임플란트 주위 조직 보존을 위한 임플란트 경부의 디자인에 관한 고찰

연구 목적: 임플란트 식립 후 변연골 흡수에 따라 임플란트 주위 연조직이 재구성되며, 이에 따라 치료의 예후 및 심미성 등에 영향을 주게 된다. 그러므로 임플란트 경부 주위 골조직 보존을 위한 임플란트 경부에 다양한 디자인이 연구되고 있다. 본 고찰의 목적은 초기 변연골 흡수의 원인과 이에 따른 임플란트 주위의 연조직 변화에 대해 고찰하고, 어떠한 임플란트 경부 디자인이 임플란트 주위 조직의 보존에 유리한 지 알아보고자 한다. 연구 재료 및 방법: Pubmed database에서 임플란트 초기 변연골 흡수의 원인과 관련된 논문과 임플란트 경부의 여러 디자인에 관한 논문을 검색하여 분석하였다. 임플란트 경부 디자인은 one piece implant, two piece implant, internal hex abutment, external hex abutment, taper joint connection, butt joint connection, scalloped design abutment, platform switching concept에 관해 검토하였다. 결과: 초기의 임플란트 주위 조직 보존에 대하여 one piece implant가 two piece implant보다 유리한 것으로 여러 임상적, 실험적 연구가 있다. Two piece implant에서는 internal hex abutment가 external hex abutment보다, taper joint connection가 butt joint connection보다 유리할 것으로 보여진다. Scalloped design abutment에 대해서는 논쟁의 여지가 있어 더 많은 연구가 필요할 것으로 판단된다. Platform switching concept은 그 원인이 명확히 밝혀지지는 않았으나 임상적, 실험적으로 초기 임플란트 주위 조직 보존에 대해 유리한 것으로 판단된다. 결론: 임플란트 경부의 디자인마다 각각의 장단점이 있고 추가적인 연구가 더 필요한 제한이 있지만 현재까지의 선행 연구들을 분석 종합해 보면 초기 임플란트 주위 조직 보존을 고려한다면 가능한 경우 one piece implant가 유리할 것으로 판단되며, 보철적인 문제나 다른 이유로 인하여 two piece implant를 고려할 경우 platform switching concept, internal connection abutment, taper joint connection을 이용하는 것이 임플란트 주위 조직 보존에 좀더 유리할 것으로 사료된다 Purpose: The peri-implant soft tissue is remodeled by the initial marginal bone resorption affecting the prognosis and esthetic result of treatment. Thus various designs on implant neck design are studied to preserve peri-implant bone. The purpose of this study is to review on the causes of initial marginal bone resorption, the configuration of peri-implant soft tissue, and the implant crestal module favorable in preserving peri-implant tissue. Materials and methods: The studies on the causes of initial marginal bone resorption and the implant crestal modules are researched and reviewed using Pubmed database. The implant crestal modules including one piece and two-piece implant, internal and external hex abutment, taper and butt joint connection, scalloped design abutment, and platform switching concept are reviewed. Results: A number of clinical and experimental studies preferred one piece implant to two-piece in preserving initial peri-implant tissue. For two piece implants, internal hex abutment and taper joint connection appear more favorable than external hex abutment and butt joint connection relatively. Controversial issues still exist on scalloped design requiring more studies on it. Although the rationale is not certain, the concept of platform switching seems favorable in preserving initial peri-implant tissue based on clinical and experimental studies. Conclusion: Each implant crestal module contains its own advantages and disadvantages with various controversial issues. In the aspect of preservation of initial peri-implant tissue, however, one-piece implant seems beneficial. In cases when two-piece implant is more appropriate due to prosthodontic concerns or any other problems, the application of platform switching concept, internal connection abutment, and taper joint connection may be favorable for the preservation of peri-implant tissues.

안내염과 대장암이 동반된 재발성 간농양

The causes of pyogenic liver abscess has been known as biliary tract disease or intrabadominal infection but the large proportions of the patients has no apparent underlying disorders. Recently colonic mucosal lesions were reported in patients with cryptogenic liver abscess and it has been suggested that colonic mucosal break may play a role in developing liver abscess in otherwise healthy patients. We experienced a patient of severe recurrent liver abscess complicated with endophthalmitis only 3 months after successful treatment of initial cryptogenic liver abscess and a polypoid colon cancer was discovered by chance. It seems prudent to proceed colonoscopic examination in patients with cryptogenic liver abscess especially when it is recurrent.

Thus the chip form control becomes a very important task for reliable turning process. The chip forum is identified using the neural network of supervised data Through the measurement of energy radiated from the chip, The feed mechanism is adjusted in order to break continuous chip according to the result of the chip form recognition and it shows a good approach ;or precision turning operation.

Baculovirus Vector System에 의해 발현된 재조합 Pseudorabies Virus Major Capsid Protein의 면역원성

The recombinant pseudorabies virus major capsid protein (rMCP) was produced by expression of the MCP gene in Sf-9 cell using baculovirus transfer vector system. Following evaluation of the immunochemical properties of the rMCP, the immunogenicity of the recombinant subunit protiens were investigated in guinea pig and swine to obtain the preliminary guide line for the subunit vaccine using rMCP and gP50. It was proved that ultrasonication and 30% ammonium sulfate was most efficient to concentrate and purify the protein. The rMCP was safe in mice, guinea pigs and piglets. In guinea pigs, rMCP mixed with various adjuvants induced substantial degree of serum neutralizing antibody titers, but revealed incomplete protecivity against challenge. In swine, the combination of rMCP and gP50 showed the higher serum neutralizing antibody titers and cellular immune responses than rMCP alone. However, the protectivity was lower in comparison with the commercial gI-deleted inactivated vaccine. We expect these results to contribute to characterization of MCP gene of Korean isolate of PRV and to ultilize as preliminary information for prodution and evaluation of PRV recombinant subunit vaccines.

소록도 官舍施設의 建築的 特性에 관한 硏究

This study is to analyze the architectural characteristics of official houses in Sorok Island. On twenty official house planes, which were constructed during 1927-1940, were surveyed and analized about house type, plane construction style and unit space scale. In the result of this study, first, official house type in Sorok Island was classified into multi-family type and one-family type, plane construction style was classified into the wooden verandah style, compact style and corridor style of the original housing form of Japanese. Second, unit plane was composed mainly of 2∼3 TADANII and an ONDOL room, stand-up kitchen style, bathroom, toilet etc. and added living room of Westem Style in the large scale plane. Third, for the seperation of rooms HUSIMA(wooden sliding doors) had been installed, and TADANII size was used 「Kyung Kahn」 scale of Japanes. In the unit space scale, multi-fanuly type is smaller size than Japanese house, and one-family type is common size.

Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

낙상으로 인한 좌심실 파열 후 생존한 환자 1례

Blunt or nonpenetrating chest injuries are common after motor vehicle accidents or falls. Blunt injury to the heart ranges from contusion to rupture. Cardiac rupture, however, is un-common and associated with a very high mortality rate ; left ventricular rupture due to falling is even more rare. We experienced a case of left ventricular rupture of the heart caused by a fall, which the patient survived. A 63-year-old man fell off a ladder and presented with hypotension from left hemothorax and pericardial tamponade. The hemothorax was relieved via an emergency closed thoracostomy but dyspnea and hypotension continued. And so, echocardio-graphy was done and pericardial tamponade was detected. Emergency left thoracotomy was done to repair the rupture site. We believe that patients with cardiac rupture who reach the hospital alive can often be saved by prompt diagnosis and immediate surgical treatment.

The prevalence of PPNG among pretreated gonorrhoeae cases isolates at the VD clinic of Choong-ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1990, 162 strains of N.gonorrhoeae were isolated, among which 80 were PPNG. The prevalence of PPNG in Seoul showed increased tendency till 1987, thereafter, it has been stationary.

The prevalence of PPNG among pretreated gonorrhea cases isolated at the STD clinic of Choong-Ku Public Health Center in Seoul has been studied and reported annually since 1981. In 1991, 123 strains of N.gonorrhoeae were isolated, among which 58(47.1%) were PPNG. In 1992, 98 starains of N.gonorrhoeae were isolated, among which 51(52.0%) were PPNG. In all, 109(49.3%) strains were found to be PPNG among 221 strains isolated between 1991-1992. The prevalence of PPNG in Seoul showed increased tendency till 1989, thereafter, it has been stationary or slightly decreasing.

This field study explored motivation and satisfaction dimensions of Senior Citizens. Adopting empirical analysis the researcher sampled seniors aged from 55 to 74, and performed questionnaire survey through direct interview. Traveling motives involved in their decision-making are (1) being just with family, friends, or relatives. (2) relaxation and recreation, (3) new experience and visiting unknown places in order. They attach great satisfaction to natural and beautiful scenery, climate of destination, etc.
