중환자실에 입원한 세균성 폐렴환자에서 반정량적 procalcitonin 검사의 유용성

Background : In pulmonary infection, serum procalcitonin levels increase rapidly, probably in response to sepsis-related cytokine release from neuroendocrine cells of bronchial epithelium and inflammatory cells. We applied procalcitonin assay in critically ill patients with bacterial pneumonia. Materials and Methods : Patients admitted to the intensive care unit (ICU) and show diffuse infiltrations in their chest X-ray were included. Quantitative bronchoalveolar lavage (BAL) culture (≥10⁴ CFU/mL) was performed in all cases on the 5^(th) day of ICU admission. We excluded patients with structural lung disease, non-infectious lung infiltrations, and atypical infections such as Mycobacterium tuberculosis, Pneumocystis jiroveci, and viruses. Serum procalcitonin levels were measured semi-quantitatively by using PCT-Q kit. Results : A total of 28 adult patients (M:F=23:5) were included: 11 (39.3%) medically-ill patients, 7 (25%) surgically-ill patients, and 10 (35.7%) burn patients. Serum procalcitonin level was <0.5 ng/mL in half of the cases (14/28) and ≥0.5 ng/mL in the remaining half of the cases. Compared to those with serum procalcitonin level of <0.5 ng/mL, patients with serum procalcitonin level of ≥0.5 ng/mL had more frequent mechanical ventilation, higher CRP/APACHE II scores/number of organ failure (P<0.05), and showed increased tendency for death (P=0.052). Positive bacterial BAL cultures were noted in 17 cases (60.7%). Of these, 7 cases (41.2%) showed serum procalcitonin level ≥0.5 ng/mL. Conclusions : High serum procalcitonin level seems to be closely associated with the severity and poor prognosis in critically ill patients with bacterial pneumonia. However, pneumonia could not be excluded with low level of procalcitonin among ICU patients.

In order to evaluate the effect of poloxamer 407 content on the dissolution profiles of pellets, diltiazem HCl (DTL) core pellets were prepared with poloxamer 407 (50∼90% w/w, with lactose as filler) using an extruder and a spheronizer. Any possible interaction between the drug and excipients was evaluated using DSC, IR and TLC. Dissolution tests were performed using USP basket method. In addition, scanning electron micrograph was performed to examine the surface roughness and cross sections. The release of DTL from the core pellets was decreased with increasing poloxamer 407 content. Cracks appeared on the surface of the core pellets with increasing the poloxamer 407 content, which may play a role on the retardation of the release of DTL from core pellets. There was no any significant interaction between the drug and excipients employed to prepare the core pellets.

For the development of antifungal agents. modification of naftifine which exhibits significant antimycotic activity was performed by replacing the naphthalene ring of it to hetero cyclic rings such as morpholine. benzothiazole. piperidine and pyridine derivatives. The synthesized compounds were tested in vitro antifungal activity against five different fungi with naftifine as a comparative antimycotic molecule. From the biological evaluation two compounds. (E)-N-(3-phenyl-2-propenyl)-N-(4-piperidinylmethyl)amine(3d) and (E)-N-(3-phenyl-2-propenyl)-N-(3-pyndylmethyl)amine(3D) showed relatively noticeable activity(MIC=50㎍/㎖). On the other hand. the other compounds had no activity.

Background: Non-melanoma skin cancers are caused mainly by prolonged UV exposure. However, there is no effective prevention other than avoiding sun exposure. Recently, ablative fractional laser(FL) treatment is actively being carried out for facial rejuvenation. Objectives: We elucidated whether the occurrence of skin tumors caused by exposure to UV can be decreased by multiple sessions of ablative CO2 FL. Methods: Two groups of hairless mice were treated with either ablative CO2 FL or nothing at 3-week intervals during the 20 weeks of UV exposure period. At the 30-week, representative tumors were taken to evaluate the type of tumor and check the mRNA expression levels of the MMP-13 and type-1 procollagen. In addition, we evaluated the change of skin barrier function at 12 weeks and 20 weeks. Results: At 30 weeks, the UV plus FL treated group(group I) showed a significantly lower average size of tumor and a lower occurance rate of tumors than the UV only treated group(group II). Tumors in the group II showed more malignant progression. And the groups II showed significantly greater trans-epidermal water loss than the group I. The group II showed significantly decreased collagen and elastic contents than the group I. The group I presented a higher mRNA level of type-1 procollagen, and a lower level of MMP-13 than the group II. Conclusion: Ablative CO2 FL can be effective for not only skin rejuvenation but also for prevention of skin tumors induced by UV.
