In this paper, implemented the algorithms that recognize the image learning the standard data by Back propagation and Hopfield network of neural network Back propagation learning method needs more hours in carrying out one-time learning due to the hopfield algorithm, but in the rate of detection per learning frequency, back propagation shows an excellence. Back propagation learning algorithm requires 25 seconds to obtain the rate of 97% detection of the image, but to detect the image over 99%, the time of 1 minute and 20 seconds is required and while the rate of image recognition is very high in initial learning, it is delayed from over 97%. To the contrary, in the case of hopield network, more hours are required to recognize 97% of the image than back propagation, but in recognizing the image over 99%, it requires shorter hours than back propagation.

Background: To examine the influencing factors on patients' satisfaction in the emergency department(ED) for quality assurance. Methods: Patients who visited to the ED were prospectively investigated from November 1 to December 31, 1997. Authors developed questionnaire to investigate influencing factors on patients' satisfaction. A Chi-square test and LISREL 7.0 were applied for statistical analysis. Results: Patients' satisfaction was significantly related to physical environment variables, accessibility variables, kindness of hospital personnel variables, and patient's trust for doctors variables. In path analysis, willingness for revisit was influenced by patients' satisfation, accessibility, physical environment, patients' trust for doctors in order, and willingness for recommendation was influenced by accessibility, patients' satisfaction, and kindness of hospital personnel in order. Conclusions: The influencing factors on patients' satisfaction are physical environment, accessibility, kindness of hospital personnel, and patient's trust for doctors. Willingness for revisit and willingness for recommendation are influenced by patients' satisfation. In spite of some limitations, the results of this study can be used as a baseline information for exploring the influencing factors on patients' satisfaction. Further comprehensive research efforts should be made on the study of patients' satisfactoin in the ED.

Multicasting transfer data from one point to the members of groups. which can be supported by using multicast tree in point-to-point network. For multicast routing a minimal steiner tree is used, and has an advantage in that only one Steiner tree is needed for a group. However, finding a minimal Steiner tree is an NP-complete problems and it is necessary to develop efficient heuristic algorithms. In this paper, we present agents and genetic algortihms for a finding minimal Steiner tree. Agents check weights among nodes and transmit these to genetic algorithms. Then, genetics algorithms build Steiner trees with these weights and try to find a optimal Steiner tree. This paper describes a tree-based genetic encoding scheme which is in sharp contrast with binary string representations usually adopted in conventional genetic algorithms, and shows the efficiency of this scheme through experiments.

Tetrodotoxin is a neurotoxin produced by about 90 species of puffer fish and causes paralysis of central nervous system and peripheral nerves by blocking the movement of all monovalent cation. Ingestion of tetrodotoxin produces clinical manifestations such as paresthesias(within 10-45 min), vomiting, lightheadedness, salivation, muscle twitching, dysphagia, difficulty in speaking, convulsion and death that expressed by cardiopulmonary arrest with loss of brain stem reflex sometimes. Tetrodotoxin prevents or delays ischemia induced neuronal death by way of following 3 mechanisms. Firstly, it reduces the energy demand of the brain tissues. Secondly, it delays or even prevents anoxic depolarization. Finally, it deminishes ischemia induced cell swelling and cerebral edema. We report a case of puffer fish poisoning which presented with cardiopulmonary arrest and loss of brain stem reflex, but completely recovered by aggressive cardiopulmonary resuscitation.

<▼1>
Supplemental Digital Content is available in the text
</▼1><▼2>
Abstract

Proteinuria is a major determinant of adverse renal outcome, and its reduction slows renal progression in glomerular diseases. However, the optimal target of proteinuria in glomerular diseases is unclear, and discrepancies in the definition of proteinuria produce ambiguous findings. Here we investigated the optimal target of proteinuria by using different definitions of proteinuria. We analyzed 574 IgA nephropathy (IgAN), 175 membranous nephropathy (MGN), and 177 focal segmental glomerulosclerosis (FSGS) cases from 3 Korean kidney centers. We evaluated the impact of proteinuria on renal outcome with 2 definitions: time-average proteinuria (TAP) and time-varying proteinuria (TVP). The endpoint was renal progression, defined as a 50% decline in glomerular filtration rate or end-stage renal disease. During a median follow-up of 57.3 months, the primary outcome occurred in 54 patients with IgAN, 26 with MGN, and 30 with FSGS. Multivariate Cox regression using TAP indicated that there was a linear association between proteinuria and risk of renal progression in IgAN. However, moderate proteinuria was not associated with an increased risk of renal progression in MGN and FSGS. In contrast, the analysis by TVP showed that the risk significantly increased in proportion to proteinuria during follow-up in all 3 diseases. Our findings suggest that TVP-based model can delineate association between proteinuria and risk of renal progression better than TAP-based model, considering that TVP reflects the dynamic change of proteinuria over time. Thus, proteinuria reduction to the lowest possible level is required to improve renal outcomes in patients with glomerular diseases.
</▼2>

Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]

Proteomics analysis of salt-induced leaf proteins in two rice germplasms with different salt sensitivity. Can. J. Plant Sci. 91: 337-349. This study was conducted to investigate salt-stress-related physiological responses and proteomics changes in the leaves of two rice (Oryza sativa L.) cultivars. Shoot growth and water content of rice leaves were more severely reduced in Dalseongaengmi-44 than in Dongjin under salt stress. The salt-sensitive Dalseongaengmi-44 exhibited a greater increase in sodium ion accumulation in its leaves than the salt tolerant Dongjin. Comparative analysis of the rice leaf proteins using two-dimensional gel electrophoresis (2-DGE) revealed that a total of 23 proteins were up-regulated under salt stress. Based on matrix-assisted laser desorption ionization-time of flight mass spectrometry and/or electrospray ionization-tandem mass spectrometry analyses, the 23 protein spots were found to represent 16 different proteins. Ten of the identified proteins were previously reported to be salt-responsive proteins, while six, class III peroxidase 29 precursor, beta-1,3-glucanase precursor, OSJNBa0086A10.7 (putative transcription factor), putative chaperon 21 precursor, Rubisco activase small isoform precursor and drought-induced S-like ribonuclease, were novel salt-induced proteins. Under salt stress, fragmentation was increased in several proteins containing the Rubisco large chain. The results of these physiological and proteomics analyses provide useful information that can lead to a better understanding of the molecular basis of salt-stress responses in rice.
