        국내 대학병원에서 정주용 Ciprofloxacin 사용의 적정성 평가

        목적 : 국내 병원에서의 정주 ciprofloxacin 사용의 적정성을 평가하고자 하였다. 재료 및 방법 : 2003년 9월 25일부터 8개의 대학병원에서 정주 ciprofloxacin을 받았던 290명의 연속적으로 모집한 성인환자를 대상으로 약제사용 평가(DUE)를 전향적으로 수행하였다. 이 연구에 사용된 DUE 기준은 미국병원약사회의 기준을 국내 실정에 맡게 수정하여 사용하였으며 1) 사용의 정당성에 관한 항목, 2) 사용 시 준수되거나 시행되어야 할 사항에 대한 항목, 3) 부작용에 대한 항목, 4) 치료 효과에 대한 항목으로 나누어 조사하였다. 결과 : 환자의 평균 나이는 54세였다. 33예(26.9%)에서만 약제 사용의 적정성 측면에서 기준을 만족하였고 91예(73.1%)에서는 사용이 적절하지 않았다. 정주 cipro-floxacin의 부적절한 사용의 흔한 원인은 수술 전후의 예방약 사용, 비뇨기계 처치 전의 예방적 사용, 열성 질환에 대한 경험적 사용 등의 순이었다. 사용 시 준수사항 의 측면에서는 사용 전 배양검사(73.8%)와 사용 중 배양검사(68.6%)가 낮은 수행 정도를 보였으나 그 외는 비교적 높은 적정성(82.4-100%)을 보여주었다. 정주 ciprofloxacin사용의 합병증은 드물었다. 치료 효과의 면에서 치료반응은 93.3%였다. 결론 : 약제 사용의 정당성 기준에 따른 정주 cipro-floxacin의 부적절한 사용은 연구에 포함된 대학병원에서 매우 흔하였다(73.1%). 그러므로 적절한 항생제 사용에 대한 교육과 제도적 장치가 매우 필요한 것으로 판단되었다. Background : To evaluate the appropriateness of intravenous (IV) ciprofloxacin usage in university hospitals in Korea. Materials and Methods : We prospectively evaluated the appropriateness of drug usage in 290 consecutive adult patients who received IV ciprofloxacin in eight teaching hospitals during the period from September 25, 2003 to November 15, 2003. Drug use evaluation (DUE) in this study was based on the modified standard by the American Society of Hospital Pharmacy with regard to the justification of drug use, critical and process indications, complications, and outcome measures. Results : Average age of the patients was 54 years. The use of IV ciprofloxacin was appropriate only in 33 cases (26.9%). Common reasons for inappropriate use of IV ciprofloxacin include perioperative uses or prophylactic injection before urologic procedures, and empirical therapy for febrile episodes. Most of the "critical indications" and "process indications" showed high rate of appropriateness (82.4-100%). However, the rate of taking culture and sensitivity test before (73.8%) and during ciprofloxacin use (68.6%) were low. Complications with ciprofloxacin use were rare. In 93.3% of cases, ciprofloxacin treatment was clinically effective. Conclusion : Use of intravenous ciprofloxacin is frequently inappropriate in tertiary care hospitals in Korea. Implementation of education program and antibiotic control system is critical in improving the appropriateness of antimicrobial use.

        제대군인 취업지원 프로그램 구안

        이 연구의 목적은 중·장기복무를 마치고 제대하여 재취업하고자 하는 제대군인들의 원활한 전직을 돕기 위한 제대군인 취업지원 프로그램의 개발방향과 개발안을 제시하는 데 있다. 이를 위해 국내·외에서 이루어지고 있는 제대군인 취업지원 프로그램의 실태를 분석하였고, 제대군인 취업지원 프로그램에 대한제대예정군인 및 제대군인과 기업체의 요구를 분석하여 시사점을 도출하였다. 그리고 이를 기초로 제대군인 취업지원 프로그램의 기본방향과 전체 흐름도, 세부 프로그램 개발안을 제시하였다. 이를 위해서 활용된 연구의 방법은 문헌연구, 조사연구, 전문가협의회 등이다. 이 연구를 통해서 제시된 제대군인 취업지원 프로그램의 개발방향과 개발안을 토대로, 일관되고 지속적인 수요자 중심의 제대군인 취업지원 프로그램의 개발이 가능할 것이다. The purpose of this study is to suggest plans and directions for developing. job-seeking support program for military veterans. The study is conducted through literature review, survey and interview. The results of this study are as follows: ⒧ basic development directions and flowchart of job-seeking support program for military veterans are suggested; and ⑵ development plans for sub-programs are suggested.

        Detection of CTX-M and Clonal Analyses using MLST in Cefotaxime Resistant $Escherichia$ $coli$ Isolated from the Han-River, Korea

        Bacteria resistant to various antibiotics have recently become an issue of the utmost importance. Resistant strains are not uncommon, even in municipal drinking water sources. The health threat posed by resistant, pathogenic bacteria has serious ramifications for both public health and agriculture. In this study, we isolated antibiotic resistant bacteria from water samples from the Han River, Korea, which is contaminated by the wastewater from many industrial complexes, hospitals, agricultural and animal husbandry estates, and from wastewater treatment facilities. We determined the degrees of resistance to various antibiotics exhibited by the isolated strains. The similarities between the isolated $E.$ $coli$ strains were examined, using the pulsed field gel electrophoresis and multilocus sequence typing, in order to trace their origins and to explore the syntechnic adaptations and pathogenicity of the various strains and relate these to their genetic sequence. A total of 25 $E.$ $coli$ strains were isolated from six stations along the Han River. All the 25 strains exhibited resistance to ampicillin. We also investigated resistance to amoxicillin, clavulanic acid, cefazolin, cofoxitin, cefotaxime, cefpodoxime, ceftriaxone, cefepime, nalidixic acid, aztreonam, ciprofloxacin, streptomycin, gentamicin, chloramphenicol and imipenem. Based on the ESBL detection, 14 strains belonged to the ESBL producing strains. The number of the clonal complex producing strains was 5 among the 14 isolated strains. The 5 strains were included in the 168, 23, 38, 469, 156 clonal complex, respectively. The rest 9 strains were not included in the clonal complex, but showed independent STs.

        <i>Ex vivo</i> imaging of basal cell carcinoma using synchrotron phase-contrast X-ray microscopy

        <P>Background/aims</P><P>There is a need for development of non-invasive methods to improve early diagnosis and screening of suspected malignant lesions. Phase-contrast X-ray microscopy (PCXM) has potential to reveal the structures inside soft tissues, and fine details can be observed without any staining or contrast-enhancing cell preparation. We aimed to investigate the possibility that PCXM can be used to explore the microscopic details of basal cell carcinoma (BCC).</P><P>Methods</P><P>Paraffin blocks of specimens from patients with basal cell carcinoma were cut with 30 μm thickness for PCXM imaging. Experiments were performed at the International Consortium of Phase Contrast Imaging and Radiology (ICPCIR) (7B2) beamline of the Pohang light source in Korea. The PCXM images were achieved by using coherent hard X-rays from a synchrotron source with no monochromatization.</P><P>Results</P><P>We could obtain images with clear edge enhancement by PCXM. The images taken with this technique showed clear anatomic details of organelles in normal skin such as epidermis, dermis and skin appendages. Most of cancerous lesions were clearly differentiated from adjacent normal tissues and the images closely corresponded to those obtained with low-magnification optical microscopy.</P><P>Conclusion</P><P>In this pilot study, we successfully demonstrated that synchrotron PCXM could be used for radiological imaging of BCC with great anatomic details.</P>

        The fracture of the humerus during arm wrestling are uncommon, but may occur in the shaft or medial condyle. In the shaft of humerus, the fractures are usually spiral and between the middle and distal third, with or without neurological involvement. A case of a comminuted spiral fracture with butterfly fragment of the distal third of the humerus is reported which had occurred during arm wrestling and was treated by open reduction and internal fixation. The possible mechanism of injury and the question of treatment are discussed.

        Prognostic Value of Pretreatment Metabolic PET Parameters in Cervical Cancer Patients With Metabolic Complete Response After Concurrent Chemoradiotherapy

        PURPOSE: Cervical cancer may recur after concurrent chemoradiotherapy (CCRT), even in patients with metabolic complete response (mCR) on post-CCRT F-FDG PET. We retrospectively analyzed the value of metabolic parameters, measured by pretreatment PET, as prognostic factors for predicting recurrence and death in cervical cancer patients with mCR. METHODS: In 61 patients who reached mCR on post-CCRT, metabolic PET parameters including SUVmax, metabolic tumor volume (MTV), and total lesion glycolysis (TLG) of the primary tumors, as well as those of the metastatic lymph nodes, were measured by pre-CCRT PET. Total MTV and total TLG were also measured from primary tumors and metastatic lymph nodes. Survival analyses for disease-free survival (DFS) and overall survival (OS) were performed. RESULTS: Eleven patients (18.0%) presented with recurrence of cervical cancer, and 12 (19.7%) died during follow-up. SUVmax, MTV, TLG of metastatic lymph nodes, and total TLG could significantly predict DFS, and nodal SUVmax could predict OS. Para-aortic lymph node metastasis also significantly correlated with both DFS and OS. In multivariate analysis, nodal SUVmax and para-aortic lymph nodes metastasis were prognostic factors for DFS, and the best predictor was nodal SUVmax. For OS, nodal MTV and para-aortic lymph node metastasis were significant prognostic factors in multivariate analysis, and the best predictor was para-aortic lymph node metastasis. CONCLUSIONS: Metabolic PET parameters, SUVmax or MTV values of metastatic lymph nodes, may be surrogate prognostic markers for recurrence or death in patients with locally advanced cervical cancer who reached mCR after definitive CCRT.

        Comparing volumetric and biological aspects of 3D-printed interim restorations under various post-curing modes

        Purpose. This study aims to compare the volumetric change, degree of conversion (DOC), and cytotoxicity of 3D-printed restorations post-cured under three different conditions. Materials and Methods. 3D-printed interim restorations were post-cured under three different conditions and systems: 5 min, 30 min, and 24 h. Three-unit and six-unit fixed dental prostheses (n = 30 for each case) were printed; ten specimens from each group were post-cured and then scanned to compare their volumetric changes. Root-mean-squared (RMS) values of the data were acquired by superimposing the scanned files with original files. Thirty disk-shaped specimens were printed to evaluate the DOC ratio. Fourier transform infrared spectroscopy was used to compare the DOCs of 10 specimens from each group. Human gingival fibroblasts were used to measure the cell viability of every specimen (n = 7). The data from this experiment were employed for one-way analysis of variance and Tukey’s post-hoc comparisons. Results. Differences between the three-unit restorations were statistically insignificant, regardless of the post-curing conditions. However, for the six-unit restorations, a high RMS value was acquired when the post-curing duration was 30 min. The average DOC was approximately 56 - 62%; the difference between each group was statistically insignificant. All the groups exhibited cell viability greater than 70%, rendering them clinically acceptable. Conclusion . The post-curing conditions influenced the volume when the length of the restoration was increased. However, this deviation was found to be clinically acceptable. Additionally, postcuring did not significantly influence the DOC and cytotoxicity of the restorations.
