        Distal Femoral Medial Opening Wedge Osteotomy for Post-Traumatic, Distal Femoral Varus Deformity

        Restoration of neutral mechanical alignment of the lower limb is an important factor in the treatment of unicompartmental arthrosis. Traditionally, medial opening wedge high tibial osteotomy has been widely performed to correct varus malalignment with unicompartmental arthrosis. However, an ideal indication for the high tibial osteotomy is the knee with metaphyseal tibial varus malalignment. The basic principle of corrective osteotomy is performing an osteotomy at the center of the deformity to prevent abnormal joint line obliquity. If pathologic distal femoral varus deformity is the cause of genu varum, the osteotomy should be performed in the distal femur. Reports of medial opening wedge distal femoral osteotomy (DFO) to correct varus malalignment are rare. We present a case of this very rare and challenging condition in a 47­year­old male, which was successfully treated by medial opening wedge DFO.

        Conceptual Study for Tissue-Regenerative Biodegradable Magnesium Implant Integrated with Nitric Oxide-Releasing Nanofi bers

        The excessive initial corrosion rate of Mg is a critical limitation in the clinical application of biodegradable Mg implantsbecause the device loses its fi xation strength before the fractured bone heals. This study suggests a new approach to overcomethis hurdle by accelerating tissue regeneration instead of delaying the implant biodegradation. As angiogenesis is anessential process in early bone regeneration, a Mg implant coated with electrospun nanofi bers containing nitric oxide (NO),which physiologically promotes angiogenesis, is designed. The integrated device enables adjustable amounts of NO to bestored on the NO donor-conjugated nanofi ber coating, stably delivered, and released to the fractured bone tissue near theimplanted sites. An in vitro corrosion test reveals no adverse eff ect of the released NO on the corrosion behavior of the Mgimplant. Simultaneously, the optimal concentration level of NO released from the implant signifi cantly enhances tube networkformation of human umbilical vein endothelial cells without any cytotoxicity problem. This indicates that angiogenesis canbe accelerated by combining NO-releasing nanofi bers with a Mg implant. With its proven feasibility, the proposed approachcould be a novel solution for the initial stability problem of biodegradable Mg implants, leading to successful bone fi xation.

        폴리에스테르 섬유 감량시스템의 제어를 위해서는 처리조 내에서 감량되는 섬유가 목표 감량치에 도달했는지를 판단해 주는 장치가 필요하다. 본 연구에서는 가성소다용액의 초농도 및 섬유 감량 정도의 정확한 측정과 감량 종료시간의 산출 및 이에 따른 종료신호를 발생할 수 있는 폴리에스테르 섬유 감량기 전용의 자동적정장치를 개발하였다. To control caustic soda treatment system for the polyester fabric reduction, we need the device to determine that the fabric has been arrived target reduction value. In this study, we develope an automatic process titrator for the caustic soda treatment system. Major functions of the target titrating system are accurate measurements of the caustic soda concentration and then producing the end point signal for caustic soda treatment system.

        비복신경이식을 이용한 긴 거리의 말초신경 결손부 수복

        The surgery of oral and maxillofacial area poses the risk of cranial nerve damage such as trigeminal nerve or facial nerve. Inferior alveolar nerve is prone to damage in the third molar extraction, implant installation, orthognathic surgery, open reduction and rigid fixation, and tumor ablation surgery. On the other hands,facial nerve is likely to be damaged or sacrificed with trauma or parotidectomy. In case of inferior alveolar nerve injury, the incidence is reported to be about 1.3%. The nerve function will almost recover in minimal damage, but it won't recover at last in total damage of a part of nerve unit. In latter cases, nerve regeneration in intended by allograft as nerve substitute or various route of merve condit. But the recovery with autograft is believed to be most relialbe mrthod in the rapair of long-span(longer than 15㎜)nerve defect. We have performed autologous sural nerve graft in the repair of nerve defect, which is caused by resection of benign or malignant tumor. Hereby we report the method of nerve harvesting, recovery of defected peripheral nerve and the complications of donor site with the discussion of sural nerve anatomy.

        하치조신경 및 설신경 손상 평가를 위한 한국인 정상 성인의 하순-이부 및 혀의 감각 조사

        In the head and neck area, there are so many sensory nerves, which are sometimes injuried iatrogenically or inadvertently so that involved patients complained of the loss of sensations. In such cases, it is important to judge the degree of injuries and regeneration of nerve for better diagnosis and treatment. Seddon and Sunderland's classification, which is commomly used, is focused on histological change and nerve conduction. As times goes by, it is difficult to access patient's sensory disturbance by this method. Until now, so many methods such as contract threshold, direction, two-point discrimination, pin prick, thermal discrimination and current perception threshold have been introduced for sensory evaluation. However, there hasn't been enough information regarding each methodology nor integrated standard methodology for the measurement. the purpose of this study is to get Korean adult normative sensory values of lower lip,chin and tongue using modified methods of contact thershold, ditection, two point discrimination, pin prick, thermal discrimination and assess degree of regeneration of sensory nerve damage.

      • SVD 알고리즘 및 HMM을 이용한 얼굴 및 눈 패턴 검출

        The studies about automatic pattern detection of the eye and face from the human image acquired by the CCD image sensor have good applicabilities in the industry, home automation, and data communication field. In general, pattern detection method consists of feature based matching and template matching. In feature based matching, the feature vector is extracted with DLM(dynamic linking matching), EBGM(elastic bunch graph matching), HMM(hidden markov model) matching and knowledge based matching using statistical characteristics. In template matching, in general, the template vector is extracted with PCA(principal component analysis). When these method applied in the face and eye detection, each method has its own merits and some disadvantage. Therefore, by combined utilization of SVD(singular value decomposition) and HMM algorithm, is expected that we can selectively make use of each methods advantage and it result in improved detection accuracy. In this paper, we proposed the method for face and eye detection, which was combined by the two algorithms, to be suitable for the high speed image processing using DSP chip or microprocessor. In the beginning, template matching was followed by a template extraction using batch SVD algorithm and then face pattern was classified and recognized by HMM algorithm which is one of feature based matching technique. Finally, eye pattern detection was performed by pattern search neural network utilizing eigeneye image.

        진행된 HIV 감염 환자에서 십이지장의 미란성 병변으로 발현한 Mycobacterium genavense 감염 1예

        Mycobacterium genauense는 최근에 알려진 미코박테륨으로서, 식욕 부진, 체중 감소, 설사, 림프절 병증 등 파종성 Myocobacterium avium-intracellulare complex 감염과 유사한 임상상을 보이며, 위장관 침범을 잘 하는 것으로 알려져 있으나 아직 국내에서 증례는 보고된 바가 없다. 저자들은 진행된 HIV 감염 환자에서 십이지장의 미란성 병변으로 발현한 Mycobacterium genavense 감염 1예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. 환자는 상복부 동통과 발열, 설사로 내원하였다. 십이지장의 미란성 병변에서 시행한 조직검사에서 항산균이 가득 차있는 거품세포가 관찰되었으며, 이 검체에서 16S rRNA 유전자의 중합연쇄반응 검사를 시행하여 Mycobacterium genavense를 확인하였다. Mycobacterium genavense, first identified in 1990, is known as a pathogen that mimics disseminated Myocobacterium avium-intracellulare complex (MAC) infection with particular propensity for the gastrointestinal tract. In Korea, no case with the organism has been reported. Herein we report a case of Mycobacterium genavense infection that manifested with erosive lesion of duodenum in a patient with acquired immune deficiency syndrome. The patient presented with epigastric pain and fever, diarrhea. Duodenal biopsy showed histiocytic infiltration with numerous acid-fast bacilli. Identification of the mycobacterial isolate by the polymerase chain reaction restriction analysis of 16S rRNA gene revealed Mycobacterium genavense.

      • 입체재단과 평면제도법의 의한 Bodice 원형의 비교 연구

        The purpose of this study was three fold: to take the measurements of dress form according to the size, to compare the size of bodice pattern made by draping and flat pattern method. and to disclose the problem of dress form through comparison of measurements of body and dress form. 163 samples of dress form size from 6 to 10 were selected and 16 items on the dress form were measured. A draping method and two different flat pattern method (pattern A and B) were used to compare the patterns. For the draping method, one representative dress form each size number 6 to 10 was selected and standard dress form which was made for this study based upon the standard size of women in twenty years of age was used. The results of the study were as follows: First, the size of dress form ranging from 6 to 10 were generally used in the universities. Particularly, the size 6 and 8 which were ideal as body sizes for college women were mostly used. Second, there were significant differences in waist dart and low-underarm dart between the draping and the pattern A. There was no significant difference in the comparison of dart between the draping and the pattern B. Third, back neck girth, back neck width, front-side line grade, and back shoulder grade were found to be greater in the draping pattern than the patterns A and B. Pattern A and B were wider than the draping pattern in front-armhole girth, total-armhole girth, front neck depth, front-armhole depth, and back-armhole depth. Front neck girths and front shoulder grade were wider in the pattern B than the draping. Fourth, when the bust girth was compared to the waist, hip, and neck girth, and shoulder width, the ratio of the dress form was similar to that of standard, but generally the measurements of each item on the dress form were smaller than the standard, showing a great difference in the shoulder width. The back neck width of the dress form was wider, and the front neck depth of the dress form narrower than the actual body. Therefore, especially it is necessary to consider the average neck size of body to make dress form.

      • 좌심실 전동순환 보조장치에 관한 연구

        본 논문에서는 정상류형 혈액 펌프의 일종인 축류형 혈액 펌프를 이용하여 혈액 펌프 시스템을 설계하였다. 이 시스템은 혈액 펌프, 신호 획득부, 인터페이스부, 신호 처리부로 구성되어 있고, 혈압과 심박수를 이용하여 혈압과 혈류량을 적절히 제어함으로써 좌심실의 기능을 보조할 목적으로 연구되었다. 전동 펌프가 회전할 때 생체내의 순환조건에 알맞도록 혈액 펌프의 회전수, 혈액 펌프에 걸리는 부하와 압력 정보를 적용하여 실험식을 만들어 제어에 이용하였으며 설계 제작된 모의 순환장치로써 실험한 결과 모의 혈관 내에서 적절한 혈압과 혈류의 제어가 가능하였고, 혈구의 변화량 실험 결과는 임시의 목적으로 사용가능함을 알 수 있었다. This paper outlines the development of a non-pulsatile axial flow type blood pump control system. By utilizing blood pressure and heart rate, this system can assist the left ventricle in controlling blood pressure and blood volume. The system is comprised of a blood pump, signal sensor, signal interface, and signal-processing component. A control algorithm is also proposed which can control non-pulsatile, continuous blood flow in the human circulatory system. To facilitate the control required fur non-pulsatile blood pump in a physiological system, an experimental control rule was developed utilizing ECG and blood pressure data, both of which are easily detectable variables in the bods. The system was then tested using a mock-up circulation system and we found that it is possible that this systems could be temporarily used in clinic.

      • 초등학생의 읽기 능력과 과학 탐구 능력 및 과학 성취도와의 관계

        본 연구의 목적은 읽기 능력이 과학 탐구 능력 및 과학 성취도와 어느 정도의 관련이 있는지를 알아보는 것이다. 최근 30년간 실험실 활동과 탐구 능력은 상당한 관심을 받으며 강조된 반면, 읽기 활동과 읽기 능력(또는 독해 능력)은 과학교육분야에서 소홀히 취급되고 있는 실정이다. 그러나 글을 읽는다는 것은 단순히 글자를 해독하는 과정이 아니라 다양한 요소들이 개입되는 사고의 과정이라는 인식 하에 읽기는 최근 일부 과학교육학자들에 의해 꾸준히 연구되어지고 있다. 이러한 연구들 중 흥미로운 것은 읽기 능력과 탐구 능력 요소들간의 유사성에 대한 찬ㆍ반 논의이다. 이에 본 연구에서는 인천광역시의 초등학교 5학년 115명을 대상으로 읽기 능력, 탐구 능력 및 과학 성취도의 관계를 알아본 결과, 이 세 요소는 유의미한 상관 관계를 나타내었다. 이는 문제해결 활동으로써 과학과 관련된 글에 대한 읽기 활동을 적절히 활용한다면 학생들에게 탐구 능력 요소들을 연습할 수 있는 기회를 제공할 뿐만 아니라 과학 지식의 습득이라는 과학교육의 중요한 두 가지 목표 달성에 효과적인 방법이 될 수 있음을 시사한다. The purpose of this study was to inquiry how much elementary students' reading ability has relations with science process skills and science achievement. Generally laboratory activity and science process skills have been emphasized, on the other hand, reading activity and reading ability have been neglected by science educators for nearly 30 years. But reading a text is not simply a process of decoding printed symbols; it is a process of thinking involved manifold factors. Under this recognition, some science educators have steadily researched into reading. The interesting thing out of them is pros and cons about correspondence between science process skills and reading skills. Therefore, in order to investigate the relationships between reading ability, science process skills, and science achievement, grade 5 students (N=115) Were selected from one elementary school in Inchon and then they were required to respond to tests (which assess reading abilities, science process skills, and science achievement). The results of this study appeared to indicate that reading ability has significant relations with science process skills and science achievement. Probably, the proper using of scientific written materials, as problem-solving activity, will provide students with opportunities that they can practice science process skills and obtain the scientific knowledge.
