며, 어디로 나아갈지 모르는 카메라의 힘이 운동하는 방향에 주목하고 있는 독특한 구조로 흘러가는 영화임을 분석하고 있다. 먼저 영화는 ‘휴일’이라는 시간을 출발점으로 삼고 있지만 쉼이나 휴식이 아니라 자본주의 체제의 지속을 위한 재생산(reproduction)의 시간임을 인물들 간의 관계를 통해 보여준다. 주인공 ‘허욱’과 ‘지연’은 가난한 연인으로 커피 한 잔 사먹을 돈이 없어 공원에서 데이트를 하고, 아이를 키울 형편이 안 돼 낙태 비용을 마련하기 위해 모욕적인 상황을 마다하지 않는다. 허욱이 낙태 비용을 마련하기 위해 친구들을 만날수록 걷잡을 수 없는 한계 상황으로 내몰리는 것을 확인할 수 있다. 허욱의 친구들 또한 심심해서 처음 만난 여자와 섹스를 하거나, 술에 취해 있거나, 목욕을 여섯 번이나 하는 등 무기력하게 시간을 보내는 인물들로 그려진다. 그런 점에서 이 영화는 ‘휴일’이라는 시간이 진정한 휴식이 아니라 재생산을 위한 시간이며, 생산활동을 하지 않는 인물들은 극한의 상황에 내몰리고 있음을 확인할 수 있다. 허욱은 휴일이 끝나갈수록 더 무기력해지고, 결국 혐오의 시선으로 자신을 바라보던 친구 ‘규제’로 인해 자기 파괴적인 증오의 감정을 표출하기에 이른다. 즉 영화는 허욱을 통해 휴일이라는 시간이 생산활동과 재생산의 세계를 파악하게 만든다. 마지막으로 본고는 사회적 불안정성이 증대되고 변동이 극심화 되어가는 1960년대 후반 기존에 자신이 누리던 지위나 위치를 상실할 경우 사회적으로 취약한 계급이나 계층은 무기력이나 증오의 감정에 더욱 취약할 수밖에 없음을 영화 속 관계들을 통해 분석하고 있다. In this paper, the movie, <A Day Off> directed by Lee Man-hee, was analyzed not as the reflection or reproduction of the society of those times, but at the level of stillness and movements, and it was argued that the film had established an autonomous area rather than had been used as the means of ideology or propaganda. In other words, it was analyzed that <A Day Off> was not a narrative- centered movie, but one focusing on the power generated in the relationships among the characters, and had a unique structure of paying attention to the direction of the camera s dynamic without knowing where it would head. First, the film begins with the time of ‘a day off’, yet it s revealed through the relationships among the characters that it is not rest or break, but the time of regeneration for the continuance of the capitalistic system. The main characters in the movie, ‘Heo Wuk’ and ‘Jiyeon’ are poor lovers, who have a date in the park because they don t have money for a cup of coffee and don t say no to insulting situations when they have to raise money for abortion because they can t afford to raise a child. We can see that the more Heo Wuk meets his friends to prepare to pay for abortion, the more he becomes forced into an uncontrollable critical situation. Heo Wuk s friends are also described as the figures spending time spiritlessly, having intercourse with a woman whom one meets for the first time because he is bored, being drunken, or taking a bath six times a day. In that sense, it can be proven that in this movie, ‘a day off’, doesn t mean true rest, but is just time for reproduction, and those not engaged in production activities are being driven into extreme situations. As the day off is drawing to its close, he becomes lethargic more and more, and ends up expressing the emotion of self-destructive hatred because of his friend, ‘Gyu-je’, who sees him with the eyes of disgust. That is to say, the film makes one figure out the time of a day off showing the world of production activities and reproduction through Heo Wuk. Lastly, in this paper, it was analyzed through the relationships in the movie that in the latter half of 1960s when social instability was increasing and changes were becoming severe, when one lost the position or status that one used to enjoy, the one belonging to the weaker class or rank in the society couldn t help but become more vulnerable to the emotions of lethargy or hatred and disgust.

        2011 대구세계육상선수권대회 남자 높이뛰기 메달리스트들의 바이오메카닉스적 특성 분석

        The purpose of this study was to perform a kinematic analysis of the high jump techniques of the three men`s medalists at the 2011 IAAF Championships in Daegu (August 27-September 4, 2011), In particular, a three-dimensional coordinates method was used to analyze the last three strides before touchdown, the touchdown techniques, and the movements after takeoff toward the bar. An analysis of the, data for the biomechanical characteristics of the world`s best high jumpers could contribute to an improvement in the performance of a national high jumper. The first conclusion of the data analysis was that the arm movements of the gold medalist, J. Williams, had a single arm form, whereas the arm movements of the other medalists were a double arm form. Second, the difference in the knee joint angles upon touchdown and toe-off was 10˚. Third, J. Williams achieved his maximum CM height after takeoff (1.26m) using the maximum flexion of his knee joint. Fourth, the foot contact duration of A. Dmitrik (0.11 s) was the shortest among the medalists, and the ratio for his transformation of horizontal velocity to vertical velocity was the greatest (75.25%) among the three. Last, the maximum CM height of T. Barry was the greatest, and his foot contact duration was the longest.

      • 발전설비계획과 송전설비계획을 연계한 통합설비계획에 관한 연구

        The competitive electricity market requires a new paradigm in the capacity planning. Market participants operate their facilities and plan new capacities for the benefit maximizing. However, the public system planner should provide the national guideline, because the power system is costly & large item. This paper presents a methodology on a generation expansion planning incorporating the transmission expansion planning as a public policy. The proposed model is formulated as a mixed integer programming that minimized operation and construction costs. This model is demonstrate by a case study.

      • 수치모형을 이용한 사행수로의 하상특성 분석

        지구촌 곳곳에서 최근의 기상이변으로 인한 수해를 방지하기 위한 많은 대책들이 나오고 있다. 그 중 국지적인 호우로 인하여 국내의 많은 하천들의 사행수로 구간에서 편수위 및 홍수위 상승에 따른 피해가 심각하여 이에 대한 연구가 증가하고 있다. 본 연구에서는 사행수로의 하상특성을 분석하기 위하여 기본적인 수치모형 방정식의 이해와 이를 바탕으로 한 최근 국내의 사례를 예로 들었다. 수치 모형으로는 현장에서 범용되고 있는 것을 채택하되, 1차원 모형으로는 HEC-RAS를 2차원 모형으로는 RMA-2모형을 이용하였다. 분석 결과 유속의 경우는 2가지 모형 공히 사행구간의 변화를 알 수 있었으나, 수위는 1차원 모형 자체의 한계성이 나타났다. 사행 구간에서는 홍수시 만곡부 외측으로의 편수위 상승과 같은 외수의 월류로 인한 위험을 가중시키는 현상이 발생하게 되므로 제반계획의 수립시 이러한 사항을 충분히 고려하여야 한다. In every globe village, numerous methods appear for detection flood damage caused by unusual change of wether lately. Due to local storm, in nationwide meandering rivers disaster of increased superelevation and flood level is seriously. In this study, for analysis bed state in meandering channel the basic numerical model equation be understand and introduced an example domestic case. In numerical models, selecting used in field adopted HEC-RAS for one-dimensional model and RMA-2 for two-dimensional model. Results of analysis, case of flow velocity recognized variation of meandering channel in two computational models, but in case of water level we find out one-dimensional model's limitations. Occasion of flood, in outside section of meandering bend, because of added overflow existing state like increasing of superelevation is occurrence, in making various plan enough consideration is needed.
