      • 녹동균 세포외막 단백질 백신 CFC-1-101의 안정성 및 면역원성 검토 : 임상 제 Ⅰ/Ⅱa상 시험

        목적 : 제일제당에서는 녹농균의 세포외막 단밸질을 유효성분으로 하는 백신인 CFC-101을 개발하였으며, 동물시험에서 이 백신의 안전성과 유효성을 입증하였다. 본 연구에서는 이 녹농균 백신의 인체에 대한 안전성과 면역원성을 평가하는 동시에 인체 접종시의 최적 투여 용량을 결정하기 위하여 제 I/Ⅱa상 임상시험을 수행하였다. 방법 : 건강한 성인 남자를 피험자로 선별하여 각 용량군에 백신투여자 6명, 위약투여자 2명을 배정하였다. 백신 투여군은 0.25mg, 0.5mg 또는 1.0mg 용량의 녹농균 백신을 7일 간격으로 3회에 걸쳐 근육주사 하였으며, 위약 투여군에게는 세포외막 단백질을 제외한 동일한 성분을 투여하였다. 백신접종 후 국소적 또는 전신적인 반응의 발생여부를 관찰하고, 혈액시료를 체취하여 백신의 역가와 유효성을 검정하였다. 결과 : 녹농균 백신 CFC-101은 모든 접종자에서 양호한 내약성을 보였다. 또한 0.5mg 과 1.0mg 백신 투여군에서는 100%의 항체양전율을 나타내었다. 생성된 항체는 녹농균 세포외막단백질에 특이성을 보였고, 녹농균 감염에 대해 방어효능이 있었다. 결론 : 이와같은 결과로부터 이 녹농균 백신은 인체에 안전하게 투여할 수 있으며, 높은 항체 생성능으로 감염방어 효능을 보이고 0.5mg과 1.0mg이 최적용량인 것으로 판단되었다. Background : We developed a Pseudomonas aeruginosa outer membrane protein(OMP) vaccine CFC-101, and the prophylactic efficacy of which has been demonstrated in animal models. In order to evaluate the safety and immunogenicity of the P. aeruginosa vaccine, we carried out a phase I/Ⅱa clinical trial in healthy male volunteers. Methods : Groups of eight volunteers, including two placebo subjects, were vaccinated intramuscularly with three doses of 0.25, 0.5 or 1.0 mg of the vaccine at one week intervals. Sings of systemic and local reactions observed after vaccination were recorded for each vaccinee for 5 days. Physical examinations were performed on days 0, 1, 7, 8, 14, 15, 21, and 42, and clinical laboratory tests were done on days 0, 3, and 21. Blood samples for assay of serum antibody levels were obtained up to 42 days after the first vaccination. Results : The vaccine was generally well tolerated by all vaccinees, showing no significant side effects. In the three dosage groups, all vaccinees, except one receiving the 0.25 mg dose, showed significant elevation in serum IgG antibody titers against the vaccine proteins, indicating 100% seroconversion in 0.5 and 1.0 mg groups. The human antibodies induced by the vaccine were specific for P. aeruginosa OMPs, as confirmed by western blot analysis and immunoprecipitation assays. The capacity of the human antisera to enhance opsonophagocytic killing activity by polymorphonuclear leukocytes and to confer protection against P. aeruginosa infections indicates that the antibodies elicited by the vaccine have protective efficacy. Conclusion : We conclude that the P. aeruginosa OMP vaccine is safe and effective for human use and its optimal dose to be 0.5 or 1.0 mg.

        한국 정신장애의 역학 조사 연구[I] : 각 정신장애의 유병률

        Objectives : This study aims to estimate the prevalence of the DSM-IV psychiatric disorders in Korean population using the Korean version of Composite International Diagnostic Interview (K-CIDI). Methods : Subjects were selected by taking multi-stage, cluster samples of 7,867 adult household residents, 18 to 64 years of age, in ten catchment areas. Total 78 trained interviewers administered the K-CIDI to the selected respondents, from June 1 to November30,2001. Results : Total 6,275 respondents completed the interview. Some 33.5% of respondents reported at least one lifetime disorder, 20.6% reported at least one-year disorder, and 16.7% reported at least one-month disorder. The most common lifetime disorders were alcohol abuse/dependence (17.24%), nicotine dependence/withdrawal (11.19%), specific phobia (5.16%), and major de-pressive disorder (4.25%). The lifetime prevalence of substance abuse/dependence (0.25%) and schizophrenia (0.16%) was very low. Nicotine and alcohol use disorder showed very high male/female ratio. Mood disorder and anxiety disorder were more prevalent among female than male. Conclusion : The prevalence of psychiatric disorders was high. In comparison with other studies, remarkable differences in distributions of psychiatric disorders across the areas and times were observed.

        한국어 판 구조화 임상면담도구 개발 : 신뢰도 연구

        연구목적: 정신장애의 정확한 진단과 평가는 임상에서뿐만 아니라 신뢰성 있는 연구를 위해서도 매우 중요하다. Structured Clinical Interview for DSM-IV(SCID)는 임상전문가들이 사용하는 진단도구로서, 비교적 짧은 시간에 정확한 진단을 내릴 수 있다. 본 연구는 한국어판SCID를 개발하고 그 신뢰도를 평가하기 위해 수행되었다. 방법: 국문학자가 포함된 번역위원회를 통하여 연구용판 SCID를 번역한 후, 정신과 의사 2인과 임상심리학자 1인에게 한국어판SCID 실시방법을 교육시킨 뒤 한국어판SCID를 이용한 면담의 평가자간 신뢰도 (interrater reliability)를 검증하였다. 면담 대상은 1999년 2월에서 3월까지 2개 병원 정신과에 치료중인 환자 90명(남:41명, 여:29명)이었다. 결과: 현재 장애(current disorder) 평가에서 주요 우울장애, 기분부전장애, 정신분열병, 알코올 남용 및 의존, 기타 물질 남용 또는 의존, 여러 불안장애들 및 섭식장애 등과 같은 대부분 장애의 kappa값은 .70이상으로 매우 높았다. 그 이외의 양극성 장애, 망상장애, 광장공포증, 감별 불능 신체화 장애 및 건강염려증의 kappa값도 .69에서 .40사이로 수용할 수 있는 정도였다. 평생 장애(lifetime disorder)에서는 양극성 장애(k=.69)와 감별 불능 신체화장애(k=.59)를 제외한 다른 모든 장애의 kappa값이 .70이상이었다. K-SCID 면담시 Ⅰ축 질환에 대하여 흔하게 다중 진단이 내려졌으며, 평균 진단 수는 1.5∼1.7개로 나타났다. 결론: 한국어판 SCID는 신뢰도가 높은 진단도구로 생각되며, 향후 정신질환의 정확한 진단과 임상연구에 유용하게 사용될 수 있다. Objectives: Accurate diagnosis and assessment for psychiatric disorders is crucial for research, as well as for clinical practice. Structured Clinical Interview for DSM-Ⅳ(SCID-RV) is a less time-consumimg and more accurate structured diagnostic interview form. It can be used by clinical professions and is known for a reliable diagnostic tool. Present study was conducted to develop Korean version of SCID-RV and to test the inter-rater reliability. Methods: The authors have translated original SCID-RV into Korean, and revised in parallel with sociocultural background of Korea. Ninety patients from two psychiatric hospitals, both outpatient and inpatient, were interviewed and rated independently by three raters. Results: The kappa coefficients for most of illnesses, such as major depressive disorder, dysthymia, schizophrenia, alcohol abuse and dependency, anxiety disorder and eating disorder were excellent(>0.70) in the evaluation of current disorders. And the kappa coefficients for bipolar disorder, delusional disorder, agoraphobia, undifferentiated somatoform disorder, and hypochondriasis were acceptable(>0.40) in the evaluation of current disorders. In the evalua-tion of lifetime disorders, the concordant rates of all the diagnoses except bipolar disorder and undifferentiated somatoform disorder were excellent. Lack of hierarchy in DSM-Ⅳ allows for multiple Axis I diagnoses. Mean numbers of Axis I diagnoses per subject assigned by the three raters were 1.5-1.7. Conclusions: Our findings confirm that SCID-RV yields highly reliable diagnoses. SCID-RV is recommended for accurate diagnosis in clinical practice and research on psychiatric disorders.

        MODAL TESTING AND MODEL UPDATING OF A REAL SCALE NUCLEAR FUEL ROD

        In this paper, modal testing and finite element modeling results to identify the modal parameters of a nuclear fuel rod as well as its cladding tube are discussed. A vertically standing full-size cladding tube and a fuel rod with lead pellets were used in the modal testing. As excessive flow-induced vibration causes a failure in fuel rods, such as fretting wear, the vibration level of fuel rods should be low enough to prevent failure of these components. Because vibration amplitude can be estimated based on the modal parameters, the dynamic characteristics must be determined during the design process. Therefore, finite element models are developed based on the test results. The effect of a lumped mass attached to a cladding tube model was identified during the finite element model optimization process. Unlike a cladding tube model, the density of a fuel rod with pellets cannot be determined in a straightforward manner because pellets do not move in the same phase with the cladding tube motion. The density of a fuel rod with lead pellets was determined by comparing natural frequency ratio between the cladding tube and the rod. Thus, an improved fuel rod finite element model was developed based on the updated cladding tube model and an estimated fuel rod density considering the lead pellets. It is shown that the entire pellet mass does not contribute to the fuel rod dynamics; rather, they are only partially responsible for the fuel rod dynamic behavior.

        Ashbya gossypii로부터 riboflavin 대량생산을 위한 배지 최적화와 유가식 배양

        본 연구에서는 Ashbya gossypii를 이용한 비타민 B2(riboflavin) 생산에 있어서 발효배지의 최적화를 위해 탄소원(포도당과 대두유), 유기 질소원(CSL외 8종), 전구체(glycine 및 glutamate)의 영향을 검토하였다. 또한 생성균주의 고농도 배양을 통한 riboflavin 생산성 향상을 위해 유가식 배양을 수행하였다. 최적 발효배지의 탄소원은 기질 저해를 피할 수 있는 3%의 포도당과 0.5%의 대두유를 사용함으로써 riboflavin 생산수율을 증가시킬 수 있었다. 유기 질소원으로는 corn steep liquor(CSL)가 가장 좋았으며, 최적 CSL의 농도는 1%였다. Riboflavin 생합성의 전구체인 glycine과 glutamate는 배양 초기에 0.5%로 첨가하는 것이 riboflavin 생산에 가장 좋았다. 세포의 riboflavin 생합성 활성을 장기간 유지시키고 균체의 균체의 고농도 배양을 위해 포도당과 yeast extract로 구성된 복합배지를 연속적으로 공급한 유가식 발효 결과, 플라스크 배양(3.5g/ℓ)보다 약 100%, 발효조를 이용한 회분식발효(5.4 g/ℓ)보다 약 20% 향상된 6.8 g/ℓ의 최종 riboflavine 생산농도를 얻었다. 이상의 결과로부터 탄소원, 전구체 및 필수 영양소(특히 비타민) 공급원인 CSL의 최적화된 공급 전략을 통한 유가식 발효와 고농도 균체 배양 기술의 확립으로 riboflavin 생산수율을 더욱 향상시킬 수 있을 것으로 생각된다. In order to maximize the riboflavin production by a mutant strain Ashbya gosspyii, the optimization of medium and fed-batch fermentation were performed. As carbon sources, glucose and soybean oil were necessary for the riboflavin overproduction. Optimal concentrations of glucose and soybean oil in the flask cultures were found to be 3.0% and 0.5%, respectively, in a complex medium containing corn steep liquor (CSL) 1%. Among the various organic nitrogen sources tested, CSL as the most effective one both for the cell growth and riboflavin overproduction. More than 1% concentration of CSL, however, was inhibitory to the cell growth and riboflavin production. Glycine and glutamate at each 0.5% concentration stimulated the riboflavin production. The riboflavin concentration was reached to 5.4 g/ℓ in the batch fermentation with the optimized medium. The riboflavin production could be enhanced upto 6.8 g/ℓ through the fed-batch fermentation employing continuous feeding of glucose and yeast extract.

        Highly-closed/-Open Porous Ceramics with Micro-Beads by Direct Foaming

        This study reports on wet-foam stability with respect to porous ceramics from a particle-stabilized colloidal suspension that is achieved through the addition of polymethyl methacrylate (PMMA) using a wet process. To stabilize the wet foam, an initial colloidal suspension of $Al_2O_3$ was partially hydrophobized by the surfactant propyl gallate (2 wt.%) and $SiO_2$ was added as a stabilizer. The influence of the PMMA content on the bubble size, pore size, and pore distribution in terms of the contact angle, surface tension, adsorption free energy, and Laplace pressure are described in this paper. The results show a wet-foam stability of more than 83%, which corresponds to a particle free energy of $2.7{\times}10^{-12}J$ and a pressure difference of 61.1 mPa for colloidal particles with 20 wt.% of PMMA beads. It was possible to control the uniform distribution of the open/closed pores by increasing the PMMA content and by adding thick struts, leading to the achievement of a higher-stability wet foam for use in porous ceramics.
