        Acute Myocardial Infarction during the Subacute Phase of Refractory and Incomplete Kawasaki Disease in a Five-year-old Boy

        Kawasaki disease (KD) is an acute, systemic vasculitis of childhood. The early mortality of KD results from coronary complications, mainly aneurysmal thrombosis with myocardial infarction, and the subacute phase of KD has the highest risk of mortality. Although there have been reports of ischemic heart disease as late cardiologic sequelae of KD in young adults, acute myocardial infarction caused by coronary complications in the subacute phase of KD is rare. We experienced one pediatric patient who developed coronary artery aneurysm and acute myocardiac infarction (AMI) during the subacute phase of incomplete and intravenous immunoglobulin (IVIG)-nonresponsive KD. The patient was given a good prognosis due to close monitoring and early recognition of AMI. Physicians should carefully monitor KD patients who do not respond to initial IVIG therapy and who show progressive coronary artery dilatation. If such a patient complaints of chest pain and the ECG shows hyperacute T waves, the physician should suspect development of AMI.

        Ileo-Pelvic Anastomosis and Augmentation Cystoplasty for Treatment of Encrusted Pyelitis in a Transplanted Kidney

        Infection stones are more likely to form after urinary diversion as the result of urinary stasis. To prevent urinary stasis due to encrusted pyelitis in a transplanted kidney,we describe an alternative a surgical treatment: ileo-pelvic anastomosis. In our patient with a transplanted kidney, the ileal conduit had previously been anastomosed end-to-side owing to renal tuberculosis with an atrophied bladder; the transplanted ureter was anastomosed to the ileum in the left lower abdomen with an ileal conduit on the opposite side. Routine check-up revealed hydronephrosis with infected pyelitis and ureteritis in the transplanted kidney. We performed ileo-pelvic end-to-end anastomosis to prevent urinary stasis by lengthening the ileal conduit and performed augmentation cystoplasty to support the atrophied bladder following tuberculosis. We suggest that this approach may be useful in similar cases.

        Radiosensitization by targeting radioresistance-related genes with protein kinase A inhibitor in radioresistant cancer cells

        Here we determined which radiation-responsive genes were altered in radioresistant CEM/IR and FM3A/IR variants, which showed higher resistance to irradiation than parental human leukemia CEM and mouse mammary carcinoma FM3A cells, respec-tively and studied if radioresistance observed after radiotherapy could be restored by inhibition of protein kinase A. The expressions of DNA-PKcs, Ku70/80, Rad51 and Rad54 genes that related to DNA damage repair, and Bcl-2 and NF-κB genes that related to antiapoptosis, were up-regulated, but the regulated in the radioresistant cells as compared to each parental counterpart. We also revealed that the combined treatment of radiation and the inhibitor of protein kinase A (PKA) to these radioresistant cells resulted in synergistic inhibition of DNA-PK, Rad51 and Bcl-2 expressions of the cells, and conse-quently restored radiosensitivity of the cells. Our results propose that combined treatment with radiotherapy and PKA inhibitor can be a novel therapeutic strategy to radioresistant cancers.

      • 1형당뇨인 및 그 가족의 심리사회적 적응을 위한 게슈탈트와 수용-전념치료 프로그램의 개발 및 효과

        아동기에 주로 발병하는 1형당뇨는 만성질환의 하나로, 1형당뇨 당사자뿐만 아니라 이들의 가족도 사회적 인식 편향 및 지지체계의 부족으로 심리사회적인 적응에 어려움을 겪고 있다. 따라서 1형당뇨 가족을 위한 치료 접근은 생물학적 치료의 한계에서 벗어나 심리치료적 중재가 병행되어야 한다. 본 연구는 1형당뇨 가족의 특성불안과 상태불안 수준을 감소시키고, 수용행동, 긍정적 자기개념과 심리사회적 적응 수준을 증가시키려는 목적을 가지고 게슈탈트와 수용-전념치료 프로그램을 개발하여 실시하였다. 본 연구는 게슈탈트와 수용-전념 치료를 통합한 프로그램을 1형당뇨인과 그 가족의 불안문제에 적용하여, 심리사회적 적응 향상의 효과를 측정하였다. 본 연구의 대상은 중학생 이상 성인까지의 1형당뇨 당사자와 보호자 41명으로, 1형당뇨인을 위한 온라인 커뮤니티를 통하여 모집하였다. 이들을 처치집단 22명, 통제집단 19명으로 나눈 후 처치집단에게 주말 하루 2회기씩 총 10회기 프로그램을 5주간, 총 40시간 실시하였다. 게슈탈트와 수용-전념치료가 1형당뇨인과 그 가족의 심리사회적 적응에 미치는 효과를 검증하기 위해 연구 참가자들의 특성불안, 상태불안, 수용행동, 자기개념, 1형당뇨 적응 수준을 측정하여 비교하였다. 연구 결과, 처치집단에서는 특성불안, 상태불안 수준은 통계적으로 유의미하게 감소하였으며, 수용행동, 긍정적 자기개념 및 1형당뇨 적응 수준은 통계적으로 유의미한 증가를 보였다. 이러한 효과는 추후검사까지 지속되었다. 본 연구 결과, 게슈탈트와 수용-젼념치료 통합 프로그램의 치료개입이 1형당뇨인과 그 가족의 특성 및 상태불안을 감소시키고, 수용행동, 긍정적 자기개념, 심리사회적 적응수준을 높이는 데 효과적인 치료중재임을 시사하고 있다. 마지막으로, 본 연구의 의의, 제한점 및 추후연구를 위한 고려사항 등이 논의되었다. Type 1 diabetes, which occurs normally during childhood, is a chronic disease. It causes difficulty for both patients and their families with psychosocial adaptation due to bias of social awareness and lack of support system. Therefore, the treatment approach to type 1 diabetes must integrate both biological treatment and psychotherapy. This study aims to reduce state and trait anxiety as well as experience avoidance of the families of type 1 diabetes patients, and improve positive self-identity and psychosociological adaptability, by developing and executing the Gestalt-ACT group therapy. This program is an integrated treatment of Gestalt and acceptance-focused treatment and was applied to anxiety of families of type 1 diabetes patients while measuring the effect of improved psychosocial adaptability. The subjects of this study were 41 type 1 diabetes between middle school students and adults with type 1 diabetes and their guardians collected through an online community for type 1 diabetes patients. The subjects were divided into the experimental group, 22 people, and control group, 19, to whom twice a week, a program was executed for 10 sessions for five weeks, total of 40-hours. In order to verify the effect of Gestalt-ACT group therapy on psychosocial adaptation of families of type 1 diabetes patients, the level of trait anxiety, state anxiety, acceptance behavior, self-concept, and type 1 diabetes adaptation was measured and compared. The result shows that the levels of trait and state anxiety, and experience avoidance were reduced statistically-significantly, while positive self-concept and type 1 diabetes adaptation showed significant increase. The treatment effect was verified as it changes the trait anxiety, state anxiety, experience avoidance, positive self-concept, and type 1 diabetes adaptation on a statistically significant level even in the test result obtained after 10 weeks after the end of the program. Lastly, the meaning, limitations, and considerations for future studies were discussed.

      • 자료흐름 분석기법을 이용한 프로그램 시험 모델

        지금까지 연구된 프로그램 모듈에 대한 시험 방법은 제어흐름을 중심으로 고려했기 때문에 오류의 검출을 보장하지 못할 뿐만 아니라 순환 구조의 경우에는 무한계의 경로가 발생하는 등의 문제점들을 내포하고 있다. 이런 문제점들을 해결하기 위해 자료흐름 분석기법을 이용한 Rapps/Weyuker 모델과 Laski/Korel 모델이 개발되었다. 그러나 이 모델들은 개별 모델에 적용하는 프로그램 실행 경로 선택 기준만을 제시했으며, 또한 각 경로 선택 기준을 만족시키는 완전 경로 산출은 제시해 주지 못했다. 본 모델에서는 이 두 모델의 개별적인 경로 선택 기준들을 통합하고, Petri Net을 이용하며, 순환을 zero-two subset 개념을 적용해서 처리함으로써 사용자에게 각 경로 선택 기준을 만족하는 완전 경로를 제시하고, 산출된 경로를 기본으로 기존 모델의 경로 선택 기준 포함 계층구조를 처리할 수 있는 포괄적 통합 모델을 제시한다.

        공공측량 성과심사 자동화를 위한 결정트리 알고리즘의 적용

        The current public survey performance review extracts samples according to the set screening ratio, and examines the extracted samples to determine the suitability or inadequacy of the survey performance. The examiner directly judges the survey performance submitted by the performer, and extracts it in consideration of various field conditions and topography for each subject. However, it is necessary to secure fairness in the examination as it is extracted with different extraction methods for each subject and the judgment of the examiner. Accordingly, in order to automate sampling for public survey performance review, the detailed sampling criteria of the reviewer were investigated to prepare a volume calculation table, and the automation of sampling using Python was studied. In addition, by reviewing items that can and cannot be automated, the application of the automated decision tree algorithm of sampling was reviewed.

        Experiment on the effect of Artemisia sieversiana extract on hair loss prevention and cell growth

        Objectives: This study aimed to examine the safety, effects on proliferation of hair papilla cells, and anti-inflammatory and antioxidant mechanisms of Artemisia sieversiana Ehrh. ex Willd. (AS) extract. Methods: Safety tests through purity testing, acute toxicity tests, and repeated toxicity tests were performed using AS extract (ASE) which had been dried for over two years. Cell culture and proliferation tests were conducted; VEGF (vascular endothelial growth factor), bFGF (basic fibroblast growth factor), and EGF (epidermal growth factor) and protein expression analyses were performed for mechanistic evaluation; and inhibitory effects of ASE on the RNA expression of testosterone, 5α-reductase, and aromatase was assessed. The anti-inflammatory and antioxidant efficacy of ASE was confirmed by measuring the levels of nitric oxide, inflammatory mediators (TNF-α and PGE2), inflammatory cytokines (IL-1β, IL-6, and IL-8), and chemokine MCP-1. Results: The safety of ASE was confirmed. The mechanism of cell proliferation in human hair follicle dermal papilla cells involved the promotion of VEGF, bFGF, and EGF expression. ASE decreased mRNA expression of testosterone, 5α-reductase, and aromatase-1 in a concentration-dependent manner. PGE2 and TNF-α production by inflammatory mediators was also significantly decreased in a concentration-dependent manner, and inflammatory cytokine and chemokine expression was inhibited. Conclusions: ASE is suggested to promote papillary cell growth at the cellular level, to suppress expression of various enzymes involved in hair cycle and cell death, and to inhibit hair loss through anti-androgen, anti-inflammatory, and antioxidant effects.

        전술데이터링크 환경에서 암호화 기법을 이용한 논리적 서브넷 구성기법

        TDL provides common situation awareness and simultaneous decision making power through real-time and near real-time information sharing to all warfighters. Accordingly, The ROK forces, which have to carry out various operational missions, make various efforts to acquire TDL for joint and combined operations in various weapons systems such as command post, ship, airplane, tanks, and etc. However, it takes a lot of money to acquire such ability. Accordingly, there is a need for an alternative for minimizing the cost increase and improving the TDL operation capability in the existing operation. In this paper, we propose an alternative method to expand the TDL through logical subnet configuration using ARIA encryption technology instead of adding individual network through the acquisition of new equipment. The proposed logical subnet configuration method shows that various logical subnets can be configured for each group on the TDL sharing information in the existing broadcast network. Through the system implementation, we confirmed the validity and feasibility of the technique. In the future, based on this paper, it is expected that further study on the ROK TDL subnet operation method and the performance improvement of each weapon system platform. Through these studies, we hope to contribute to the improvement of ROK military mission performance and economic military operation.
