        Transcatheter arterial chemoembolization (TACE) has become an effective alternative treatment strategy for patients with inoperable hepatocellular carcinoma (HCC). Although TACE is relatively safe, acute respiratory distress syndrome associated with pulmonary lipiodol embolism is a rare and potentially fatal complication. We report a rare case of acute respiratory distress syndrome after TACE for inoperable HCC. A 75-year-old man, with huge HCC in right lobe, was treated by TACE for the first time. Seven hours after uneventful TACE procedure, he felt dyspneic and his oxygen saturation recorded by pulse oximetry (SpO2) fell to 80% despite of applying non-rebreathing mask. He underwent mechanical ventilation with a protective ventilatory strategy. We experienced a case of acute respiratory distress syndrome after TACE for HCC.

        고주파수 근사 이론을 이용한 결함으로부터의 초음파 산란장 해석

        본 논문에서는 체적형 결함과 균열형 결함에 대한 초음파 산란 현상을 모델링하기 위한 두 가지 이론을 설명하였다. 동탄성 Kirchhoff 근사 (EKA)와 기하학적 회절이론 (GTD)이 각각 원주형 기공과 반무한 균열에 적용되었다. 이 두 이론은 고주파수 근사법으로 알려져 있다. 모델 결함들에 평면파가 입사하는 경우의 2차원 동탄성 산란 문제를 고려하였으며 산란장을 반사계수와 회절계수의 항으로 구하였다. 원거리에서 산란파의 변위에 대한 입사파 변위의 비를 관찰 방향의 함수로 구했으며 그 결과를 경계요소법과 비교하였다. This paper describes two different theories used to model the scattering of ultrasound by a volumetric flaw and a crack-like flaw. The elastodynamic Kirchhoff approximation (EKA) and the geometrical theory of diffraction(GTD) are applied respectively to a cylindrical cavity and a semi-infinite crack. These methods are known as high frequency approximations. The 2-D elastodynamic scattering problems of a plane wave incident on these model defects are considered and the scattered fields are expressed in terms of the reflection and diffraction coefficients. The ratio of the scattered far field amplitude to the incident wave amplitude is computed as a function of the angular location and compared with the boundary element solutions.

        비선형 음향효과에 의한 고온 재료의 파괴인성 열화도 평가

        고온 장시간 사용재의 열화로 인한 파괴인성의 저하를 초음파 방법으로 평가하기 위하여 Cr-Mo-V 강을 실험실에서 인공 열화처리하여 열화로 인한 손상 기구를 규명하고, 열화도에 따른 초음파 파라미터의 변화를 체계적으로 관찰하였다. 초음파 파라미터로서 속도, 감쇠와 보다 최근에 개발된 비선형 음향 파라미터를 사용하였는데, 비선형 음향 파라미터가 가장 민감한 변화를 보였다. 또한 전기비저항 측정 결과와 비교할 때 두 방법 모두 전체적으로 유사한 거동을 보였다. Charpy V-notch 충격시험으로부터 구한 파면양상 천이온도(FATT)와 비선형 파라미터 사이에는 비교적 좋은 상관관계가 성립하였으며, FATT와 파괴인성(KIC) 사이의 관계식에 근거하여 열화된 발전설비 재료의 파괴인성을 초음파 비선형 파라미터의 측정을 통하여 간접적으로 구할 수 있음을 보였다. In order to develop an ultrasonic evaluation method for properties degradation of high temperature materials, a number of Cr-Mo-V steel samples were heat-treated, and their damage mechanism was examined. Ultrasonic parameters such as velocity, attenuation, and more recently developed nonlinear acoustic parameter were measured. The nonlinear acoustic parameter was found to be most sensitive to material degradation mainly attributed to the precipitation of impurities in grain boundaries. When compared to the electrical resistivity results, the nonlinear parameters showed similar behavior. There existed a relatively good correlation between the nonlinear parameter and the fracture appearance transition temperature (FATT) obtained by Charpy V-notch impact test. Based on the relationship between the FATT and the fracture toughness (KIC), correlation between the nonlinear parameter and KIC was established.

        Tumor-infiltrating lymphocytes ('1°ILs) interact most closely with tumor cells and thus are more likely to reflect tumor host interactions accurately. But it is unknown whether such T cells are nonspecific inflammatory cells or a subset of specific host immune responses. In this study, there was no clear correlation between the infiltration of T lymphocytes in stomach cancer and the overexpression of c-ErbB-2 or increasing class I MHC expression on tumor cells. A positive correlation was seen between the presence of TILs in the tumor and tumors with diploidy by flow cytometric DNA analysis. The proliferative responses of Ills stimulated with IL-2, anti-CD3 mAb, or both were examined. When compared to normal mucosal-associated lymphoid tissue lymphocytes, the proliferative response of TILs to high dose IL-2 was minimal. A similarly poor response to anti-CD3 mAb plus IL-2 was also observed. The freshly isolated TILs exhibit reduced ability to proliferate in response to IL-2, anti-CD3 mAb or both. The microenvironment of the tumor suppresses the proliferative capacity of the TILs. The mechanism of this suppression remains unknown. It could be mediated by suppressor cells, by soluble substances within the tumor, or both. To examine this question, supernatants of stomach cancer cells (SNSNU-1) were tested for the presence of immunosuppressive factors. Human peripheral blood T-cells and tumor-draining lymph node lymphocytes (TDLNL) were incubated for 3 days with SNSNU-1 and then assessed for proliferative responses to PMA, anti-CD28 mAb, or both and for the inducibility to express IFN- r or IL-4 mRNA to PMA. Peripheral blood T-cells pretreated with SNSNU-1 were unable to proliferate in response to PMA, anti-CD28 mAb or both. SNSNU-1 also produces inhibitory activities of TDLNL proliferative response to PMA or anti-CD28 mAb and PMA (49%, 52%, respectively). In contrast, culture supernatants obtained from HEp-2, K562 or Daudi showed normal proliferative responsiveness of peripheral blood T-cells and TDLNL by PMA, anti-CD28 mAb or both.

      • 결핵균 30 kDa 항원과 Triton X-100 Solubilized Protein 항원에 의한 대장암 주변 림프절 단핵구의 활성화

        Tumor-draining lymph node mononuclear (TDLMN) cells are specifically sensitized to the growing tumor but such cells are deficient for mediating an antitumor response. In this study, we examined the feasibility of using mycobacterial 30 kDa or Triton X-100 solubilized protein (TSP) antigen to stimulate mononuclear cells of colon cancer-draining lymph node for the generation of cell mediated immune effector cells. The proliferative response of TDLMN cells stimulated with mycobacterial 30 kDa or TSP antigen was determined by ^(3)H-thymidine incorporation assay. The proliferation of TDLMN cells to mycobacterial 30 kDa or TSP antigen was significantly increased in PPD (+) patients, but a poor response to the 30 kDa or TSP antigen was observed in PPD (-). The expression on γδ T cells to mycobacterial 30 kDa or TSP antigen was assessed by flow cytometry. The γδ T cells from PPD ( + ) patient responded only to 30 kDa antigen but to TSP antigen. An investigation of cytokine mRNA expression was undertaken using reverse transcription-polymerase chain reaction (RT-PCR) to follow TDLMN cells stimulated with the 30 kDa or TSP antigens for 5 days. The IFN-γ and TNF-α mRNA expression was only induced in TDLMN cells of PPD ( + ) patient in response to the 30 kDa or TSP antigen. The IL-2 mRNA expression was induced in both PPD (+) and PPD (-) in response to the 30 kDa or TSP antigen. But the IL-4 mRNA expression was not induced in response to the 30 kDa or TSP antigen. These results suggest that the 30 kDa and TSP antigens may serve as biologic response modifier for the generation of cell mediated immune effector cells.

      • HMG-CoA 환원효소 억제제에 의한 ICAM-1 유전자의 발현조절

        Background : ICAM-1 act as one of major adhesion molecules in the atherosclerotic lesion. ICAM-1 expression is mainly regulated at the level of transcription and depend on IFN-γ signal transduction pathway in which the STAT1 transcrption factor is a critical intermediate. IFN-γreceptor not only initiates tyrosine 701 phosphorylation of STAT1 by Jak1 and Jak2, but also phosphorylates serine 727 through the activation of Raf-1/MAP kinases. HMG-CoA reductase inhibitors have anti-atherosclertic effects, beyond normalization of hypercholesterolemia, by directly acting on endothelial cells, macrophages and vascular smooth muscle cells. HMG-CoA reductase inhibitors suppress the synthesis of isoprenoid intermediates such as geranylgeranyl-pyrophosphate or farnesylpyrophosphate. These effects results inhibitors suppress the synthesis of isoprenoid intermediates such as geranylgeranyl-pyrophosphate or farnesylpyrophosphate. These effects results inhibition posttranslational farnesylation and geranyl-geranylation processing of small GTP-binding preoteins and inhibition of normal signaling activities. Method : We made several 5'-deletion constructs of rat ICAM-1 promoter and analyzed the promoter activities by measuring the luciferase activity after transfection into ECV304 cells and smooth muscle cells. We checked the level of total and phosphorylated STAT1 protein by immunoblot analysis using specific antibodies. Results : Lovastatin inhibits IFN-γ-induced ICAM-1 gene expression in the ECV304cell. The cells pretreated with PD98059, MEKK inhibitor showed significantly low ICAM-1 RNA induction with IFN-γ stimulatio. IFN-γ induced phosphorylation of tyrosine 701 was not significantly changed by the pretreatment of lovastatin. But lovastatin suppresses IFN-γ-induced phosphorylation of ERK1/ERK2 which are responsible for the seine 727 phosphorylation in STAT1. Conclusion : We showed that HMG-CoA reductase inhibitors, lovastatin, suppresses IFN-γ mediated ICAM-1 gene expression through the inhibition of transcription. HMG-CoA reductase inhibitor suppresses IFN-γ-induced phosphorylation of serine 727 in STAT1 through the modulation of MAP kinases.

        The various clinical features of tuberculosis are mediated by diverse cytokines produced by various immune cells which are initially triggered by M. tuberculosis antigens. CD4+ T cells can be classified into two subsets according to the patterns of cytokines they produce; Thl cells give rise to cell-mediated immunity and are characterized by the production of IL-2 and IFN-y, whereas Th2 cells are more efficient in mediating antibody production and secrete II-4, IL-5, IL-6 and II-10, Thl cells can control Th2 cell and vice versa. Peripheral blood mononuclear cells of healthy, cured and chronic refractory tuberculosis patients were stimulated with PPD, TSP and PHA antigen, and lymphoproliferative response and expression of II-2, IL- 10, TNF-α mRNAs were measured. Lymphoproliferative responses to PPD, TSP and PHA antigen were depressed in chronic refractory case compared with others expression of IL-2 mRNA was depressed in chronic refractory case stimulated with all antigens. Expression of IL-10 and TNF-α were depressed in cured and chronic refractory cases stimulated with PPD and TSP antigens.
