        Characterization of tissue conditioner containing chitosan-doped silver nanoparticles

        Purpose: Development of a latent antimicrobial soft liner is strongly needed to overcome a possible inflammation related with its dimensional degrade or surface roughness. Modified tissue conditioner (TC) containing chitosan-doped silver nanoparticles (ChSN) complexes were synthesized and assessed for their characterizations. Materials and methods: ChSN were preliminarily synthesized from silver nitrate (AgNO3), sodium borohydride (NaBH4) as a reducing agent and chitosan biopolymer as a capping agent. Ultraviolet-visible and Fourier transform infrared spectroscopy were conducted to confirm the stable reduction of nanoparticles with chitosan. Modified TC blended with ChSN by 0 (control), 1.0, 3.0 and 5.0 % mass fraction were mechanically tested by ultimate tensile strength (UTS), silver ion elution and color stability (n=7). Results: At 24 hour and 7 day storage periods, UTS values were not significant (P>.05) as compared with pristine TC (control) and silver ion was detected with the dose-dependent values of ChSN incorporated. Color stability of TC were influenced by ChSN add, with the higher doses, the significantly greater color changes (P<.05). Conclusion: A stable synthesized ChSN was acquired and modified TC loading ChSN was characterized as silver ion releasing without detrimental physical property. For its clinical application, antimicrobial test, color control and multifactor investigations are still required.

        은이 첨가된 생체 활성 세라믹 복합체 연구

        은 콜로이드 용액을 이용하여 은이 침적된 생체활성 세라믹 복합체를 제조하였다. 제조된 은 콜로이드 용액과 생체활성 세라믹 복합체의 물리적 특성은 각각 X-선 회절분석기, 라만분광기, 전자현미경으로 분석하였다. X-선 회절분석 자료에 의하면 은 나노입자의 표면에 염소이온이 화학적으로 결합한다는 사실을 알 수 있었다. 또한 전자현미경 분석에서는 은이 침적된 생체활성 세라믹 복합체의 표면에 염화은이 균일하게 분포하는 것을 알 수 있었다. 따라서 생체활성 세라믹 복합체 표면의 염화은이 생체활성 세라믹 복합체의 하이드록시아파타이트 형성을 강하게 방지한다는 사실을 확인할 수 있었다. Ag-Doped bioactive ceramic composites were prepared by colloidal silver solution. The physical properties of colloidal silver solution and Ag-Doped bioactive ceramic composites were characterized by Scanning electron microscopy(SEM), X-Ray Diffractometer(XRD) and Raman spectrophotometer respectively. According to XRD, we have identified that the chloride ion was chemically attached silver nano particles. SEM studies showed that silver chloride phases were homogeneously distributed on the Ag-Doped bioactive ceramic composites surface. Finally, we concluded that the silver chloride phase on the Ag-Doped bioactive ceramic composites surface was strongly prevent formation of Ag-hydroxyapatite.

        키토산이 첨가된 의치상 레진 복합체의 항균 특성 및 물성에 관한 연구

        In this study, we proposed a modified polymethylmethacrylate (PMMA) denture acrylic impregnated with chitosan as a novel antimicrobial denture base material. We fabricated the samples of chitosan-PMMA denture acrylic (CPD) into four types (1.0 , 3.0 and 5.0 % and 0 %; control) according to the mass fraction (w%) of chitosan blended to pristine PMMA powder with methylmethacrylate (MMA) liquid by the ratio of 1.7 g/ml. For mechanical evaluations, surface morphology, degree of conversion, flexural strength and color changes were tested. The antifungal effects were evaluated in vitro as a number of viable cells (CFU; colony forming unit) in retrieved fungal suspension on samples after 24 hour incubation. A significantly reduced CFU was exhibited at 5.0 % CPD (p < 0.05) by the 40.9 ± 2.1 % inhibitory effect. CPD exhibited the similar microscopic surface texture and no statistical differences (p > 0.05) of the degree of conversion and flexural strength values as compared to pristine PMMA. Color differences were detected at 3.0 and 5.0 % of CPD by the reference value of 2.69. Within the limitation of present study, CPD could be a possible intrinsic antifungal denture material with the proper mechanical characters for a denture biomaterial. For clinical application, future studies including color stability, anti-adherent mechanism and the long-term effect were still required. 키토산이 투여된 의치상 레진 (CPD: chitosan-PMMA denture acrylic)의 항균특성 및 물성을 평가하기 위해 키토산 분말을 PMMA 분말에 0 (대조군), 1.0, 3.0 및 5.0 w% (질량 분율)로 첨가한 후 혼수비 1.7g/ml로 MMA용액과 중합하였다. 항균특성은 Candida albicans (ATCC 14053) 희석 균액 100 μL를 시편표면에 접종하고 24 시간 배양 후 회수 된 진균 현탁액의 생균수 (CFU; colony forming unit)로 평가하였고 키토산 분말첨가에 따른 의치의 물성변화는 중합전환율, 굴곡 강도 및 색조안정성을 통하여 측정하였다. 5.0 % CPD에서 대조군에 대한 유의성 있는 진균 부착 억제율 (40.9 ± 2.1 %)이 관찰되었고 키토산 첨가에 따른 중합전환율, 굴곡강도 수치의 통계적 차이는 나타내지 않았으나 (p=0.05) 3.0 w% 이상 투여 군들에서 농도비례적으로 허용 색조변화 기준치 (ΔE =2.69)를 초과하였다. 본 연구의 키토산-의치상 레진 복합체는 적절한 물성과 내재적 항균특성을 가진 치과생체재료로서의 가능성을 도출하였고 임상 적용을 위하여 색조 안정성 개선을 포함한 명확한 항균작용기전 그리고 장기 효과 등의 연구들이 계속 진행되어야 할 것으로 사료된다.

        상아질접착제와 열순환에 따른 유동성 레진의 전단결합강도 비교 연구

        Statement of problem : Limited research on flowable resin has been undertaken on its application directly on dentin associated with the adhesive systems. Purpose : This study was to evaluate the shear bond strengh and fracture aspect of flowable resin on human dentin with various types of dentin bonding adhesives with thermo cycling effect. Materials and methods: Filtek-Flow(3M ESPE, USA) was used as flowable resin and Eighty human molars were randomly divided into 4 groups : three dentin bonding adhesives (Scotchbond-Multipurpose : 3-step contentional system, One-Step : One-bottle system. Prompt L-Pop : All-in-one, self-etching primer) and 32% etching treatment without bonding adhesive as a control group. For evaluating their durability of bonding, each group was subdivided : storaging in the water at 37$^{\circ}C$(24 hours) and thermocycling (0$^{\circ}C$-55$^{\circ}C$, 30 seconds intervals, 1000 cycle). Shear bond strength tests were performed and resin-dentin interface and fracture mode were observed. Results were analysed by one-way ANOVA and Scheffe's multiple range test. Results and Conclusion : 1. At 0 cycle, the mean shear bond strength of One-Step exhibited the highest value of all groups(p<0.05), and there were no significant differences between Prompt L-Pop and Scotchbond-Multipurpose, Scotchbond-Multipurpose and control(p>0.05). After 1000 thermocycling, One-Step exhibited higher value than other groups(p<0.05), and there were no significant differences among other groups (p>0.05). 2. The shear bond strength of each group was significantly decreased after thermocycling except Scotchbond-Multipurpose (p>0.05). 3. The most common failure mode was adhesive type and mixed type, next in order.

        주관절 강직의 관절경적 치료

        Purpose: Management of the stiff elbow by arthroscopic procedure is an effective but technically demanding. Our purpose was to review the specific arthroscopic maneuver which can be useful for the stiff elbow. Materials and Methods: A stiff elbow that is refractory to conservative treatment can be treated surgically to remove soft tissue or bony blocks to motion. The olecranon or coronoid osteophyte and loose bodies have been removed arthroscopically with good results and rare complications. Results and Conclusion: For the successful arthroscopic management of elbow stiffness, it need to knowledge and skills for debride contracted tissue and preserve vital anatomic structure. 목적: 주관절 강직의 관절경적 처치는 효과적이지만 술자의 능력이 요구되는 것이 사실이다. 이 종설의 목적은 주관절 강직에 대한 관절경적 처치 방식을 기술하고자 한다. 대상 및 방법: 비수술적 치료에 반응하지 않는 주관절 강직의 경우 운동 제한을 유발하는 연부 조직 및 골성 물질을 제거하게 된다. 이때 주두, 구상 돌기의 골극과 유리체의 제거는 합병증의 빈도는 낮고 좋은 결과를 보장한다. 결과 및 결론: 관절경을 이용한 주관절 강직의 치료에서는 구축된 조직을 절제하고 중요한 해부학적 구조물들을 보호하기 위한 지식과 숙련된 기술이 필수적이다.

      • 뇌졸중으로 인한 편마비 환자에서 슬라이딩 재활기구를 이용한 하지 운동의 효과

        Background: The purpose of this study was to determine the effects of the Sliding rehabilitation machine¢c(Kyungpook National Univ, Korea) in patients with hemiplegia caused by stroke. Method: Forty hemiplegic patients with stoke were enrolled for in this study. Experimental group (n=20) were trained with the sliding rehabilitation machine for 20 min/day, 5 days/week, for 2 weeks. Using the rehabilitation machine, the paretic lower limb could be forced to use selectively compared with the nonparetic lower limb. Control group were trained with conventional physical therapy. Functional Ambulatory Category (FAC), lower extremity Fugl-Meyer assessment (FMA), Brunnstrom stage of leg, Berg Balance Scale (BBS), lower extremity Motricity Index (MI), and lower extremity muscle power by hand-held dynamometer were measured before and 2 weeks after training.Results: Significant improvement of the lower extremity Motricity Index, and the lower extremity muscle power was observed in experimental group (p〈0.05). FAC, BBS, lower extremity FMA were improved in both groups (p〈0.05). Conclusion: The sliding rehabilitation machine will be effectively used for lower extremity muscle strengthening in patients with hemiplegia.
