Nanoxel-PM(TM), docetaxel-loaded methoxy-poly(ethylene glycol)-block-poly(d,l-lactide) (mPEG-PDLLA) micellar formulation was prepared in an effort to develop alternative, less toxic and efficacious Tween 80-free docetaxel formulation, and its pharmacokinetics, efficacy, and toxicity were evaluated in comparison with Taxotere(R) in preclinical studies. The mean diameter of the Nanoxel-PM(TM) was 10-50nm and the polydispersity of samples exhibited a narrow size distribution and monodisperse unimodal pattern. Pharmacokinetic study in mice, rats and beagle dogs revealed that Nanoxel-PM(TM) exhibited similar pharmacokinetic profiles (C<SUB>max</SUB>, AUC, t<SUB>1/2</SUB>, CL, V<SUB>ss</SUB>) to Taxotere, and the relative mean AUC<SUB>t</SUB> and C<SUB>max</SUB> of Nanoxel-PM(TM) to Taxotere(R) were within 80-120%. Furthermore, excretion study in rats demonstrated that there was no statistically significant difference in the amount excreted in feces or urine as an unmetabolized docetaxel between Nanoxel-PM(TM) and Taxotere(R). Its pharmacokinetic bioequivalence resulted in comparable anti-tumor efficacy to Taxotere(R) in human lung cancer xenografts H-460 in nude mice as well as in lung, ovary and breast cancer cell lines. Several animal toxicity studies on Nanoxel-PM(TM) compared with Taxotere(R) were carried out. In single dose rat and dog model and repeated dose mouse model, both Nanoxel-PM(TM) and Taxotere(R) exhibited similar toxic effects on hematology and body weight gain. On the other hand, vehicle related hypersensitivity reactions and fluid retentions were not observed when Nanoxel-PM(TM) was administered, unlike Taxotere(R), in the beagle dog study. Based on these results, it is expected that Nanoxel-PM(TM) can reduce side effects of hypersensitivity reactions and fluid retention while retaining antitumor efficacy in cancer patients. Currently, Nanoxel-PM(TM) is under evaluation for bioequivalence with Taxotere(R) in a multi-center, open-label, randomized, crossover study.

Airborne spread of nosocomial bacterial, fungal or viral infections is known to occur, but its exact incidence is unclear. Although air sampling is not recommended as routine procedure, special situation such as contamination of hospital building by flood can make air sampling useful surveillance method. In September 1990, flood made our hospital immersed into water. The authors performed air sampling before and after disinfection of the hospital to observe the degree and microbiologic pattern of air contamination. Disinfection procedure was performed by using the solution of chlorine and phenol mixture. Before disinfection, air culture revealed 1201 CFU/㎥ in total microbial count which changed to 862 CFU/㎥ after disinfection. Only the degree of fungal contamination showed significant decrease after disinfection. Microbiologic identification showed that Gram positive cocci is the major contaminant. But the basement area which was completely immersed into water contained mostly Gram positive rod before disinfection. Air sampling may not be a useful method in evaluating the efficacy of disinfection.

Cefuzonam(Cefnam) is a new parenteral third generation cephalosporin with strong bactericidal activity against gram-negative and gram-positive organisms and is stable against various B-lactamases. To evaluate the clinical efficacy of cefuzonam nosocomial infection, we performed a porspective open trial of cefuzonam in 20 patients with various bacterial infection;9 with pneumonia,5 with urinary tract infection, 4 with bacteremia, 1 with cholangitis and I with acute osteomyelitis. The etiologic organisms isolated in enrolied patients were S. aureus(4), MRSA(7), S.epidermidis(1), E.coli(2), K.pneumoniae(2), S.marcescens(3) and E.cloacae(1). Most patients received 1.0g of cefuzonam every 12 hours for at least 5 days. Nineteen patients were cared or improved after cefuzonam treatment and clinical efficacy rate was 95%. microbiologic eradication was achieved in 17 patients(85%). Overall response rate to cefuzonam was 95%. We also performed in vitro susceptibility test of cefuzonam and other antibiotics against 04 strains of gram-positive and gram-negative organisms. Cefuzonam was effective against MSSA, S.epidermidis and all tested gram negative organisms, but not effective against MRSA according to the in vitro data. Cefuzonam is a safe and effective agent for the treatment of nosocomial infection and has a unique activity against gram-positive organism besides gram-negative and anaerobic organisms.

Environmental monitoring by measuring the air concentration of solvent mixtures should be the most useful method in the exposure assessment. But the solvent mixtures in air can be difficult to measure. In order to improve the method for air measurements of solvents, the author developed a simultaneous determination method for mixtures of 23 solvents using gaschromatography. And also for the purpose of assessing occupational exposure to solvent mixtures, the author applied this method to measure the air concentrations of solvent mixtures in industrial setting. The best condition of this method was 35℃ -150℃ for column temperature, 250℃ for detector and injector temperature with capillary OV-1 column at 0.2kg/㎠ if inlet pressure. And The recovery rates were 90% and over for 16 organic solvents including toluene and 70% and less for 4 organic solvents including ethanol. In raw materials(adhesives, diluents) of 3 industrial settings(paint manufacturing industry, chemical products industry & fishing products industry), the major components was aromatic hydrocarbons (toluene, o-, m-, p-xylene), and the number of detected items among 23 solvents were 4-16. The Cn/Nn-value(2.02) at one unit of adhesives developing department in chemical products industry exceeded permissible exposure limit(1), and Cn/Nn-values demonstrated a remarkable range(Cn/Nn-values were 0.04-2.02 for charcoal tube and 0.02-0.68 for passive sampler).

A general method for fabricating transient plasma structures with high-intensity laser pulses is developed to gain fine control over laser-plasma interactions. These structures have been used as programmable photonic devices in the development of laser-wakefield accelerators, soft X-ray lasers and plasma nonlinear optics driven by multi-terawatt laser pulses. Plasma ramps are used to control electron injection in laser-wakefield accelerators, plasma waveguides are used to enhance the efficiency of soft X-ray lasers by orders of magnitude and periodic plasma structures are used to achieve quasi-phase matching in relativistic harmonic generation. By scanning the interaction length with the same plasma-fabrication method, tomographic measurements are carried out to resolve the injection/acceleration process in laser-wakefield accelerators and amplification processes in X-ray lasers and relativistic harmonic generation. A theoretical analysis and a computer simulation are also carried out to provide insightful pictures of these processes. These research works show that by controlling plasma structures with optical fabrication methods, laser-plasma interaction can be engineered to expand and enrich the frontier of high-field physics.

Generally fatty liver is attributed either to chronic alcoholism, diabetes mellitus, or obesity. Based upon this commonly held clinical brief, this study was conducted to investigate the contributing factors of fatty liver and odds ratio (OR) of known contributing factors. A sample of 310 male participants, who visited at Seoul Paik Automated Multiphasic Health Testing System from November 1991 to December 1991, was separated into 112 cases and 198 controls by ultrasonographic fin ding. There were statistically significant difference between fatty liver and normal in triglyceride (TG), body mass index (BMI), alanine aminotransferase (ALT), high density lipoprotein cholesterol (HDL-C), fasting blood sugar (FBS), alcohol consumption, low density lipoprotein cholesterol (LDL-C), total cholesterol, gamma-glutamyl transferase ($\gamma$-GT), duration of alcohol intake and alkaline phosphatase (Alk.P)(P<0.01, P<0.05). The statistically significant elevated odds ratio were noted for TG (4.48, confidence interval (CI) 2.66-7.55, P=0.000), alcohol consumption(3.24, CI 1.56-6.23, P=0.002), BMI(3.05, CI 1.87-4.97, P=0.000), and FBS (2.59, CI 1.53-4.40, P=0,000). In summary, it is suggested that the fatty liver could be preventive by avoiding such deleterious factors as high fat diet, alcohol and obesity.
