        The purpose of this study was to develop a new ^13C-urea-containing capsule for diagnosis of H.pylori. The urea-containing capsules were prepared with various diluents such as polyethylene glycol (PEG), microcrystaline cellulose, sodium lauryl sulfate and citric acid. The dissolution test, ^13C-urea breath test and stability test were then performed on the capsules. Microcrystalline cellulose and sodium lauryl sulfate retarded the initial dissolution rates of urea. However, PEG increased the initial dissolution rates of urea. Furthermore, two formulae composed of PEG,[^13C-urea/PEG (38/1.9 mg/cap)] and [^13C-urea/PEG (38/1.9/1.9 mg/cap)] had the maximum DOB value. about 16 at 20 min, while the formula composed of only 38 mg ^13C-urea had the maximum DOB value at 30 min. The results indicated that PEG improved the sensitivity of ^13C-urea in the human volunteers. The capsule [^13C-urea/PEG (38/1.9 mg/cap)] was stable for at least six months in 25 and 37℃. Thus, a PEG-containing capsule, [^13C-urea/PEG (38/1.9 mg/cap)] would be a more economical, sensitive and stable perparation for diagnosis of H. pylori.

        Early Recognition of Pediatric Strokes in the Emergency Department: Epidemiology, Clinical Presentation, and Factors Impeding Stroke Diagnosis in Children

        Purpose: Strokes are challenging to diagnose in pediatric emergency departments (EDs) as level of suspicion is low and atypical presentations are common. We analyzed clinical features, epidemiology and factors of delayed identification in arterial ischemic strokes (AIS) and hemorrhagic strokes (HS). Methods: Single-center retrospective cohort study of children aged between 29 days and 18 years old diagnosed with stroke between July 2016 to June 2021. Results: Among 36 children, 11 (30.5%) had AIS, 25 (69.4%) had HS. Median age for AIS was 9 years (interquartile range [IQR], 2 to 9) and HS 9 years (IQR, 1 to 11.5) (P=0.715). Focal neurological deficit was seen in 72.7% of AIS and 20% of HS (P=0.006). Only 18.2% of AIS and 52.0% of HS presented within 6 hours of symptoms. Median time from symptom onset to ED presentation was 24 hours (IQR, 12 to 28) for AIS and 7 hours (IQR, 1.8 to 48) for HS (P=0.595). Most (85.6%) arrived by own transport. Median time from presentation to neuroimaging was 7 hours (IQR, 0.9 to 7) for AIS and 4.8 (IQR, 1.3 to 16.8) hours for HS (P=0.376). Eleven patients, 9/25 (36.0%) HS and 2/11 (18.2%) AIS, did not have stroke as differential diagnosis at ED (P=0.714). Common initial diagnoses were viral illness or headaches. On univariate analysis, age <1 (odds ratio [OR], 17.5; 95% confidence interval [CI], 1.2 to 250.4; P=0.035) and absence of focal neurological deficit (OR, 13.091; 95% CI, 1.5 to 117.9; P=0.022) were significant factors for delayed identification. Conclusion: Index of suspicion for pediatric strokes among caregivers and clinicians should be increased. Public awareness campaigns are recommended.

      • Effect of Grifola frondosa polysaccharides (GFPS) on the improvement of clinical symptoms in letrozole-induced polycystic ovary syndrome (PCOS) rats

        This study was to investigate the ameliorative effect of GFPS on clinical symptoms in letrozole(1 mg/kg)-induced PCOS rats. The female SD rats were classified into 4 groups as follows: the normal group (gavaged with saline), the PCOS group(ingested letrozole daily for 21 days). Normal group and metformin group gavaged with letrozole followed by metformin (265 mg/kg) and GFPS(600 mg/kg), respectively. All rats were weighed every 3 days until the end of the experiment. The ovaries, perirenal, abdomen and periuterine fat were removed and the relative weights (g/bw×100) were calculated. Serum levels of FSH, LH, E2, T, TG, TC, LDL-C were measured by ELISA and calculated the LH/FSH ratio. GFPS reduced follicle number and increases granulosa cell layer thickness and luteal number. GFPS also reduced the increase of body weight, ovarian and visceral fat weight in PCOS rats (P<0.01). In addition, GFPS reduced the serum levels of T, LH/FSH, TC, TG, LDL-C and LH, and elevated the serum levels of E2 and FSH in PCOS rats. In conclusion, GFPS can improve follicular development and restore body weight, ovarian weight, serum hormone levels and lipid profile in PCOS rats.

        Ginseng berry polysaccharides on infl ammation-associated colon cancer: inhibiting T-cell differentiation, promoting apoptosis, and enhancing the effects of 5-fl uorouracil

        Background: Ginseng is a commonly used herbal medicine in treating various medical conditions. Chronic gut inflammation is a recognized factor for the development of colorectal cancer (CRC). In thisproject, Asian ginseng berry polysaccharide preparations were used to assess their effects on CRC andrelated immune regulation mechanisms. Methods: Ginseng berry polysaccharide extract (GBPE) and purified ginseng berry polysaccharideportion (GBPP) were used to evaluate their activities on human HCT-116 and HT-29 CRC cell proliferation. Interleukin-8 secretion analysis was performed on HT-29 cells. Naive CD4 cell isolation and T-helper celldifferentiation were performed and determined using flow cytometry for Th1 and Treg in addition to cellcycle and apoptotic investigation. Results: GBPE and GBPP significantly inhibited interleukin-8 secretion and cancer cell proliferation,inhibited CD4þIFN-gþ cell (Th1) differentiation, and decreased CD4þFoxP3þ cell (Treg) differentiation. Compared to the GBPE, GBPP showed more potent antiinflammatory activities on the malignant cells. This is consistent with the observation that GBPP can also inhibit Th1-cell differentiation better, suggestingthat it has an important role in antiinflammation, whereas Treg cells hinder the body’s immuneresponse against malignancies. Supported by cell cycle and apoptosis data, GBPE and GBPP, at variousdegrees, remarkably enhanced the anticancer activities of 5-fluorouracil. Conclusion: Data from this project suggested that Asian ginseng berry potentially has clinical utility inmanaging enteric inflammation and suppressing CRC through immunomodulation mechanisms.

        Impact of a Dust Storm on Characteristics of Particle Matter (PM) in Guangzhou, China

        A strong dust-storm (23-25 April, 2009) occurred in the provinces of Inner Mongolia, Gansu, and Shanxi, North China. Cities along the storm path (from north to south: Xi’ning, Lanzhou,Chengdu, Changsha, and Guangzhou) all experienced a sharp increase in particle matter (PM10) concentration. This is the first case that an Asian dust storm hit Guangzhou in Southern China. The impacts of dust storm on the characteristics of PM were investigated using samples collected in Guangzhou during 27-29 April, 2009. In addition, the mass concentration and chemical composition during a normal non-dust period (12-14 May, 2009) were compared with those in dust period. The results show that the concentration of PM10during the dust episode (0.231 mg m−3) was twice higher than that in the non-dust episode (0.103 mg m−3). Chemical analysis showed that concentrations of metal elements, enrichment factors of metal elements, and soluble ions during the dust episode were very different from those of non-dust. The total concentration of metal elements content in PM10 was 53.5 μg m−3 in the dust episode, which is about two times higher than that in non-dust episode (28.5 μgm−3). Increases in concentrations of Na, Ti, Zn, Cu, and Cr ranged from zero to 100% during the dust episode. However, the enrichment factors in non-dust episode were higher than that in dust-storm period,indicating that the above five chemicals originated mainly from local sources in Guangzhou. The concentrations of K, Mg, Al, Fe, Mn, V,and Co increased by over 100% in the dust episode, indicating their origins of remote sources. In the dust period, some water-soluble ions increased in PM10, but the main components in PM10 were SO42−,NO3− and NH4+. At last, we assessed the sources of dusts by analyzing synoptic situation and back trajectories of air mass in Guangzhou, and demonstrated that the main source of the dust storm was from Mongolia.

        Number marking in Colloquial Singapore English

        Number marking in Colloquial Singapore English (CSE) has often been described as sporadic or random. This description stems from the fact that plural marking is largely optional in CSE, combined with a lack of precise characterization of its optionality. We show that CSE number marking is not random but systematic; plural marking is obligatory on a noun that occurs with a number-neutral determiner in Determiner+Noun sequence, and optional elsewhere. We propose a principle called Avoid Ambiguity Principle and derive the optional but not-so-optional nature of CSE number marking from an interaction between the principle and the structure of a nominal expression. Unlike CSE, English displays obligatory number marking. We argue that this is a result of the two languages differing in two respects-(i) English always projects D, whereas CSE does so optionally; (ii) English D comes with unvalued [αpl], whereas CSE D comes with valued [+/-pl]. These language-specific parameters, coupled with the Avoid Ambiguity Principle, capture the differences between the two languages. Under our analysis, English transformed to CSE by a parameter resetting in category D when it came to Singapore.

        Correlation analysis of factors influencing the electronic unit pump cycle fuel injection quantity under overall operating conditions for diesel engines

        Electronic unit pump (EUP) can satisfy both diesel engine emission legislation and fuel economy by improving injection pressure and numerical control. Fluctuations in cycle fuel injection quantity (CFIQ) of EUP determine the coherence and stability of the EUP fuel injection system. The EUP simulation model is developed in the AMESim environment. The method for the simulation experiment is designed in the MODDE environment using the design of experiments method. The results of the simulation reveal the variation laws of correlation between parameters with interaction or no interaction under overall operating conditions of diesel engines. In addition, the results also show the EUP system is a complex nonlinear system. Under overall operating conditions, all the characteristic parameters,such as fuel supply pressure, cam profile velocity, control valve lift, injector opening pressure, injector needle lift, and injector flow coefficient,have significant correlation with CFIQ. The interacting first-order factors exhibit the most significant correlation with CFIQ. The self-interacting second-order factors have significant secondary correlation with CFIQ.
