mediastinal teratomas are encountered commonly in adult life, and rarely in childhood. Characteristically, these mediastinal teratomas are located anteriorly with only rare examples in the posterior mediastinum. The cystic teratoma usually behaves as a benign neoplasm, but the solid [non-cystic] teratoma is frequently malignant. We experienced a case of large anterior mediastinal cystic teratoma in a two-years old boy, which was treated by complete surgical excision with good result.

Tetrahydrobiopterin (BH4) is an essential cofactor in NO synthesis by endothelial nitricoxide synthase (eNOS) enzymes. It has been previously suggested that reduced intrahepaticBH4 results in a decrease in intrahepatic NO and contributes to increased hepatic vascularresistance and portal pressure in animal models of cirrhosis. The main aim of the presentstudy was to evaluate the relationship between BH4 and portal hypertension (PHT). Onehundred ninety-three consecutive patients with chronic liver disease were included in thestudy. Liver biopsy, measurement of BH4 and hepatic venous pressure gradient (HVPG)were performed. Hepatic fibrosis was classified using the Laennec fibrosis scoring system. BH4 levels were determined in homogenized liver tissues of patients using a highperformance liquid chromatography (HPLC) system. Statistical analysis was performed toevaluate the relationship between BH4 and HVPG, grade of hepatic fibrosis, clinical stageof cirrhosis, Child-Pugh class. A positive relationship between HVPG and hepatic fibrosisgrade, clinical stage of cirrhosis and Child-Pugh class was observed. However, the BH4level showed no significant correlation with HVPG or clinical features of cirrhosis. BH4concentration in liver tissue has little relation to the severity of portal hypertension inpatients with chronic liver disease.

Alcohol and the Metabolic Syndrome in Korean WomenWon Ki Hong, M.D., Jong Sung Kim, M.D, Ph.D.†, Jin Gyu Jung, M.D., Sung Soo Kim, M.D., Chan Il Park, M.D., Keun Bae Kim, M.D., In Wook Jung, M.D. 연구배경: 적절한 음주는 심혈관 질환의 이환율과 이로 인한 사망률의 감소와 관계가 있다. 대사증후군은 심혈관 질환의 위험도를 높이는 질환으로 알려져 있다. 음주는 대사증후군의 위험을 높이는 데 영향을 주는 인자로 여겨지고 있어 여성의 음주와 대사증후군 간의 관계를 알아보기 위하여 시행하였다. 방법: 2005년 3월부터 5월까지 대전광역시 소재 일개 대학병원 종합검진센터에서 종합검진을 받은 262명의 여성을 대상으로 하였다. 대사증후군의 진단은 National Cholesterol Education Program, Adult Treatment Panel III의 기준에 근거하였고, 그 중 허리둘레는 세계보건기구 아시아-태평양 기준에 따랐다. 음주상태는 하루 음주량을 조사하여 비음주군, 적절음주군(1잔 이하), 과음군(1잔 초과)으로 분류하였다. 대사증후군의 구성요소들과 음주와의 연관성을 조사하였고 음주상태에 따라 대사증후군의 빈도를 비교하였다. 결과: 여성 적절음주군은 중성지방이 비음주군에 비해 유의하게 낮았다 (P<0.05). 여성 음주군들은 고밀도지단백이 비음주군에 비해 유의하게 높았다(적절음주군 P<0.05, 과음군 P<0.05). 적절음주군은 비음주군에 비해 대사증후군의 위험이 낮게 관찰되었다(Odds ratio=0.43, 95% Confidence interval: 0.22~0.84, P=0.013). 결론: 여성에서 하루 1잔의 적절한 음주는 대사증후군의 위험이 낮았다.

Types of foundation of high rise buildings are primarily determined by loads transmitted from super structure, soil bearing capacity and available construction technology, The use of deep foundation of the buildings considered in this study due to the fact that rock of enough bearing capacity is not found down until 90~l00m. When a concentration of high soil pressure must be distributed over the entire building area, when small soft soil areas must be bridged, and when compressible strata are located at a shallow depth, mat foundation may be useful in order to have settlement and differential settlement of variable soils be minimized. The concept of mat foundation will also demonstrate some difficulties of applications if the load bearing demand directly carried down to the load -bearing strata exceeds the load -bearing capacity. This paper introduces both the analysis and design of mat type foundation for high rise buildings as well as the method-ology of modelling of the soil foundation, especially, engineered to redistribute the stress exceeding the soil bearing capacity. This process will result in the wide spread of stresses over the entire building foundation.

A rapid progress has been made over last decade in the state-of-the-art earthquake structura1 engineering towards a better understanding of both the earthqauke ground motion and structural response. These efforts seek to ensure that there will be no serious injury or loss of life in the event of earthquake, and that structures can be built at minimum cost. The design of structures in general, concrete structures in particular, to resist strong ground input motions is not a simple matter, and analytical models for such structures must be developed from a design perspective that accounts for the complexities of the structural responses. The primary obj ective of earthquake structural engineering research is to ensure the safety of structures by understanding and improving a design methodology. Ideally, this would require the development of an analytical model related to a design methodology that ensures a ductile performance. For the accurate assessment of the adequacy of analytically developed model, experiments conducted to study the inplane inelastic cyclic behavior of structures should verify the analytical approach. The fundamental goal of this paper is to present and demonstrate experimentally verified analytical methods that provide the adequate degree of safety and confidience in the behavior of reinforced concrete structural components. This study further attempts to extend the developed modeling techruque for use by practicing structural engineers for both the analysis and design.Plication of the relaxed diaphragm through left thoracotomy was done and result was excellent as seen on Fig. 5. Cause of eventration of the left hemidiaphragm was due to paralysis of the left phrenic nerve which was tested during thoracotomy.
Design and Evaluation of Telematics User Interface for Ubiquitous Vehicle

In the ubiquitous computing environment, a ubiquitous vehicle will be a communication node in the vehicular network as well as the means of ground transportation. It will make humans and vehicles seamlessly and remotely connected. Especially, one of the prominent services in the ubiquitous vehicle is the vehicle remote operation. However, mutual-collaboration with the in-vehicle communication network, the vehicle-to-vehicle communication network and the vehicle-to-roadside communication network is required to provide vehicle remote operation services. In this paper, an Internet-based human-vehicle interfaces and a network architecture is presented to provide remote vehicle control and diagnosis services. The performance of the proposed system is evaluated through a design and implementation in terms of the round trip time taken to get a vehicle remote operation service.
