      • F-82 The effect of forced expiratory volume in one second on the treatment response of small cell lung cancer

        The impact of chronic obstructive pulmonary disease (COPD) and pulmonary function on the treatment response of small cell lung cancer (SCLC) has not been studied. The objective of this study was to compare the clinical characteristics and treat-ment response of SCLC patients with and without COPD and investigate predictors related with pulmonary function of mortality in patients with SCLC. This was a retrospective multicenter study performed between January 2011 and December 2015. In all, 147 SCLC patients treated with chemotherapy and had pulmonary function test were enrolled. Patients were divided into the COPD group and the non-COPD group. The overall survival (OS) and progression free survival (PFS) were compared, and predictors of worse OS were analyzed using Cox regression analysis. COPD was present in 54.4% of all SCLC patients. There were no differences in clinical characteristics except pulmonary function test between COPD and non-COPD group. OS and PFS (log-rank test, p = 0.165 and p = 0.071, respectively) were not different between COPD and non-COPD groups. In a multivariate analysis using Cox regression model, extensive disease (hazard ratio [HR] = 2.59, 95% CI: 1.60-4.19) and low the forced expiratory volume in 1 s (FEV1 <80%) (HR = 1.85, 95% CI: 1.15-2.96) were independent risk factors for shorter survival. COPD was no impact on the treatment response of SCLC, but low FEV1 was predicting factors for poor clinical outcome.

        언어 경관 연구: 제주의 사례를 중심으로

        This study aims to investigate the languages used in linguistic landscapes in Jeju, Korea. It analyzes the frequency of using the various languages, the use of language by the type of business, the four part writing types(Homophonic, Mixed, Polyphonic, and Monophonic) Backhaus, 2007: 91 , and multilingual signs in linguistic landscapes in Jeju. Signs were collected in Chilseong-ro, Nuwemaru-gil, and Lee Jungseop-gil, the main urban streets in Jeju. According to the survey, English is mainly used in both monolingual and multilingual signs in the three areas. In the types of business, clothing uses English signs the most. In multilingual signs, Hangul and English are most often used together. There is a code preference between Hangul and English. When English is a dominant language, Hangul is less visible and mainly offers transliteration. When Korean is a dominant language, English mainly offers information. This study suggests that there is positive symbolism in English. The positive symbolism of English in Jeju has influenced on the change of linguistic landscapes.

      • F-74 Eosinophilia at stable COPD is correlated with eosinophilia at acute exacerbations

        Purpose: Peripheral blood eosinophilia has been suggested to be a useful marker of sputum eosinophilia during chronic obstructive pulmonary disease (COPD) exacerbations. In previous study, eosinophilic exacerbations experienced better clinical outcomes than did those with neutrophilic exacerbations in COPD patients. In stable COPD, airway eosinophilia is related with increased steroid responsiveness. However, the correlation of eosinophilic inflammation at stable COPD and exacerbations has not been investigated. Patients and Methods: This was a multicenter retrospective study conducted in six university affiliated hospitals in the Republic of Korea from 2010 to 2014. Blood samples were obtained at the time of acute exacerbations and stable disease at outpatient clinic before or after admission in COPD patients. For blood eosinophilia, a cut-off level of 2% was used. Results: Overall, 347 COPD patients with severe exacerbations were admitted to hospital were included. Of these, 207 and 130 patients were classified into the eosinophilic and non-eosinophilic groups based on serum eosinophilia at stable COPD. The proportion of males was higher in eosinophil group (P = 0.019). The proportion of eosinophilia at the time of AE was significantly higher in eosinophil group (34.8% vs. 20.0%, P = 0.003). There was a significant positive correlation of eosinophil counts between stable COPD and AE COPD (r = 0.156, P = 0.026) Conclusion: We show that eosinophilia at stable COPD was correlated with eosinophilia at acute exacerbations.

        김선(金䥧)의 <주행백수(舟行百首)> 연구

        With the publication of Damjeong Series(藫庭叢書), researches related to the Damjeong group became active. The world of poetry and literature by Kim Ryeo's brother, Kim Seon(金䥧), is also being newly illuminated. Among Kim Seon's various literary works, I particularly paid attention to 100 poems written while riding a boat on the Han River titled “Juhaeng Baeksu(舟行百首)”. It is inferred that this work was created on his way to Yeoju before the major criminal case of Gangicheon(姜彝天) took place in 1797. At that time, Kim Ryeo(金鑢)'s brother-in-law Lee Woo-shin(李友信) and his friend Kim Geon-soon(金建淳), who had been martyred in 1801, were living in Yeoju. His journey started from Seoul to Yeoju and traveled by boat on the Han River for 4 to 5 days. In particular, he has created many poems in the area of Wolgye(月溪), Yanggeun(楊根), and Yeogang(驪江). The composition of the series of 100 poems is as follows. First, it vividly captures the waterway of the Han River, especially the experience of a boat going up dangerous rapids. Second, it is a refreshing description of the commercial water transport of the Han River that was active at the time. Third, it captures the lives of the people along the Han River and expresses the romance of a boat trip. Fourth, it reveals his own conflict over entering the office and sighes at the impermanence of life. “Juhaeng Baeksu” is travel poems on the Han River and poems of folklore along the Han River. The tradition of creating a series of 100 poems in the form of o-eonjeolgu(五言絶句) on the Han River waterway seems to have been transformed using Tang poet Jeongi(錢起)’s “Ganghaengmuje(江行無題)”. Examples include Kim Seon's poems and Jung Yak-yong(丁若鏞) and Shin Pil-young(申弼永)'s travel poems on the Han River. 담정총서(藫庭叢書)의 출간으로 담정 그룹 관련 연구가 활발해진 가운데, 김려의 아우인 김선(金䥧)의 시문학 세계도 본격적으로 조명되고 있다. 김선의 다양한 문학 작품 가운데 특별히 <주행백수(舟行百首)>를 주목하였다. 이 작품은 김선이 1797년 강이천(姜彝天)의 옥사가 일어나기 이전 여주를 방문하는 길에 창작된 것으로 유추된다. 당시 김려의 동학이자 매형인 이우신(李友信)과 1801년 순교한 김건순(金建淳)이 여주에 살고 있었는데, 김선은 출사의 갈등을 겪으면서 한강의 수로로 이들을 방문하는 장쾌한 여행을 하였던 것으로 보인다. 김선의 여정은 서울에서 출발하여 여주에 이르는 한강 수로를 4~5일에 걸쳐 여행하였으며, 특히 월계-양근-여주 여강 일대에서 많은 시를 창작하였다. 오언절구 100수의 장편 연작시 구성을 살펴보면, 첫째 한강 수로의 형세, 특히 위험한 여울을 거슬러 올라가는 뱃길의 체험을 생생히 담은 작품들, 둘째 당시 활발하게 펼쳐진 한강의 상업적 수운(輸運)을 묘사한 작품들, 셋째 주행의 낭만과 민풍(民風)을 포착한 작품들, 넷째 진퇴의 고민과 인생의 무상함을 토로한 작품들로 이루어져 있다. 오언절구라는 짧고 간결한 형식을 자유롭게 이어나가면서 한강 수로 여행에서 김선은 보고 듣고 느끼는 다양한 체험들을 시작화 하였다. 이런 점에서 <주행백수>는 한강 기행시이면서 한강의 기속시이다. 한강 뱃길의 여행 체험을 절구 형식으로 100수의 연작시로 창작하는 전통은 당나라 전기(錢起)의 <강행무제(江行無題)>를 전례로 삼아 변용한 것으로 보인다. 김선의 <주행백수>를 비롯해 이후 정약용(丁若鏞)과 신필영(申弼永)의 한강 기행시가 그러한 예이다.

        조선 후기 장동(壯洞) 김문(金門)의 부자간 수창시(酬唱詩) 연구

        This paper examined the meaning and significance of the poetry exchanged between father and sons of Jangdong Kim's family. The father and sons of Jangdong Kim's family enjoyed writing poems together. Kim Suhang wrote poems while traveling or living with his sons in Yeongam, Cheolwon, and Jindo, the exile sites. He also wrote poems while traveling or living with his sons in Baegunsan Mountain and Sambuyeon, where his sons stayed. Kim Suhang’s six sons also inherited his father's poetic tendencies. Kim Changjip exchanged poems with his son Kim Jekyum at exile Geoje Island, Kim Changhyeop exchanged poems with his son Kim Sungkyum at Baekunsan Mountain, and Kim Changhyup exchanged poems with his son Kim Yangkyum at Baekdam and other places. In these poems, the fathers and sons sometimes became comfortable and pleasant companions, sometimes consoling each other. First of all, they became friends who wrote poems together. They naturally embodied Jin shi(眞詩) in the process of writing poems in earnest about the emotions and the scenes they felt while traveling through the mountains and about the feelings they felt when they spent their daily lives with their family. 본고는 부자(父子) 사이의 한시 수창(酬唱)이 매우 활발하였던 조선 후기 장동(壯洞) 김문(金門)을 대상으로 부자간 수창시의 양상과 그 의미를 살펴보았다. 김수항(金壽恒)은 유배지 영암, 철원, 진도에서, 또 둘째 아들 김창협과 셋째 아들 김창흡의 은거지인 백운산, 삼부연 등에서 함께 생활하거나 여행하면서 여러 아들과 활발하게 한시를 수창 하였다. 이러한 양상은 다음 세대로 이어졌다. 첫째 아들 김창집(金昌集)은 유배지 거제도에서 김제겸(金濟謙)과, 김창협은 백운산, 미호 등에서 김숭겸(金崇謙)과, 김창흡은 백담 등에서 김양겸(金養謙)과 한시를 수창 하였다. 부자가 산수를 여행하며 주고받은 대량의 산수기행시들을 통해, 장동 김문의 아버지와 아들은 때로는 편안하고 즐거운, 때로는 위로가 되는 삶의 동반자가 되었으며, 무엇보다도 부자는 좋은 시우(詩友)가 되었다. 이들 문인은 가족과 함께 일상을 나누는 정감과 산수를 직접 밟으며 보고 느낀 정경을 진솔하게 한시로 써나가는 과정에서 진시를 자연스럽게 구현해 나갔다.

      • TP-48 : Thematic Poster ; Analysis of Positive Lymph Node on CT or PET-CT by Using EBUS-TBNA in Pneumoconiosis Patients Suspicious of Lung Cancer

        Background: Pneumoconiosis patients often show false positive lymph node (LN) involvement by computed tomography (CT) or positron emission tomography (PET)-CT, which is difficult to distinguish from malignant involvement of lung cancer. Objective: The purpose of this study was to evaluate positive mediastinal LN on CT or PET-CT by using EBUS-TBNA in pneumoconiosis patients suspicious of lung cancer. Methods: Twenty-two pneumoconiosis patients who were suspicious of lung cancer between May 2009 and May 2013 were analyzed in the study. Results: All patients had at least one positive mediastinal LN on CT or PET-CT. Microscopic examination by EBUS-TBNA revealed dark pigmentations of LNs from all patients. Among 22 patients, 16 (72.7%) patients were diagnosed as lung cancer. Of these, only 7 patients were confirmed as malignancy or atypical cells at least 1 lymph node by EBUS-TBNA. A total of 42 LNs were analyzed. Of these, the number of pathologic confirmation as malignant, atypical and reactive LNs were 5 (11.9%), 3 (7.1%) and 34 (81.0%), respectively. The mean value of size and SUV were not different comparing malignant and atypical LNs with reactive LNs (11.93±4.24 vs. 10.04±3.82, P=0.718; 6.67±2.99 vs. 5.15±2.21, P=0.923). Conclusion: In patients with pneumoconiosis, mediastinal lymphadenopathy can be misdiagnosed by CT or PET-CT. The size of LNs and the SUV value in the PET-CT did not have an effect on differentiating malignant LNs from reactive LNs. Therefore, procedure like EBUS-TBNA should be considered to evaluate suspicious lymph node involvement.
