A Multi-Center Educational Research Regarding Breastfeeding for Pediatrics Residents in Korea

Purpose: Pediatricians have a significant responsibility to educate mothers about the importance of breastfeeding. However, there have been minimal efforts in the courses of resident training in Korea. The purpose of this study is to evaluate the change in knowledge and attitude before and after a 4-week breastfeeding educational intervention among multicenter residents. Methods: Prospective interventional educational research was designed for residents at eight training hospitals in Korea. Institutional reviews were obtained in each hospital. The education curriculum consisted of 14 courses regarding breastfeeding theory and practice. These materials were used to teach pediatric residents for 4 weeks. Knowledge-based tests were administered before the course, and re-tests were administered after the course using different test items of similar levels. Test scores and survey responses were compared before and after the intervention. Results: A total of 73 residents (1st year 20, 2nd year 23, 3rd year 16, and 4th year residents 14) from eight training hospitals completed the intervention. Their average age was 30.3±2.9 years, 17 (23.3%) were male, 22 (30.1%) were married, and eight had more than one child of their own. The mean pre-test score was 61.8±13.4 and the mean post-test score was 78.3±7.5 (P<0.001). The inter-grade difference in the score was significant in the pre-test (P=0.005), but not significant in the post-test (P=0.155). There were more responses of obtaining confidence after the intervention (P<0.001). Conclusion: In our study, pediatric residents showed improvement in their knowledge and confidence level after 4 weeks of the breastfeeding curriculum. CDMA 망을 이용한 차량 자가진단 및 원격제어

반도체외 정보통신기술이 자동차에 적용되면서 텔레매틱스라는 새로운 산업분야가 생겨났고, 텔레매틱스 서비스를 위한 다양한 기술들이 연구되고 있다. 텔레매틱스의 많은 서비스 중에서도 차량의 자가진단과 제어부분에서 많은 연구가 이루어지고 있으며, 다양한 방법들이 제시되고 있다. 이들 방법 중에 이동전화를 이용한 제어방식으로는 음성인식과 DTMF를 이용한 방법들이 연구되었지만 여러 가지 문제를 내포하고 있어 활용되지 않고 있다. 본 논문에서는 임베디드리눅스 기반의 CDMA모듈을 내장한 단말기를 설계하고 사용자 인증을 위한 인증절차와 단말기간의 통신을 위한 프로토콜을 설계하여, 무선데이터통신과 SMS서비스를 이용한 차량자가진단 및 원격제어방법을 제안한다. A new industry field called telematics was brought into bring as a result of The advance of semiconductor and computer communication. Lately, several researches and technologies have been developed to provide various telematics services. Specifically, lots of researches and alternative methodologies have been done and presented on the area of a self-diagnosis and a remote control of a vehicle, which are part of many different telematics services. The speech recognition and the DTMF(Dual Tone Multi Frequency) are representative vehicle-control methods via cellular phone, but these two methods are too problematic to be utilized in the telematics. This paper designs a mobile authentication process and protocol for communication between telematics that have CDMA module running the embedded linux. Besides, it proposes a self- diagnosis technique and a remote control method that exploit the wireless data communication and the SMS service on the CDMA network.

The purpose of this study was to investigate the effects of the perceived relationships of the adolescents with their parent on their peer relationships. The data were collected from 385 male and female high school students in Seoul through the questionnaire method. The instruments used for this study were the Parent Bonding Instrument(PBI) and the Index of Peer Relations(IPR). The data were analyzed by the frequencies, percentiles, means, standard deviations, Cronbach α, t-tests, oneway ANOVA, Pearson`s correlations and stepwise regression, using SPSS WIN 10.0 program package. Major results of the study were as follows: 1. An analysis of data revealed that degrees the relationships of the respondents with their parents and peer were higher than moderate. 2. The adolescents` peer relationship was affected by father`s occupations, family structures, parents` marital status and the economic status of the family. 3. The stepwise regression analysis yielded the results that relationship father`s caring(β=-.364), parents` marital status(β=.150) and economic status of family(β=.130) were significant factors in explaining the adolescents` peer relationship. Based on the results of the study, needs for effective father education and enhancement of stable marital relationships were addressed.

MPLS망에서 RSVP와 DiffServ연동모델의 성능분석

The explosive growth of the internet traffic has imposed tremendous stress on routers, particularly, in the core network. Today's IP services reflect the unpredictable and undifferentiated packet loss and jitter characteristics of traditional best-effort routers. For above requirements, it is required not only the high-speed transmission of the packet, but the new routing function, the guarantees of QoS and bandwidth, the supports of user-requested services MPLS is proposed by the IETF and many other research institute as the solutions for this problem. In this paper, analyze performance of QoS Model applied RSVP over DiffServ to MPLS network. this model is each RSVP applied access network and DiffServ applied core network. In Result, It is amiss that basis MPLS net uses to core network because the average packet loss rate and average delay time are increased if traffic increases. Experiment result was seldom difference in case of network that apply each RSVP, DiffServ, RSVP over DiffServ. RSVP over DiffServs case packet loss rate and delay time were low than DiffServ and the packet loss rate is high because comparing than RSVP but delay time was very low.

In this paper, we propose a new 3-dimensional cooperative authoring tool the environments. In this method, we clearly define the roles of authors Involved In the cooperation and extend the functionality of the each client system, and thus reducing the overload in server systems and/or the network bottleneck than existing client-server models, while minimizing the locking delays of the existing pessimistic locking mechanism. In addition, we use scene-graphs of Java 3D to represent the rapidly changing scenes vividly 본 논문에서는 새로운 3차원 협동 저작 기법에 대해 설명한다. 이 방법에서는 협동 저작에 참여하는 작업자에 대한 명확한 역할 구분과 클라이언트의 기능 확장을 통해, 기존의 클라이언트-서버 구조가 갖는 서버 부담 및 네트워킹 병목 현상을 완화하는 한편, 기존의 '비관적 잠금 기법'에서 발생하는 작업 공유에 따른 불필요한 대기 시간을 줄인다. 또한 자바 3D의 '장면 그래프'를 이용하여 장면을 표현함으로써 실시간 장면 정보의 변화를 빠르고 쉽게 반영하였다.

상악동을 침범하는 역위성 유두종의 증례보고

The present study reports a case of inverted papilloma of the nasal cavity and infiltrating into the maxillary sinus. Inverted papilloma is an uncommon and locally aggressive benign tumor of the sinonasal region. The patient, 51-year-old male, presented with unilateral nasal obstruction and periodic swelling on the palate without pain. Enhanced CT scan revealed a heterogeneously enhancing solid mass in the nasal cavity and infiltrating into the right maxillary sinus, as well as an incidental, secondarily infected residual cyst m the periapical area of the right maxillary canine. The sinonasal mass was revealed as an inverted papilloma on histopathologic examination.

알레르기성 자반증 환자에서 캡슐내시경으로 공장궤양을 진단한 1예

Henoch-Schönlein purpura (HSP) is the most common form of systemic vasculitis in children. Palpable purpura, arthralgia, arthritis, abdominal pain and renal involvement are the major clinical manifestations. Gastrointestinal involvement is related with abdominal pain and bleeding. We described a 71 year-old female experienced acute exacerbation of HSP presented with gastrointestinal bleeding. She was hospitalized for hematemesis and diagnosed duodenitis by esophagogastroduonenoscopy (EGD). Duodenitis was improved at EGD checked in 7 days. She still complained of melena and abdominal pain. There were no abnormal findings at sigmoidoscopy. Jejunal ulcer and purpura were diagnosed by capsule endoscopy. Symptoms were relieved after administration of systemic steroid. But she needed renal replacement therapy for 3 months. Small bowel ulcer diagnosed by capsule endoscopy in patients with HSP was rarely described in Korean literature. This case suggests that capsule endoscopy have a role in diagnosis of small bowel ulcer and its severity in HSP with gastrointestinal symptom.

당뇨환자에서 발생한 두개저의 침습성 아스페르길루스증 1예

두개저의 침습성 아스페르길루스증은 매우 드물지만 치명적인 질환으로 침습성 아스페르길루스 부비동염에 합병될 수 있다. 침습성 아스페르길루스증의 치료 원칙은 병변의 외과적 적출술과 장기간의 항진균요법으로 되어 있으나, 적절한 치료 기간과 치료 반응의 경과 판정에 대한 지침은 없는 실정이다. 저자들은 부비동염 증상을 주소로 내원하여 아스페르길루스종을 진단받고 4개월 전에 수술받은 적이 있는 당뇨 환자에서 두 개저의 침습성 아스페르길루스증을 진단하였으며, 병소의 부분적인 수술적 제거와 5개월 이상의 전신적 항진균제 사용으로 환자의 임상 경과가 호전되었으며 치료전과 치료 과정에서 시행한 갈륨스캔에서 방서성동위원소의 섭취가 점차 감소하여 일치하는 소견을 관찰할 수 있었다. 이에 저자들은 당뇨를 포함한 면역저하자에서 발생하는 아스페르길루스 부비동염은 조직 침습 유무에 대한 정확한 진단이 매우 중요하며, 추후에도 침습성 아스페르길루스증으로 진행할 수 있음을 유의해야 할 것을 강조하며, 또한, 갈륨스캔이 그러한 감별과 치료 경과 판정에 유용함을 경험하여 보고하는 바이다. Invasive aspergillosis of skull base is a rare but potentially lethal disease which complicates fungal sinusitis and requires surgical debridement, and long term antifungal therapy. However, optimal duration of antifungal therapy and standard method to monitor of clinical response are not known. A 55-year-old diabetic female patient presented with headache and otatgia 4 months after surgery for non-invasive aspergilloma of sinus. Brain MRI findings revealed bone destructions at the skull base and mastoid process. Mastoidectomy and tympanoplasty were performed, and the resected specimen showed fungal hyphae invading the tissue. Amphotericin B was given for 9 weeks (a total dose of 3.3g) with oral itraconazole, followed by itraconazole (400 ㎎ daily) alone for another 12 weeks. Serial gallium scans, taken before treatment and at 8th and 16th weeks following therapy, showed gradual reduction in uptake of the lesions. We emphasize that non-invasive Aspergillus sinusitis may progress to invasive disease, and strong suspicion of invasiveness is mandatory, especially in immunocompromised patients including diabetes mellitus. Also, we suggest that gallium scan is useful for determining the presence and extent of the disease, and monitoring clinical response following therapy.

급성 단순하부요로감염의 경험적 치료로써 레보플록사신 3일요법의 효과

목적 : 급성 단순요로감염의 원인균에 대한 내성증가에 따른 경험적 1차치료의 선택에 대한 어려움으로 인하여 최근 미국의 Infectious Disease Society of America(IDSA)에서는 이에 대한 가이드라인을 제시하였다. IDSA의 가이드라인에 따르면 그 지역의 TMP-SMX에 대한 내성균주의 비율이 10-20% 이상일 때는 급성 단순요로감염의 경험적 1차치료제로 플루오로퀴놀론제를 선택하도록 하였다. 이에 저자들은 급성 단순하부요로감염 환자에 있어서 레보플록사신에 대한 임상적, 미생물학적 유효성 및 안전성을 알아보고자 하였다. 방법 : 시험은 전향적, 비맹검, 공개, 비비교, 다기관 임상시험으로 시행되었고 전국 5개 대학병원을 방문한 56명의 급성 단순하부요로감염 환자에게 레보플록사신 100㎎을 1일 3회, 3일간 경구 투여하여 1-3일 후에 추적 관찰하였다. 결과 : 평가가 가능하였던 53명의 환자 중 50명의 환자에서 치료 후 5-7일 후에 증상이 소실되었거나 호전되어 94%의 임상적 유효성을 보였으며, 51명의 환자에서 치료 후 1-3일 후에 원인균이 제거되어 96%의 미생물학적 유효성을 보였다. 약물 관련 이상반응은 2%의 환자에서 나타났으나 이로 인한 치료 중단 사례는 없었으며 심각한 생화학적 또는 혈액학적 이상반응은 나타나지 않았다. 결론 : 레보플록사신 100㎎ 1일 3회 3일 요법은 임상적, 미생물학적 유효성이 뒤어나며 안전한 치료요법으로 급성 단순하부요로감염의 경험적 1차 치료에 적합할 것으로 생각된다. Background : The increasing frequency of clinical failure of uncomplicated urinary tract infections (UTIs) may be due to emerging resistance to commonly prescribed antimicrobials. Infectious Diseases Society of America guidelines state that uncomplicated UTIs should be treated empirically with a fluoroquinolone, if the proportion of trimethoprim/sulfamethoxazole-resistant isolates in the community is >10%-20%. The clinical efficacy and safety of levofloxacin were assessed in patients with acute, uncomplicated lower urinary tract infection. Methods : The study was a multicenter, prospective, open label, non-comparative, non-blinded trial. Fifty-six patients with acute, uncomplicated symptomatic lower urinary tract infections from five university hospitals were treated with levofloxacin 100㎎ tid for 3 days and were followed up for 1 to 5 days. Results : Fifty-three of the 56 patients were evaluable. Ninety-four per cent of patients were symptomatically cured or improved by the fifth to seventh day after therapy started. Bacteriological eradication of initial pathogen was achieved by 1 to 3 days in 96%. Drug related adverse experiences were seen in 2% of patients. None necessitated cessation of therapy. No significant biochemical or hematological abnormalities occurred. Conclusion : These results demonstrate the clinical efficacy and safety of empirical 3-day levofloxacin for acute, uncomplicated urinary tract infections.
