Glutathione peroxidase 3 as a serum biomarker of postoperative relapse in patients with lung cancer

Glutathione peroxidase 3 (GPx3) is anti-oxidant extracellular protein. It has been known that the down-regulation of GPx3 is related to the progression and proliferation of cancer. We prospectively collected serial serum samples at baseline, 3, 6 and 12 months after surgery during the year 2013. The analysis was performed using the following statistical methods: t-test, paired t-test, time-dependent receiver operating characteristic curve (ROC) analysis and Cox proportional hazard regression analysis. Total 165 patients were enrolled. Fifty-eight (35.2%) patients were confirmed relapse. The mean changes of GPx3 were significantly higher in relapse group at 3 months (0.30±0.04 vs. 0.21±0.04 μg/mL, p=0.023), 6 months (0.30±0.05 vs. 0.17±0.04 μg/mL, p=0.039) and 12 months (0.30±0.05 vs. 0.18±0.04 μg/mL, p=0.044). In time-dependent ROC curve analysis, the mean GPx3 change at 3 months and 6 months could predict the relapse from 18 months to 54 months after surgery (AUC, 0.611-0.676) and from 9 to 54 months (AUC, 0.646-0.708). The mean relapse-free survival of the higher mean GPx3 change at 3 months (>0.302 μg/mL) and 6 months (>0.305 μg/mL) groups were significantly shorter than lower change group (35.12 vs. 46.98 months, p=0.006 and 36.0 vs. 45.86 months, p=0.031). Multivariate Cox regression analysis shows that higher GPx3 change group at 3 months (hazard ratio [HR] 3.122, p<0.001) and 6 months (HR 2.28, p=0.022) were independent factors for relapse. Serum GPx3 value after surgery may be the useful biomarker for monitoring of relapse in lung cancer

Purpose: As part of implementation of “Framework Convention on Tobacco Control” of world health organization and financial penalty for the purpose of smoking rate reduction, government began to raise the tax of tobacco from January 1, 2015. In Korea, most smoker is men, and smoking rate is very high compared to other OECD countries. So, in this article, we try to analyze change in smoking habits of adult in Korea after increasing tax rate of tobacco. Method: for the study, we had used national health and nutrition survey 6 th. The subject is current smoking male over the age of 19. Smoker is defined as current smoker who smoke over 100 cigarettes in a whole life. In this group, smoking rate, reason for quitting smoking, association between smoking cessation and increasing tax rate of tobacco and smoking habit change after increasing tax rate of tobacco were analyzed. Result: when comparing the smoking rate before and after increasing of tobacco tax rate, adult male smoking rate was decreased 39.3% (407 out of 2154) in 2014 to 34.4% (413 out of 2352) in 2015. People who had tried to stop smoking(2008), they said reason for quitting smoking is ‘concern about health’ (763, 38%), ‘problem of health (470, 23.4%) and ‘expensiveness of tobacco’ (131 6.5%). Conclusion: after tax rate increase, male smoking rate is decreased, and there is some effect of increase price of tobacco to quitting smoking, especially in low income level. Therefore, not only non-price policy, but price policy of tobacco should be also considered for successful national smoking cessation strategy

Pulmonary sequestration is a rare congenital anomaly of the lung in which it is separately supplied from the aorta or one of its branches. Bilateral pulmonary sequestration is very rare, particularly in adults. In bilateral pulmonary sequestration, resection of both sides is usually recommended if both sides are infected and symptomatic. We report the case of a 37-year-old female patient with bilateral intralobar pulmonary sequestration treated by staged bilateral lower lobectomy.

Background: This study is aimed to investigate the feasibility of liquid biopsy for detection of ALK rearrangement and predictive value for ALK inhibitor treatment. Methods: We retrospectively analyzed formalin-fixed paraffin-embedded (FFPE) tissue from previously confirmed FISH-positive (n=199) and -negative (n=920) specimens. Total RNA were extracted from FFPE blocks, plasma and platelets. Fusion RNA of EML4-ALK was detected using PANA qPCR EML4-ALK fusion gene detection kit, initially developed for tissue genotyping. Results: FISH-positive (n=33) and FISH-negative (n=28) cases were finally enrolled. In validation data compared with FISH, liquid biopsy (plasma- or platelet-positive) had higher sensitivity (78.8%) and accuracy (78.7%) than tissue RT-PCR. Platelets showed slightly higher sensitivity than plasma (69.7%vs.63.6%). Higher positivity for liquid biopsy was shown in subgroup of delayed blood sampling since diagnosis more than 6 months (plasma,85.7%;platelet,87.0%). In 26 patients treated with crizotinib, platelet-positive subgroup showed longer duration of treatment (7.2 vs 1.5 months, p=0.090), longer median progression- free survival (5.7 months, 95% CI 0.0-16.7 vs 1.7 months, 95% CI 0.5-3.0; p=0.028), higher overall response rate (70.6% vs 11.1%, p=0.011) and disease control rate (88.2% vs 44.4%, p=0.028) than platelets-negative subgroup. Conclusion: Plasma and platelets are valuable sources for liquid biopsy in detection of ALK rearrangement and showed favorable sensitivity despite using tissue-based RT-PCR kit. Platelets can be useful for predicting treatment outcome of crizotinib.

Background: Treatment efficacy of osimertinib was assessed in previously untreated patients with advanced or metastatic NSCLC harboring EGFR activating mutation detected from ctDNA combined with tumor genotyping. Methods: CobasV2 and PANA Mutyper were used for ctDNA genotyping. Patients with EGFR activating mutation detected from ctDNA were enrolled and received once-daily administration of osimertinib (80 mg). Primary endpoint was objective response rate (ORR), and secondary endpoints were ctDNA test sensitivity, progression-free survival (PFS), duration of response (DoR) and safety. Results: ctDNA of 29 patients was positive for activating EGFR mutation and 19 patients were enrolled. Median age was 70 years (range 32-84) and majority of patients had brain metastasis (15/19, 78.9%). ORR was 68.4% (13/19) and DCR was 94.7% (18/19). Patients with exon 19 deletion showed more favorable ORR than patients with exon 21 L858R/L861Q (10/11, 90.9% vs. 3/7, 42.9%, p=0.032). DCR of brain metastasis was 100.0% (15/15; CR=3). Sensitivity of ctDNA tests for activating EGFR mutation was 74.4% when using both tests and 61.5% (Mutyper) or 64.1% (CobasV2) with either tests. Only one patient experienced drug-related interstitial pneumonia of grade ≥3 and resulted in drug discontinuation. Survival data including PFS was not matured (7/19, 36.8%) and the analysis will be followed. Conclusion: Osimertinib had favorable efficacy in the first-line treatment of NSCLC harboring EGFR activating mutation detected from ctDNA combined with tumor genotyping, even though in patients with old age and brain metastasis.
Concordance Between Artificial Intelligence and Multidisciplinary Tumor Board for Lung Cancer

Background: IBM Watson for Oncology (WFO) is a cognitive computing system which helps physicians quickly identify key information in a patient’s medical record, surface relevant evidence and explore treatment options. We evaluated the level of agreement between WFO and multidisciplinary tumor board (MDT) for lung cancer. Methods: From January to April 2018, newly diagnosed lung cancer cases in Chonnam National University Hwasun Hospital were retrospectively examined using WFO version 18.4 according to four treatment categories [surgery, radiotherapy, concurrent chemoradiotherapy (CCRT), and palliative setting]. Concordance between MDT and WFO was analyzed by Cohen's kappa value. Results: Total 132 cases were enrolled in this study. The concordance rate occurred in 92.4% (k=0.881, p<0.001) for the all cases. The strength of agreement was very good in stage 1 (92.4%, k=0.855), stage 4 NSCLC (100%, k=1.000) and extensive disease SCLC (100%, k=1.000). In stage 3 NSCLC, the strength of agreement was good (80.8%, k=0.622). And the concordance was moderate in stage 2 NSCLC (83.3%, k=0.556) and limited disease SCLC (84.6%, k=0.435). There were discordant cases in surgery (4/29, 13.8%) and CCRT (5/33, 15.2%), but discordance was not occurred in radiotherapy and palliative care. Conclusion: Treatment recommendations made by WFO and MDT were highly concordant for lung cancer cases especially in stage 1 NSCLC and metastatic stage. However, WFO was just an assisting tool in stage 2-3 NSCLC and limited disease SCLC, so shared decision making may be more important in this stage.

Background: The aim of this study is to investigate the feasibility of liquid biopsy using plasma and platelets for detection of ALK rearrangement. Methods: We retrospectively analyzed the FFPE tissue and blood sample to detect ALK rearrangement using multiplex RT-PCR from 30 advanced NSCLC patients. EML4-ALK fusion RNA was detected using PANAqPCRTM EML4-ALK fusion gene detection kit. Results: Twenty-eight patients were FISH positive and two were negative. Liquid biopsy(plasma or platelets-positive) had higher sensitivity(96.4%) and accuracy(93.3%). Platelets showed slightly higher sensitivity and accuracy than plasma(82.1 & 83.3% vs 78.6 & 76.7%). Median proportion of positive cells in FISH was higher in subgroups of liquid biopsy with positive result(Plasma, 30.0 vs 15.0%; Platelets 30.0 vs 20.0%), but it was not statistically significant (p=0.062 and 0.104). In 18 patients with crizotinib treatment, platelets-positive subgroup showed a tendency of longer duration of treatment (7.2 vs 1.5 months) and higher response rate(57.1 vs 0.0 %), but the difference was not significant (p=0.071 and 0.092). However, platelets-positive subgroup showed significantly higher disease control rate than platelets-negative subgroup treated with crizotinib(85.7 vs 25.0%, p=0.044). Conclusion: Plasma and platelets are a valuable source for liquid biopsy using RT-PCR technique in detection of ALK rearrangement, and they could play a supplementary role in diagnosis of ALK-positive NSCLC. Furthermore, platelets, especially, may be useful for predicting the treatment outcome of crizotinib.
