A successful treatment of infants and children with congenital heart defects depends upon several factors: diagnosis must be accurate, the operative technique, including techniques of perfusion and myocardial preservation, must be suited to the needs of patients and intensive cares, including accurate anesthesia, must be applied through all stages of investigation and treatment, i.e. before, during and after the operation. Recently we performed a open heart surgery successfully for a six-years old girl who had suffered from frequent upper respiratory tract infection and mild to moderate exertional dyspnea with congenital heart defects (ventricular septal defect, Kirklin type II, with patent foramen ovale). The operation and postoperative course were very smooth. And now we report this with review of literatures.

산화막을 갖는 지르칼로이 산화의 흡착물 영향

The oxidation behaviors of Zircaloy in air are studied by measuring the weight gains of specimens. The oxidation study of Zircaloy in air is relatively few, comparing to the enormous amount of works of Zircaloy corrosion in water or steam. Researches about oxidation mechanism in air, exposured Zircaloy claddings to the LiOH which is added to the primary coolant, and to NaCl which is from the salty coasted wind have not been carried out yet. To depict integrities of these Zircaloy claddings of spent fuels, Zircaloy specimens produced by W/H were oxidised at the temperature range between 400℃ and 500℃ in the electric furnaces. 8 types of the specimens are prepared. ① etched Zircaloy, ② NaCl adsorbed, ③ LiOH adsorbed, ④LiF adsorbed, ⑤NaF adsorbed, and ⑥ KF adsorbed Zircaloy specimens on the pre-existing oxide. NaCl, NaF, KF, and NaF accelerate the oxidation rate of Zircaloy cladding in air. Care should be taken in the management of spent fuels not to be exposed to salty air. Adsorbates like NaF can be used as a oxidation enhancer in volume reduction of Zircaloy claddings.

외상환자의 중증도분류를 위한 Triage Score와 Triage-Revised Trauma Score의 비교

Background: The triage-revised trauma score(t-RTS) is used for triage in trauma patients in many countries. Recently, the triage score was developed as a new triage method for trauma patients in Korea. The objective of this study was to assess the accuracy of triage for the t-RTS and the triage score. Methods: The medical records of 1575 consecutive trauma patients visiting the Emergency Center, Masan Samsung Hospital, from March to August 1999 were retrospectively analyzed. The triage score and the t-RTS were taken from the charts completed by the emrgency room doctors and nurses. The accuracies of the two triage methods were compared by using the undertriage and the overtriage rates, receiver operating characteristic(ROC) curve analysis, and a correlation analysis with many physiologic, anatomical, biochemical, and mixed scores(Glasgow coma scale, injury severity score, TRISS, APACHE II and III scores, operation, survival, etc.). Results: The undertriage rates for the triage score and the t-RTS were 22.8% and 38.2%, respectively(p≤0.001), the overtriage rates were 6.3% and 3.1%, respectively,(p≤0.001), and the accurate triage rates were 91.3% and 91.7%, respectively. The areas under ROC curves for the triage score and the t-RTS were 0.871±0.033 and 0.800±0.039, respectively(95% confidence interval, p<0.001). In general, the correlations of the two triage methods with many physiologic, anatomical, biochemical, and mixed scores were moderately high, but there were no statistical differences between the correlation coefficients of the two triage methods for those scores. Conclusion: The triage score was equal or superior to the t-RTS for triage in trauma patients. The former was simpler and could be used easily by emergency medical personnel. Therefore, the authors conclude that the triage score can be used instead of the t-RTS for triage in trauma patients.

Permanent magnet brushless motors are increasingly being used in high performance applications. The advantage of using a BLDC motor is that is can be controlled to have the speed-torque characteristics similar to that of a permanent magnet DC motor. In addition, the BLDC motor has low ineria, large power to volume ratio, and low noise as compared with the permanent magnet DC servo motor having the same output rating. This paper is proposed the design method of Outer Roter BLDC for a blower fan by the magnetic equivalent method. In order to compute and estimate the performance parameter of Outer type BLDC, the dynamic model and charateristics analysis of BLDC is simulated by Matlab.

심내막상 결손증(Endocardial cushion defect)은 선천성 심장기형의 약 3% 정도를 차지하는 비교적 드문 질환이며, 복잡한 동반 기형을 나타내고 있고, 그 수술 방법에 있어서도 많은 논란이 있으며, 사망율도 높은 질환인 바, 저자들은 지난 1년간에 3예의 본 증례에 대한 수술 치험에서 술후 3∼6개월간에 만족할만한 임상적 추적 결과를 얻었기에 보고하는 바이다. In Pusan Paik Hospital, Inje Medical College, there were two complete endocardial cushion defects (ECD) and a partial one, which were successfully repaired in 1986. In a patient of partial ECD, primum atrial septal defect was closed with pericardial patch. In a patient of complete ECD, associated with secundum ASD, pulmonary stenosis and Down's syndrome, the atrial and ventricular septal defects were closed separately with bovine pericardium and Dacron patches respectively, and then pulmonary stenosis was relieved by transannular patch widening in addition to valvotomy and infundibulectomy. In another patient with complete ECD, small interventricular communication was closed with simple suture with pledget, and primum ASD closed with pericardial patch. In all patient, the mitral clefts were closed with three or four 5-0 Prolene interrupted sutures. Transient A-V dissociation developed postoperatively in two patients, and heart failure in complete ECD with Down's syndrome was overcome with medical treatment.

Aspergillus niger 글루코오스 산화효소의 유도 발현

Glucose oxidase catalyzes the oxidation of β-D-glucose to gluconic acid. It is necessary to be obtained maximum glucose oxidase activity because the enzyme is used commercially for various applications, in particular extracelluar glucose oxidase. For high activity of extracellular glucose oxidase, we used the enzyme from Aspergillus niger and YEP medium. Extracellular glucose oxidase activity as high as 7.5Uml^-1 was obtained with sucrose as carbon source and peptone as nitrogen source. The result of testing a various of medium condition was obtainable the highest enzyme activity in condition of culture medium of 1% sucrose(w/v), 2% peptone(w/v) and 1% yeast extract(w/v) at 25℃. Our work indicate an economically attractive process for enzyme production. In addition Aspergillus niger glucose oxidase was purified by DEAE-Sephacel chromatography. The activity of the enzyme proceeded at pH 5.5 and the effective substrate of the enzyme was only glucose.

To optimize the preparation process of Jeung-Pyun, an experiment was derived, using a central composite design, to find the optimal mixing conditions. The addition of Paecilomyces japonica powder, the volume of Tak-Ju and the second fermentation time were independent variables, with the pH, volume, color, sensory and texture properties of the Jeung-Pyun selected as response variables. As the Paecilomyces japonica powder and fermentation time were increased, the b values and volume after the second fermentation also increased. The color of the Jeung-Pyun became deeper with increases in the Paecilomyces japonica powder and fermentation time, and was deepest on the addition of 30% Tak-Ju. The pH increased with increasing fermentation time of the Jeung-Pyun, and on the addition of 3% Paecilomyces japonica rapidly increased more than with the other groups. The optimal mixing conditions for the best quality Jeung-Pyun, with restricted conditions above a b value of 3, a color intensity above 3, and a volume of 40㎖ and a pH of 4.02~4.04 after the second fermentation, were 4.0%, 20% and 45 mins for the Paecilomyces japonica powder, Tak-Ju and second fermentation time, respectively.

결절성 황색종과 동반된 제 Ⅲ 형 과지질단백혈증 1예

We report a case of type Ⅲ hyperlipoproteinemia associated with tuberous xanthoma, which is improved by treatment with lovastatin and bezafibrate. A 34-year-old female patient visited our clinic for evaluation of multiple yellowish-brown papulonodules on the kness, elbows and palms. Serum lipid examination showed an abnormal elevation of serum cholesterol and triglyceride. Lipoprotein electrophoresis showed a single peak at pre-β and β portion without separation. The biopsh specimen from a nodule on the knee and a papule on the palm revealed characteristic findings of xanthoma. We diagnosed the case as type Ⅲ hyperlipoprotei-nemia. After 3 months of treatment with diet restriction, lovastatin and bezafibrate, the serum levels of cholesterol and triglyceride were reduced to normal level. The skin lesion of the palm showed complete regression and ones on both knees and elbows were moderate improvement.
