경계요소법을 이용한 위상변이 마스크의 단차 효과 분석

3차원 위상변이 마스크의 단차 효과를 분석하기 위해 투명 경계조건, 주기적인 경계조건, 및 연속조건을 가진 경계요소법을 광 리소그래피 공정 시뮬레이션에 새로이 적용하였으며, 해석적인 해와 참고문헌의 결과와 비교함으로써 구현된 모듈의 정확성을 검증하였다. 또한, 기존의 rigorous coupled wave analysis에 의한 방법에 비해 수렴성과 계산 시간 측면에서 경계요소법을 이용하는 것이 더 효율적임을 확인하였다. 끝으로 비교적 간단한 위상변이 마스크와 다층-위상변이 마스크에 대한 최적 설계 과정을 기술하였다. The boundary element method was newly implemented into an optical lithography simulator so that it could evaluate rigorously the topological effects of 2dimensional phase-shifting masks. Both transparent and periodic boundary conditions were applied for the method, and the continuity conditions were used for treating interface nodes. The accuracy of the module developed for simulating aerial images was verified by comparison with analytic solutions and published results. In addition, it was found that our simulator would be more efficient than the conventional method based on the rigorous coupled wave analysis in views of the convergence and the calculation speed. Finally, the optimal design of two phase-shifting masks was performed.

선형적 위상배열 코일구조의 시뮬레이션을 통한 민감도지도의 공간 해상도 및 필터링 변화에 따른 MR-SENSE 영상재구성 평가

Parallel imaging technique can provide several advantages for a multitude of MRI applications. Especially, in SENSE technique, sensitivity maps were always required in order to determine the reconstruction matrix, therefore, a number of difference approaches using sensitivity information from coils have been demonstrated to improve of image quality. Moreover, many filtering methods were proposed such as adaptive matched filter and nonlinear diffusion technique to optimize the suppression of background noise and to improve of image quality. In this study, we performed SENSE reconstruction using computer simulations to confirm the most suitable method for the feasibility of filtering effect and according to changing order of polynomial fit that were applied on variation of spatial resolution of sensitivity map. The image was obtained at 0.32T(Magfinder II, Genpia, Korea) MRI system using spin-echo pulse sequence(TR/TE = 500/20 ms, FOV = 300 mm, matrix = $128{\times}128$, thickness = 8 mm). For the simulation, obtained image was multiplied with four linear-array coil sensitivities which were formed of 2D-gaussian distribution and the image was complex white gaussian noise was added. Image processing was separated to apply two methods which were polynomial fitting and filtering according to spatial resolution of sensitivity map and each coil image was subsampled corresponding to reduction factor(r-factor) of 2 and 4. The results were compared to mean value of geomety factor(g-factor) and artifact power(AP) according to r-factor 2 and 4. Our results were represented while changing of spatial resolution of sensitivity map and r-factor, polynomial fit methods were represented the better results compared with general filtering methods. Although our result had limitation of computer simulation study instead of applying to experiment and coil geometric array such as linear, our method may be useful for determination of optimal sensitivity map in a linear coil array.

Characteristics of Meniscus Tear in Tibial Plateau Fractures

목적: 경골 고평부 골절은 슬관절에 다양한 문제를 발생시킨다. 본 연구는 관절경하 경골 고평부 골절에서 반월상 연골 손상 양상을 확인하고자 한다. 대상 및 방법: 2007년 3월부터 2010년 3월까지 경골 고평부 골절을 진단 받은 39명의 환자 중 33명 환자를 대상으로 골절 고정 시내 관절경을 시행하였다. Schatzker 분류에 따르면 1명의 I형(3.3%), 19명의 II형(53.3%), 4명의 III형(13.3%), 2명의 IV형(6.6%), 2명의 V형(6.6%), 5명의 VI형(20%) 소견을 보였다. 결과: 25명의 환자(75.8%)에서 외측 반월상 연골 파열 소견을 보였다. Schatzker II형 환자 19명 중 18명의 환자(94.7%)가, Schatzker III형 환자 4명 중 3명의 환자(75%)가, Schatzker VI형 환자 5명 중 4명의 환자(80%)가 반월상 연골 파열 소견을 보였다. 반월상 연골 중 가장 많이 손상된 부분은 외측 연골의 전각부분이다(22/25). 25명의 반월상 연골 파열 환자 중, 한 명을 제외한 모든 환자에서 반월상 연골 변연부 수직 종파열 양상을 보였으며, 이는 대부분의 경우에 있어 관절경하 봉합술을 시도할 수 있었다. 결론: 경골 고평부 골절에 있어 관절경하 반월상 연골 손상에 대한 파악 및 치료에 유용하며, 경골 고평부 골절환자에서 관절경을 권한다. Purpose: Tibial plateau fractures cause a variety of problems in the knee joint. The purpose of this study was to investigate the characteristics of the meniscus injuries in tibial plateau fracture arthroscopically. Materials and Methods: Thirty-three out of 39 consecutive patients diagnosed with tibial plateau fractures underwent arthroscopy between March 2007 and March 2010. According to Schatzker classification, there were 1 type I (3.3%), 19 type II (53.3%), 4 type III (13.3%), 2 type IV (6.6%), 2 type V (6.6%) and 5 type VI (20%) fracture patterns in 33 patients. Results: Twenty-five cases (75.8%) had lateral meniscus tears. There were 18 meniscal tears in 19 cases of Schatzker type II fractures (94.7%), 3 meniscal tears out of 4 cases of Schatzker type III fractures (75%) and 4 meniscal tears out of 5 cases of Schatzker type VI fractures (80%). The most commonly affected site (22/25) was the anterior horn of the lateral meniscus. Of the 25 documented meniscal tears, all but one were vertical longitudinal tear at meniscocapsular junction so most cases are amenable to arthroscopic repair. Conclusion: Arthroscopy for the meniscal injuries in tibial plateau fractures is a valuable diagnostic and treatment tool, we recommend arthroscopy in tibial plateau fracture.

Unusual Late Life-Threatening Arterial Bleeding after Salvage Total Laryngectomy Following Chemotherapy and Radiotherapy

본방사선 치료 후에 후두 절제술을 시행할 경우 인두피부누공, 감염, 출혈 등 상처부위 합병증의 가능성이 높다. 저자들은 최근에 항암치료와 방사선 치료 후에 재발한 성문상부암에 대해 후두 전적출술을 시행하였고, 수술 66일째 생명을 위협하는 인두의 동맥 출혈을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Primary Leiomyosarcoma of the Submandibular Gland

평활근육종은 주로 사지와 몸통에 주로 발생하며, 악하선의 원발성 평활근육종은 매우 드물다. 악하선의 원발성 평활근육종은 증상과 방사선 검사 모두에서 비특이적이며, 치료는 광범위 절제술이다. 최근 저자들은 악하선에 발생한 원발성 평활근육종을 광범위 적출술을 시행한 후에 술후 방사선 치료를 시행하여 치험하였기에 문헌 고찰과 함께 보고하는 바이다. 악하선의 평활근육종은 드물지만, 방사선 검사에서 경계가 분명하고 조영 증강이 잘되며, 세침흡입검사에서 방추세포가 발견될 경우에 감별 진단으로 고려해야 한다.

Goltz 증후군 1례

저자들은 특징적인 진찰 소견과 피부 조직 생검에서 제 I형 교원질 유전자 발현의 감소를 보인 Goltz 증후군 1례를 경험하였기에 보고하는 바이다. Goltz syndrome(focal dermal hypoplasia) is a rare disorder characterized by ectodermal and mesodermal dysplasia described in 1962 by Goltz. In Korea, one case of Goltz syndrome was reported in 1994. The inheritance mode is mostly X-linked dominant. Skin abnormality is the most common manifestation including hypoplasia of the dermis. Skeletal involvement such as syndactyly, polydactyly, scoliosis, kyphosis and spina bifida occulta may be present, also ocular and dental abnormalities are reported. Radiologic findings are the osteopathy and striation of the long bone. We experienced a case of Goltz syndrome in a 9-year old female who was presented with right side hypotrophy, focal dermal hypoplasia, ocular(anidria, microcornea), dental(oligodontia, amelogenesis) and skeletal(syndactyly) abnormalities. Skin biopsy was performed and showed decreased expression of type I collagen gene with Northern blotting.

농촌지역 노인들의 사상체질에 따른 영양상태

The purpose of this study was to investigate the food habits and nutrient intake, among the elder according to their Sasang constitutions. The investigation group was composed of 79 elder men and 117 elder women in Sungjugun Kyungsangbukdo. Data were collected from 1 July, 2002 to 31 July, 2002. In conclusion, diet habits and nutritional intakes were poor in the elderly older than 65 years of age so that they need regular food intake habit and balanced nutrition to obtain enough nutrients to maintain healthy lives. And the following suggestions are made. Firstly, multi-directional studies are needed to improve nutritional intakes, prevent diseases, and improve quality of life in the elderly. Secondly, in-depth studies are needed with various and more people related with Sasang constitutions. Thirdly, we need to screen for risk factors of poor nutrition in each constitution for systemic maintenance of nutritional status and health management in the ever increasing elderly population. For this purpose, an objective investigative tool befitting the situation in Korea should be developed. Fourthly, comparative studies on diet habit and nutritional status are needed by investigating the relationship between Sasang constitutions and diseases. Fifthly, food menu should be proposed according to Sasang constitutions to be utilized as basic data of longevity study.
