비침습적 산전 검사 도입 이후(2014-2023) 국내에서 시행된 14,011건의 양수검사에 대한 임상 및 세포유전학적 소견

배경: 양수 검사는 1970년대부터 산전 진단에 중요한 역할을 해왔 다. 한국에서는 고령 임신의 증가와 비침습적 산전 검사의 도입으 로 산전 진단과 관리에 큰 변화가 있었다. 본 연구는 지난 10년간의 양수 검사의 임상적, 세포유전학적 경향을 분석하며, 특히 NIPT 도입 이후의 변화에 주목했다. 방법: 2014년부터 2023년까지 지난 10년간 시행된 14,011건의 중기 임신 양수 검사 사례를 후향적으로 분석했다. 연간 검사 건수, 검 사 적응증, 임신부 연령, 임신 주수, 세포유전학적 결과, 그리고 염색체 이상 발생률을 조사했다. 결과: 양수검사 건수는 2014년 2,928건에서 2023년 862건으로 감 소한 반면, 염색체 이상 발생률은 3.9%에서 15.8% 로 증가했다. 2014 년부터 비정상적인 모체 혈청 선별검사가 가장 흔한 검사 적응증으 로 유지되었으며, 2020년부터는 비침습적 산전 검사 결과 이상이 두 번째로 흔한 적응증이 되었다. 염색체 이상은 비침습적 산전 검 사 결과 이상이 있는 경우에서 가장 높은 비율(41.2%)로 발견되었 으며, 염색체 이상 병력이 있는 경우(11.0%)와 초음파 이상 소견이 있는 경우(8.9%)가 그 뒤를 이었다. 총 14,011건의 양수검사 중 1,307건(9.3%)에서 염색체 이상이 발견되었다. 이 중 수적 이상은 81.2% (1,061건), 구조적 이상은 18.8% (246건)를 차지했다. 가장 흔 한 염색체 이상은 21번 삼염색체(48.0%)였으며, 그 다음으로 18번 삼염색체(13.4%), 클라인펠터 증후군(5.1%), 트리플×증후군(3.5%), 터너 증후군(2.4%) 순으로 나타났다. 결론: 본 연구는 비침습적 산전 검사 도입 이후 지난 10년간 국내 에서 시행된 양수검사의 임상적 및 세포유전학적 경향을 분석하 였다. 연구 결과는 고위험 임신에서 양수검사의 지속적인 중요성 을 강조하며, 포괄적인 산전 평가 전략의 필요성을 시사한다. Background: Since the 1970s, amniocentesis has played an essential role in prenatal diagnosis. In Korea, increasing maternal age and the in- troduction of non-invasive prenatal testing (NIPT) have significantly influenced prenatal care. This study aims to evaluate clinical and cytogenetic findings from amniocentesis over the past decade, highlighting the changes observed following the adoption of NIPT. Methods: We retrospectively analyzed 14,011 cases of second-trimester amniocentesis over 10 years (2014–2023) to explore annual distribu- tion, indications, maternal age, gestational age, cytogenetic results, and chromosomal abnormality rates. Results: The number of amniocentesis cases decreased from 2,928 in 2014 to 862 in 2023, whereas the incidence of chromosomal abnormalities increased from 3.9% to 15.8%. Since 2014, abnormal maternal serum screening has remained the primary indication, while abnormal NIPT re- sults became the second most common indication starting in 2020. Chromosomal abnormalities were most frequently identified in cases with ab- normal NIPT results (41.2%), followed by those with a history of chromosomal abnormalities (11.0%) and abnormal ultrasound findings (8.9%). Among the 14,011 amniocentesis cases, 1,307 (9.3%) exhibited chromosomal abnormalities, with numerical abnormalities accounting for 81.2% (1,061/1,307 cases) and structural abnormalities representing 18.8% (246/1,307 cases). Trisomy 21 was the most frequent abnormality (48.0%), followed by trisomy 18 (13.4%), Klinefelter syndrome (5.1%), triple X syndrome (3.5%), and Turner syndrome (2.4%). Conclusions: This study identifies key trends in the clinical and cytogenetic findings of amniocentesis in Korea over the past decade. Despite the increasing adoption of NIPT, amniocentesis remains essential in managing high-risk pregnancies and ensuring comprehensive prenatal assessment.

The purpose of this study is to investigate the impact of job demand on employees’ social network service addiction and how it influences their job satisfaction and organizational citizenship behavior. And we explore if occupational self-efficacy can moderate the relationship between job demand and social network service addiction. In the results, first, job demand increases social network service addiction. Second, social network service addiction decreases job satisfaction and organizational citizen behavior. Finally, employees’ occupational self-efficacy decrease the effect of job demand social network service addiction.

<P>Repetitive transcranial magnetic stimulation (rTMS) or transcranial direct current stimulation (tDCS) has been used for the modulation of stroke patients' motor function. Recently, more challenging approaches have been studied. In this study, simultaneous stimulation using both rTMS and tDCS (dual-mode stimulation) over bilateral primary motor cortices (M1s) was investigated to compare its modulatory effects with single rTMS stimulation over the ipsilesional M1 in subacute stroke patients. Twenty-four patients participated; 12 participants were assigned to the dual-mode stimulation group while the other 12 participants were assigned to the rTMS-only group. We assessed each patient's motor function using the Fugl-Meyer assessment score and acquired their resting-state fMRI data at two times: prior to stimulation and 2 months after stimulation. Twelve healthy subjects were also recruited as the control group. The interhemispheric connectivity of the contralesional M1, interhemispheric connectivity between bilateral hemispheres, and global efficiency of the motor network noticeably increased in the dual-mode stimulation group compared to the rTMS-only group. Contrary to the dual-mode stimulation group, there was no significant change in the rTMS-only group. These data suggested that simultaneous dual-mode stimulation contributed to the recovery of interhemispheric interaction than rTMS only in subacute stroke patients. This trial is registered with NCT03279640.</P>
Synthesis and characterization of regioregular alkyl-substituted poly(3,4-ethylenedioxythiophenes).

<P>In order to increase the solubility of PEDOT, we introduced an alkyl chain into polymer backbone. Depending on the position of the alkyl bond, the steric hindrance between the alkyl chains can also leads to regioregularity in the polymer. The regioregular polymers can easily occur self-assembly, both in solution and in the solid state, resulting in highly ordered two- and three-dimensional polymer architectures. A series of soluble regioregular alkyl-substituted PEDOTs were synthesized using the Grignard Metathesis (GRIM) for the purpose of study on regiorandom and regioregular PEDOT. The ethylene proton adjacent to the oxygen atom of regioregular methoxydodecyl-PEDOT appears at 3.51 ppm, whereas the corresponding peak of regiorandom methoxydodecyl-PEDOT appears at 3.42 ppm.</P>

<▼1><P>Supplemental Digital Content is available in the text</P></▼1><▼2><P><B>Abstract</B></P><P><B>Background:</B></P><P>Bojungikki-tang, a herbal medicine for strengthening the digestive system, has been widely used in East Asian countries since the Yuan Dynasty in China. Moreover, Bojungikki-tang has reportedly exhibited anti-allergy effects in many clinical trials. This systematic review will assess the effectiveness and safety of Bojungikki-tang for allergic rhinitis (AR).</P><P><B>Methods and analysis:</B></P><P>Eleven databases, including Asian databases, will be searched for studies conducted through October 2017. We will include randomized controlled trials (RCTs) assessing Bojungikki-tang for AR. The risk of bias will be evaluated using the Cochrane risk of bias assessment tool, and confidence in the cumulative evidence will be evaluated using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) instrument.</P><P><B>Ethics and dissemination:</B></P><P>This systematic review will be published in a peer-reviewed journal and will also be disseminated electronically and in print. <P><B>Abstract</B></P> <P><B>Introduction</B></P> <P>The detection of <I>BRCA1/2</I> mutations is important because PARP1 inhibitors are approved for germline and/or somatic <I>BRCA</I>-mutated advanced ovarian cancer. Next-generation sequencing (NGS) is increasingly used in clinical practice for <I>BRCA1</I>/2 mutations. The purpose of this study was to consider several conditions of NGS <I>BRCA1/2</I> assay applicable to clinical laboratory tests, in particular for using formalin fixed paraffin embedded (FFPE) ovarian tissues.</P> <P><B>Materials and methods</B></P> <P>We selected 64 ovarian cancer patients and performed Oncomine™ BRCA assay using FFPE tissue. Effect of FFPE sample quality was analyzed by NGS quality parameters including deamination metric. Somatic variants were selected by removing germline variants of peripheral blood and interpreted as pathogenic, variants of unknown significance, and false positive.</P> <P><B>Results</B></P> <P>We found a positive relationship between the number of variants over the deamination metric and FFPE age (<I>P</I> < 0.001) with a cutoff values of approximately 0.7 and 60 months, respectively. When comparing NGS results with Sanger sequencing, NGS misreported 3 of 15 variants using default parameters which were corrected after changing parameters. We detected somatic variants in eight patients and classified them into pathogenic (n = 3), VUS (n = 3) and false positive (n = 2).</P> <P><B>Conclusions</B></P> <P>This study is important for improving <I>BRCA1/2</I> mutation detection capabilities of NGS analytical pipelines and strategy to overcome their limitations using FFPE tissue in ovarian cancer patients.</P>

As the game technology become advanced, the interest in realtime simulation based on physical become increased. In this paper, we examine the example and the technology of the 3-dimension physical engine and the example of the ocean game. And we study the movement of buoyant objects in the ocean in this paper. The movement of buoyant objects in the existing game engine has few application examples as well as study result. Therefore, in this paper we study the physical-based exercise equation and the application example about movement of buoyant objects to make up for these weak points.
