      • 심한 골병변으로 발현된 기능성 낭종성 부갑상선 선종

        낭종성 부갑상선 선종과 심한 골병변을 동반한 부갑상선 기능항진증은 매우 드문 질환으로서, 저자들은 양측 고관절의 통증을 초기 주소로 내원한 환자에서 고칼슘혈증과 부갑상선 호르몬 증가, 골병변의 방사선적 소견을 통해 부갑상선 기능항진증을 진단하고, 경부 초음파와 컴퓨터 단층 촬영, 부갑상선 스캔검사 및 수술중 부갑상선 낭종액 검사 등을 통해 기능성 부갑상선 낭종의 한 종류인 낭종성 부갑상선 선종을 진단하고 수술적 제거를 통하여 정상화된 1예를 경험하였다. A cystic parathyroid adenoma is rare. A case of primary hyperparathyroidism, with the cystic formation of a parathyroid adenoma and a severe bony lesion, is reported. A 52-year-old male was admitted due to pain in both hips and for evaluation of hypercalcemia. The plasma level of the intact parathyroid hormone (iPTH) was elevated to 1424 pg/mL. Ultrasonography and the computed tomography revealed a parathyroid cyst on the left thyroid lower pole. Parathyroid scintigraphy detected a parathyroid adenoma. A radiograph showed a subperiosteal bone resorption on the phalanges, and a brown tumor (osteitis fibrosa cystica) on the femur shaft was noted. A surgical excision of the parathyroid adenoma was performed. The PTH level in the cystic fluid was increased. A histological examination confirmed a cystic parathyroid adenoma. The PTH level was normalized after the operation (J Kor SOC Endocrinol 18:214-220, 2003).

        The purpose of this study were to develop Knowledge Management System for the nurse and to identify its effectiveness. The program was developed in development of the contents and programing. And the system was operated to 85 nurse for three weeks. Data collection were performed during the period of May 1, 2005 to December 30, 2005. The data was collected from 85 nurses using questionnaires. In order to ascertain the validity of clinical application of the system, Prior experience of the computer mediated communication, user satisfaction and utilization were analyzed. The instruments for the study were satisfaction(Kim, 2000), utilization(Kim, 2002). The collected data were analysed by descriptive statistics, using SPSS WIN 11.0 program. The results showed average score which is the satisfaction(6.09) showed middle satisfaction scores. It is concluded that if Nursing Knowledge Management System is operated considering all the above factors, It wound be one of the best qualified continuing clinical method for nurses.

      • 한국인 제2형 당뇨병 환자의 골격근에서 인슐린 신호전달체계의 결함

        연구배경: 제2형 당뇨병에서 나타나는 인슐린 저항성은 간, 지방, 근육 같은 말초조직에서 인슐린의 작용이 떨어지는 것을 말한다. 제2형 당뇨병의 발생기전에 인슐린 저항성과 인슐린 분비능 저하가 같이 관여함은 잘 알려진 사실이다. 인슐린의 세포내에서의 저항성을 규명하기 위한 인슐린의 세포내 신호전달체계에 대한 연구가 활발히 진행되고 있으며 몇몇 연구에서 인슐린 저항성을 가진 골격근육에서 IRS와 관련된 PI3-kinase의 활성감소와 Akt kinase의 활성감소를 보고하고 있으나 아직까지 명확하게 그 기작이 설명되어지지 않고 있다. 본 연구에서는 정상성인, 제2형 당뇨병 환자를 대상으로 경구당부하검사 및 인슐린 클램프검사를 시행하고 인슐린 클램프 검사시 대상인의 근육을 채취하여 인슐린 신호전달 체계(IR-β, IRS, Akt(PKB, Rac) kinase, GSK-3)를 연구하였다. 방법: 연구대상자는 한국인으로서 경구당부하검사상 정상인 및 당뇨병 환자 각각 11명, 9명을 대상으로 하였으며, 정상인은 건강인으로 과거력상 당대사에 영향을 줄 질환이 없고, 현재 당대사에 영향을 줄 약물 복용 및 다른 소견이 없는 경구당부하검사상 정성 내당능을 보이는 대상자로 하였다. 당뇨병환자는 모두 제2형 당뇨병 환자로 이환기간이 만 5년 이내인 경우로 하고, 인슐린으로 치료하는 대상자는 제외하도록 하였다. 대상자의 연령, 체질량지수, 체지방량, 공복시 총콜레스테롤, 중성지방, 고밀도지단백 콜레스테롤 및 HbA1c, 인슐린, C-peptide를 측정하였고, "HOMA model"을 이용하여 베타세포의 기능와 인슐린저항성 정도를 평가하였다. 정상혈당클램프 검사(euhlycemic hyperinsulinemic clamp test)를 시행하고 인슐린 투입 전과 인슐린 투입 30분후, 두차례에 걸쳐 대퇴부 근육생검을 실시한후 western 법으로 IR-β, IRS, Akt kinase, GSK-3의 인산화량을 측정하였다. 결과: 정상인 11명과 당뇨병 환자 9명을 대상으로 하였으며 대상자의 평균 연령, 평균 체질량지수, 체지방량, 허리/엉덩이 둘레의 비(waist hip ratio; WHR)는 두 군간에 차이가 없었다. 평균 공복혈당은 정상군 98.0±0.3㎎/dL, 당뇨병군 208.1±16.5㎎/dL(p<0.05), HbA1c는 정상군 5.4±0.5%, 당뇨병군 9.2±0.6%(p<0.05)였다. "HOMA model"을 이용한 베타세포의 기능과 인슐린저항성은 정상군 56.4±8.5%, 1.4±0.2, 당뇨병군 72.2±52.3%(p<0.01), 10.2±6.3(p<0.01)였으며 정상혈당클램프 검사상 포도당 이용률은 정상군 8.2±0.6㎎/㎏/min, 당뇨병군 3.7±1.1㎎/㎏/min(p<0.01)로 정상군과 당뇨병군간에 유의한 차이를 나타내었다. Western blot법으로 IR-β, IRS, Akt kinase, GSK-3의 인산화량을 인슐린 투입전과 인슐린 투입 30분후 측정하였을 때 IR-β에서는 정상군이 103.9±2.3에서 241.3±18.6, 당뇨군이 108.9±2.2에서 198.7±6.3으로 증가하였다(p=NS). 이는 인슐린 투입후 당뇨병군이 정상군과 비교하여 인산화량의 증가정도가 통계적인 차이가 없었다. IRS에서는 정상군이 111.6±7.3에서 295.6±17.2, 당뇨군이 114.5±6.1에서 222.0±23.2로 증가하였으며(p<0.05), 인슐린 투입후 당뇨병군이 정상군과 비교하여 인산화량의 증가정도가 약 24% 감소하였다. Akt kinase에서는 103.4±6.0에서 416.8±29.5로 증가하였으며(p<0.01), 인슐린 투입후 당뇨병군이 정상군과 비교하여 인산화량의 증가정도가 약 43% 감소하였다. GSK-3에서는 정상군이 107.7±6.7에서 595.7±28.1, 당뇨군이 104.3±4.8에서 443.3±12.9로 증가하였으며(p<0.01), 인슐린 투입후 당뇨병군이 정상군과 비교하여 인산화량의 증가정도가 약 25% 감소하였다. 결론: 제2형 당뇨병 환자에서 정상인과 비교하여 인슐린 자극후 IR-β는 인산화량의 증가정도가 큰 차이가 없었으나, IRS, Akt kinase, GSK-3에서는 인산화량의 증가 정도가 감소되는 것으로 보아 상위 인슐린 신호전단체계부터 결함이 있는 것으로 생각된다. Background: The glucose uptake rate is the limiting step in glucose utilization and storage. The failure of insulin to stimulate glucose uptake in muscle appears to be a primary defect of insulin resistance. This study was undertaken to examine the effect of physiological hyperinsulinemia on the phosphorylation of the insulin receptor (IR-β), insulin receptor substrate (IRS), Akt kinase and GSK-3 in isolated skeletal muscle, in people with type 2 diabetes(n=9) and control subjects(n=11). Methods: 75g OGTT and euglycemic hyperinsulinemic clamp test were done. And vastus lateralis muscle was obtained before and 30 min into the euglycemic clamp. Western blots were performed for tyrosine phosphroylation of insulin receptor substrate (IRS) and phosphorylation of the insulin receptor(IR-β), Akt and GSK-3. Result: There were no statistical differences in the mean age, BMI and body fat between the control subjects and diabetic patients. The fasting blood sugar and HbA_1c in controls and diabetic patients were 98.0±0.3 and 208.1±16.5ng/dl, and 5.4±0.5 and 9.2±0.6%, and 1.4±0.2 in the control subjects, and 72.2±52.3%(p<0.01) and 10.2±6.3(p<0.01) in the diabetic patients, respectively. The insulin resistance from the euglycemic hyperinsulinemic clamp test were 8.2±0.6㎎/㎏/min and 3.7±1.1ng/㎏/min in the control subjects and in the diabetic patients, respectively(p<0.01). Compared with the normal controls, insulin-stimulated IR phosphorylation was no different to that in the diabetic patients. However, insulin-stimulated IRS phosphorylation, insulin-stimulated Akt phosphorylation and insulin-stimulated GSK-3 phosphorylation were reduced in the diabetic patients compared with the normal controls by 24, 43 and 25%, respectively(p<0.05). Conclusion: In Korean type 2 diabetic patients, the insulin resistance may be due to the impairment of the upstream insulin signal molecular network. Further studies will focus on determining whether these signaling defects are the cause of the development of insulin resistance, or secondary to the altered metabolic state, associated with type 2 diabetes mellitus (J Kor Soc Endocrinol 17:685∼697, 2002).

        In order to obtain the basic data for artificial maturity control of 'Mibaek' peach, GA and ethephon were foliar sprayed 4 weeks prior to harvest, and their effects on fruit maturity and quality were examined. GA and ethephon tended to decrease the fruit weight except 100mg ㆍ L-1 GA+50 to 100mg · L-1 ethephon treatment. GA increased the fruit firmness, and ethephon at higher concentration increased the fruit drop. Harvest date was advanced for 7 to 8 days by ethephon while was delayed for 6 to 8 days by GA treatment, regardless of concentration.

        사람폐암세포주 (PC-14)에서 Cyclosporin A에 의한 Adriamycin 내성의 극복

        Cyclosporin A and verapamil were tested using MTT assay to evalute the modification effect on the resistance to adriamycin in a human lung cancer cell line(PC-14) and its resistant subline(PC-14/A). PC-14/A was derived by the continuous exposure of PC-14 to incremental concentrations of adriamycin. PC-14/A was 2.5 times more resistant to adriamycin in terms of ICso than PC-14. Cyclosporin A alone, at a concentration of 2.5㎍/㎖, inhibited the growth of PC-14 to 68.3%. 2.5㎍/ ㎖ and 5.0㎍/㎖ of cyclosporin A showed an increase in the cytotoxicity of adriamycin (p<0.01) with 5.0㎍/㎖ being greater than 2.5㎍/㎖(p<0.01). Excluding the direct cytotoxic effect, however, cyclosporin A did not increase in the sensitivity of PC-14 to adriamycin but only showed an additional cytotoxic effect with adriamycin. Verapamil (up to 6.0㎍/㎖) did not inhibit the growth of PC-14. 3.0㎍/㎖ of verapamil did not increase the cytotoxic effect of adriamycin. The combination of cyclosporin A and verapamil with adriamycin enhanced the cytotoxicity of adriamycin, but the result was similar to that of cyclosporin A with adriamycin. 5.0㎍/㎖ of cyclosporin A modified the adriamycin resistance of PC-14/A(SR, 3.2). However, 3.0㎍/㎖ of verapamil did not significantly reverse the adriamycin resistance of PC-14/A. The modified effect of the combination of 5.0㎍/㎖ of cyclosporin A and 3.0㎍/㎖ of verapamil was similar to that of 5.0㎍/㎖ of cyclosporin A alone in PC-14/A. These results demonstrate that cyclosporin A has an additional cytotoxic effect with adriamycin in PC-14 and PC-14/A and has overcome the acquired resistance to adriamycin in PC-14/A. They also suggest that cyclospoin A may have the therapeutic potential in the treatment of human lung cancer.

        Over the decade, rapid progress has been made in the application of gene manipulation technology to aqaculture for the genetic improvement of aquacultured species. Two basic fields of genetic improvement involve the endocrinal control such as control of spawning seasons, induced and multiple spawning and cryopreservation of gametes, and the gene manipulation using chromosome engineering, sex reversal, hybridization, selective breeding and gone trasnfer techniques. Chromosome engineering of induced polyploidey, gynogeneris and androgenesis have been extensively investigated in fish. The major goal of thease stueies has been to develop methods for the incressing productibility by controlsex and by rapid establishment of inbreeding lines. Sex reversal techniques has been applied to the species which has sex related significant differences in growth, behavour pattern, breeding time, body shape and size etc. so, culture of monosex population is highly desirable. Pyusiological sex reversal by hormonal treatment or genetic sex reversal by matings between specipic genotypes have been carried out. Hybridization in fish are performed in the hope of buliding the desirable characters, according to some parameters such as viaility, growth potential, sexual maturation and disease resistance. Maternal or paternal genomes can be manipulated before and/or after hererospecipic inseminations. Selective breeding has been carried out to improve the performances or obtain fish products most desirable to the producer by modifying genotypic frequences using specific mating system such as inbreeding or outbreeding and by changing alle frequences toward ones more favorable to the beeder's golds through accumulation of the favored alles. Gene transfer technology has been introduced, aimed at the production of transgenic fish showing the improved charateristics and performances such as enhanced growth, disease resistance and cold resistance in their environment. Researches of transgenesis in fish are focused on the identification of desiable piscine genes, development of expression vector, optimization of introducing techniques, stable expression and transmission to germ line.

      • 한국 청소년에서 혈청 Adiponectin 농도와 비만도와의 상관관계

        연구배경: Adiponectin은 지방세포에서 특이적으로 발현되고 분비되는 adipocytokine이다. 아직 adiponectin 과 소아 비만과의 관련성을 연구한 보고는 많지 않다. 본 연구에서는 한국 청소년을 대상으로 혈청 adiponectin농도와 비만도 및 심혈관계 질환의 위험인자들과의 상관관계를 보고자 하였다. 방법: 2003년 4월 서울특별시 서대문구 소재의 한 중학교에서 12∼15세의 무작위 선발된 164명을 연구 대상으로 하여 신장, 체중, 허리둘레, 체지방을 측정하고 체질량지수를 계산하였다. 또한 공복채혈을 통하여 콜레스테롤, HDL-콜레스테롤, 중성지방, 혈당 및 인슐린 농도를 측정하고 혈청 adiponectin 농도는 ELISA 방법을 이용하여 측정하였다. 결과: 혈청 adiponectin 농도에 따라 삼등분하여 저 adiponectin군, 중간 adiponectin군, 고 adiponectin군으로 나누고 각 군간에 신체계측치 및 임상적 특성을 비교 분석하였다. 저 adiponectin군에서 다른 군에 비해 체질량지수와 허리둘레가 의미있게 크게 나타났다. 또한 저 adiponectin군에서 공복혈당 및 공복혈청 인슐린 농도가 높게 나타났으며, HOMA-IR 값도 높게 나타났다. 저 adiponectin군에서 총콜레스테롤과 중성지방치는 높게, 그리고 HDL 콜레스테롤치는 낮게 나타났다. 다변량회귀분석을 시행한 결과 혈청 adiponectin농도와 가장 상관관계가 높은 변수는 허리둘레와 성별로 나타났다. 결론: 소아에서도 성인과 마찬가지로 혈청 adiponectin농도와 복부비만 및 심혈관계 질환의 위험인자들과의 강한 음의 상관관계를 보임을 알 수 있었다. 향후 사춘기 성호르몬의 변화가 adiponectin에 미치는 영향 및 소아비만의 치료에 따른 adiponectin의 변화 등에 대한 연구가 필요하리라 사료된다. Background: Adiponectin is an adipocytokine that is highly specific to adipose tissue. In contrast to other adipocytokine, the adiponectin levels are decreased in obesity and/or type 2 diabetes. There are few studies regarding the correlation between the adiponectin concentration and obesity in children. Thus, whether the serum adiponectin concentrations are associated with adiposity in children was investigated. Methods: One hundred and sixty four subjects were selected from the participants in an ongoing study on the relationship between birth weight and insulin resistance in children. The current weights, heights, body fat percentages, waist circumferences, blood pressures, lipid profiles and insulin resistance, by the HOMA method, were measured in all the subjects. The serum adiponectin concentrations were determined by a validated sandwich ELISA, using a human adiponectin-specific antibody. Results: The serum adiponectin concentration was negatively correlated with the body mass index, waist circumference, systolic blood pressure, insulin resistance by HOMA and serum triglycerides, and positively correlated with the serum HDL cholesterol level. The serum adiponectin concentrations in the boys were significantly lower than in the girls. In a multiple regression analysis, the serum adiponectin concentration was strongly associated with waist circumference and gender. Conclusion: It is concluded that there was an inverse relationship between the serum adiponectin concentration and abdominal adiposity in children. However, further studies on independent gender differences on adiponectin are needed (J Kor Soc Endocrinol 18:473∼480, 2003).

        In football games of late, owing to the development of 'tactics', 'strategy' and skillful defense ability, it grows very hard to get a goal, so 'the draw' is very freguent. As a result, penalty kick becomes often the core of victory or not. Accordingly, Which team can get more goals in penalty kick becomes exculusive victory factor in modern football games. After the comparative studing the accuracy and goal-making in case of 'Inside-kick' and 'instep-kick' which is ofter used in each game, the concussion is as fallows : 1.In accuracy-survey which target is used in penalty kick (in case of 'open-skill'), in side-kick is much more superior and accurate to Instep-kick. 2.In case of wrong-direction from the target A, Instep-kick gets more frequent miss kicking than Inside-kick. Miss-kicking ratio 33 miss kicks armong 310 kicks in Instep kick 72 miss kicks armong 310 kicks in Inside kick 3.Goal-king ratio in close skill 263 successful kicks armong 310 kicks in Inside kick 243 successful kicks armong 310 kicks in Instep kicks As showing the above, there seems be only 20 goals-difference at a glance but the difference is very meaningful considering many games are decided only by one goal-difference, 4.at the situation of facing with the goal keeper, the heason why there is not any conspiquous difference between Inside kick and Instepkick is that Instepkick can be kicked comparably with ease from the four parts of both corners and Instepkick has a lot of grounders when the foot is swing it is dynamically easy to kick the ball to the ground with pushing the sole of the foot. The reaction time of goal keeper may be considered with the advantage which Instepkick is not to be easily exposed to the goal keeper and easiness which goal keeper can defend the ball because the movement to down part is shorter than that in upper corner part. 5.In penalty-kick, through this survey, psycholgatical factor kicks works as improtant varialiles. As a resilt, being unconscious of goal-keeper, if one shoot the ball against target B with intensity and accuracy, lie gets a goal more easily.

      • 2-microphone method을 이용한 자동차용 내장재의 임피던스 측정 실험

        Sound absorption and impedance are two important parameters in identifying porous materials' behavior. To reduce the interior noise of automobiles effectively, the above parameters should be known in detail. Impedance tube method with two microphones is performed to obtain the impedance of absorbing materials. By switching two microphones, more accurate impedance results are obtained. Flow resistivity, one of four Biot's parameters, is measured and by applying Delany-Bazley's law on the measurement results, impedance values are calculated. Impedance results from both the impedance tube method and flow resistivity with Delany-Bazley's law are compared with and show reasonable results in certain frequency bands. The computer simulation with SYSNOISE is also conducted to verify the impedance measurement results.

      • 테니스 포핸드 발리와 백핸드 발리의 운동학적 분석

        This study is to analyze the kinematic variables of forehand and back- hand volley in tennis. Five male tennis players are used as subjects, two S-VHS video cameras set in 60frames/sec were used for recording volley motion. Coordinated raw position data through digitizing are smoothing by low-pass Butterworth filtering method at a cut off frequency 6Hz, and the direct linear transformation(DLT) method was employed to obtain 3-D position coordinates. The conclusions were as follows; 1.In the swing time, forehand volley was longer than backhand volley in phase1, but forehand volley was shorter than backhand volley in phase2, forehand volley was shorter than backhand volley in the total swing time. 2.Backhand volley was longer than forehand volley in displacement of Y axis direction and the height of CG was similar to the forehand and backhand volley in total swing phase. 3.At the point of impact, the resultant velocity of racket head was similar to the forehand and backhand volley, and the maximal resultant velocity of racket head occurred before impact. 4.In the angle of the frontal trunk, the wrist and the knee, forehand volley was larger than backhand volley in each event, and each segment angle increased before impact to except the wrist angle of backhand volley. 5.In the angular velocity of the wrist, forehand volley was positive addition and backhand volley was negative addition at impact.
