간암 환자에 합병된 Citrobacter braakii 패혈증 1례 보고

Citrobacter species is a gram-negative bacilli that can cause opportunistic infections in immunocompromised hosts. Citrobacter braakii refers to the genomospecies 6 of the Citrobacter freundii complex. There are no detailed studies on infections caused by this newly identified specific genetic species in Korea. We herein report a case of a patient with hepatocellular carcinoma who, after undergoing transcatheter arterial chemoembolization, developed biloma which later progressed to C.braakii sepsis and did not respond to treatment. To our knowledge, this is the first reported case in Korea on C. braakii infection resulting in septic shock in a patient with malignancy in Korea.

Culdocentesis is a procedure that checks for abnormal Quid in the space just behind the vagina (cul-de-sac). First, a pelvic examination is done. Then, after exposing the postehor vaginal fonix with a bivalve vaginal speculum the posterior lip of the cervix is grasped with a tenaculum. The cul-de-sac is then entered through the postehor vaginal wall with an 18-20 gauge needle with a syringe inserted. As the cul-de-sac is entered, suction is applied, and the intraperitoneal contents are aspirated. In the recent past, culdocentesis was commonly used in the evaluation of a vahety of common intraperitoneal conditions, principally, the hemorrhage associated with ectopic gestations and ruptured ovarian cysts the presence and/or drainage of purulent associated with pelvic inGammatory disease, and as a screening procedure to facilitate early diagnosis of ovarian neoplasms. Usually 16-18 gauge needle with a syhnge attached was used for culdocentesis. But we have experienced a case of therapeutic culdocentesis with 16 gauge angioneedle in patient of hemoperitoneum. So, report it with a bhef review of literature.

A highly sensitive and selective fluoresence high-performance liquid chromatography using 1-bromoace-tylpyrene as a pre-labelling reagent was applied to the determination of cinnamic acid and glycyrrhetic acid at p mol level. The optimum conditions for the derivatization such as concentrations of KOH, 18-crown-6, and 1-bromoacetylpyrene, reaction temperature and reaction time were investigated. The derivatives were separated on a reversed phase column using the mixture of acetonitrile, methanol and water as the mobile phase. The effluent was menitored by fluorometer. Linearities of calibration curve were obtained between 0.5 p mol and 2.0 p mol. The detection limit of cinnamic acid and glycyrrhetic acid was 0.05 p mol in a 20 ㎕ of injection volume.

ISDN User Part(ISUP) is a defined signalling information control function required a setup/release/supervisor control of circuit-switched calls, and made by monolithic structure(Single level : CCS level 4). A new type of ISDN User Part is hot issues recently. It is possible that it should have minimum impact on the control signalling system and should be modular and structured, against for circuit-switched using new services and broadband ISDN services as a packet mode service and a multimedia call. This paper describes the traditional Monolithic ISUP and the new design concept, the Separated ISUP.
중, 고등학생의 한국판 상태-특성 불안척도의 적용 연구

Objectives : This study aimed to investigate the validity and standardize the Spielberger State-Trait Anxiety Inventory (STAI) in middle and hi? school students and find the factors which influenced their anxiety. Methods : 1,033 middle and hi? school students were selected randomly from 7 regions around the country. The State-Trait Anxiety Inventory (STAI) was applied in all subjects and the Beck Anxiety Inventory (BAI) was applied with it to calculate the coexistence validity. To find the causes of adolescent anxiety, Beck Depression Inventory (BDI), Subjective Happiness Scale, The Satisfaction With Life Scale, Rosenberg Self-Esteem Scale (RES), Parent-Adolescent communication family inventories, In family social scene were all applied and correlation with STAI was investigated. Results : Reliability issues, such as internal coherence, the coherence between specific phobia and anxiety state were Satisfactory and the coexistence validity with BAI was high. The result of factor analysis shows that factor structures were similar to previous studies. High school students showed higher anxiety levels than middle school students, and girls higher than boys. For socio-economical factors, Poverty and poor grades related to higher anxiety. Among psychological factors, happiness and Self-esteem showed high correlation wi anxiety. Conclusion : The State-Trait Anxiety Inventory (STAI) showed high validity and reliability in middle and hi? school students and proved itself a reliable scale. When treating their anxiety, age, sex, socio-economical and psychological factors should all be considered.
치매 및 경도인지장애에 대한 인지선별검사 4종의 타당도 비교

Objectives The authors have evaluated the validity of four currently used screening tools of cognitive function in Korea for dementia and mild cognitive impairment in a community elderly population. Methods We screened 5,085 non-randomly sampled elderly over age 60 in the Gwangju city, Gyeonggi Province. Among those, 927 persons were examined further for evidence of dementia and mild cognitive impairment (MCI). A baseline questionnaire on sociodemographic characteristics and four major screening tests (K-MMSE, MMSE-K, MMSE-KC, and K-HDS) were administered. For the evaluation of validity, we have analyzed the ROC curve and calculated the sensitivity and specificity of each test at its optimal cutoff points. Results ROC analysis of the 3 MMSEs and K-HDS has shown that area under the curve (AUC) was between 0.897 and 0.923 for dementia. Sensitivity and specificity were 0.768 and 0.870, respectively, when the cutoff line was set at 17/18 in K-MMSE. The other two MMSEs had optimal cutoff lines at 16/17 with sensitivity of 0.768 and specificity more than 0.90. K-HDS had its optimal cutoff line at 18/19 with higher sensitivity (0.874) and lower specificity (0.755). Compared with dementia, the four screening tests showed poor validity on MCI, Sensitivity and specificity was in the range of 0.733-0.756 and 0.840-0.855, respectively. Conclusion The validity of the screening tools which are currently being used in Korea to screen for MCI was not significantly different among tests. Because of the difference in the population norm and z-score estimation, use of the common norm is advisable.
