        Background : In recent years, gonorrhea has been panedemic and remains one of the most commom STDs in the world, especially in developing countries. Objective & Methods: For the detection of a more effective therapeutic regimen and assessing the prevalence of PPNG, we have been trying to study the patients who have visited the VD Clinic of Choong-Ku Public Health Center in Seoul since 1980 by means of the chromogenic cephalosporin method. Results: In 1996, 139 strains of N. gonorrhoeae were isolated, among which 53(39.0%) were PPNG. Conclusion: Our results suggests that after a peak of 74.3% in 1993, the prevalence of PPNG in Seoul is gradually declining.

        색상 측정 기기를 이용한 복합레진 적층 수복과 단일 수복의 색상 비교 분석

        Objectives: This study analyzed the difference in color caused by different thickness in enamel layer of composite resins when applied with single and layering placement technique, and evaluated if the results agreed with the shade guide from the manufacturers to verify reliability of the color matching process of the manufacturers. Materials and Methods: For single composite resin samples, 6 mm diameter and 4 mm thickness cylindrical samples were fabricated using Ceram-X mono (DENTSPLY DeTrey) and CIE L*a*b* values were measured with spectrophotometer. Same process was done for layering compositie resin samples, making 3 dentinal shade samples, 4 mm thickness, for each shade using Ceram-X duo (DENTSPLY DeTrey) and enamel shade resins were layered in 2 mm thickness and CIE L*a*b* values were measured. These samples were ground to 0.2 mm thickness each time, and CIE L*a*b* values were measured to 1 mm thickness of enamel shade resin. Results: Color difference (△E*) between single and layering composite resin was 1.37 minimum and 10.53 maximum when layering thicknesses were between 1 mm and 2 mm and 6 out of 10 same shade groups suggested by manufacturer showed remarkable color difference at anythickness (△E* > 3.3). Conclusion: When using Ceram-X mono and duo for composite resin restoration, following the manufacturer's instructions for chossing the shade is not appropriate, and more accurate information for Ceram-X duo is needed on the variation and expression of the shades depending on the thickness of the enamel.         이중혈류유발 심폐소생술이 심정지를 유발한 개의 단기 생존율에 미치는 영향

        Background and Objectives: We previously reported that, compared with standard cardiopulmonary resuscitation(S-CPR), better hemodynamic effects could be achieved by simultaneous sterno-thoracic cardiopulmonary resuscitation(SST-CPR) in which we compressed the sternum and constricted the thorax circumferentially during the systolic period by using a device. This study was designed to assess whether SST-CPR, compared with S-CPR, improve the survival rate of dogs with cardiac arrest. Subjects and methods: Twenty-five mongrel dogs(19∼31㎏) were enrolled in this study. After four minutes of ventricular fibrillation induced by an AC current, animals were randomized to resuscitate with either S-CPR(n=13) or SST-CPR(n=12). Epinephrine(1 ㎎) was injected into the right atrium every three minutes after the beginning of CPR. Defibrillation was attempted after 6 minutes of CPR. Standard advanced cardiac life support was started if defibrillation was not successful. Results: SST-CPR resulted in significantly(p<0.001) higher systolic arterial pressure(91±47 vs 47±24 ㎜ Hg), diastolic pressure(43±24 vs 17±10 ㎜ Hg), coronary perfusion pressure(35±25 vs 13±9㎜ Hg), and end tidal CO2 tension(9±4 vs 3±2 ㎜ Hg). Two of 13 animals(15 %) resuscitated with S-CPR and six of 12 animals(50%) resuscitated with SST-CPR survived until 12 hours after cardiac arrest(p<0.05). Donclusion: SST-CPR, compared with S-CPR, improves the short-term survival rate in canine cardiac arrest.

        한국인 성인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 비교

        본 연구는 비만도에 따른 한국인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 특성을 파악하기 위하여 시행되었다. 이를 위하여 계명대학교 의과대학 동산의료원 수면클리닉에 수면장애를 주소로 내원하여 수면다원검사 후 치과에서 측모 두부방사선계측사진 촬영을 한 87명의 성인 환자들을 체질량지수(BMI)와 수면무호흡지수(AHI)에 따라 비비만 단순코골이군(Non-obese, simple snorers), 비만 단순코골이군(Obese, simple snorers), 비비만 수면 무호흡군(Non-obese, OSA patients), 비만 수면무호흡군(Obese, OSA patients)의 4군으로 나누어 비교하였다. 그결과, 4군 중 비만 수면무호흡군의 수면무호흡지수가 가장 컸으며, 비만 수면무호흡군보다 비비만 수면무호흡군의 하악각이 더 크고 혀 길이는 더 작았다. 또한, 비비만 수면무호흡군보다 비만수면무호흡군의 설골이 더 전하방에 위치하였고, 수면무호흡지수에 영향을 미치는 기여 인자는 비만 수면무호흡군에서는 혀 길이, 비비만 수면무호흡군에서는 설골의 후방위치였다. 이처럼 비만 수면무호흡 환자와 비비만 수면무호흡 환자의 측모 두부방사선계측학적 특성과 기여 인자가 다르게 나타나므로, 치료방법도 따라서 다르게 선택해야 할 것이다. 비만 수면무호흡 환자들에게는 먼저 체중감량이 권고되어야 할 것이고, 비비만 수면무호흡 환자들은 폐쇄부위에 따라 구강 내 장치나 Nasal CPAP (continuous positive airway pressure), UPPP (uvulopalatopharyngoplasty) 등이 추천될 수 있을 것이다. Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.

      • 퍼지 추론을 이용한 분류 벡터 양자화기의 성능 개선

        In Classified Vector Quantization(CVQ) for image coding, the block classification pondering the block characteristics will be the most important procedure if we consider system performance. But it does not take effect because classification of conventional CVQ followed act on a heuristic and statistics method. In order to solve the problem of conventional methods, i.e, for block classification effectively we propose a fuzzy inferencing algorithm to improve its performance. To do this, fuzzy rules must be generated in the first. Our rules are based on energy distribution in transform domain with respect to spatial variation. Ultimately fuzzy inferencing of this paper apply these fuzzy rules for efficient block classification on a input image. As a result of efficient classification, our proposed method shows that blocking effect is decreased and PSNR(peak to peak signal to noise ratio) is increased by about 1 ∼ 2 dB for 0.2 ∼ 0.6 bpp range, when the performance is compared with Aizawa's scheme^[9].

      • 정신분열병 환자의 집-나무-사람 검사 반응과 정신병리

        Objectives: This study was conducted to compare the characteristics of schizophrenic subjects with those of normal subjects in the H-T-P test. Materials and methods: Fifty-six schizophrenics and 53 normal subjects were examined and scored with the H-T-P test. And their drawings were scored by using 85 objectively scorable items developed by the author. Results: The House : The schizophrenic subjects depicted less frequently, the houses of 2 or more floors, doors, ways or pave stones leading to the doors, door knobs, windows, accessory trees and animals, chimneys, smoke, and fences. The pictures of House drawn by schizophrenic subjects showed slanted houses. The Tree : Contrary to the expectation, keyhole trees and the looping lines of the trees occurred with low frequency in the schizophrenic group. The Person : The most striking differences of the two groups were the sexual depictions, moving objects, smiling face, appendages of clothes (eg. button, pocket, belt, texture etc.), and the constructional acuity of bodies, which occurred less frequently in the scizophrenic group. Another difference is the absence of ears in the pictures of the schizophrenic group. Conclusions: In conclusion the H-T-P test can identify the psychological contents, cognitive functions, and social attitude of the schizophrenic subjects, based on the contents and structures of their drawings.

        두가지 혈류 유발방법에 의한 새로운 심폐소생술(이중 혈류 유발 심폐소생술)장치의 개발

        Background: There have been many efforts to augment blood flow during cardiopulmonary resuscitation. These efforts have focused on maximizing the effect of cardiac pump or thoracic pump alone. However, considering that the heart is the biggest blood reservoir and increase of intrathoracic pressure can generate blood flow, simultaneous exploitation of both mechanisms may have synergistic effect. We hypothesized that simultaneous chest constriction in addition to sternal compression by standard CPR may have additive hemodynamic effects by preventing deformation of the chest and increase of intrathoracic pressure. Methods and results: we built a new mechanical device to perform compression and thoracic constriction simultaneously. The device consists of two main elements. Piston in the center is to depress the sternum. Strap is to constrict the thorax circumferentially. Strap is attached to both sides of the piston. When the piston is pushed down, it depresses the sternum and pulls on the thoracic strap. To determine strap width to produce optimal hemodynamic effect, we measured hemodynamic parameters with variable widths of strap in two dogs after induction of ventricular fibrillation. Result of the experiment showed that 10cm wide strap was determined to be most effective. We also determined optimal depth of compression to produce maximal hemodynamic effect with animal experiments using two dogs. Animal experiments showed that the highest aortic pressure could be generated when the stemum was depressed to 5 cm. Cardiopulmonary resusciation using a new device could generate higher systolic aortic pressure, coronary perfusion pressure and end-tidal carbon dioxide tension in comparison with standard cardiopulmonary resuscitation in a pilot animal study using two dogs. Conclusion: New cardiopulmonary resuscitation method using a mechanical device designed by us could perform sternal compression and simultaneous thoracic constriction, and generate better hemodynamic effects than standard cardiopulmonary resuscitation in pilot animal experiments.

      • Epoxy-Amine-PGE系의 力學的 性質

        epoxy수지와 함께 경화제로는 ethylene diamine(EDA), hexamethylene diamine(HDA), diethylene triamine(DETA)등을 사용하였다. 한편 첨가제로 phenyl glycidyl ether(PGE)를 사용하여, 이의 함량변화에 따른 각 hardner system의 力學的 性質에 대한 效果를 實驗하였다. 이를 위해 여러 가지 경화조건과 혼합비율에 따라 인장특성과 충격강도등을 측정하여 이들의 함수관계를 알 수 있었고 또한 이 composites의 역학적 성질이 최적화되는 조건을 고찰하였다. The purpose of this study is to research into the effect of PGE(Phenyl Glycidyl Ether) on mechanical properties of Epoxy-Amine-PGE system. For measurement of the mechnical properties, tensile properties and impact strengths are measured in accordance with an ASTEM method. Mechanical properties of Epoxy-Amine-PGE system have been investigated as a function of PGE content. The properties of the network structure are predominantly controlled by appropriate choice of PGE content. Results are as follows; 1. In three amine hardener systems, tensile properties increased but impact strengths somewhat decreased as a PGE content was increasing. This result may be due to effect of PGE which decreases system cross-linking density as "chain stopper". 2. Tensile strengths of Epoxy-HDA-PGE system were higher than those of Eposy-EDA-PGE system as a whole. This result may be due to decreasing of cross-linking density in system with increasing of molecular weight of hardener. 3. In Epoxy-DETA-PGE system, tensile strength had a maximum value and impact strength had a minimum value at PGE-12 phr.

      • 기호학적 입장에서 본 탈춤의 연극성

        이글은 Tadeusz Kowzan과 Erika Fisher-Lichte의 이론을 원용하여 탈춤의 연극성을 규명한 작업이다. 언어/춤, 언어/상활, 언어/제스츄어 등 여극 기초간의 대립적 양상에 의해 탈춤의 골계적 비극성이 드러나는 것으로 파악하였다. 이 독특한 연극성이 현대의 마당극에도 적용되어야 비로서 새로운 한국 연극의 기틀을 마련할 수 있을 것이라고 판단된다. The concept of theatricality has undergone a historical change. How to define theatricality is a controbersal matter among contemporary theater critics. Un this regard the semiotic approach provides an efficient tool for clarifying the unique theatricality phenomena drown the audience in a burst of laughter tinted with tears.

        As You Like It에 나타난 Fool 플롯 연구

        To the Renaissance audience, the prevailing attitude on love and nature in pastoral was an ambivalent one. It would have presumably required an uncommon courage and foresight to perceive this problematic relationship. Moreover it would have been more than a creative job for a playwright to develop a new technique of dramatizing it. Shakespeare seems to have fulfilled this requirement. By developing Vice into Fool, Shakespeare established the Fool convention more firmly than any other contemporary writer did. The significance of the Fool convention is to be fully understood in terms of the Fool's role to place every characters in his/her own background. The fact is that contemporary minor writers presented a puppet shepherd and shepherdess estranged from his/her natural background or idealized on a pure intellectual level. While Shakespeare took a concise measurement of every psychological distances of the characters in experiencing nature and succeeded in constructing vivid characters. Touchstone and Jaques, unseen in Lodge's Rosalynde, play typical fools. Especially Touchstone shoots his bolts behind a stalking-horse hiding his character, which is traced back to the character of Vice as an ambidexter. He transforms himself so brilliantly, depending on the opposite speaker, that one is simply fascinated. But his transformation in AYL contributes to revealing the Weltanschauung of other characters so critically that each is individualized and organically merged into his own background. In conclusion, the Fool convention made it possible for Shakespeare to achieve three goals : the synthesization of various views of the Elizabethan age, the divulgence of the problematicity of pastoral view on nature and love, and the characterization in depth.
