Background : Angiogenesis seems to be essential for hepatocellular carcinoma progression. It could be evaluated by observing the CD34 expression. In the present study, we studied to define whether or not angiogenesis has the correlation with clinicopathological characteristics of hepatocellular carcinoma. Methods : In 46 cases of hepatocellular carcinoma, we examined the CD34 expression in tumor and surrounding non-tumorous liver tissue using immune histochemistry. Microvessel density was also calculated. Results : The expression of CD34 was diffuse and strong in all hepatocellular carcinoma tissue and sparse in adjacent area of the surrounding cirrhotic regenerating nodule. The mean microvessel density was not significantly different between in the group of Edomondson's grade 1-2 (n=30; 25.9±15.5) and the group of Edmondson's grade 3-4 (n=16; 33.1±11.8; P=0.085). And it was not significantly different either between the patients (n=27) with tumor smaller than 4.6cm (30.6±15.1) and the patients (n=19) with larger than 4.6cm (25.3±13.6, P=0.225) in diameter. However overall survival was worse in patients (n=22) with high microvessel density (>28.4) than in patients (n=24) with low microvessel density significantly (P=0.043). Conclusion : The expression of CD34 may serve as the diagnostic tool in differentiating hepatocellular carcinoma from benign regenerating nodule. Although microvessel density showed no significant correlation with size and histopathological grade of hepatocellular carcinoma, angiogenesis may be used as a prognostic marker.

Primary malignant lymphoma of the thyroid is a relatively rare malignant tumor of the thyroid gland. It is known to be frequently associated with Hashimoto's thyroiditis. In Korea, 8 cases of primary lymphoma of the thyroid have been reported. All patients were over 64 years of age and the size of masses was over 5cm. A 230year-old woman complained of diffuse goiter. High resolutional ultrasonography revealed 0.6x0.3x0.7cm sized focal low echoic lesion at the deep portion of the left lobe and 0.9x0.5x0.8cm sized focal low echoic lesion at the mid-portion of the right lobe of the thyroid gland. We performed ultrasound-guided fine needle aspiration on the both nodules. The cytologic finding of the nodule in the right lobe suggests malignant nodule and that of the left lobe showed Hashimoto's thyroiditis. Near total thyroidectomy was performed and confirmed a tumor of low-grade mucosa-associated lymphoid tissue which size was 0.8cm. She was treated with combination chemotherapy (cyclophosphamide, oncovin, epirubicin, methylprednisolone and bleomycin). We report this case with a review of literatures.

Lithium has been established as a drug useful for the treatment of manic depressive disorder. It is now well recognized that long-term administration of this drug is associated with various antithyroid effects such as goiter, and subclinical and overt hypothyroidism. However, it has been associated less commonly with thyrotoxicosis. Recently we experienced a case of thyroitoxicosis during lithium therapy. A 24-year-old man treated with lithium carbonate 900 mg-1,200 mg/day for manic-depressive illness for four weeks. He then complained of nervousness, palpitation, tremor, heat intolerance, and sweating. Neck pain was not noted. At that time the results of thyroid function test were consistent with hyperthyroidism: T3 568.8 ng/dL, TSH 0.01 mU/mL, FT4 6.0 ng/dL, but 24 hr radioiodine uptake was 0.3%. We suspected this case as lithium induced thyrotoxicosis and discontinued lithium administration. After discontinuation of lithium thyrotoxic symptoms were subsided. One month later, thyroid hormon levels became normalized: T 100.2 ng/dL, TSH 0.06 mU/mL, FT4 0.97 ng/dL and 24hr radioiodine uptake was 16%. We report this case with review of literatures (J Kor Soc Endocrinol 13:629-633, 1998).
